Fantastic experience: Honestly, I was nervous moving from the hospital to this PT facility; however, as soon as I got here, the warmth and kindness of the staff made me feel comfortable, and assured me that I was in excellent hands. I found the staff consistently sweet and at the same time highly skilled. There are too many wonderful staff to name all, and at this point, I sure do wish I had a list, here are a few: The nurses, Megan, Ravneet, and Martina, PT instructor Roman gave me confidence and encouragement, Kevin in OT, wound care Daniel-you are the absolute best! All the nurses assistants, thank you, special thank you to Andrea and Flor. So sorry for anyone I forgot to mention. Thank you, all!
Fantastic experience: I love the kind and thoughtful and hardworking therapy department. I like nurses but could use a little bit more communication but when they are in the rooms they are very nice and thorough. My CNA helped me out when I needed a shave and I really appreciate that.
Positive experience: I've been here a week and it's fairly decent. One of my favorite staff members is Gracie. Everyone tries to make your stay as pleasant as possible.
Positive experience: Very clean rooms, I would love to give a shout out to the wonderful and through Katt the CNA also Harman the wound nurse does a great job.
Negative experience: DO NOT PUT YOUR LOVED ONE HERE!! I wouldn't even put my worst enemy in this awful place!! It looks like a hospital straight out of a horror movie! My dad was placed there after surgery on his femur and when I went to visit him he looked scared and pail! His call light didn't work and and when I tried asking the CNA a question, she was extremely rude and wouldn't even look at me! The food looked like puke and the whole place smelled like a porta potty! It was very dim and dirty. The bed was so thin, that after 3 nights of sleeping there, my father had bed sores on his back side. The CNAs or nurse didn't even notice them! This place should be shut down!! Luckily I was able to have my father relocated to a beautiful place in Tracy with an amazing staff.
Fantastic experience: I love the friendly and caring staff my roommate is very nice. Lastly very clean and taken care of rooms
Negative experience: Crazy place to work most staff are bullies from janitor to RN unless your part of they're facilty. Station 3 nurses are bullies..start my day with vital sign then off to give showers. 2 nurses are complaining resident needs care while I was giving shower then intentionally ask me to retake vitals coz it's too high nor too low..on station 2...2 cna that's works there for 2 decades are mean when ask for stand by pt transfers suppsed to be 2 cna assistance but just ignore you sitting laughing in nurse station and don't help so I just managed to do it on my own.its also one of the dirtiest no supplies so un organized nasty bathroom and breakroom that I endure for 2 weeks.I'm here to come to work not to make long life friends...guess what the facility DNR me for no reason.
Fantastic experience: Very kind and caring staff, call button is always answered, therapy department is through and great.

Fantastic experience: This facility has great staff and are always very welcoming. Patient care is their #1 priority. I had a loved on here who was there until he passed. They treated him so well and always communicated with our family. As a Healthcare vendor, I continue to visit this facility and appreciate their amazing staff for trying their best to go above and beyond.
Negative experience: I’m a delivery driver and got an order to deliver here. The place is horrifying. AC broken, just have portable units in the halls. It’s been over 110 for two weeks. The place is poorly lit and looks like it’s half abandoned. It’s like something out of a horror film. If I reach that age I would rather die than end up in a place like this.
Negative experience: Do not send your loved one here. This place needs to be investigated. Period.
Negative experience: If I could give 0 stars I would. My mother in law was sent to this place and is NOT getting the care she needs. Can’t get ahold of the administrator Ammon or anyone in charge to get my mother in law in her own room so she can rest and come home. Tried calling back after my initial complaint with no one answering. Poor care. Do NOT take your loved ones to this place.
Negative experience: DO NOT SEND YOUR LOVED ONE HERE. My dad was sent here after surgery for rehabilitation and we pulled him out of here less than 24 hours after he got there. He was short of breath (He had surgery on his throat/neck area and was in a neck brace) and while on FaceTime with my mom (who was at work) he told her “I can’t breathe good” He pressed the button for the nurse and my mom was shouting for them through the phone. No one acknowledged them or came to help. At that point my mom told him to bang on anything and try to yell. It took them OVER 5 minutes to come assist him. Instead of giving him an air mask they asked questions about anxiety, if he was hurting and was ignoring my mom who was panicking. My mom is yelling at them through the phone that “He needs air and cannot breathe, get him some air and then he could talk” The nurses point the phone at the ceiling so my mom could not see my dad anymore and continue to stare at my dad as he starts to pass out. My mom is hearing his breathing struggle and is screaming at the nurses to “Save my husband!!!” When they finally get an air mask on, my mom asks if someone can stay in the room with him and monitor him until his breathing is stable. She was told that they can stay up to 5 minutes then they would have to leave. They start to leave the room and my mom asks my dad if there is a nurse in the room with him and he (While still on the verge of going unconscious) says “No.” They never came back in to check on him. My mom stayed on the phone for an hour to calm him down and to make sure he didn’t have another episode where he could not breathe. After she calmed him down and he said he could breathe again he told her that she could hang up. She was terrified that no one would come back into check on him. As soon as she was off work she went to the facility still in work clothes to speak with them directly. He needed to be sent back to the hospital (THANK GOD) and left the facility. My sister seen his condition while he was at the facility where they had OVER-MEDICATED him and said that he looked and acted like a zombie, he was not there. During that visit they transferred him back to the hospital, she saw that he started to improve almost immediately after he left and was alert and aware. My dad is not a fearful person but told my sister AFTER he was transferred back to the hospital that he “Did not feel safe and was afraid for his life at that facility” The hospital is now saying that they recommend he go back to THAT rehabilitation center (Almond Vista Health Care) We are bringing him home, our family will be round-the-clock caring for him where he will not be left alone. THIS PLACE NEEDS TO BE SHUT DOWN IMMEDIATELY AND KAISER NEEDS TO STOP SENDING PATIENTS HERE. Your loved ones are in DANGER here!

Fantastic experience: This has been my 4th skilled facilty and I would rate it my favorite (the best). The care & food was fantastic. I would come again with no hesitation. Physical therapy was encouraging & motivating to help me with mobility. Thank you nurses and Ben in PT for all you did.
Positive experience: Nurses & CNA were very attentive. I would come again. Loved physical therapy, Roman & Jason was great.
Positive experience: I've been to most of the skilled nursing facilities in Modesto for rehab and this is one of the best. The food is decent, sometimes even good lol. That's saying a lot. I was well cared for and judging by the lack of parents yelling for help. So we're the long term residents. I was here when it was Wnddor and it was a pit. Extreme improvement since. I'm sure it happens, but I never wheeled by a room reeking with the odor of soiled diapers pouring out. If you've been to some of these places,you'd understand. Bit boring. If there were activities, I didn't know about them. Overall. A clean and well run place. I'll come back if I have to do this yet again. Side note: not a great selection of alternatives if you don't want the main dish, but at least they have alternatives.
Fantastic experience: My spouse recently had double knee replacement surgery. I'm grateful for the staff. The facility overall is very clean, the care he is receiving is better than acceptable. Everyone is friendly kind and very helpful. The food is . His recovery will be another week or so. But in my heart I know he will be fully functional once released thanks to the nurses, PTs, dietary staff, housekeeping staff and administration.
Fantastic experience: This amazing Hospital has a Great Team of amazing Nurses and STaff and My Mom Irma Huffman has been there since 2016 and it was called Windsor and my Mom has been getting Great care and if there is anything that happens I really don't blame them for She's been Loved and cared for she was in another place across town and I moved Her there my Mom is not easy to take care of i know this because I was her caregiver at Home and it's not easy but anyway she's really been taking care of Great.
Fantastic experience: I have been a patient here 4 or 5 times now to recover from surgery or for nursing care for various reasons and I have to say that I am so glad I have this place to rely on during difficult recoveries. It may not be the fanciest but I couldn’t ask for better care. The nursing staff and therapy team are wonderful and have treated me with respect and understanding to my, what can be very challenging, medical and rehabilitation needs. I also want to shout make a shout out to the kitchen staff. Thank you for putting up with my picky dietary wants/needs. Thank you everyone for giving me a safe and compassionate place to come to during some trying times.

Great staff and great nutritionist. 2nd stay was recently under new owner. Rt knee replacement surgery. Experience again great. Pt 5x-7x weekly. Again staff and meals were great. Would I stay Again. Absolutely .

Fantastic experience: Hands down the best place to put your loved one. The staff is caring and will help you with any questions you have. I’ve had family at other places and I can’t even compare them to Almond vista. I won’t take my family anywhere else.
Fantastic experience: Don’t read the negative reviews they are old and about the old facility. Almond vista is now new and improved and they are renovating everything to make it seem newer and better for the workers and residents. The workers are super nice and caring to everyone. So please give almond vista a chance I promise you won’t be disappointed.

Fantastic experience: Excellent Place for recovery. They provide therapy 7/day a week which is excellent for my loved one. He was able to d/c home in timely manner. Nursing staffs were great. They were able to help my loved one with respect & dignity.

Fantastic experience: At first I was very skeptical because of the rating and reviews. Until my mom’s rehabilitation. She was in the best hands. Her staff was very attentive and compassion towards her. She was talking how much food was given. Visiting is only one person because there’s a lot of elderly and by appointment only. It was wonderful to hear their was Bible study and worship praising King Jesus, available.
Fantastic experience: People please don't believe everything you read as far as all the negative reviews they couldn't be more wrong. I've been here 12 day's and on my way home and I wouldn't be going home so soon if it wasn't for all the warm caring staff of Almond Vista. The therapists that motivated me to want to get better and I am. The kitchen staff who made me a lot of grilled cheese sandwiches over the last 2 weeks thank you they were great! And to the staff in Yellow Zone you know who you are THANK YOU FOR TAKING THE BEST CARE OF ME! ❤
Fantastic experience: My grandmother was placed here on hospice. The care and compassion they showed her and our family was outstanding. I would highly recommend this facility.
