The Springs Health & Rehabilitation Center

Fantastic experience: My husband was admitted to The Springs for some extra care after surgery at a traditional hospital. Our experience with The Springs was excellent from the first person that greeted us to the last person that we saw as we said our goodbyes. Everybody there is extremely professional, but also extremely caring. We felt a personal connection with every member of the staff that we came across. Especially the team that helped Rich the most. That would include Rita and Beatrice, both CNA‘s, Aaron and Susanna, both from physical therapy and occupational therapy, and Francis, who helped my husband with wound care. Being in an extended stay in a nursing facility can be very difficult for the patient and the family. Everybody involved in my husband‘s care, felt like family to us and helped him heal that much faster. I can’t say enough good things about The Springs.

Fantastic experience: My dad was there for two weeks and for the most part he had a great experience. The only thing that was a problem was that many times when he needed help with the restroom, he would push the buzzer and it would take a while for anyone to come in. We’d even have to look for someone to help. I’m just thankful a family member was there the majority of the day to make sure this was taken care of. He enjoyed the food. Overall he was happy with his stay

Fantastic experience: The Springs is truly a God sent and an answered prayer. I had my mom go there for some physical therapy and rehabilitation after recovering from the hospital. The nurses and staff are kind, compassionate, understanding, knowledgeable and extremely helpful. During my moms stay, we had to bring her sad news. Her husband had passed away. Every single staff member was there for my mom including any emotional need. They actually said my mom was an inspiration to them. Those words stuck with me and strengthened me and I wasn't even the patient! Lol. What I am getting at is The Springs not only cares for their patients... but people in general. I wish I could give The Springs more than a 5 star review. If I had my way I'd give them the sun, moon and stars! Thank You Jesus for giving us The Springs.

Fantastic experience: My mom was at The Springs for about a month after she broke her hip and had surgical repair. The facility is clean, the people are caring and kind, and communation with family was great. My mom didn't care for a lot of the food, but there are alternative options that were sometimes better. My mom may give it a lower score but she was the one asked to do painful things. She did say that sometimes she felt like it took too long to get help. I saw staff members constantly moving, so I'm sure they are doing everything they can. My mom loved the hairdresser there and that was a special bonus. They have recreational activities but she did not partake in any. She had a special fondness for Jackie. She is so sweet and kind. Dalila was great, and always greeted us with a smile. I didn’t learn other staff member names but I would like to thank all of them. I'm appreciative of the good care they gave my mom. It was one less worry while she was there.

Fantastic experience: Wow!!! Where do I begin!!! Well i will tell you that this was the first time anyone in my family needed to stay at a rehabilitation center and we were so nervous to leave mom somewhere other than home. I can tell you from my heart that The Springs exceeded my expectations!!! This place is Amazing, they thought of everything and every single person here displays kindness and caring ❤️ if you ever have this need, this is your place!! Do not go anywhere else, if you want The Best!! I will highlight a couple folks that mom.said were fabulous to her and thanks to them, she was happy and recuperated much quicker: Manielle Calder was so attentive & gentle to mom, she wanted to bring her home , Myra Valdez was also just so nice and pleasant. She will always remember her. her therapists Angelo & Andrew were Amazing and pushed her to advance and get the job done, they would play her favorite songs and were just so amazing. I will also say that for me, the daughter caring for mom, Gregory(social worker) was so informative, kept me up to date and met all my requests. Communication was by far The Best with him. There should be a Gregory at every center and last but not least, Perla(case manager) was also so pleasant and kind and made.sure she addressed all my questions. If I give more stars, this place would get a 10 from me THANK YOU FOR CARING FOR MY MOM ARMIDA WITH THE LOVE AND KINDNESS YOU ALL POURED!! we are Thankful beyond words!!!

Fantastic experience: I chose The Springs when my mom was released from the hospital. The moment I walked in I was impressed. My mom thrived at The Springs. The care was exceptional, the food was great and the place was always clean and always smelled good. The staff treated her like a queen. They had daily activities that my mom enjoyed such as painting and crafts. The grounds are very nice and I would sit outside with her at the tables to get some sun. Between the daily physical therapy, the quality care, and the good food she improved rapidly. I can’t say enough good things about The Springs.

Negative experience: I call this place on a Sunday to speak with a patient. these people don't answer their phones. And then, when they answer the phone, they transfer you and nobody answers the phone I don't know what kind of place this is but I wouldn't put My family and there and then they pick up and hung up on me.

Fantastic experience: I could not be more pleased with my experience at the Springs! The overall atmosphere of caring and respect was truly amazing! From the nursing staff, the accommodating kitchen, physical therapy, literally every component of the facility that came together, made for a fantastic 9-day stay. I will choose the Springs again if the situation dictates! 5 Star!!!!!

Fantastic experience: Mom was here for 3 weeks, she was very well cared for and very happy. The CNA, Rita and Ivy were absolutely wonderful. Also Yvonne the social worker was very helpful. Would definitely recommend The Springs.

Fantastic experience: I've had negative experiences with other facilities when my mother needed to stay and recover after an illness. This time my father-in-law needed a place to recover. The Springs has the best staff. The are very professional and friendly. They made him feel comfortable which made me feel comfortable leaving him there. It's beautiful and clean. I can't say enough about the staff. They were very attentive and answered all my questions. I would highly recommend this place.

Fantastic experience: This is a very nice facility with a helpful and friendly staff. They helped my mom for 3 ½ weeks to get strong enough to return home. The physical therapy was very good, but only once a day. More often would have been better.

Fantastic experience: The facility was clean & bright & a positive environment. The staff was kind, helpful & pleasant. The PT & OT was there everyday to help me get better & stronger so that I could get home soon. We had one issue with the call light button but when I addressed it to the administrator he was very quick to fix the issue! I would recommend The Springs Rehabilitation Center to others for sure.

Fantastic experience: My father is 96 and he's been at the Springs for approximately 45 days, and I know he is receiving xlnt care. Staff keeps me updated regularly. And he never fails to mention the nursing and physical therapy staff,.as well as the quality of food. Thank you to all.

Fantastic experience: I would like to Thank everyone from Dr Patel to Lynn, our case manager to all the Nurses, Therapy workers and all of the rest of the hands on staff. The Springs is a Top Notch facility in Murrieta. I would recommend the Springs to any and everyone nearby or out of the area. My mother actually asked a few time if she had to leave, instead of " When can I come home: This is a testament of care coming from a patient. Fantastic experience: This place is absolutely fantastic. All of the workers love the people and serve with extreme kindness. It is a very clean facility. The staff are attentive and professional. The nurses, PT, OT and speech therapists were all so patient and kind. The CNA’s were outstanding!!! The servers and activity (dining area) are some of the nicest and most patient people I have ever met. They made my mom’s experience incredible. I will never forget all the love and kindness shown to me and my family. I would recommend this facility to anyone.

Fantastic experience: The CNA’s were great! They worked their tails off! The LVN’s also were diligent & truly cared about you! Physical therapy rocked! Dylan & Martin made my recovery easy & beneficial with their tips for home! Kayla went above & beyond in getting me a potty chair when the company dropped the ball on delivery, Kayla went & picked it up. The only bad thing was Joshua & the food! He couldn’t get me a renal diet to save his life. Idk who built the trays of food but they weren’t knowledgeable in renal diets

Fantastic experience: When i was told i had to go to rehab i said i will only go to The Springs. I heard good things about the care there. My stay there was wonderful i highly recommend it . The whole staff was competent and caring.The facility was clean and organized.The food was not always to my taste but there was always something on the tray i could eat. Staff that did a great job . Fantastic experience: DAISY, GIGI, ANN, DON, KYLE, ANDREW!!!! K. SANCHEZ! DENISE!!! The Dietician, OT, PT... you all listened, adjusted, persevered, and served my family member w care! I tried to remember everyones name because I really wanted you to know that you mattered to me, the way I wanted my mom to matter to you. Humans...thats who we are together sharing our talents w kindness and love. I stayed involved during my moms stay and you never once rolled your eyes at me ( at least not to my face! Fantastic experience: My brother and sister said that the care that was given for our mom was excellent, in fact, my brother said that he could not say enough good things about how well that she was cared for. We were truly blessed to have had our mom stay at The Springs Health and Rehabilitation facility.

Fantastic experience: I was extremely well cared for by a truly caring and capable staff of nurses, assistants, and physical therapists. Most importantly, I could see and feel the love of God flowing through both the place, and the people. A bit of culinary correction might be in order, yet the entire experience was a blessing.

After she was transferred there from the hospital they took her in like family. The communication and the care was great. Terri from the Springs was very helpful, and she was so knowledgeable and professional. I would highly recommend The Springs.

Fantastic experience: Amazing facility! Management, nursing staff and rehab team is awesome. They had my grandma walking and transferring herself again in a matter of a week and a half and also managed other issues she was having very well! Fantastic experience: I am so grateful for the care that my mom recently received when she stayed weeks at The Springs. So much so, that I asked to speak with who was in charge to tell them what a wonderful job they were doing. I was a little got up in emotion and forgot to mention a few things. Every single time I came to see my mom the facility was spotless. My mom was treated with respect and dignity. You never realize how vulnerable your parent or love one can be until you're in a situation where you have to trust their care to someone else. Every staff member from the receptionist to the nurse the individuals that cleaned and cared for her to the therapist were so amazing. They have patience and compassion and what they provide for your loved one is priceless. The case workers were so diligent on keeping me informed and when something needed to be adjusted or changed the communication was there to make that happen. Thank you so much for everything! You have no idea how much it has meant to my family!

Fantastic experience: The outside of the facility is very inviting. When I entered the lobby, I was greeted by a friendly receptionist. She helped us locate my husband's room. The gym looks well equipped for the rehabilitation program that you have for your patients. My husband was very impressed with the wonderful staff. They are all very friendly and all seem to be very knowledgeable as to how to rehabilitate patients to be able to walk again. The food was very good, even though all items on the menu were not given to some patients. For the most part the place is cleaned continuously. One time the public bathroom needed to be cleaned and the next time I used it, it had been cleaned. My husband just raved about how accommodating the nurses and all the staff were to him during his stay at "The Springs."

Fantastic experience: The staff was very helpful and friendly! It was a great idea to have a main point of contact who was Lynn for us to help with my mother’s care, questions and issues. Gemma was friendly but it was not helpful that she did not return calls, and that caused a lot of anxiety whether the medical insurance was submitted for my mother. Lynn, the CNA’s, and Physical Therapy are the stars of The Springs!

Fantastic experience: My mother was recently a patient in The Springs and had a very good experience. The facility was clean, fairly new, all the staff was helpful and mom got the rehabitation she needed to recover. 5 stars.

Fantastic experience: An outstanding rehabilitation center. My 91 year old mother was so fortunate to be transferred to this facility from the hospital. Her care was immediately taken forward by just about everyone on staff. Not only did the caring staff took care of my Mom, but they made sure I understood what was going on with her care plan. I always was able to get all my questions answered by someone on the staff. Administrators were constantly notifying me with updates and changes to visitation guidelines. Nurses and CNA'S were there for my Mom anytime she needed help, they were kind, patient and empathized with my Mom's health issues. Physical therapy staff were unbelievable always encouraged her to get up and move and at times she didn't want to get up and do therapy but they kept coming back thru the day to encourage her, and therapy was fun, never forced and therapist always showed patience and respect to my Mom. I would definitely recommend this facility to anyone.

Fantastic experience: My husband was transferred to The Springs unable to walk and in 3 weeks he was walking strong with a walker and was able to transfer self from bed, toilet, shower chair and back from wheelchair and walker. He also learned to transfer to car as well. He had no complaints and always told me how nice the staff was to him. I found staff happy, friendly and plentiful. He never waited for someone to come when he turned his call light on. All the staff from CNA’s, Therapist, RN’s and Case Managers where there for questions, answers and teaching. They all kept me informed with phone calls and text messages every step in my husbands recovery. Both my husband and I feel blessed that a bed had opened for him there, I really feel he would not have progressed as rapidly as he did anywhere else.

Fantastic experience: The dining area staff were very special and helpful especially Denise. The facility was always clean and the front desk staff was very competent. The physical therapy staff was always encouraging and upbeat.

Fantastic experience: The Springs is an excellent facility. It was clean and provided good food and had great visiting policies. The care that was given to my Dad by his RN’s and CNA’s while he was there was wonderful. The staff was friendly and very helpful. Everyday I was greeted by Cyra, who made checking in so easy and efficient. She was so sweet and kind. Terri and Rosett were quick to resolve any issues and provide answers and actions when needed. Debbie made sure she checked in on my Dad and showed genuine interest in his concerns. Two special CNA’s, Nicole and Zoe, went above and beyond. They were fantastic! Last but not least, a huge thank you to Lynn for always listening and making things happen like they should. Your persistence is appreciated. I would definitely recommend The Springs. ❤️

Fantastic experience: Highly recommend The Springs. The facility is new and clean and beautiful but most importantly the staff is wonderful. Everyone from the beginning with admissions to the front desk staff and to the nursing/care staff. Felt very assured that my dad was well taken care of here. Previously had him at another local facility that I was not happy with. So glad that I transferred him here.

Fantastic experience: My Mother came here after her surgery. Because she needed some Help with getting Stronger and walking again. I was very hesitant at first, because my Mother has never been in a Rehabilitation center. But all the Staff and Doctors took really good care of my Mother. Every need was taken care of. They were all Friendly and really listened to any concerns I had. It's only took my Mother 1 week and 1/2 before she returned home. They really Helped with her Recovery and played a Great role in Helping her get stronger to return home.

Fantastic experience: My family was comfortable leaving our mother at the Springs facility due to the professionalism compassion and even love of your staff. I'm in my late sixties and starting to forget names but I wish I could remember all the names of all the people that we're helping us in 23b 4 Lenora Woods others came by and they stepped in to help to, stepping up to the plate even if it wasn't there room. Our mother came home Monday and is doing well do to the help rehabilitating our mother. Thank you so much for your heartfelt service. Don't change.

Fantastic experience: My 84-year-old mom was at “The Springs” for almost two weeks. I was so impressed with the way the case manager, doctors, and other staff always communicated with each other and with me, and any concerns I had were promptly addressed. For example, we thought my mom might have a UTI, so a urinalysis/culture was done within the hour, and the next day the doctor as well as the nurse called me with the preliminary results and care plan. While there, I had to take my mom out of the facility for an appointment, and the physical therapists were right on time, waiting for me to arrive so they could help my mom into her wheelchair and into the car. The CNAs were quick to respond when called for, and helpful and gentle. There were some issues with the kitchen not always sending what my mom had ordered, and the phone in her room had a broker ringer that did not get fixed while we were there. Also, I did not like that the staff put my mom in incontinence undergarments “just in case,” even though she had no issues with incontinence. After a day or two, she refused to keep wearing them. However, I felt like the excellent medical care more than made up for these issues. I would give “The Springs” 4.5 stars, but I’m rounding up to 5!

Fantastic experience: Overall, my experience was great. The Staff, from doctors, nurses, LVN's, CNA'a, PT/OT, Activities, students & janitorial/housekeeping, etc. were professional, friendly, caring, thorough and knowledgeable. Even had assistance from some of the students in training and they did a great job too. I would, also, like to attribute the speed in my recovery was due to the commitment of the staff to our recovery. I'm not sure what questions you have, however, I would like to comment on the food service. For the most part the meals were creative and quite a variety. If I may, I'd like to suggest not to overcook the vegetables. It seems like they were steamed twice to keep them warm. Sorry they weren't edible. After being there a week, Viviana, from the kitchen asked my likes & dislikes. I mentioned the vegetables and they did improve. Bacon is another thing I had requested crispy. The majority of the time it was barely cooked. For almost my 3 weeks there I said I don't drink milk and didn't care for the oatmeal. I kept returning them to the kitchen and they kept giving it to me. I think that would be a significant waste of revenue if other patients felt the same way. I know they prepare a lot of meals & I don't know the answer to fix it, but not all meals arrived hot. For some reason I would get toast & bagels for almost every breakfast. One or the other should be sufficient. Unfortunately, I assume, because of the hot food, the toast would be kind of soft instead of crunchy. Also, sometimes I'd get butter & sometimes not. I will say I didn't starve and the staff was good about providing alternate snacks. Please know I'm attempting to make things better, not complaining and definitely attempting to assist in a better reputation to hospital/rehab food service. Especially, since you are a relatively new facility. P.S. I did make an effort to memorize about 90% of the staff names. However, by the 3rd week it was challenging. I, seriously discovered that it takes a lot of people to care for those of us who need it. I'd love to mention names, however, I wouldn't want to forget even one.

Fantastic experience: My experience with The Springs was not for me personally but for a close friend... The experience was completely positive in the outcome... Facility is clean, well maintained, excellently trained staff and would not hesitate to recommend this facility... I can only give a 5 star rating but deserves higher...

Positive experience: Overall wonderful care and staff. Issue is short staff causing too many patients per caregiver. Thus patients don't receive as much needed attention. Pushing the "red" button for a nurse is not sufficient. Someone actually needs to go to the nurses station to find help. Food is less than desirable. Appearance and quality very poor. By the time food tray is received by patient, it's cold. Overall surroundings were kept clean. Nice areas outside for visits.

Fantastic experience: From the owner down to those who keep The Springs clean and protected this is an A1 facility. My mother was under great care and very well taken care of. A lot of those on staff deserve kudos but in particular We had a extremely good experience with her nurse Rebecca! I can understand why there would be a long waiting list in order to get into The Springs! Job well done. Wendell Peoples

Fantastic experience: Although it’s not perfect,I am really happy here. I came in six months ago and have seen a change in the growth of this newly opened facility. It runs much smoother now. The nursing stuff is pleasant and take really good care of me. We have very good activities and also something for you to always be a part of. The quality of the food has gotten really better, much more since Erich has been here. If I have a concern I can go to anyone and they listen to me. I am glad I am here.

Fantastic experience: My mom was in this skilled nursing facility for about two weeks. She had been hospitalized with a stroke and then went to this facility for rehab and follow up care. It was an absolutely wonderful experience for her and our family. The facility is beautiful, new, spotless clean, has amazing staff and has a lot of amazing amenities. We enjoyed visiting with my Mom in her room and enjoyed taking her around in her wheelchair throughout the facility. The patio area was wonderful for visiting when we had a larger group of family together and wanted to enjoy the fresh air and have a more private setting for visits. I cannot say enough about the rehab department!!!!! They did such a great job! Not only was the gym, state of the art, but the rehab staff all went above and beyond with care. They worked with my mom a couple times a day for weeks and helped her get her strength back and Taught her a lot of things that she can continue to practice at home. Her nurses were great and the directors were great as well. I was able to attend the care plan meeting where all of the department heads went over her plan of care, and they all followed through with everything promise during the meeting. The administrator, Wes Jones also exceeded all expectations. He took the time to visit with my mom personally and with us family members. He kept us updated with her care. He even allowed my mom and a few family members to use his office for lunch one day because it was a bit warm on the patio. The personalized touches that this facility offered was wonderful to see. We will definitely recommend this facility to others and we are so grateful that during such a hard time, my Mom was comfortable and happy there. —- Nicole Norman

Fantastic experience: Erin at the front desk is highly professional, friendly and informative and my moms room LVN Marrisa is so sensitive, kind and a total professional. She was so nice to my mom ! I was there yesterday (3/14/2021) and felt a great sense of kindness also from the nursing desk. I look forward to my mom constantly gaining strength and being able to return home. A+ to this facility.

Fantastic experience: The Springs facility was amazing! Erin gave me a wonderful tour on 8/27/19. I was impressed with the patient rooms and all the amenities, PT & OT facilities, and the daily menu. The staff that I met seemed very caring. After the tour, Erin suggested that I contact Colleen at The Pavilion in Sunny Hills for more information on that facility and talk about options for my family member’s long-term care. I spoke to Colleen on 9/17/19. She was very helpful in talking through my specific situation and giving her honest and professional opinion. I appreciated her honesty. It gave me peace of mind and focus on what my goal would be for my family member’s care. I contacted The Springs on 10/3/19 and spoke to Tracy and Erin. Tracy answered my general questions and then transferred me to Erin. Erin informed me of the status of the facility opening up to long term care patients and made sure she had all the information needed to contact me to evaluate my family member (i.e. insurance, services, etc.) to see if The Springs would be able to accommodate her needs. With all that said, the true test will be when the facility is fully occupied and larger staff. If The Springs administration and medical staff are able to care efficiently for the patients/residents, then that would make the wait well worth it.

Fantastic experience: My stay at the Springs has been wonderful. I arrived here 12 days ago, for rehabilitation, due to a leg fracture. Each and every staff member has worked tirelessly to help me with my ultimate goal of returning to my home. They are my health care providers, my friends and my family. I highly recommend this facility.
