Lake Pleasant Post Acute Rehabilitation

Fantastic experience: Staff were friendly, kind, courteous, and very knowledgeable. I actually enjoyed myself while there for 2 months. As a medical professional myself, I held the staff to a higher standard and they consistently exceeded it. My deepest thanks for the care I received.

Negative experience: How are the reviews of this place so high?? The staff is nice, but my dad had to ask for food. They forgot his breakfast, brought him 1 piece of pizza for lunch, and never gave him dinner. No breakfast the next day either. So a slice of pizza in 36 hours?? THANK GOD he has family to visit & feed him. The people there who don't have that must be starving. Skeleton crew on the weekends. He fell once and laid on the floor for 30min before someone helped him. When my sister asked if he could get his pain meds, they said he could only have Tylenol...because they were looking at the wrong chart!! If he hits the nurse call button, it takes 30 minutes for someone to come by... He's there to rehab, but he's left in his bed 99% of the time. This place is disgraceful and abusive.

Fantastic experience: I love the staff here! Everyone has been so welcoming and caring since my arrival. It has been challenging for me to feel comfortable since having to move away from my husband, but the Lake Pleasant staff has helped me feel better about my entire situation. Thank you Lake Pleasant for truly caring about my well-being!

Fantastic experience: Del did such a wonderful job evaluating the therapy that I would need. She was not only helpful but also friendly and made me laugh with her family stories. Austin told me I was the strongest patient in the building and he convinced me that memory about the things I have to do at home are most important.

Fantastic experience: I would like to recognize the whole staff there. My dad was there for a few days but unfortunately tonight my dad got rushed to the hospital. All of you did your best to help my dad and I greatly appreciate that. But tonight with my dad’s complications I would like to especially recognize Becky and Liliana! You both responded with urgency and awareness. But the whole team from front end to back end, yall are great! From the bottom of our heart, THANK YOU!!! ❤️

Fantastic experience: I sent my father to this facility for respite care. OMG, when I tell you that this facility went above and beyond for my father and Myself this was the first time my father ever had suggested that he wouldn’t mind staying at a Facility long-term But he said he can’t leave me I was having problems with transportation, picking my father up Lake Pleasant was very very accommodating and overly helpful. I want to take the time out to say thank you to all the staff And miss Virginia was amazing She helped us with the extended stay and was very very supportive when it came down to the transportation I just wanna take the time to say thank you and my father will be returning. This will be his permanent place to come to when I need a break or for a long-term care if he decides that this place five stars.

Fantastic experience: When my Dad got to this facility he was very sick because of the lack of care he received at another facility, Coming to this place was the best decision I could have made for my father. He’s back walking talking and making jokes. I’m so greatful for the nurses and staff that had the patience and care to give my dad. Big ups to one of his nurses Erin she is always at work and as busy as she is she never leaves my dad or anyone else hanging. Thank you all for everything you do and nursing my dad back to health.

Fantastic experience: Very nice place! Very clean and the staff is nice and attentive. Had no problems during my love one’s time here. Highly recommend!

Negative experience: Not attentive at all. Nurse call light would be left on for 30 + minutes When pain meds were requested (since they wouldn’t put on a schedule) they would still take over 2 hours to get them to me . (With asking multiple times within that time period!) Causing the pain to become severe . Some night staff is good A lot day staff is the big issue Biggest complete is the staff being so inattentive to patients especially when it comes to pain medicine. I would give 1 star but some of the night staff were great with medication . On time .

Negative experience: Don't send your loved ones there, no care, medicines, diaper changes, less than two weeks got nasty U.T.I. and bed sores.

Fantastic experience: Thank you for taking care of the only family I have left. The nursing manager I think she is called I finally got to meet her today and say thank you because the nursing aid on that hall said the manager is part of why she is so passionate about what she does.

Negative experience: My mom was transferred here out of surgery due to a bad foot infection. She was back in the hospital within 24 hours. This place is a NIGHTMARE. They found out she had vision problems - turned the light off in her room and left her there. She needed antibiotics - they said it'd be there by morning. Afternoon comes around, they still don't have it, blame the hospital, and just say they'll extend her stay. ANTIBIOTICS DO NOT WORK THAT WAY. YOU HAVE TO CONTINUE THEM OR THE INFECTION WILL COME BACK. Neglect seems to be the status quo and when you ask for help, they sigh like you're BOTHERING them. Seriously, don't send anyone here - family, friends, coworkers, ENEMIES. It's that terrible.

Negative experience: Clean place. They are severely understaffed. Negligence is pretty much guaranteed. Do not be fooled by the 4.0 rating.

Negative experience: Would give 0 stars if I could. Unqualified "nurses" who know nothing about their patients or equipment in the room. ALL of my dad's clothing, prescription glasses, slippers, charger to his phone and more were lost and/or stolen. He came home with such bad diaper rash because they let him sit in pee and diarrhea for hours (he has chronic ibs-D and they KNEW) They sent him WITH ALZHEIMER'S THEY KNEW ABOUT to a doctor appointment ALONE which ended up being a complete disaster. The ONLY good things about this facility were Director of Nurses, Jen and Kina. Life savers. Beautiful women. We will never use this facility again and we will never recommend it to anyone that actually loves and cares about the person being placed there.

Negative experience: Really unintelligent staff, don’t trust your family here if you really love them and want them cared for by competent health care workers that care.

Negative experience: place is dirty theres stains on the floor you have to share a room with someone unless you get lucky that theres no one else in there yet when i was visiting you could hear patients yelling and the food looks non edible also if you dont eat your food you dont get nothing else and its thrown away and one of the nurse was checking my grandmas blood presser and she had a injury on her elbow and the lady put it on her elbow where her injury was you also have to share a bathroom with someone my grandma waited an hour and a half to get pain meds dont ever go here wouldnt wish it on anybody

Fantastic experience: This rehab facility was a great experience for us! My family has had experience with Sun City Post Acute (didn’t know it was 1 out of 5 Medicare stars before we went but that story is for another day) down the street and Lake Pleasant. Lake Pleasant wins every time. The nurses are some of the sweetest people and my loved one wanted to take his therapist home with him.

Fantastic experience: The staff is so kind!

Negative experience: My 86 year old mother was there for almost three weeks after suffering from an infection for a week in the hospital. She and her room mate both had negative things to say about this place. No one picks up the phone except the receptionist and when I left messages for the head of the physical therapy and the doctor to try to get some answers they never called me back. My mom has dementia so I had a few questions about her condition. The only way to get information is to go there and stand at the nurses counter and they will answer tell you what is in the report. One of the CNA's was just rude. This person seemed miserable and should seek a career change for sure. My mother needed help and had her call light on for an hour before she came. The rooms are just bleak...my mother's bed was shoved in a corner so she could stair at two grey walls. Don't let the five stars fool you...this place take in homeless people and mentally disturbed individuals. My mother's room mate told me these individuals walk up and down the halls at night making noise. She said she gets scared that one of these characters would come in her room at night. While I was visiting my mother I took her outside on their patio. A patient in a wheel chair was sifting through the cigarette garbage cans for old cigarette buts to smoke. The food is also pretty bleak. Better read the menu because you wont recognize what the meat is. I give two stars for some of the hard working staff that seem to do their best.

Fantastic experience: I was just discharged from Lake Pleasant Post Acute Rehabilitation earlier this week. I can say that I feel so much stronger and am back to my prior level of function from before my knee Injury. It truly feels like the nurses and aids care for their patients. The nursing management team that they have there is outstanding and professional. If I ever need inpatient physical therapy again I would definitely come back to this facility.

Fantastic experience: I recently had my mom at the Lake Pleasant rehab facility, the staff was great and very caring. I feel that my mom got great physical therapy and nursing care while being the facility. I would recommend this facility to family and friends.

Negative experience: Terrible place! nobody knows what's on. Nurses stand in hallway talking, raising voice at 2 am.. while patients are trying to sleep. They lie about the care they give to our loved one.

Negative experience: This place is trash, nurse Leanne if that was even her real name was incredibly rude. I don’t understand how some one like that is even a nurse. The patient I was visiting had to call 911 and when I let Leanne know she literally laugh. This place is a joke please don’t come here.

Positive experience: This is my 4th facility over the last few years & so far the best. Even though I don’t give 5 stars I still would recommend it. The missing 5th star is due to some of the nursing staff - that when you initiate communication for clarity regarding care, they are quite disrespectful in their speech in tone & interrupting you as if you are a child- one of theirs! Asylum-like. But most of the time things are great & so are they as well as wonderful reception & charge nurses!

Fantastic experience: This place is AMAZING. All their staff, at all levels, care about their patients. My dad was here for a little over a week after major surgery and they made sure he had a full understanding of how to do things on his own before going home. They pushed him to get up and get stronger even when he didn't want to. Ashley was so sweet and made sure not only my father but us family members were taken care of and had a clear understanding of my father's medical needs.

Fantastic experience: I just want to give credit to this facility with how caring their employees are. My mom said she had a great experience here. The CNA’s attended to her every needs, we’re so appreciative for that. Also i would like to point out how great their receptionist is. The young lady said her name is Kena, she was so helpful and friendly. She didn’t make my family & i feel like a burden at all with the questions we had.

Negative experience: Too many bad things to type. Lost all my belongings when they moved me to another room. Stayed on them for 3 days prior to release and it has been another week

Fantastic experience: I was recently a patient at Lake Pleasant for rehab after my hip surgery. The staff there were so helpful. Physical therapy was amazing and took great care of me and helped me to get back to my normal life. Great care by everyone. I will highly recommend them!!

Fantastic experience: I have been employed with Ensign now for about 11 years. The last 6 years have been here at Lake Pleasant. I truly enjoy coming to work. The staff is like family and our team truly cares for the residents. We also have one of the best Rehab departments.

Fantastic experience: For me... great healthcare starts and finishes with the people. That's what drew me into this facility and captured my heart. From administration, to the floor staff, to the dietary, housekeepers, and therapy staff. Everyone really cares about helping patients and eachother. It's a true family here that each patient becomes a part of. The facility is clean, well lit, and the daily activities bring smiles to everyone's faces. I've worked a lot if places but I can say with certainty the culture here is infectious. I would honestly stay here myself or bring my family here if we were ever in need if these kind of services.

The CNA’s here are amazing people, Sarah especially goes above and beyond for all her residents and ensures all their needs are met. Nothing but good things to say about this place!
