Fantastic experience: My loved one has only been there two months now and I am very pleased to say that the staffing there are very polite and informative. My loved one is unable to speak for himself, or move around for comfort, but I can see the quality of care and attention that is given meets my standards. I use to be a CNA and I know quality care when I see it. Even the facility never smells bad at all every time I visit. That’s how I first know of the quality of care the staff provides. And the newly remodeled front lobby is so inviting. The kindness of the staff and the quick response when I call for assistance for my loved one is top notch. They call my mom or me to let us know what they need to do to keep us informed of doctor’s orders or just to get permission to cut his hair or shave his facial hair. I am so pleased and thankful for all the staffing and for the quality that of care they provide. YOU ARE APPRECIATED by me (a former CNA), my family and my friends. Thank you Park Avenue facility.

Negative experience: They didn't give my dad his medication for 3days. He's completely blind and they didn't find that out til almost a week later. There were patients outside by the street and no one watching them. Please do not put your family members in here!!!

Fantastic experience: The new administrator, Arthur Rios is doing an awesome work at the facility from significant positive changes to residents care, listening to patients concerns, addressing them to residents and relatives appreciation, actually practicing open door policy for staff for ideas and implementation to infrastructure upgrades. These in turn provides positive work environment, residents and staff happiness and productivity.

Positive experience: Everyone in Sub accute unit was amazing to me for the two months I was there. Everyone was so attentive and made me feel special. Once I was moved to unit 1 for only 1 week I wanted to run out, I was transferred at 8 pm and no one came into my room until close to 11. I had a horrible experience in that last week. I know staff was changing so I do hope it works out.
We place every effort in providing quality care and effective communication to residents at Park Avenue Healthcare. Our Administrator has an open-door policy and encourages residents to reach out when questions or concerns arise. Thank you.
Negative experience: WARNING!!! My dear friend is in this facility... here is his post from 2 days ago!!! This morning our breakfast was served COLD. My tray was served with 2 spoons that we're stuck together with cheese and not completely cleaned properly before service. Pictures will be included. My roommate, Ernest Culp, was given a single serving of one waffle and a single sausage. When he asked his CNA to go to the kitchen and ask for 2 more waffles and sausages, the CNA returned and said that the kitchen had no more food for the breakfast. My roommate got up dressed himself and went to the kitchen where he confronted the kitchen staff and was ultimately given two more Eggo frozen waffles, uncooked and with two hard boiled eggs. This is unacceptable. The kitchen staff pretends to be ignorant and they do not read the tickets that are printed for all the residents meal needs Also, as of today we have been confronted with patio doors being locked. These patio doors have always been open to the residents in order for us to go out and enjoy the day. However, the alleged use of cannabis has prompted Rockport to enforce closure of the patios. For as long as I have lived here (7 years) these patios have always been open. I was told that a key would always be available for those of us who do not smoke. But experience has shown me that no one will know where a key will be at any time. There will always be an excuse. Please don't EVER let those you love end up in a place like this.... This is the way.

Negative experience: I will tell you about these people . My son was hit by a car in LA . We live in AZ , I had no idea he was in CA. He never woke up , they had him here as a John Doe fOR EIGHT MONTHS!! TELL ME HOW HIS CARGIVERS DID NOT SEE HIM NAME THAT IS TATTOOED ON HIS STOMACH ?? HUGE TATTOO! OF HIS VERY NAME ! And he lays there alone nidentifed for 8 months until his death . Alone . How incompetent are you people? No one made one bit of an effort to find nhis family. Im so mad I cant even type. Thespeople are wrong and EVIL. THEY ARE UP TO SOMETHING WHY ELSE WOUKD THEY DO THIS .MONEY . IT MUST UNVOLVE MONEY . LOW LIFES . YOU PEOOLE ARE GOINGDOWN UM TURNIG YOU IN .
Fantastic experience: At first I was hesitant and unhappy to arrive here. I have been here over 2 months. All my needs are met. The staff and DON all attend to my needs and concerns. Negative experience: Our good friend and father is left in pain from 12 hours to days at a time before anyone will even attempt to give him his pain meds. He is in agonizing pain and it's like they don't even listen to his calls. The doctor will not even speak with out family member/friend so they do not know the doctors name nor how to reach him. They also gave the excuse that there was no meds of his type in the facilities. This was a bold face lie. This place also does not clean dishware leaving our family member to have actual thick mold growing in their dishware. I'll stop there for now.

Negative experience: My sister was sent there for recovery from surgery and wound care. Her wound only got worse and she is now back in the hospital facing more surgery. Her medications were given irregularly or not at all, the antibiotics were not given as prescribed and the wound care was nonexsitant. She was not helped to take a sponge bath or to wash her hair. When she arrived in the late evening her wound was seeping and soaking her gown which she removed and rang for assistance for 2 hours with no response. She sat there in paper underwear and a sweater wrapped around her until her daughter showed up the next morning. There is little to no care given to the patients. How can they still be open and collecting money from insurance for services not provided.Really it's less than 1 star.

Negative experience: Horrible place !! my brother picked up my mother yesterday and now she is in hospital in the intensive care unit with a breathing tube down her throat due to neglect on this facility part!!! They kept telling us she is fine but she lost 50 pounds in 4 months of being at this facility!!! Every time we would go to see her or try to call her she was completely unresponsive due to the fact of either the wrong medication or too much medication that is still being investigated!!! And she also picked up lice at this facility and their solution was to shave her head!! Please do not put your family members here

Negative experience: This place should not exist. Not only that, but the staff is so incredibly evil, cold hearted and cruel. I thought I was going to lose my mother and they treated me as if I was a nuisance because I would call to check in on my mom. As if they were so annoyed. They have caller id so they would chose when to answer my calls. I would go days without being able to contact someone. My mother left there in an emergency back to the hospital. She was malnourished, dehydrated, got a UTI and bed sores. My mother was there to be cared for, not abused. She was unable to change herself, feed herself or talk so she could not stick up for herself. I would show up and all she can express to me were her tears. My mother was a honored nurse for 50 years at San Antonia and for her to be treated the way she was should call for serious action. Station 2 is a horror house. And, as a grieving daughter, Susan would stand in the hall and stare at me with her arms crossed as I would say goodbye to the other poor residence that have fallen victim to this facility. GOD, PLEASE HELP THESE PEOPLE. I am so grateful to have got my mother out of there and saved her LIFE!

Fantastic experience: This facility is so great! Great for patients to regain rehabilitation, the employees are always so helpful and quick to responding to patients. I would definitely recommend!
Fantastic experience: In the four months I’ve been here this place has changed a lot and gotten better for a lot of good reasons they worked very hard to change it around and make this a better place to be treated very fairly by a kind of LVN named Karolina Christina Daisy and a few others like Emilita Lena are wonderful also social worker Ashley is also a great social worker Brianna is great!!!
Negative experience: My family member has been waiting for over 2 weeks to see an actual Doctor, the nursing staff are doing the best they can to treat his sudden illness. If this keeps up I will be calling 911.

Negative experience: This is the worst place to bring one of your family members ,,they don't respond when needed ,they don't have enough nurses on Shift.They don't even care about the patients . If you have your loved ones there you need to check on them every day to make sure they're giving them proper Care. I can't believe that this place has not got reported or clothes down by now my mother is there And we would like to take her somewhere else but we can't after we put her there..

Negative experience: I HAVE BEEN LEFT HERE BY THE WOUND CARE NURSE FOR AN HOUR NOW, MY WOUNDS OPEN AND BLEEDING AND OPEN TO AIR, FLIES, AND LORD KNOWS WHAT. THIS IS NOT RIGHT! NURSE HAS BEEN CALLED NUMEROUS TIMES, BUT HAS NOT APPEARED. THIS SHOULD HAVE BEEN BANDAGED FIVE MINUTES AFTER DOC SAW ME. THIS IS NOT RIGHT!

Fantastic experience: My dad was here for rehab, the staff is very helpful especially Brianna, it’s tough seeing your father going through pain but thank god he’s much better, the TV has been an issue but they are working to fix it. The staff(nurses) are great takjj in big care of dad!! Thanks!!!!!
Negative experience: I thought this facility was going to the one to help my mother but after 2 years i see different. The staff at station 3 are the worst cause they pay no attention to their patients. I hope they get right soon. I got to bring moms home because of the filth and rudeness.

Negative experience: I used to think this was a nice place but my opinion has changed over the year, my grandpas nurse that he normally has in another country rite now and should be back soon. Since she has been gone I've noticed my grandpas care decline, we visited him Sunday and his Clothes are filthy along with his body, his has skin flakes everywhere as if he has been changed or bathed in a long time, in fear of Retaliation we never reported it, we don't want my grandpa to be mistreated,also because my grandpa has dementia and he would never be able to tell us if somebody was hurting him because the staff that were there Sunday seemed like some elder abuse nurses the way they were handling the patients did not give me a good feeling in my heart when I left since then I've been feeling sick in regards that day. Now today is Wednesday and my grandpa is still in the same clothes he has since I changed him Sunday! Also still not bathed. This is probably when the place smells so bad like urine and poop.
Negative experience: This is the worse facility they no longer are contacted with Pomona Valley Hospital. That should be reason enough not to place a family member at this facility.
Positive experience: They have taken really good care of my Mother for almost a year. They call me immediately with any changes, have great recommendations, and I can see the comfort they bring my mom. Every nurse or CNA I've dealt with has been professional, caring, and honestly did their best to care for my loved one. As another reviewer mentioned, it looks "under construction" there are ceiling panels missing because they're sealing the roof against leaks. No big deal. Doesn't affect the care. I'm very happy with Park Avenue
Fantastic experience: Wow, staff do go above and beyond, despite the place looking like it is (under construction). My father only stayed there for about 2 weeks. Rehab department and nursing staff were very nice. The two gentlemen in the front office, Kenny and especially John (some people called him JV depending on what he is doing that day I guess- he wears scrubs one day, but wear a button up shirt and a tie for the most part) them two are just as wonderful as the other other staff. Will bring my father there again if he ever needs a nursing home again.

Positive experience: I go to Park Avenue to bring the celebration and Jesus from the Catholic Church to the people there who can't go to church. I love bringing God's word and Jesus to them and they love having me. It's always clean and the people are always happy .
Negative experience: This place lets visitors stay until they want. The problem is the employees. They take to long to help patients. My grandma needs help using the restroom. I had to go and ask for help. One of the employee said, "She choose the wrong time to use the restroom, we are on our lunch". I was furious. They do come and check on her but not when she calls them for help.
Negative experience: This place is terrible... my grandma is here and she's constantly telling me how she feels alone. The help here is very upsetting I can ask for a nurse and not get one for about an hour because they all take their breaks together. It's under construction right now and it's so dangerous I don't understand how they could still let patience live there with those conditions. I've requested that my grandma get moved to a new center but her insurance isn't budging right now. The nurses sit at their desks and eat plates of food on their breaks and there's rarely anyone avaible during those times to assist people who really need it. The only positive thing I have to say is my grandma is actually taking her medicine daily .
Negative experience: Since 2000-a family member has been at facility. Our experience is: cna yell at patients. cna cus at patients. it's dirty, they do not practice courtesy and respect with patients. we are never call when there are changes at facility. this facility has for last 5 years been rated and the bottom of all other Rehab, Healthcare facilities!
