Positive experience: My wife got admitted here on April 9th for a compression fracture on her lumbar vertebrae. She's in terrible pain and needs help getting up and using the restroom. The 1st night we were here she pushed her call button to get help. It took over 30 minutes for someone to get in to help. In fact, I went out to voice my concern that maybe her call button was broke because no one was bothering to check on her. That's when she came in to help her. If I didn't go out who knows how long it would have been. Since then, my wife tells me it's pretty normal for her to wait for long periods of time to be assisted. Today, the cna came in to assist her to the commode then left. My wife waited over a half an hour on the commode before she came back to help her back in bed. Her legs were numb from being in an uncomfortable position for that long. I would give this establishment only one star but most of the staff are very friendly and professional. The RNs and all the therapist's have been friendly and very helpful. Most of the food, I am told, is pretty good. It seems to me that they are under staffed. I've been told that the nurses are responsible for 30 patients. Some of the CNAs seem to be here just for a paycheck and not really care about the patients. Some of them, not all, seem put out when they have to do anything. I chose this establishment based on all the positive reviews I read on Google. So far, I am greatly disappointed. We have been here for 4 days so far, hopefully the response time gets better and I will update my review. As for now regretfully, I can only give it 2 stars. Update: my wife's health seems to be declining while in this facility. This morning, she became extremely nauseous. She voiced her concern to a male staff member, i think he's the cna, not sure. He told her, "We just gave you your meds. You always feel this way after you take your meds." and left. I have been with her most of the time, she never felt nauseous after taking her meds. I am requesting that she be taken to the hospital since her health has been declining since being admitted here. It's been over an hour since she called me to come down to help her. She told me she was nauseous and had to go to the bathroom. I rushed down and helped her off the bed and helped her to the commode. Her roommate just informed me that my wife was waiting 3 hours to get help going to the bathroom. TOTALLY UNACCEPTABLE!!! Update: The head administrator and the director of nursing met us in the room to discuss our issues and our concerns. They listened and seemed genuinely concerned and empathetic. They assured us that they will be investigating the matters thoroughly and cracking down on these issues. We will be staying until wife is strong enough to go home. Will change my rating if we notice a positive improvement. More updates coming... I was going to up my rating to 4 stars because after speaking to the administrators, we did see a positive change in response time etc. Today, after taking my wife in a wheelchair outside for fresh air and coming back to the room, she needed help onto the commode for a bowel movement. The cna told us she could put her there but she would have to sit there for over an hour because she was getting off soon and didn't have time to come back to help her. This was at 5 pm. I believe shift change is at 6 pm. It's now 6:20 pm and she is still waiting for assistance. Also, I was told before I arrived today, wait time was a lot longer than usual.

Positive experience: *Final Update* My mother has since been discharged, and I am updating the review to four stars. The management and staff are accountable, but I docked one star due to some of my mom’s clothing had gone missing. I think the biggest issue they face is that they were severely short-staffed (like most medical facilities) which causes some things to fall in the cracks. If you are going to be visiting your loved one frequently, I think it’s easier to stay on top of what’s happening. I myself am dealing with my own health issues which limited my ability to visit. If your loved one is also not 100% fluent in English, make sure they ask for an interpreter (I was a medical interpreter) which they have to provide by law. That will also ensure smoother communication. Finally, as far as the food provided they don’t have much diversity in their offerings. If you’re used to American cafeteria food, you’ll love it though. *UPDATE* The owner reached out to me and apologized for the unfortunate first impression. He also assured me that this was not the kind of standard he wishes to se in their facility. I have changed my rating to three stars because accountability matters, especially with regard to the elderly. Tonight is the first night my mother spent here, and the attendant call button went unanswered for a long period while she suffered severe acid reflux. She had to call me, and in turn I had to call their nurse station at 6:20 AM to get someone to assist a 78YO woman with three broken ribs. When the nurse finally came, she made no effort to alleviate the stomach pain my mom was experiencing. My mother had to get up to ask for some warm water, which was terribly inconsiderate given she’s got the broken ribs and stomachache—and in a lot of pain. Plus, she is a fall risk, and should not be getting up. A cup of warm water is a small ask. Additionally, her daily medications that were supposed to be given to her was not provided as directed by the doctor. Some meds are thrice a day, and she has told me a lot of the medications have been forgotten. The medications included her acid reflux medication. I asked for the nurse to call me back because I was afraid I might be a language barrier, but I didn’t get a call back. As I write this, I’m afraid the care staff will retaliate and be even more neglectful.

Fantastic experience: This nursing home provides outstanding physical and occupational therapy services in the area! The food is absolutely delightful, making every meal a pleasure. I enthusiastically recommend this facility any day of the week! The staff is exceptional, delivering care with genuine warmth and professionalism. You surely won't be disappointed!

Fantastic experience: My mom has been at Alpine since June. Not been easy and coming everyday I have seen how everyone and everything goes. No doubt under staffed and that part is stressful if I’m honest worrying about my mom. The food has been really decent and I have no complaints. I have interacted with 60 nurses probably and want to shout out to as many as I can remember names of. Liz, Tasha, Destiny, Jenny, Joe, Brandon, Tina, Judith, Briana, JD, Sam, Mari, ( another girl Alena? Or Lena? kinda taller girl, thin build ) and I am sure I am missing more and sorry I don’t have all the names. Maybe I had one girl who wasn’t up to professional par however it’s been really good overall despite short handed staff that is pressed and delivered for my mom by and large every day. I think the ones who I remember the most for their efforts above and beyond are Liz, Destiny, Judith, and Joe. Thank you for all you do I appreciate the WHOLE staff and think you all do amazing job that I know is really tough. Coming in and signing in with Debbie and Oscar was professional and fast as well. This is my second part as my mom passed at Alpine on 1-14-25 at night. Just wanted to say thanks to Joe, Amber, McKenzie, Alyse, Brandon and Liz…and special thanks to Jerry and Melanie as well as Mary and Destinie for coming over to her room and checking on my mom. These people were great and loving to my mom. As my heart is broken right now I’m very thankful for these hard working beautiful people. Nothing bad to report on how she was treated. I’ve already said it but my opinion is just please get them help as staff are very professional and kind but pressed with amount of work on each person’s plate… THANK YOU ALPINE! I visited my mom over 200 times over these last months. Appreciate your care. Thanks Debbie and Oscar for making it easy to get in daily. I’m sure I missed some names. Sorry! Thank you! Fantastic experience: Facility looks great! I’ve had a family member here for quite a while now so I’ve seen it while under construction and now fully finished! One constant has always been Sam’s warm and welcoming personality! Love the place! Definitely recommend for families with members who need long term skilled nursing!

My complaints has been answered with more blankets which is not a solution. my next move is calling the health department. Updates..... evidently my complaints did not get to management but I am now in a room and comfortable with heat.thank you Alpy

Fantastic experience: My mom was at Alpine a few months ago so when Sam called me this weekend and asked me to rate Alpine I was happy! Not only was the admissions absolutely effortless, the staff was compassionate, the facility was beautiful and clean but everyone from Admissions to the nurse practitioners answered every question we had along the journey. Fantastic experience: The journey to place my father at a skilled nursing facility was challenging, but I felt at ease when I met with Sam as she answered all my questions. My dad is home now after 3 weeks at Alpine and is back to his old self.

Fantastic experience: My aunt has recently returned home from Alpine Skilled nursing and is thriving! I can truly say that the staff and team at Alpine are miracle workers. From administration to the staff, they all worked so well with my aunt and made her feel at home while there. My aunt was there for about 2 months or so and gained some close relationships with the staff. Negative experience: Understaffed! I could hear patients yelling for help and nobody was around. CNA’s and nurses are great, doing the best they can with the huge load they are given. Brenda, the discharge coordinator, is passive aggressive and a poor communicator. I’ve already filled out a complaint form. The owners need to hire more help and pay a living wage to the nurses and CNA’s

She can also explain more about how much we pay our staff and the issue of staffing calling out for work. We just want to make sure you have the facts on this since there’s a lot of false information out there!

Our staff, management included, work around the clock to ensure our residents are cared for properly. We’re sorry you had a bad experience last night. We will do our best to make sure it doesn’t happen again!
Negative experience: My dad was there for several months past a heart and back issue. Communication was hard and while he struggled there in his recovery, we had to work hard to find his personal belongings. Upon arrival to see him, staff was dismissing guests and not very willing to help. We ultimately moved him and he is flourishing. We requested his items be shipped and paid to dollar to have them shipped. Items were opened (medicine bags) and he was missing several items like his hearing aids due to “one tape” packaging. While some care members were responsive and caring, we found many to be dismissive. I was on the phone with my dad as he waited for help for one hour and heard them telling him to hang tight to only get someone to tell me they have been telling him to go back to us room for an hour. I don’t recommend unless you can be there often. We lived too far away (5 hours then), so could rarely help on the ground level.
We are so sorry to have missed the mark for you, Diane. This is certainly not the experience we want you to have. We will look into what might have happened.
Negative experience: They treat night shift staff horribly so don't be surprised if your loved ones aren't getting the care they need. Very sad display for the appreciation on the nurses here for an expensive facility... When you don't take care of the nurses they don't take care of the patients and eventually high turn over... Sad place to have your family member. Keep checking

Fantastic experience: I came to Alpine needing Physical Therapy to help me walk on my own. Just shy of three weeks I was discharged. I must mention those that helped on such a positive journey. Sheila (Nurse), Andy (PT), Rachel (PT), Billie (PT), Joe (PT), Anna (CNA), Peter (Nurse), Debbie (Receptionist), Crystal (Activity Director) Braden (Activity Assistant), Joe (Nurse). Fantastic experience: Have nothing but positive things to say about this facility! Everyone from the admissions staff to the nurses are so welcoming and willing to assist to the best of their abilities. They recently did some renovations and the upgrades really send this facility over the top. Choosing alpine was an easy decision and I highly recommend them to anyone in need of this type of care!

Positive experience: I was at Alpine for 30+ days and had a good experience! The staff was great! The nurses and especially the CNAs who have the toughest job, always showed great patience and empathy! In addition, the PT staff was AWESOME! They do a great job and made me feel confident in going home and getting around by myself. There are a few people I would like to "shout out" as they made my stay much more comfortable. CNA's: Sabina, Natasha (night shift) Anna E, Hilda, Olga, Rhett, Seth, Keith,. Jerry! I'm sure I'm missing some!! They do a great job! Nurses: Mary, Stephanie, Johnathan, Shane, Joann, Amber (ALL of them!) Sam (admissions) the most positive person! Always with a smile and a Good Morning! She is so helpful! Dani, Brenda so helpful as well! The only complaint I have is the staff shortage, I literally waited 30+ mins for my call light- so thankful I wasn't on the floor hurt! Overall it was a good experience and I got out of it what I put in! The goal is to get home better than when I arrived and I did, so Thank You!!!

Fantastic experience: I had a family member that stayed here at Alpine following some recent health issues. Can’t say enough good things about this facility. From the administrative staff to the nurses and CNAs, everyone was so nice and professional. It truly helped in the overall health and recovery of my family member.

Negative experience: I received word from the night nurse that my mother was being sent to the hospital because she seemed to be in sharp decline tonight due to a number of factors. Since my mother has been here her health has declined rapidly. Tonight at the ER the nurse and staff were SHOCKED at how extremely dehydrated my mother was and how dirty and ill kept her hygiene was. The nurse showed me a coccyx sore and areas of her body where the diaper had cut into her skin because it was WEIGHED DOWN WITH URINE. I saw the drenched dripping diaper hey lifted off her and it looked like it WEIGHED several pounds. She had TWO eyes with conjunctivitis and they mentioned she had urine stains all over the blankets on TOP of her when she came in. She was so dehydrated her urine was cloudy and brown and she has a UTI. I can't imagine how these people could let my mother be in the bed like that without changing her diaper. I wonder how they would feel if they had to lie there in that urine soaked filth? The ER folks started her on antibiotics for the UTI immediately and noted she might also have pneumonia. The hospital nurse and assistants were enraged and considered filing an elder abuse report. I could not believe it myself when I saw her from the waist down. If they had taken proper care of her she might not have needed to go to the damn ER. To top it off with the night nurse Susan in the area my mom stays at was extremely rude and defensive, trying to interrupt me by giving all kinds of excuses and then even telling me sarcastically to go ahead and file an elder abuse report. I couldn't believe it. I don't know what the hell happened to this place but I remember it being ten times better some years back when my dad stayed there. All I know is they better not ever let me catch my mom in such squalor or I'll find a way to do a lot more than just file an elder abuse report. It says a LOT when the ER nurse and staff is outraged at my mom's condition. This place needs to ante up because if my mother's health declined this badly in such a short period of time there must be something very wrong. And there's just NO excuse for what I saw tonight at the ER
Hi Ms. Lloyd - I hope your mom is doing better today. We expect our staff to take care of our Residents in the best possible way, and we are so sorry you had this experience. We have started our own internal investigation to figure out what happened and uncover the facts. Our investigative procedure includes watching hallway cameras, interviewing staff, and analyzing the Resident's chart.

Positive experience: I can’t say enough positive things about Sam. We had a rocky start with my mom’s admission on a Sunday (not a normal admission day) personally I think this was more the hospitals fault. Sam helped us and my mom get settled and truly went out of her way to help us all feel more comfortable with the situation. Staff is kind and helpful and we appreciate all their help. Facility is very clean, could use a some art in the rooms but I’m sure they will get to that as the remodel moves forward. Many more days to go but so far we are happy

Fantastic experience: This is a great place to work, I would recommend it to anyone. The staff here is very nice and friendly and helpful and are always available when you need help or ask them a question. The residents are very nice and very funny.
Negative experience: I really don't understand how after three years more isn't being done to spread COVID in a center, extra precautions should be made during an outbreak especially with fragile patients. The father of my children survived a horrific accident and is now on life support because of carelessness.

Negative experience: As a former hospice nurse I am disgusted by the care provided to my kid’s dad so far. We were told when he was transferred on Wednesday he would received a PT assessment the following day… his first PT session and assessment was today, 3 days in. They also took until 11pm last night to even shower him after telling him for days they’d help him with one. He has some very severe head injuries and has fallen multiple times already while in here, nurses don’t respond to the “blinking light outside the door” which is the call button so he has to use his cell phone to call us for help in the middle of the night and day, but we are an hour away! It was also disturbing to sit and hear other patients scream for the nurse over and over for insanely long periods of time. Staffing issues need to be addressed and the quality of care needs to drastically improve for patients with such severe rehabilitation needs. He’s not just a dementia patient that can be “ignored” he’s a young father that needs to get better to resume his role in their lives and not decline due to negligence!
Hi Danielle — thank you for providing specific examples in your review so we could follow up with each Department. Generally, we are very sorry for missing the mark on your expectations. It is always our desire to provide exceptional service, which can be very difficult to do in healthcare. We can’t provide any specifics, so we will provide general statements about your concerns and follow it up with that we have talked to the Resident who does not share your same concerns. All Residents are evaluated by the appropriate therapies within 24 hours. After the evaluation, a treatment plan is created, which is executed right away. Our best practice is providing 2-3 showers a week (often Residents come to us from the hospital where they never received a shower or bed bath during their stay), and as-needed partial baths.
A Nursing Home/Rehab Center as large as Alpine has a variety of Residents receiving care for different reasons at any time. Some of our Residents suffer from behavioral health issues, while some are simply with us for a few days after surgery to get stronger and go back home.
Our staffing ratios are much different than a hospitals pattern, but that’s how the system is designed. Residents benefit the most from Nursing Home care when we (facility staff and families) all work together for the Resident.
Again, thank you for sharing your concerns, and we are confident we can provide exceptional service to our Residents. In the future, if you have any additional concerns, please ask to speak to the Administrator or Director of Nursing who can address your questions/comments right away.
Fantastic experience: My Uncle was admitted to Alpine from the hospital and stayed with them until he passed away. He could be a giant pain in the butt and the very nice folks at Alpine were nothing but kind to him. They helped me every step of the way in not only ensuring he was comfortable and taken care of but with all of the paperwork that goes with this situation. I would highly recommend Alpine.

Negative experience: This place had no compassion for my Mother. Would not give her a shower for over a week even when she would ask everyday, several times a day. I had to call them and share our frustrations. They claimed that she was refusing, however, I know better. She was there for three weeks and only received 3 showers. They claimed she was to received one every Monday and Friday, however, that did not happen. Food was terrible. I pray and send positive energy should one of their love ones need care and help that they receive better treatment than my Mom.

Negative experience: Really badly organized and complete lack of. care when promised. No plan or scheduling of physical therapy when that's what he went there for. Just awful. Completely awful. Important medications don't come on time and are missed and they didn't even have.
Hi Laura - it is my understanding you and your loved one were ready to go since the moment you arrived. Healing and regaining strength take time, which is why this is done in a skilled nursing and rehabilitation center and not in the hospital. Sometime we do miss the mark on customer service, and we do apologize for that. We hope you and your loved one are healing successfully at home.
Fantastic experience: * Update, my father passed away while at Alpine and he was treated with the utmost respect and dignity right up until the end. I honestly cannot say enough about everyone at Alpine, from the Administrator to the sweet girl who cleans the rooms and everyone in between who touched my dad’s life during his short stay. Thank you from the bottom of my heart.* My dad has been here for a week and so far I can’t say enough about the wonderful staff and caregivers. Starting with Sam who, from our very first meeting, showed her compassion toward my father’s situation. The bubbly personalities of Katie, Caroline and Rachel can quickly calm a stressful situation and Liz and Brittany are just so kind. And Kevin! A great big teddy bear of a guy who is so gentle and respectful of my dad. He even holds his hand while feeding him. Carol and Kelli at the front desk are wonderful greeters and so sweet. There are so many others who’s names I haven’t learned or can’t remember. A big thank you to everyone at Alpine for taking care of my dad and keeping him safe.

Negative experience: If i could rate these people a 0/10 i would. They are negligent and do not care for the people in there. It is always hard to find a nurse or assistant. My father was able to walk and some what talk when entering this facility and they had him just sit in the bed to where he was no longer able to walk 5 months after. They are liars and just tell you what you want to hear. My father ended up passing in this facility and i beg you not to send your loved one here.

Negative experience: My mother was in this dump for almost two months. I wanted to have her transferred after the first week when I found out that they had not given her so much as a bed bath in that time but she didn't want to have to go through the quarantine process again which BTW was a joke since they had her in quarantine with another new patient and the nurses and CNAs that were "caring" for her came and went. They don't even have blood pressure cuffs, the food is a joke, like parents gave their teenagers money to buy food but they spent it on pot and then had to come up with meals using crackers, ketchup and sliced cheese. They withheld her medications because they said that they had to come from their own pharmacy but then had to wait almost five days for the pharmacy to give them her meds. And they are not suites, they are shared rooms even though over half of the facility is unused. I'd give it zero stars if I could.
Hi Daniel, we always appreciate constructive feedback, however, I am not sure this sort of review is conducive for a productive and engaging conversation. Nonetheless, I will address a few points:
1) Quarantining. This is a mandated process that we must follow for any new Admission into the building. We are monitored, surveyed and potentially fined by the state and federal governments for not following their guidance for healthcare facilities. We will, however, make sure all new admits are offered multiple bed baths during their first few days.
2) Nursing supplies. Our staff is equipped with nursing supplies, so I am not sure why you were informed there are no blood pressure cuffs? This is a critical supply and our Residents have their vitals taken 2-3 times a day.
3) The food. We have found that food preferences vary greatly amongst our Residents, which makes sense. However, we do know there was a lot of room for improvement on the food, so we recently launched a brand new menu, retrained our kitchen staff, and have seen a lot more satisfaction from Residents.
4) Medications. There could be a few things that happened here, such as us never receiving an order from the transferring facility or hospital for the medication. By law, we can not administer medication without a signed order from a physician. This could take a day or two.
5) Rooms. We have a mix of one-bed, two-bed, and three-bed suites. Depending on the situation, a Resident may or may not have a roommate. We find if Residents are with us for a while, a roommate provides socialization and motivation to get better.
Fantastic experience: AMAZING! My father was at Lakeside for many weeks and I visited him daily. As a result, I got a first-hand look at how much the staff cared for him. They were fabulous! I was invited to participate in the therapy sessions (physical, occupational and speech) and was impressed at how they were encouraging him to push while still making sure he was safe. Once the COVID lockdown hit, I still called daily and spoke with staff regarding all aspects of his care. They always took the time for me, and never made me feel as though I was bothering them. They relayed my messages to Dad and made sure that he still had company. Although it was difficult not being able to see him daily, I was comforted by the fact that I knew he was in good hands. Everyone there was caring, professional and really cared about my father. THANK YOU to the staff – you made a huge difference!

Negative experience: They never answer their phone. Then when you do by lucky chance get ahold of someone they tell you the person you need to talk to isn't there and that you have to call back. Very frustrating.
Hi Vanessa, we definitely understand how frustrating this might be. Alpine (previously named Lakeside) is now under new ownership and one of the first projects is a complete upgrade of the phone system. The system is from the 1980s! This installation should have a very positive impact on our telephone answering services.
Negative experience: Wist medical facility I've ever stayed at... Medicine is always 2-4 hours late... Nurses are rude and mean... And the food ... Omg the food is something of a nightmare.... If tou love you're family member do not bring them here .... Gave me constant panic attacks being there... This facility should honestly be investigated by the state...
