Fantastic experience: We moved my dad here for long term care at beginning of January 2025. He had previously resided at a memory care facility a few miles away for a few years. We came to the sad fact that money was running out and we had to choose a Medicaid facility for my dad’s continuing care. I’m beyond grateful for the staff at Southampton. My dad has been here for 5 mos and the level of care he’s received at this facility is better than the care he received at the other facility. My dad is always dry and clean. His room never smells of urine. His teeth are clean. He’s bathed regularly. The staff has never been rude to him, and when I come to visit I’m always greeted with a smile by staff. The facility isn’t flashy like some of the other facilities in the area, but honestly to have the peace of mind that your loved one is being cared for and treated kindly is worth way more. Today I was visiting my dad in the common area. The staff began playing music. A few residents started dancing and not long after a few members of staff began dancing with the residents. It was clear they were all having fun and enjoying about 20 min of dancing on late Tuesday afternoon. It was really heartwarming to observe! Fantastic experience: I would like to give a shout out to the beating heart of rehabilitation at Southampton Rehab, the staff of Physical, Occupational and Cognitive Therapies (PT/OT/+). The staff that I worked with included Althea Strozier, Jaclyn Barry, Lance Ranek and Omar Alkhadra. Jaclyn is like the sunshine that lights up the room with her caring and watchful eye. When she addressed me from across the PT/OT room, reminding me to stand up straight, it was coming straight from her heart. To top it off, Jaclyn always carried with her a kind sense of humor to keep the atmosphere in the PT/OT room fun and easy. Her confidence in and execution of her craft is unquestionable. Lance has a backbone quality about himself. He always guided his therapy with a sense of awareness of how my body was reacting and responding. He knew the right therapies for me, and just how far to push me before crossing over into too much. Lance has a seemingly endless depth of knowledge as a PT. Additionally Lance was a pleasure to work with. He was always 100% supportive. Omar, like Jaclyn and Lance, has a confident and deep understanding of his craft. He is especially kind and personable. Like Lance and Jaclyn, I was always glad to hear that I would be working with Omar. Thank you Omar for your fine work and support! Working with Althea was always fun. Althea works on cognitive strength, using mind puzzles to work one’s brain-power. Being 65 years old, I’ve known about the usefulness of puzzles like these for a long time, but having never tried them, didn’t understand the potential benefits! I’m a believer now and have some in my cart on Amazon. I want to stay as sharp as I can, and Althea has opened that door for me. Also Althea made the process fun and funny. And she brought herself to the table in an open and available way. Thank you Althea, Omar, Jaclyn and Lance for providing me with a focused and vigorous rehab experience to help me on my way back to my life sooner rather than later!

Positive experience: Front desk staff friendly. Nursing staff doesn't acknowledge visitors to the facility. I stood at the desk and said" excuse me" twice.Nursing staff continued to laugh and talk amongst themselves. I was acknowledged by 1 staff member that alerted the others that I was needing assistance finding a resident.Nice facility. Always clean. My mother is pleased with her care and rehab.I am also pleased with her progress.

Fantastic experience: I would like to give the most respect to Ms.Flo. Flo is the Assistant Director of Nursing at Southampton Health & Rehabilitation Center on Forest Hill Avenue, in Richmond. Ms. Flo is one of the most genuine, hard worker, awesome RN that makes Southampton what it is today! Not only does she have the best interest for our residents, but everyone! Ms. Flo is what I call, “The Fixer” she makes things right without a pause! Never a dull moment; beautiful smile and when she speaks, the sincerity comes out! Fantastic experience: Please note, I visit my love one today on third. I walked in the biilding looking amazing. As I were visiting, the housekeeping was so polite and identified who they were and ask if I needed anything and could she be as assisting Good job to the housekeeper.

Positive experience: My mother said the food server male knocked something off her tray had to ask server to pick it up to which server did but tossed it on her tray knocked over another item on the floor which she asked was he going pick it up he stated he didn't have time all this and tossed it on her tray. I really do like your staff very caring helpful and polite but tossing my mother food items unacceptable

Fantastic experience: My experience with this facility was awesome on my first day I’m 40 years old and never thought I would be in this type of place so I was not thrilled to be here but from the time my stretcher enter their building I felt so welcome the stay who helped me check in (I’m so sorry don’t remember names just yet as this was my first day) were awesome they made me feel so comfortable and put me to ease about being here Jessica(LPN) was amazing she was very thorough this place gets all of my 5 stars I wish I could give more

There are too many kind and dedicated professionals to mention here, yet I thank them for their dedication and professionalism. But I must also give much credit to Ms. Saundra Bates for bringing kindness, joy and love to all within her presence. Fantastic experience: The staff at Southampton really made our stay as pleasant as possible considering the circumstances. My 98 year-old grandmother was a resident there for her last six months. Unable to fully recover after hospitalization, the wonderful folks at Southampton treated her with love and kindness, dignity and respect. They excelled at compassion and communication, doing everything they could to make her comfortable. Nurses Aiisha, Jessica, and Felicia were engaged and treated us like family. CNAs Tiaa, Pam, Tania, and Jameeka cared for her like she was their own grandmother. They were all advocates for us during this difficult time. Susan was incredibly helpful on the business side, helping us navigate Medicare and Medicaid. We could not have asked for a better SNF experience. Thank you all so much!

Negative experience: I WISH I COULD GIVE THEN NO STARS>>>Very unorganized, smells horrible, dirty and my dad is not being bath or changed daily. My dad has dementia and the are making him sign using his initials to paperwork he doesn't understand. This place is not worth sending your love ones too. The right hand doesn't know what the left hand is doing. No respect for religion either....My dad is a Rastafarian and a nurse had the audacity to cut my father‘s hair after being told during admissions that this was against his religion if they had any questions they should’ve asked this was upsetting..

Positive experience: I've been here for 3 monthsI've been here for three months and I have seen tremendous improvements in my physical health and many good people that helped me along the way but someone that I have really enjoyed that is not a part of my hands on care is Nura she goes to everyone's room two or three times a day just to check and see if everything is running smoothly she sees a problem and tries to handle the situation she does it with a smile and always a pleasant word Also I have my dream team is what I call them but they are absolutely the best Jameka dada and Taylor they work from 3:00 to 11:00 on the first floor when they are here you know you are going to have a good night they work great with each other when one needs something they always have each other's back even if they are not working together trust me you still have a great aide they are all kind and friendly I think they are the best

Fantastic experience: Sharae House, Admissions Assistant and Susan Cook, Business Manager have been very helpful in assisting my mother during this period of adjusting to life in a Skilled Nursing Facility (SNF) at Southhampton. Their unique understanding in dealing with the difficulties family members face with navigating admission of a loved one to a Nursing home is commendable and greatly appreciated. Also, I want to say thank you to all the staff, nurses, custodians, doctors, social workers and any other workers who have helped with my mother's care. My mom is looking better and seems to be doing better for being 91 years old.

Fantastic experience: We would like to share our appreciation to Teresa Johnson and Gina Willis for their extra help with Scott Champion, providing first, a warm and informative welcome to Southampton. During his stay, Scott needed a hair cut and beard trimming. Teresa and Gina arranged for the task and actually did it themselves. My brother was so excited and happy he danced in the hallway after it was done! Scott has limited communication and understanding. This was very important to him to get done and continued to pull on hair and say "hair cut" everyday. Fantastic experience: I can’t begin to say enough positive things about Southampton Rehab and Healthcare! After suffering a fall last January, and no longer able to take care of himself, my brother was shuffled from facility to facility, as his coverage under Medicare would run out and they’d want to move him along. Prior to his fall, noting his declining condition, I had sought the assistance of Richmond City Social Services for my brother, and to be blunt, they were ineffectual at best. Nothing came of my efforts, and it took his near fatal fall for him to begin getting the help he needed. Even so, nothing was guaranteed, and after a year of uncertainty and stress, he eventually arrived at Southampton. I was immediately impressed when I walked in the door and have been each time I have visited since. It always smells fresh and clean, and staff is attentive, exceptionally friendly and helpful. And they seem sincere, like they understand and truly care about the well-being of their patients. In particular, I would like to mention Business Manager Susan Cook, who somehow managed to secure Medicaid for my brother at the 11th hour and prevent him from being shuffled off again. Her skill in navigating the system literally saved my brother’s life, as without the comprehensive care he is receiving at Southampton, he could not survive. A huge thank you to the Southampton Team, for giving me back my peace of mind!

Fantastic experience: I am so pleased with the care that Pauline Jones received in your facility. The patience that was provided for her. The nurses and aids that assisted with her health. I mostly appreciate Carol Skeeco making her feel loved everyday of her stay. On the scale of 1-10 for her service, I rate her a 10 on Pauline Jones and with other patients that I saw when I visited . When I made calls to the facility , they were very friendly and helpful. I thank you Carol, for taking such good care of my Aunt.

Positive experience: I visit my cousin Pauline Jones several times at this facility. The experience I seen was always amazing. She was well kept and the staff at this location, were very helpful. Carol Sheeko in the activity department shows how she cares about the clients. I really appreciated the staff at this location while she was there.

Fantastic experience: My most favourite time of the day, any time of the day, is when Ms. Carol Jones-Sheeko who works in the Activities Department comes around. She is always a breath of fresh air, a bright light of sunshine. She will lend an ear, if needed, or offer help if there is something that she can help with. Fantastic experience: I have known Saundra Bates for a few years. She is a good friend to us, my husband and me. We've been very blessed with the wonderful care she has given to my Father, Jack Martin. At times, she would pop in on her days off, and call us so I can talk with my dad. I live in California, and she knows I don't get to see him except for calls she makes or calls my siblings would make. This has been a great encouragement to me. Fantastic experience: I’d like to mention what a jewel Saundra Bates is. She has always been wonderful doing the various activities as Life Enrichment Assistant, but she also shows special kindness to our dad, which means a lot. All the residents seem to love her, and she always has a smile on her face.

Positive experience: This place has gotten better with the new management, food has gotten better when they actually stick to what they post however she rarely ever gets the pb&j and banana that's always on her ticket, the laundry service is the worst I've ever seen. It still needs work but it's slowly improving.

Negative experience: I've been patient here for three months and I would not recommend it for anyone! However I have had a great experience with the therapy team, the administration team is a different story.. do your research before committing here! There have been too many issues to list there always seems to be mis communication or no communication at all l wish you luck in your search for a facility

Fantastic experience: Ms. Sherea has been an amazing Ambassador. She is very timely in her response and in answering my questions and concerns. My mother has had a very rocky road to recovery from a car accident. Sherea seems to really care about the patient and family.

Fantastic experience: Dr. Shelley Johnson and other staff members met with my mother-in-law and me and several other people who have family members at Southampton during the regular monthly meeting. The staff who were there, and especially Dr. Johnson, did a very good job of addressing the concerns that were expressed. They explained how they were working to meet challenges with specific solutions. I came away with renewed confidence my father in-lw would get the care he needed.

Negative experience: This place is disgusting the rooms are small you cant even turn a wheel chair around they are short staffed the elevator is slow as hell there no outside area but a patio to sit on the place feel like a jail do not send your family there

Fantastic experience: I want to take this opportunity, thanking the building Administrators, Ms. Thomas for such and amazing job she does. I visit this facility and it’s amazing! I ask who’s the person that keeps the building nice looking, the floors look amazing, so shiny, no smell as many I’ve notice of a not so great of a smell! The welcome I’ve received, I was amazed! They informed me the new director of Housekeeping who keeps that building clean can say, what a great job the Administrator and the Housekeeping Director! They both are doing an an awesome job of this facility.

Fantastic experience: My sisters, Flomesha Hogan and Tamia Hogan work there. I saw the pretty flowers out and asked about it. They explained they’re under new management. I’m excited for the patients. If they’re beautifying the grounds I can only imagine what’s being done on the inside!

Negative experience: Not worth 1 star, they will neglect your love ones. While they are in the hall talking and being ghetto.

Fantastic experience: Antoinette Shaw and Gwendolyn Tucker in the MDS department welcomed me with such a warm and welcoming attitude. Very positive outlook about the facility and the team spirit here.I know I am going to truly enjoy working here. The MDS team smile was very contagious and they explained their roles here and it was very helpful

Positive experience: Nyasia is wonderful. While I was in limbo as to where to go, she worked hours upon hours trying to coordinate everything. She kept checking in with me and even though I'm already here, she still has seen to my needs. People this place would fall apart without: Roz in Central Supply. She *always* kept everything in my room well stocked, especially after l informed her some of the nurses were 'borrowing' from my stock of briefs. Felicia, Day Nurse usually on the first floor. When I hear her voice I know I'm going to be alright that day. She knows all her residents, what they take etc. She has a great bedside manner. If I have issues with another nurse she'd encourage me to come to her about it. Felicia doesn't play. I ❤️ Felecia Jessica, Nurse usually on the first floor. She is wonderful. Always has a smile and keeps morale going while more and more residents are admitted; more and more is expected of these nurses. It felt like at one point Jessica may as well live here.

Fantastic experience: Good morning: I came in to visit a friend before work. I walked in, and the greeting I received, wow was awesome, never received this in time of visiting facilities! The place looks great, the housekeeping doing their thing, nice and clean, and ask how am I doing, wow! I will spread the word of this facility! Thank you everyone!

Fantastic experience: We had our first Family Council meeting last week at Southampton and I met the department managers of the facility. I was very impressed by the Director, Ms. Regine and her genuine desire to make sure we knew who our points of contact were for any and all needs. She was so professional and knowledgeable, but still welcoming and friendly. It was very reassuring and helpful to meet face to face with everyone who was in charge of caring for my Aunt…from Lisa at Rehab and Steven in the Dining Room, to Paula with transportation and David with maintenance…they were all very thorough in answering all of our questions. I’ve been most impressed with Nyasia (Director of Admissions) and Susan (Back Office/Billing) who were nothing short of miracles working with me to get my Aunt’s admission paperwork completed! Nyasia answered phone calls and text messages without delay and stayed in constant communication with me during the entire process. Susan followed up with case workers and kept me informed on the progress and next steps. I will forever be grateful for them and the level of support they provided during a hectic and confusing time for my family. I also have to mention the awesome nurses on the first floor, Aisha and Marsha, who are top notch in what they do and the support they give to the residents and the families! Thank you ladies for being the best! Now that my Aunt is on the 3rd floor, I’ve been working with Keisha (unit mgr) and she is amazing! She sat with the family for over an hour until 9pm one night answering any questions we had and making sure any concerns we had were documented and addressed. Keisha even followed up a few days later with a phone call to give me updates on some of my requests! Three of the 3rd floor nurses really stood out for their pleasant attitude and helpfulness…Beverly, Sherita and Tamantha. They are all very receptive when I speak to them about anything I notice regarding my Aunt’s care and Sherita always has a smile when I see her! I know that no facility is perfect and Southampton still has opportunity for improvement, but I give it 5-stars for their dedicated staff who are willing to partner with the families and ensure our loved ones get the best care.

Fantastic experience: I really appreciate the services given at Southampton. Ms Thomas the Executive Director is known for driving service, patient and employee excellence. Fantastic experience: My visit today, was unsuccessful in seeing my friend, yet the assistance and personal attention I recieved at the Nursing Desk was superb...Jessica was very discreet about the reason I decided not to visit my friend, but with such a pleasant personality and consideration for me as a Visitor that Jessica displayed, it was truly truly appreciative and only hope that the staff appreciate her as I did....Thank you

Fantastic experience: The customer service and care here are truly exemplary, and I am grateful for the dedication and kindness shown by the entire team. I had an exceptional experience at Southampton. From the moment I walked in I was greeted by Sonja at the front desk. I was introduced to Sharae the concierge, who greeted me warmly and made me feel at seen along with the admissions director Nyasia, she was incredibly helpful and guided me through the entire process admission process with such understanding and compassion. I had the pleasure of meeting Ms. Regine the administrator and welcomed me in like family. Nydra and Nura the social workers, were incredibly sweet and welcoming outstanding in their support, offering valuable assistance and understanding every step of the way. The customer service and care here are truly exemplary, and I am grateful for the dedication and kindness shown by the entire team.

Fantastic experience: I wanted to take the time out to shout out the 3rd floor at Southampton Rehabilitation & Healthcare Center. Since joining the team the customer service I have received from the staff on the 3rd floor has been exemplary. I would like to thank Nurse BJ, Nurse Faith, Nurse Beverly, Nurse Beasley, Nurse Gina, Nurse Tracey, CNA Theresa, CNA Tasha, CNA Miss Barbara, CNA Bradley, CNA Banks, CNA Nedra, CNA Shawnae, CNA Cooper just to name a few. Though it's been a short while I look forward to growing with you all as a team and providing quality care to not only the 3rd floor residents but, to ALL the residents at Southampton. Fantastic experience: I visit SRH frequently and I'm always welcomed witha smile and a "Welcome To Southampton" by all the from the front desk to the Facilty Administrator. Nurse Jessica is always there to answer any questions and takes care of my mother as if it were her own.

Fantastic experience: After suffering a life stopping stroke on July 1st I entered a Very welcoming, clean, and inviting southampton rehab & health facility for approximately 38 days, my overall experience was very positive and life changing, the team received a 5 star from me, with special thanks to miss Tia and Miss Taylor CNA'S that made a big difference in my stay, they were caring, loving, and concerned at every turn about your comfort, i also praise the entire body of CNA's at southhampton they all work hard, the team of very professional and well experienced RN's, and LPN's, with special thanks, to Keshia who was extremely confident, professional, and knew it seemed every aspect of medications its reactions, purposes, and well defined with all my doctors notes regarding my needs she was always willing to go the extra mile to make sure any questions concerns were met with in a reasonable time frame, lol she always had snacks on her med cart even if she had to provide herself, i have the utmost respect and admiration and thanks for this team of professional nurses, the administrative staff was wonderful constantly coming around making sure I was being taking care of, they created a portfolio of me that was accurate and always updated, special thanks to Nigra who went the extra mile to make sure my insurance and benefits were updated, to reflect my particular need, thank you to each and every admin personal everyone was great impacting my positive stay at southhampton, based on my progress and needs, the physical, occupational, and speech therapy team were excellent much was gained including being able to walk well enough to go home after suffering a stroke only 2 months ago, much thanks to that team and special thanks to Lance who seemed to take under his wing from the beginning, I appreciate you very much, I appreciate each and every one of you, i will miss PT, OT, ST very much, in conclusion I would recommend Southhampton rehab and health as your 1st choice, and southhampton rehab and health will have a place in my heart forever, again my thanks and appreciation to each and everyone at southhampton, Sincerely, Gary Tate

Positive experience: The only downside is the food is sometimes cold and I have to ask someone to warm it up. The tea is weak. Since I have been here I have had to ask repeatedly for bed pads, eventually I got them. They are working on renovations hopefully it gets done soon. Sometimes maintenance would take a while to fix things. Ms Nydra, the social worker, was very pleasant, she was able to explain things to me. When I call her at night, not meaning to worry her, she was always there for me with my concerns and needs.

Fantastic experience: Since I been here at Southampton they have treated me with respect. When I have an issue I go to Nydra the social worker and Regine the administrator. My therapists Quintasha and Lisa are great at helping me do my therapy. Overall everyone has been helpful during my stay here.

Fantastic experience: Marsha and Jessica have been such a huge help to my husband and me. They are great! The physical therapy team has been wonderful!

Negative experience: I’m not sure how to describe my loved one experience there but Sharae was helpful and understanding. Also there were some who seemed to care.

Fantastic experience: Southampton Rehab & Healthcare Center has been a tremendous help to our family as we quickly have had to adjust to our mother’s sudden health related needs over the past few weeks. While this transition is emotionally challenging for mom and us as a family, members of the team such as Regine, Nyasia, Paula, Nurse Jessica and other clinical and dietary staff have extended their time, active listening ear, patience, sincere care and understanding to go above and beyond to assist. Additionally, the admission and visitation processes are supported by technical and virtual enablement tools helping to make tasks easier and quicker to manage. As we move forward in partnering with this team to care for mom, we are beyond grateful for their services. Thank you for all you do!

Fantastic experience: My aunt was at this facility and honestly I was not expecting much. However, the experience was great. Most of the staff were friendly and welcoming. What really made a difference was the director (Ms. Regine) she greeted us and alway had time to chat. I felt that she really cared and she knew her patients. This made all the difference in us choosing this facility.

Fantastic experience: I was at Southampton for approximately a month after a 9 day stay at Chippenham Hospital from falling and breaking my left femur. The staff was very kind and knowledgeable about taking care of me with my broken leg . I also have PSP which is an. Offshoot of Parkinson’s. They were a little less knowledgeable about that, but we all worked toward the common goal of getting me well enough to go home. I loved the staff and the attention they paid to me and not hovering. Would recommend this facility to others with short term ailments.

Fantastic experience: My sister resided at Southampton and previous facilities prior and I can tell you from experience you will not find a better Executive Director than Ms. Regine! She is World Class in the customer service she provided to my family (taking time to listen and resolve concerns) and the example I saw her set for her staff. You don’t see many in leadership positions in the trenches with their staff but Ms. Regine is! On various occasions I witnessed her on the floor, interacting with residents and encouraging staff. This building may not be the newest and most modern but Ms. Regine is a diamond.

Fantastic experience: Paula who works in medical records, she has been so helpful when I needed answers to certain questions and if she don't know the answers she will find someone who does know the answer and get right back with you, now that is service with a smile, Thanks so much PAULA.

Fantastic experience: Nyasia provided great customer service. My pizza was delivered to the facility but got lost so nyasia contacted the pizza place and got them to make me a fresh pizza and personally picked it up herself. That goes a long way. Thank you very much. I recommend Southampton outstanding service
