Alden Debes Rehabilitation & Health Care Center

Fantastic experience: A couple of years back I had a very close relative of mine transferred to this facility due to behavioral health and other issues. I cant say that I was not skeptical. However, over time I was quite impressed with the care and empathy shown that my Great aunt received here from the CNA's, Nurses and management. With team work we were all able to get her back to baseline. The issues she was having at one point that could have been prevented were avoided. I would like to give a special shot out to the Director Of Behavioral Health Rachel. Throughout the entire stay Rachel was very consistent, empathetic, resourceful and most of all communicative. Her response time was always top tier. In addition, another special shout out to Blanca, another staff member who I believe was a CNA. Blanca, you are appreciated with love. I just wanted to leave a few words and give credit where it is due. Alot of the time, we always see negative feedback but people fail to provide positive feedback when it is due. Thank you for all that you have done for my family providing the care you once had before and during the transition of my loved one! You rock!

Negative experience: Wow, reading all these bad reviews, they are so accurate!! My loved one was here and had same experiences. We had to take him out!!! He was neglected and ended up contracting 2 different infections at the same time!!! Please don't take your loved one here!!!

Negative experience: The Staff only thinks of there Paycheck. The Aides stand in the Hall n Chat while Call Buttons are going off. Food was either Luke Warm or sometimes Frozen in the Center. For 5 days in December (2 yrs ago) there was No Heat, they said the Broiler was Broke. I definitely would Not recommend this place.

Fantastic experience: I was here for rehab every one on staff and residents and cafeteria were wonderful great place great food.

Fantastic experience: The staff here took amazing care of my mother during her stay after being diagnosed with CHF. Everyone was very Friendly and Motivating to her, We appreciate all the work and advice given to us even after she left. HIGHLY RECOMMEND!

Fantastic experience: My mom was a patient a couple of times these past two months. The staff was friendly, dedicated and hard working. This is not a profession for everyone. Did she have to wait a few times for help getting to the rest room... yes. But that was only because they were busy helping other patients. As soon as they could they were in the room to answer the call. As far as the facility... the room was very clean with a couple of large windows. Only minor complaint was the furniture. It needs to be replaced with something that would be hardier. The PT and OT were both professional. The guys in the dialysis wing were very good! The dinner options were good and the treat cart lady was a hoot! She goes around with a cart selling treats to the residents who just need a little something for the afternoon. Besides the rehab part of her stay, there were also opportunities to socialize which even included a fun game of bingo. My family would highly recommend Alden Debes as a place for rehab.

Negative experience: I wish I could give this place 0 stars. While there are some employees at this facility that are worth something and very sweet, they sadly don’t outweigh the horrible things my family experienced with our loved one in this facility. The staff are so rude and insufferable. Staff would come in when my loved one turned on their call light and turn it off just to not come back. I was told by one of the good employees that unfortunately if we wanted to make a complaint that the facility would clean up their acts for the inspections until they were over. To put the cherry on the cake, they violated treatment guidelines including an DNR. Overall please don’t take your loved ones here, whether it’s the rehab side or extended stay.

Negative experience: My grandma was there for rehab and her call lights were never answered in a timely matter especially when she had to use the restroom and her medications were never given on time. When meds are due at 10pm they should not be given at 3am. She received very poor care.

Fantastic experience: I enjoyed my stay here. The staff are wonderful and communicate very well! I will be back for another stay. I was treated very well.

Fantastic experience: Well it's been seven months since I been off dialysis and a year now of being sober since I came here I have to say if it wasn't for the staff and there team I would have not made it this far even though that I'm still here for a little while longer I have to say they really do care for everyone here and there family to make sure that you're comfortable so with that being said thank you to all the wonderful people here who make us feel welcomed may God continue to bless this staff and there team

Fantastic experience: My mother was admitted into rehab and stayed throughout the process before coming a long term resident. She has had a great experience with everyone that has helped her through the process. The entire staff has went out of there way to assist and help her make the transition. Thank you to everyone!

Fantastic experience: I had a family friend with her father here for end of life, hospice care. They have a beautiful hospice designated area in their facility with very compassionate, well trained staff. We cannot thank them enough for making his end of life the best it could be. Highly recommend.

Fantastic experience: My father has been at Alden Debes for rehab and now he is in hospice. My father has a great experience with the nurses and CNA's who enjoy taking caring of my father and helping him when he asks. The staff always greets our family everyday with a smile and a cheerful hello when we arrive to visit my father.

Fantastic experience: I have been here since last year of April for dialysis they have very great team of people who care and do there job with patients and they work hard at what they do. Me truly appreciate what they do for me because you have some people who don't care because it's not there family member but what if the table was turned around you would want that same help from that nurse or that CNA to take care of your family so with that being said they work with you you and your family to make sure you get proper care the management team works hard around here to make you feel welcomed so I don't give them a five star I give them a hundred percent for going beyond what they do here for US they treat us like family and that's what matters so come see for your self and stop letting people tell you otherwise nothing but love from me

Negative experience: Alden Debes management (Jacob) led me to believe my mom would be cared for and treated with dignity. She was not. She left their care with bedsores and blood clots. Communication with agencies, families, doctors was unacceptable. For example, I was called and told my mom was switching rooms because her roommate had died. I was unaware she had a roommate and Jacob assured me she would have a private room. At the very least a phone call to let the family that my mom would have a roommate would have made the phone call that the roommate had died much less confusing.

Fantastic experience: Excellent Facility, very clean and exceptional nursing care.

Positive experience: I have just started my journey of PT and healing at a wonderful place Alden Debed every one is helpful and tactful except a nurse name Tory In a consultation with her over a missed wound treatmentt she change the subject to me getting argumentative and walk out the door.

Negative experience: Keep your family far away from here. Whenever you have a concern it's blamed on the family member. My father has had same gown on for 3days. Bed is full of crumbs from food. They smile at you but it's an act. The management is deceptive at best. They lied to my face about not using bedrails but my father'sroomate had one. I have lovely video and pictures for Illinois to receive.

Negative experience: My dad was there 3 weeks. Good luck getting what you need done. They don't answer call buttons. Wound care not done as it was suppose to be.Theroom had NO BLINDS on windows. You can't a administration or social worker to answer a phone to save your life. I tried to get him transferred and no one would answer phone and fax info to do it.run dont walk.

Fantastic experience: I came to Alden Debes twice I had both (R) & (L) knee Replacements done....I love this place I was on the 300 wing the nurses were nice & helpful....The Physical Rehabilitation Department was nice and understanding....I would recommend this place to Anyone, Family, & Friends....I want to thank them for taking care of me.

Negative experience: neglects patients, tennis shoes stolen out of her room, never offered a shower, re-hab was taken to re-hab and left her sitting in a chair for 25 minutes. Someone going through her personal items ar 430 in the morning. Ask to see nurse, sometimes they might show up.

Negative experience: This place is a nightmare they admitted my father after he broke his hip they left him with pee and poop on his bed didn’t even clean him and I just learned they haven’t been giving him his insulin and other meds that he must need. The so called “nurses” sit at the desks talking away, I witnessed someone screaming help me or im going to hurt myself the nurse just sat there at her desk saying I’ll be there in a minute by far this experience is the worst.

Negative experience: If you love your family member, don’t let them stay here.

Fantastic experience: After breaking my leg, we determined that I needed the help of a skilled nursing facility. I am glad that I was able to get a bed at Alden Debes subacute care. The staff is caring and conscientious and ancillary services like the social worker have been very helpful with what could be an overwhelming experience. Thanks to Teresa (social worker)and all of the CNAs and nursing staff here. Also need to acknowledge how clean the housekeepers keep the entire place. Would you believe they pay attention to all surfaces where dust could gather?

Fantastic experience: My mom recently passed away at Alden Debes in Rockford IL. Let me start out by saying I was not impressed at first with the building itself. What the building lacked the staff sure made up for. As mom's illness progressed let's just say she wasn't the sweet old lady she use to be. Fantastic experience: The place treated my mom like she was family. Every time me and my dad comes to visit, She would be up dressed and in good spirits. The building would always be clean and the staff were very helpful. I also heard their wound care staff are amazing as well.

Fantastic experience: My mother went there for rehab after being in the hospital. They were awesome,extremely caring and understanding. The entire facility from the nurses,sides and the therapy staff. I would take any family member there for rehab loved the therapy department very professional and knowledgeable.

Fantastic experience: My Mother went in there for two weeks after a shot stay in hospital. The staff and the rehab center were fantastic with her. They were able to get her stronger to be able to come home. Treated her with respect and also respected my Father and I when we came to see her.

Negative experience: Horrible communication!! You can't get any staff to answer phone or return phone calls. Can't get any issues resolved because you can't get in touch with anyone.

Negative experience: Someone very close to me died as a result of the neglect demonstrated here. If you read the positive reviews about this place, they come from the staff that work there. Almost as if there are forced to. It’s a similar script on each one.

Negative experience: THIS PLACE IS STRAIGHT FROM A NIGHTMARE. PEOPLE SCREAMING FOR HELP, LAZY STAFF AT THEIR DESKS CHATTING AWAY WITH ONE ANOTHER IGNORING THE PEOPLE THEY ARE SUPPOSED TO TAKE CARE OF. I’VE SEEN SOME BAD PLACES IN MY TIME BUT THIS PLACE TAKES THE LEAD AS THE WORST PLACE EVER. FILTHY, ROTTING ODOR, POORLY MAINTAINED STRUCTURE AND RUDE STAFF ARE JUST SOME OF THE KEY POINTS I NEED TO MENTION.

Negative experience: My father was put in here for rehab. The room that he was in had no thermostat on the wall. It was just bear with wires so he was cold most of the time sometimes they would shut the door and then the room became very hot. The door to his room had to be held open with a chair. Otherwise the door slam shut. They would get him up for rehab in the morning and then nobody ever showed up afterwards to get And move him, he just laid in bed. He needed a catheter put in after three attempts. It was finally inserted. He was kicked out of the rehab because of insurance, but after a week of that catheter being in place he went to the Doctor Who said he had an infection. He was taken to the emergency room where he was found to be septic. This nursing home does not offer to bathe your family member. I would not recommend this to anyone, I would not even put my dog in here

Negative experience: 10/4/22 I fell backwards in the bathroom on a hard floor I fractured my pelvis and hip bone not being able to move for 6mo's only sleep on my back I had to call the Ambulance myself because they refused to call for me Alden Debes made me sit in a wheelchair for 8hrs daily even though I was in alot of pain, they didn't give me anything to eat for 2 days, because I couldn't sit up straight, they were rough with me when they took care of me in bed, they made me take a shower when I should had gotten a sponge bath. Now that I'm stronger again all I ask for is a tranfer to and from the shower and they won't do that twice a week Theres a hallway called the (B.B) stands for Bad Behavior that has locked doors so they can't get out the staff are Really Rough with them their when they don't need to be So I would suggest putting your family and friends Somewhere Else to get Good Care and Being Cared for Respectfully. 2) A male conselor walked in on me when I was naked when I was trying to get dressed without knocking and people that live here doesn't knock either when your in the shower Me That's Called (RESPECT) eve takingshower.

Negative experience: Worst place ever! My mom went for rehab and almost died here! She was soiled every day and I found her on the floor while the DON has his feet kicked up laughing on his personal cell phone!!!!! They let ppl starve, and there is no good help there period! They lie about everything they say they'll do! Also a staff member told me they have bed bugs and roaches! This place should be shut down period. Someone could get better care at a animal shelter!!! ENJOY THE INVESTIGATION YOU'LL BE GETTING!!!

Fantastic experience: I previously worked for this facility for approximately 2 years and trusted all of the staff in their patient care and responsibility, so much so I even checked my grandmother into the facility after her surgery because I knew she would be in excellent hands!!!

Fantastic experience: Trying to find a place to put my mother-in-law during covid for rehab after a knee-replacement surgery was difficult. This facility stepped up and got her in immediately, even going so far as to paint a new room for her. They took excellent care of her and were so conscious of COVID protocol to keep her safe. She had nothing but great things to say about the staff and their attentiveness. I would definitely recommend to anyone needing good care for their loved one.

Fantastic experience: This was my first nursing job after i graduated nursing school. I was able to grow and advance with this company and now i'm a nurse consultant. It was an awesome place to work.

Negative experience: My dad was released after falling in the bathroom at the nursing home. He can barely walk. He also had to wear a diaper. He would push the button for someone to take him to the rest room. No one came. He had soiled the diaper 3 times before anyone .came to do anything. The wheel chair they gave him will not fit threw doorways at home. I was told by the nursing home social worker. That they would set up doors.and give him additional help. No, that didn’t happen.

Fantastic experience: I have worked at Alden Debes for 19 years. I wouldn't have stayed all these years if it wasn't a great place to work. As Activity Director I take all long term Residents concerns monthly. Most concerns are an easy fix. I watch the nurses and cna's daily show compassion to our Residents. I am proud to be an Alden Debes employee and will continue to do my best to enhance the lives of our Residents!

Fantastic experience: As a nurse who has had the experience of working in several other rehab and long term care centers, I can honestly say Alden Debes is a great place ! The staff is caring and compassionate. The care given is unquestionably some of the best I’ve encountered. At Alden I’ve been treated like family and not just a random employee. Highly recommend this facility to anyone seeking placement, employment, or who needs a rehab stay!

Fantastic experience: As the admissions director since 2013 and 3 years as the liaison prior, I can tell you that we love our facility and residents. Your loved one is not a patient...they are our family. We come to work everyday striving to make a difference in each and every family members life. I am proud that Alden Debes has been my other home for all of these years.

Fantastic experience: I've worked at Alden Debes for over 7 years and have loved it. It's not always perfect, but it has been a great enough experience to stay, work multiple positions and advance within the company. I have worked with some great people along the way as well including some amazing nurses and CNA's.

Negative experience: This place does not listen to clients or families. They do not pay attention to who signing contracts also. This organization is not organized at all.

Negative experience: This place is a nightmare! If you have another choice please choose it. My great aunt (rest her soul) has just passed away alone in this foresaken place because they would not alow family to visit her. When we did visit her we noticed the complete lack of care or compassion. I worked at an amazing nursing home called Pinecrest Community for 6 years where something like this would have never happened. I would never speak badly about a place of business enless I felt passionately about it. Do not use Alden Debes.

Fantastic experience: Do You have to place your loved one in a home? Temporarily or maybe long term? We the staff understand that it is most difficult for you as the family to make this decision. We strive to rehabilitate so they can return home. No one wants to be in a home, no one can care for your loved one like you can. We strive to be a close 2nd to their family. When you visit, leave a kernal of knowledge with the staff; ie mom likes to get up after breakfast or she likes to watch the sun rise, some want to call home at a specific time...this helps with their transition as well. Sometimes there is conflict within the family as to who is the poa and decides that the resident needs to stay in a home, when others in the family do not agree. Money issues or insurance... Please leave those conflicts at the door. This is a conflict for them. When you visit they know their bodies cannot function as they used to. They want to bake or make you lunch. They want you to stay awhile, bring the kids. Come with a smile, call them on the phone, write them a letter. Steal them away for an ice cream, we have a few areas for the family to bring them a snack meal or visit with a closed door to shut out all and allow them to be greedy with their time with the family. You would be amazed what a letter or card will do. It creates instant bragging rights for this granny on the block; especially if photos are involved. Know that we at Alden care. I for one have been doing so for 18 years. I love all my Alden family. Drop by and see us, drop that letter to that aunt mom grandpa, father ect.. (most are from a generation before email). We hope to see you soon ...:)

Fantastic experience: Very good to wife and myself

Negative experience: Horrible management! Lies lies lies.. They say they will Email you and weeks later after you Repeatedly ask for an update on a family member they IGNORE YOU, my brother is there and when Something is wrong like him in pain they ask him but he won't respond to them due to stroke but he calls me and can communicate with me it takes a couple of minutes to find out what's wrong back I call the nurse to let them know where his pain is after speaking to him then they take him to the hospital once they see a problem like his catheter etc... Nobody gives a real update on anything , when your there in person it's immediate attention to him and updates but if your not I'm town forget about finding out anything on your loved one, this place is horrible for management the director and the nurse admin is horrible at their job , they lie and Are Not honest, I have sent several emails asking for a update , we had a arrangement and they will... Stay Away! No communication from them .. the nurses taking care of the patient are ok, but the Management is Horrible, they Lie and will keep information from you. If I could give it a zero I would on account of the Management .
