Fantastic experience: I’ve been to a few homes over this past year and this has been the best place so far. They have several fun activities every day you can choose to go to, my favorite has been Bingo. Vlad is the chef in the kitchen and he has been great making sure I get the food that I need for my diet (I have several medical conditions that each have their own diet so it can be difficult for me to find things I can eat). Donna has been great at checking in with me several times a week to make sure I have everything I need and to see what she can help me with for when I get ready to leave here and go home. Jake, the P.A., and Dr. Galindo have been helpful with my transition from the hospital and making sure I have all my correct RX’s, as well as helping me with my pain control. They even allow my mom to stay with me in my room (which is very spacious) so she can help me. I can’t say enough good things about this place.

Fantastic experience: I have been resident at monument Southlake for the last few months, staff is really engaged and involved in my care. I have felt very cared for during my time here. Nurses and CNA's have been really compassionate with me I am very appreciative of all that they do. I want especially thank my OT Denise that comes up with plans that improve my daily life. Transport Jeff is always on time. Lastly thank you to Riley and Chris helping me with my TV all the time.

Fantastic experience: This place has apart in saving my life. When I had given up on fighting to live the atmosphere the nurses, CNAs and other staff members at Monument Health brought hope back into my life and that little bit of hope caused me to fight even harder to live and I'm forever in debt to them. Fantastic experience: The staff here are great, they're not just caregivers, but also friends to the residents and their families. I often see the nurses, their onsite social worker, the recreation department and the rest of the management team going above and beyond, making a difficult time a little more bearable by helping the residents feel seen, heard, and loved.

Fantastic experience: My name is Bruce and I am a resident here at Monument Health South Salt Lake. I would like to share one of my experiences that I went through here at this Care facility. I am a type 2 diabetic and for the longest time my diabetes has been out of control I've always had high glucose levels which unfortunately led to the amputation of my right leg below the knee. To make a long story short I ended up here at Monument Health South Salt Lake because I had an infection in my residual limb. The nurses here got my diabetes someone under control the best they could because of how long I left it neglected. I had a low glucose moment it was pretty low I couldn't lift my arms to feed myself pudding to raise my glucose levels Angela one of the CNAs here actually kind of cradled my head and spoon fed me pudding and helped with some orange juice to raise my glucose levels and which rate it did. That is just one of the moments that Monument Health South Salt Lake has I 100% believe saved my life. There is no way I can do this by myself I am grateful and thankful to all the employees here from Chaz, The Administrator Melonie one of the fine nurses that see over the residents that live here and the friendly night receptionist.

Negative experience: This place is horrible it's like being on an alien planet and a prison complex. They have put the lights at so bright in the hallways in the bedrooms in the recreation room the lights are so bright you have to wear dark sunglasses because you feel like you're being interrogated by aliens The Food is filth the food is the is worse than a high school cafeteria food the food is disgusting and filthy. And the staff here treats you like garbage because they are overworked and they're not happy to work here you are treated like a piece of Filth by the staff you don't want to be here to be sure to like garbage by the people who are supposed to help you

Negative experience: This place is a joke. I am a resident currently and there are so many issues. I am diabetic and it is not diabetic friendly at all. I am basically managing my own diabetes. They need to provide insulin to fill up my pump, and they don't until hours or even days later. I keep snacks in my room to get sugar in me when im having a low blood glucose episode because if i waited around for them i would probably die, theyre so slow. Their hallways smell. Housekeepers clean your room however detailed they please to. We can buy food for them to keep refrigerated for us, but when we ask for it it disappears, and residents don't have access to those fridges. Only staff does. Many residents don't eat the food. It lacks flavor and it rarely tastes alright. My roommate needs help showering and when she asks for help, many of the staff is rude, and hurry her. Luckily I don't need help, just towels since I don't have my own. But I need to ask for them a long time in advance. I've had a light that needs it fixed by maintenance for at least two months, and keep reminding them, but till this day nothing has been done. My family takes care of my laundry since they lose laundry here all the time. They throw away bottles of shampoo and other things if they're almost empty, but maybe you still had a few uses out of it. I've had things stolen or go missing, and so has my roommate. Most of the staff is trying, you can tell. But even they say that the company only wants a profit. It is said that they are being bought very recently, and I hope they make needed improvement.

Positive experience: Facility is a little bit run down and definitely needs some new paint, but I just finished my first round of nursing Clincal here. And it’s safe to say I learned these nurses and CNAs do so much I myself have been a cna for 10 years. It reminded me of how hard and over whelming long term care is I did it for 6 years and I forgot how much you do for someone (I could never go back I love working In the emergency department to much now) but i appreciated Quinn RN for taking me under his wing. And Kaylee the med tech she was super helpful! Overall a great experience ! An would recommend schools to have their clinical site here for nursing and CNA students

Negative experience: Sandstone of south salt lake abuses their patients! I was driving home this evening and saw an elderly man laying on the ground. His walker was off to the side. I pulled up and asked him if he was okay and he told me he fell and was trying to get to the white car. He seemed very altered, and I asked where he lived and he told me a room number is all. He was only oriented x2 knowing his name and his room number is all. The sandstone facility was over a mile down the road. When I got the man back to the facility and contacted them, the facility had no idea he was missing, and told me he was supposed to be in locked down memory care. Each employee there was playing on their cell phones, had earbuds in, and did not care that their residents were outside not being monitored. The nurse on shift was not paying attention and was listening to music through her earbuds. When I asked for a supervisor, they were quick to yell at me and shrug me off. This facility was so run down, and no one seemed to give a care about the patients there. This elderly man needs to be seen in a hospital, and the nurse told me “he’s fine. I’m in charge now, and I have it handled.” He was clammy and feverish, and seems as though he hurt his hip when falling. I realize I should have just called police and taken him to the hospital, but I was acting out of panic seeing him laying on the ground at 10pm in south salt lake. I will be reporting this to Utah adult protective services elderly abuse, and contacting the managers of this facility. Do NOT send your loved ones here:

Negative experience: You will find that the facility had employees post their reviews to up thier scores online. DO NOT FALL FOR IT!!! Not a single review posted from a client or family has been good. I was placed here due to a nurse care manager from a hospital that didn't know anything about the facility. This is one of the worst facilities I have ever been in. Medication is not given on time, they will let you sit it your room or bathroom for long periods of time before ever answering a call light. When a call light is answered, most staff act like they are put out having to answer the call light at all. There were many times while receiving my treatments that after the alarm went off on a machine that it was just ignored. On times that I was able to get around without having anything hooked up to me that I find multiple staff members sitting at the nursing station refusing to get up and answer any lights unless they felt like it. I had to be there where they were just to get Medication. The room I was in was freezing cold as so was the entire hall and other rooms. Even the staff did away how cold it was on the 500 hallway. Temperatures are so cold I personally couldn't sleep comfortably or without constant pain. The atmosphere and treatment of clients even caused me to be deeply depressed. The food is really bad with very few exceptions. Highly salty, sometimes too spicy, extremely high in carbohydrates and fats. Therapists are rude, inconsiderate and always think that they have the right to talk to you anyway they please. I have already had to contact my surgeon about the care and the lack of instructions followed through by him. Even after my surgeons follow up with the facility the care is still very substandard. I do not recommend that anyone place a loved one here. After working in facilities myself this is one of the worst.
Fantastic experience: Anyone who cares for my mom as long as your facility did before she passed deserves the best review. She was happy here, she made friends at Avalon and will miss all those who helped her make life a little better. I even started visiting every day before my mom passed, was able to eat lunch with her and feel comfortable spending time with my mom at your facility. Thank you, i will miss my daily visits and seeing all the kind faces there.
Fantastic experience: The staff is friendly and professional. They really looked out for my wife and tended to her wound dressings and treated her like a member of the family. The lady in charge of the place, I believe her name was Jasmine went above and beyond when my wife had problems with her wound vacuum. Now my wife is back on her feet again enjoying life. Thank you Avalon for being so kind to us and being there in our time of need.
Positive experience: It's a very nice facility, and treats it's residents with respect, caring and kindness, something rare in an extended care facility. It has very good food, with cloth tablecloths and napkins. I'm only 58 yrs. old and will be here only a few weeks, but I'm very impressed with the doctors and staff in addition to the place itself!!
Negative experience: They almost killed my father by accidentally giving him someone else's medication and the staff stole the few thousand he had in cash and they continue to steal everything he owns. I can't buy him a phone or tablet because the next day, it's gone.

Negative experience: My mom just had open heart surgery, she diabetic, needs dialysis, and has an infection called C. Diff. Her room and everything around is supposed to be clean to control spread of infections. The wheelchair they gave my mom had several cigarette burns and had caked on food and crust from I don't know what... AND this was on her 1st day here at Avalon. She just got her oxygen removed a couple of days ago and they put her in a room where she can smell cigarette smoke from outside. She was admitted at 4pm yesterday and when I went to visit her at 7:30pm, she was still waiting for her food. THEY FORGOT ABOUT HER! This place is filthy and smells. IF YOU CARE ABOUT YOUR LOVED ONES, DON'T SEND THEM HERE!!! Update! This facility goes by Sandstone now. I wonder why.

Negative experience: Don’t leave your loved on here. I spent 2 weeks her one year during Christmas. I did not get the mds I needed, and because of it could not sleep after 1am. So I laid in cold room all by myself in the middle of the night. The head nurses were uncaring in a cold way. The aids were the nices people.

Fantastic experience: Avalon Valley rehabilitation is outstanding as we experienced their care for our son in long-term care. Their guidance and commitment is also thoughtful and compassionate. Nursing care is wonderful as well.

Fantastic experience: I've worked with this facility for over a year. If you or your loved one needs Skilled Nursing services, this is a gem in Salt Lake City! I've never seen a team work so hard and they do it because they care for each and every resident. They take such a variety of residents and they have done an amazing job. No facility is perfect, but when you bring up issues they will take you seriously and try their best to fix it. This place is AMAZING!!!
Negative experience: My husband rang the call button to get someone to take him to the restroom; he needed help to get out of the bed. No one came! After waiting 30 minutes, he could no longer wait. He went to the restroom in the bed. The nurses aide came in and he explained what happened, and then she left. Another nurses aide came in he told her and then she left. An hour later the nurse finally came in and she had one of the nurses aides come to clean him up. The aide was rough and my husband asked her to be gentle, as he had and infection and catheter; and she continued to be rough. This neglect and lack of care for the patient is unacceptable. THE OWNER'S RESPONSE IS A JOKE! I HAVE CONTACTED THE AVALON CORP. AND THE DIRECTOR, OF AVALON VALLEY. THE DIRECTOR AS OF 9/17/21 HAD NOT CONTACTED MY HUSBAND OR APOLOGIZED TO HIM! HOW CAN YOU DO AN INVESTIGATION OF AN INCIDENT WITHOUT TALKING TO THE VICTIM! This is NOT a place that cares about their patients, as proven by the LACK of response to my husband. 9/19 They tried to give my husband Metformin yesterday, and his medical records clearly state he is allergic to Metformin. Today they are trying to give him Jardiance which he doesn't take! What's up with this? Is it too much to expect that a skilled nursing facility gives the patient the correct medication?

Negative experience: DO NOT PUT YOUR LOVED ONES IN HERE!! I didn't want to give ANY stars to Avalon Valley Rehabilitation, 2472 S. 300 E., SLC, UT!!! My 82 yr. old father was there in July 2013 for antibiotic IV therapy after his hospitalization for sepsis due to a hip replacement infection. He is too old to survive another major surgery. Avalon's rigid routines are more important to them than patient care. Dad couldn't even get drinking water without ice! He wasn't allowed independent exercise (his habit). Encumbered by tubes and wires, he could not get up quickly enough to use the restroom when urgency arose. No one would clean his dentures for him necessitating soft foods, which were not normally served. No one applied/removed his compression stockings in a timely fashion for his heart-related edema. His Rx sleep medication necessitates IMMEDIATE sleep, as to wait has the opposite effect, making sleep difficult. Avalon insisted on giving this to him at 4:30pm (too much light, noise, etc) at the latest or after midnight, usu. 2-3am, due to staff shift changes. They insisted on watching him take it so he couldn't save it for later. He contracted C. diff (diarrhea & vomiting) yet Avalon continually denied this (& kept giving him stool softeners!) until, desperate, I called Dad's dr. His health was deteriorating (& depression was setting in) so we took him out of there, opting for a home health nurse & family care, as he lives alone. Avalon is in the business of warehousing/imprisoning people until their health ins. runs out. If patients improve, it is in spite of Avalon. I visited Dad daily to assist him w his needs. Pity the souls there who have no one to advocate for them :(

Negative experience: Do not trust Avalon valley rehabilitation center very disappointing my mother gets neglected anywhere’s from 10 minutes to 14, 20 hours at a time and the caregivers nearly fight each, she is not receiving her essential medicines people who work there need memory care I feel like reporting them, it is unacceptable shame on you Avalon valley rehabilitation

Fantastic experience: Jeff the administrator is awesome to work with. Did everything he could to get us here. Plus they let our dog come visit every day. Kathy the receptionist is always happy and has fresh cookies every morning. Mindy the Director of nursing helped us a lot with getting my husband here. Willow the business office helped us with the financing. Tristin in admissions helped us get all the information and helped get my husband here. The Cna's and nurses are awesome to work with. Very friendly staff. Everyone took very good care of us to the food and housekeeping. I would recommend this place to everyone and anyone.

Positive experience: Staff is nice and friendly. Nurse has always gotten my medications on time. Dinning rooms are always nice and friendly. Very cleaned as well. Dietary is always nice and friendly, and my meals are always the way I like them. Residents are always friendly. The atmosphere is amazing, everyone is like a family here.
Fantastic experience: I like the service, the nurses and cna's are always able to help me whenever I need them. Amy the nurse is the best at getting me the medications on time and is very friendly with me. Plating of the food is very neatly plated and looks beautiful. The staff is very friendly and amazing to be with. This place is like a huge family. The CNA are great with 1 on 1 care. Therapy department is amazing at what they do to get me back on my feet.

Fantastic experience: How do I locate a resident of one of the Utah based Avalon Health care facilities? My fiance larry e Jones was admitted to one of your facilities upon release from hospital but hospital only has corporate address listed as release information
Negative experience: Irresponsible with their patients. No information is told to families when it’s dire. No one told us our family member’s health was deteriorating. It took one nurse going under the table for us to get our family member to an actual hospital where it was too late for them. A week after leaving Avalon they passed away. Don’t trust this place with your loved ones.

Negative experience: They will give you the run around, leave your loved one feeling hopeless, schedule meetings and simply "forget" to show up. If their care is anything like admissions STAY AWAY FROM HERE. I feel sorry for anyone that has to resort to playing the game these people play with a total disregard for other peoples time & concern, mainly Tristan. I would give them no stars
Fantastic experience: The staff here is very friendly, keeps me informed on my Husbands progress in the day. The staff is really loving to my husband and to me. They try there best to take him out side to have a cigarettes. Tristin in admissions is very nice and friendly and outgoing. She always can help us with any issues or questions we have. I am so blessed for this care center to take care of my husband in times when I can't. The food is good. The receptionist picks up calls on the first ring and always get me to where I need to go.

Fantastic experience: My husband just left re-hab at Avalon Valley. We really love and appreciate and the people who saved his life and made it so he could come home again. As always, the food is never as good as home cooked to order. We love the Administrator and his wife, and the excellent staff who cared for him. Thank you to all for a great experience. We even gained a new family member, a kitty, from one of the agency CNAs Thanks Tina, Harley is doing great!

Negative experience: No stars would best apply to this place! Do not take your loved ones to this facility. Nurses Tina and Margaret are the worse. They will literally sit and argue with you on the phone if you call in to ask for an update. I have even filed a formal complaint after my fathers stay. Never will I let him come back to this facility. 20 minute wait for them to come and get my father out of the bathroom is unacceptable.
Negative experience: My husband was there for respite care for 5 days. He is on hospice care. He is on 8 liters of oxygen and his concentratior was kept getting turned down to 6. He is deaf and his call button wasn't working for at least 2 days, no-one checked to see why when he asked about it. He asked for his pain meds and was told no. He also uses a bipap at night and was only hooked up to it once. Would not recommend this facility.
Positive experience: I did my CNA clinicals here and though understaffed the majority of the workers were great. I read the reviews before clinicals, so I was super nervous, yet all the bad things said in the reviews wasn't true (from what I seen) besides the long wait to answer call lights, but remember they are understaffed with maybe 2 CNAs on each hall

Positive experience: I have been with Avalon for almost a year and the residents and staff are just wonderful. The staff are positive and easy to work with and environment in the facility has improved. You can most certainly tell that the staff care about the residents. Definitely happy to be apart of the Avalon team.
Positive experience: My grandmother has been staying here for the past 6 years. The staff are lovely to her and to all of the patients I have seen them interact with. I disagree with many of the reviews on here. From what I have seen over the past 6 years, the staff is friendly and the place has always been clean. The only reason I five 4/5 stars is because it can sometimes be difficult to get somebody on the phone.
Negative experience: Horrible place. They ignore the room call/ alarm lights for hours. Understaffed and inexperienced hires. Going here is a death sentence. Please respect your loved ones enough to find a proper care facility. We did my grandfather a great disservice when we allowed him to be transferred here. :(
Negative experience: I was there as a short term rehabilitation patient after a total knee replacement, needless to say I had an additional problem from pain medication, had a blockage for 6 days which required a additional surgery not related to the original reason I was there in the first place. The place smelled, food was ok, medical staff?? What medical staff, a nurse that hands out medications, never seen a doctor the whole 10 day's I was there, I wouldn't recommend this place to my worst enemy. I'll never use their services again.
Positive experience: I did my CNA clinicals here and though understaffed the majority of the workers were great. I read the reviews before clinicals, so I was super nervous, yet all the bad things said in the reviews wasn't true (from what I seen) besides the long wait to answer call lights, but remember they are understaffed with maybe 2 CNAs on each hall
Negative experience: Don't go here, or take your loved ones here. The food is horrible, they ignore the call lights/alarms for more than 30 minutes or more. The staff often fails to follow through with getting medication and other things (maybe because they are insufficiently staffed?). I worry anyone staying here will be neglected.
Negative experience: The first night my husband was there well no one ever came to talk with him. Second day they served breakfast on a dirty tray and food was half on plate and half on tray. They gave him no silverware to eat with. That night they forgot to serve my husband dinner. Next morning shower time. Went into the the closet to get towel and wash cloth. The wash cloth was dirty and had blood on it. That took the cake I took my husband out of there. This place is a bad place and I have all the pictures to prove what I say. I'm doing this yes for my husband and for all the people there who can't talk for them self. This is wrong. I seen the people press there bell to have a nurse to see if they need some thing and no one would go because they are under staff. My husband asked for a glass of orange juice they brought it but the cup was dirty so he didnt drink from it. They came and asked if he wanted more ice in his cup and he said yes then asked if he wanted water in it and he said yes so she went to the rest room to full it up in the sink in stead of a fountain. Guess if it was a emergency. I worked in a nursing homes and I know what has to be done to take care of sick people. My husband has a infection and I didn't need him to get another one. They want to make it up to my husband, but how do you make up the experience you showed him when he got there..
Negative experience: This place sucks. No one answers the phones or seems to care about the patients. I believe the people that work here at avalon are just there for a paycheck nothing else.
Negative experience: My wife did CNA clinicals here for 3 days. I can say, just from the stories she told me, this place is horrid. There are many patients with many bed sores, an indication the patients don't get adequate care. Also, there are patients with serious infectious diseases (MERSA) that should be kept confined, but instead these people were allowed to walk the halls freely, with no warning to the other residents or staff (or CNA 's there for training). There should be signs on the outside of the rooms of patients with these serious diseases to prompt people to don proper protective equipment, there are not, which happens to be a violation of state medical procedures. My wife was appalled at the overall lack of adherence to the procedures she spent the previous 3 weeks learning. I would not recommend this place to my worst enemy.
Negative experience: The very first day, no one in the facility answered the phone! I couldn't get ahold of my mother. A very dingy, outdated facility. Don't send your loved ones there.
Negative experience: My son was sent to Avalon by the university of utah hospital. For physical therapy after a serious car accident. He had been there for 6 days and never received physical therapy. So I spoke with administration, and was told that he had been receiving therapy. Which is not true I was with my son everyday upon arriving. Also he had a lot of problems, his osteo bag was always leaking he would ask for a nurse and would be told by staff, the nurse would be in shorty which would take at least half hour or longer. There under staffed and there services are really poor and would not recommend this facility. And admitted him to another facility.
Negative experience: People die prematurely at this facility. Pharmacy is routinely late by many hours. Supply and maintenance are rarely available on evenings and weekends. Patients complain of diarrhea after eating the food. Numerous hazards confront patients due to poor staffing. Doctors rarely evaluate patients or respond to urgent medical needs. Management increasingly assigns administrative tasks to the nurses and is more interested in playing the blame game than in solving problems.
Negative experience: This place sucks for anybody care for your elders
