Fantastic experience: I love this place especially for my dad who is 91 years old. His name is George Padilla and I love him with all my heart and soul. I especially love certain people that work there my number one favorite is named Fred. He’s a CNA and he’s awesome. He’s also tall dark and handsome and I love the way he treats my dad. Then there’s J a lovely lady that works in front when you first walk in she’s awesome and I appreciate anything she does for me. There’s also Adriana S. Who also is very awesome. She’s a very strong young lady. I say that because she’s able to pick my dad up and put him in his bed. He’s got to weigh at least 115 pounds. There’s a lot of people there that I really really appreciate God bless all of you Always.❤️❤️❤️❤️❤️❤️❤️ Lorraine Gonzales
Negative experience: This place was awful I just like peace and quiet. And to be helped in a reasonable Amount of time . Very short staffed but that is ubiquitous The food was OK most of the staff was good good Gd

Positive experience: Today they had a picnic out on the patio. Food was good and great music and entertainment. Staff was awesome. It was hosted by a real sweetheart “JAN”!!! ❤️
Fantastic experience: I came from the hospital. What a difference. The food was great. The staff especially the nurses were outstanding. The ladies that maintained the room were delightful. The patio picnic was awesome.

Fantastic experience: They work very hard and do all within there abilities to take cars of their patients.. even when disrespected they are very professional and get their job done... much respect to the workers... it's not an easy job but it's done with care and love... continue great work
Fantastic experience: Can I start off with sayin I read all the comments an all places have there problems n I placed my family member here at valley an can I say when they say, don’t judge a book by its cover . It’s so true cuz my family member was there for therapy n they did so good with him it was hard at first but they very gave him encouragement to keep moving I was so happy to see that so thank y’all at valley.
Negative experience: DO NOT SEND YOUR LOVED ONES HERE UNLESS YOU WANT THEM TO DIE OR BE MISTREATED, ABUSED. This place deserves negative stars. Valley Healthcare Center is the WORST nursing, rehab home I have ever visited. My father was transferred here the late night of February 20th, on tbe 21st we showed up at 9am visiting hour's. He was here less than nine hours, when we walked in his three week old colostomy bag had NOT been emptied at all, his bile was all over his open incision bandages on the opposite side of the colostomy bag. There was no damn sense of urgency in any damn RN, Wound care nurse, CNA, or LVN in this place. Let me add he was just discharged from a hospital after recovering from Sepsis Shock. The DR they have on staff came in and changed orders on how his wound was to be treated, which ended up being something that caused more issues. My mother was granted permission to stay the night on the 21st. The CNA on duty told her "we don't empty the colostomy bags, you need an RN and they leave at 10pm" so no wonder he was covered in bile the next morning. Every hospital we've been. In has been a CNA's responsibility to empty the colostomy bag. On the 22nd he was rushed to emergency room, where we were told the type of wound dressing their medical dr had switched him to is like a bleach solution and while yes it kills Ecoli or bad bacteria its also an old technique and much more harmful than beneficial to the abdominal open wound. He will never return to this place. His roommate I felt so bad for. He had a dr appointment which this place does organize and transport its patients to. The gentleman was back around noon, the CNA Connie left him in the uncomfortable transport chair longer than he needed to be. She said "I have to feed people" food trays had already been passed out. About thirty minutes later he asks again. And again she says she has something else to do. When I stepped out the room to see if I could find someone to help the patient, I saw Connie sitting on a chair on her phone. The gentleman sat in the transport chair for one and half hours after returning from his doctor visit. You constantly see the staff just standing around talking to one another about personal stuff, sitting on chairs placed randomly through the halls on their personal cell phones. Seems like this is where patients are sent to just die and be forgotten about. We are now in a worse situation with my father, this place did not help us. The staff is not knowledgeable, dont give a damn about anyone. Even heard them make comments on the 21st while they were in and out of the room cleaning and changing his wound and colostomy"we're having to earn our keep today" also the wound care nurse stuck her finger in his stoma to "clean" it. His stoma collapsed, hence the need for the emergency trip the very next day. Now we are in the hospital and he is fighting for his life. All the sorry statements, or we just started our shift, we didn't know, can't replace a loved one or make the situation say any better. He was here for less than 48 hour's. This place needs to be shut down. I have much more I could say about this place and the horrible employees but you should get the jest of how horrible of a place this is and try to avoid this place at all costs.
Fantastic experience: Want to give a BIG SHOUT-OUT TO GLENDA❤️ !!! She was amazing taking care of my mom. Also, nate, Tanisha, Sam have all been amazing along with a few other nurses the place is very clean and the CNA are very patient Joel at physical therapy was amazing also I couldn't ask for a better nursing home thank you all

Fantastic experience: When I was a patient here oh what fun I had in activities and I love all the office workers and some of the nurses and cna even love when Brad throw stuff for the patients and I was name queen of valley plus I was puck as vice president , though Irma Mitchell no longer here she was a awesome activity worker along with Rini and now passion is awesome, and all the physical therapy worker work well with me Be here tomorrow either, if I had to have rehap again I’ll choose valley again ❤️❤️❤️❤️❤️❤️and my friend Ms j is a awesome reception and Mrs kerrysha is a hard worker ❤️
Fantastic experience: A HUGE shoutout & thank you to Ms. Jay & Melinda. Todays family visit with my mom was awesome. Jay had the room beautifully set up & Melinda had my mom ready and waiting for us. These moments are always so meaningful and precious and I’m an so incredibly grateful for the two of them making it happen. Seeing my mom smile makes my heart smile❤️

Fantastic experience: The facility is incredibly clean, and all of the nurses are very friendly. The housekeeping completes any cleaning jobs in a timely manner. And the lady at the front desk always greets with a smile
Fantastic experience: I am currently an LVN student and attend clinicals at this facility and the staff and patients are awesome! They nurses actually allow you to learn and we love that we get the chance to do wound care for T and coco. Jay is a wonderful first impression when coming into the facility. The patients are always grateful for anything we do to help them. This clinical site has been very helpful in my learning experience in becoming an LVN.
Fantastic experience: I absolutely loved my experience here at valley! All the nurses, CNA’s, therapists work so hard to make sure the patients here are taken care of. I would also love to mention jay (front desk), she always lights up my days!!
Fantastic experience: The staff at Valley Healthcare Center are so kind and provide exceptional care for my uncle. Recently we had a birthday party for my uncle at Valley; they reserved a really nice large room and Jae even helped decorate before the party. They made the effort to ensure my uncle was well groomed and was dressed appropriately. They also provided coffee, water, and punch. They went way beyond what was necessary and were so cooperative. They are a dedicated group of caring professionals.
Fantastic experience: I had my grandmother here n it was the best place for her at the time she had got there n it took a little time for her to adjust, but luckily she had a roommate that made her feel safe n at home an I’m glad she had that thank you valley

Fantastic experience: My dad (Julio Lavin) was sent here from what others would consider a 5 star facility that didn't bath, walk or even think to do physical therapy after having a massive stroke resulting in losing all movement from neck down to feet. After arriving here he received daily physical therapy that tested his limits as time went on, he received food that was necessary for diet restriction due to blood pressure, and found great friends in nurses and other patients. He left after 5mnths here and has regained total control of his body. I just want to thank the staff, therpists, nurses, management for all the hard and effortless work you put in to bring my dad back to himself, and especially the sweet lady in the front that consoled me during hard times at visits. You guys are now family, you're awesome and I'd recommend 10/10 sending family here to get treated.
Negative experience: Unbelievable my mom is a current patient there the staff is rude . Very disrespectful I went this weekend to visit my mom and she was sitting in feces for over 3 to 4 hours we didn’t even know who her nurse was finally we get a nurse to Come in only to be told that she’s not her nurse and he’s on lunch . Well she’s covering for him why didn’t she clean her up knowing she had visitors I’ve worked in the health care field for years and I know how it is . Don’t send your love ones here they will be treated badly . She does have incredible other nurses there than I love but 3-11 need to get there at together.
Negative experience: If I could give zero stars I would. The main problem was that the staff neglected to provide critical medication designed to prevent seizures, among other things. This was after multiple forms and attempts to communicate this to them (phone, txt, email, in person, etc.) We saw other patients on the floor and when we brought it to the staff's attention, found that it was ignored until about the 3rd staff that was informed. Our family member was found soiled nearly every time we went to visit and he would frequently look neglected (he was frequently hungry/thirsty, incorrect diet, fingernails needed trimming, overgrown beard, specific smells, etc.) The list goes on. If you don't want your family member to last long, this is a good place to bring them to. We got him out of there as fast as we could.

I've been there four times for Rehab on my knees the staff were good to me
Negative experience: My first night there I took my current medicines. However they took it upon themselves to ignore those and go by the one list they had access to… about 2 years ago ,. But due to my hearing aid not working correctly, I could not understand which meds I was / was not being given. I had to wait till I got home to discover they had taken me off half my meds! Without my permission! Also few people on this staff wear masks. My roommate said I had just missed Covid.! Yet, some people when I would ask them to put on a mask, they would just leave the room and not come back.. There are a few good people who work there. The physical therapists, occupational therapist, nurse Isabelle, cna gina, and a few of the housekeeping staff. ,
Fantastic experience: I am writing this review about Valley Healthcare Center because it is well deserved!!! There are SO many good people who work at this facility. From the administration team (Brad, Ben, Jaime, Kelly, Ramona and her beautiful daughter Jay) to the nurses and supporting staff. I have been very pleased with how they took care of my loved one! Any situation that needed to be handled... it was HANDLED! It's always clean and inviting when I visited. I didn't have to worry about my loved one because he shared satisfied comments about nurses and staff who took good care of him! Jay at the front desk is SO caring and kind. She remembered me each time I visited and checked in on my loved one to be sure he was feeling well and if he needed anything. I would recommend this center to any family who made need assistance with a loved one!! Thank you to all at Valley Healthcare Center for AMAZING care!!!

Fantastic experience: My mom was a patient at Valley Healthcare for 3 weeks while convalescing from a respiratory issue. She has stayed in a few over the years and she couldn't say enough great things about how they treated her here. The staff was nice and took very good care of her needs. We would recommend this place to other family members.
Negative experience: If I could give negative stars I definitely would. The smell of urine slaps you in the face as you walk in. They try to mask it with freshener instead of keeping things clean. The head nurse is very rude and unprofessional. The sheets were dirty and had someone's crumbs on them. Totally disgusting. The lady was yelling for butt cream non stop. All the head nurse said was it's shift change at 11:30pm ish hard to believe. Needless to say after 10 minutes of disgust I took my friend that's in her 80s home. I would not with good conscience leave anyone at that place. This place should be shut down. It's horrible how people are treated.
Negative experience: Needs improvements in care, supervision, and training the CNAS as well as implementation and retention of good staff. As most nursing homes some have there strengths and weaknesses that need to be looked at for ongoing overall better care and retention of keeping good employess!!!!
Negative experience: My grandmother is living there and the clerks will NOT ANSWER THE PHONE . Please dnt place your grandmother or father or elder family here .
Positive experience: 5 star facility. Not as bad as other places ive been. The LVNs are pretty reasonable doe the most part. if you accept employment there it is at your own risk. My car has been broken into twice. So if you are willing to risk paying for vandalism then Valley Healthcare wouldnt be a bad place to be.

Negative experience: Please do yourself and your loved one a favor and DO NOT send them to this place. I can give you numerous reasons why, but let's cut to the big one. My mother contracted Covid 19 from this place and is now hospitalized and in very bad shape. They supposedly tested her the day before she was released. No one ever called with any kind of results, good or bad. And now my life is in danger as well. This place will keep people for as long as the insurance will pay, then out they go. Seriously, my mother will probably die now, and my future...
Negative experience: Had to discover that current Primary Dr. Is next door where husband (VETERAN) PASSED AWAY. Was not impressed of Valley Healthcare in San Bernardino, Ca. Appearance is unhealthy. Don't know why Insurance Company send me where husband passed away without knowing about what to find out. Primary Dr. may know about it. His 2nd wife send him there. Was never informed about his location neither about cause of death. Veteran Administration stated: I don't exist in their system!!Ugh. Most people aren't aware to ✔ the senior Rest Home RATINGS! ALWAYS CHECK THE 4 OR THE 5 RATINGS! 1 OR 2 RATINGS STAY CLEAR!
Negative experience: So far my husband can't stand it , he wants transfered out of there but theres no availabilities anywhere. Im going down there now to see why he has fallin again . The rn i talked to on the phone said he went to his knees , but they still consider it a fall . I asked her why he was moving his self from bed to wheel chair , and he has a fall risk band on , she said if he dont call us we dont know , but thats the thing he does call and NO BODY COMES ...

Negative experience: The only reason they get 1 star is because it won't let me leave a comment if I dont. RUN THE OPPOSITE DIRECTION FROM THIS FACILITY!!!! This place is disgusting from the moment you walk in it smells of urin and poop. The facility is dirty and run down. My father is there with 2 broken legs and they placed him in a bed that is BROKEN and has no way of moving or repositioning himself, I'm afraid he'll get bed soars, since Staff doesn't come around to help him reposition, Staff dose not tent to patients individual needs at all. The staff is very unorganized, sloppy looking and doesn't seem to know what's going on.Staff does not answer call button when patient presses it. Staff isnt very friendly seems cold towards patient's and rude to families. My father had to call me from his cell phone after i had just left from visiting so I could call the nursing desk cause they wern't answer call button to let me know they only gave him a little bowl of soggy warmed up veggies for dinner and that he was hungry but no one would help him, he's also diabetic and needs to eat but they failed to realize that. Do not have a pharmacy on sight so patients don't always receive important meds. When needed. I fell like my father is either going to get hurt worse there or get sick on top of what his current condition is because of how things are there. He's only there for temp. Stay we are talking to his social worker to let her know the condition there to get him out asap, but I felt so uneasy, unsafe and just bad leaving him there until we can get him moved. DO NOT SEND LOVED ONES HERE!!!!!
Fantastic experience: My Two Bestest of friends had the pleasure of healing and retraining along with rehabilitation at Valley. I would trust the staff with their care, if my 2 friends needed to ever return for any other reasons, including care for myself also!

Fantastic experience: The staff has a good heart for the patients and they have a good work ethic. Things are not perfect but the staff does follow through with addressing concerns from families and patients.
Fantastic experience: Facility is calm and the hallways smell good actually! Staff is nice and helpful. Therapy team is very good and the long term residents seem to really enjoy the activities programs.

Negative experience: run, don't walk to another facility....*if* you can ! The facility BARELY functions at all. THREE hours to "find" the intake paperwork from the Hospital that we HAND DELIVERED to them when we got there !!! Patient NOT IN THE SYSTEM after a conformation call 24hrs ahead.... therefore NO PAIN MEDS AUTHORIZED for patient.... avoid it !
Negative experience: Very interestin reviews! Husband (a VETERAN) was placed at 1680 N Waterman Ave., san Bernardino, Ca where he appears to have died at 15:45 p.m.or a.m. Now I'm under observation as the Cancer Survivor with NP further observation. My primary Dr. is also on Waterman Ave. but on the E Waterman Ave. Moved from LA County this year 2017. Strange.

Negative experience: I didn't like anything about this place, the nurses were incredibly unprofessional and their care was very poor towards the parients, (Nurses like the ones I encountered exist everywhere, unfortunately, but since I experienced this first hand, I have the right to say something) also, the patients rights were often ignored. Not good, not good at all.
