Haven Post Acute

Positive experience: My mom was here for 2 weeks for rehab post lumbar fusion surgery and we were very pleasantly surprised given the area. Although the intake process was rough because it was the weekend and staff were limited, the rest of my mom’s stay was very accommodating and met our needs. The facility itself was clean and well kept as far as nursing homes go, no bugs and the smell was relatively pleasant. Their PT/ OT team, Isaiah and Leticia, were beyond amazing. We loved every moment with them as they treated my mom with love and care throughout her stay. Their admin team, including Daisy, Theresa, Maria, and Reuben went above and beyond to get my mom’s care covered through insurance and helped us feel supported every step of the way. The nursing staff are truly the backbone of everything, they were so attentive to my mom and I, I just couldn’t have done it all without them. Jon and Florence were the nurses that stood out to me, but my mom had a whole list of her favorites there who she adored. We are so grateful for everyone there for putting up with our many questions and concerns. As a pharmacist who has worked in longterm care myself, I was truly impressed by how compassionate and attentive everyone was there. I couldn’t have done it without you all. Thank you

Fantastic experience: Thank you for the great service wishes that I could remember everyone's name but I can't but everyone was wonderful. The wound care team was very gentle and helpful. The therapy team was amazing.

Fantastic experience: I would like to thank the Social Worker Director and everyone there at Haven Post Acute. They extremely went out of their way to acomadate my friend Ruben Saldana.

Positive experience: I absolutely appreciated my significant others nurses! However he was in the open air patio with other patients and only one attendant for everyone there. He needed a walker to walk with assistance or a wheelchair, unfortunately he apparently got up and walked to the dry fountain he got one foot in the fountain before he fell (backwards) hit his head. Because of this event he had two large lumps on his head one in the front and one in the back. He was transferred to a hospital had to have a tube to drain the lump had another stroke, and passed away. I am not saying his death was necessarily their fault, however the day of the accident I was with him and I was flashing him cards he could tell me the numbers and suites on the cards. In my opinion he was showing great progress. The accident happened and he passed! I miss him so very much! He passed on June 9th 2023 and I miss him so very much

Fantastic experience: Very nice and clean, all staff was helpful, nice and very attentive to needs of my mother in law. Highly recommend.

Negative experience: 90% of the staff who work here is kind & nice but just not competent, distractable & badly understaffed. I was given the wrong medicine , I personally reported to maintainance that the toilet was stopped up but it wasn't fixed until they came into my room for something else the next day & I reminded him of my toilet which was now more stopped up than ever, it took 3 HOURS to get some pain medicine, the nurses didn't understand the doctor's orders & it took 3 HOURS for the nurse to call an ambulance to take me to the ER for my incapacitating headache after brain surgery to remove a tennis ball sized tumor. These are just a few of the horrors I experienced there. I was undermedicated & was supposed to be referred to a pain doc which never happened. I saw my primary doc one time in 3 weeks. I was assigned another Dr for my last 4 days there & never saw him. My insurance company, Scan, would only pay for the cheapest place. It is NOT a rehab facility, it's a nursing home. I NEVER should have been sent there!

Negative experience: Mom was placed in this facility for rehab. While she was there she fell out of the bed. There are no rails on any of the beds. I repeat NO RAILS ON THE BEDS! So they lower the bed to near floor level, then they put work out mats for a softer landing the next time. This, of course, puts water out of reach, so the solution to that when she got dehydrated was an IV, which Mom wouldn't tolerate. The place is older and wasn't very clean and often smelled like urine. The staff was helpful, with a little push, but more than happy to ignore you. They lost her hearing aids right away and when we replaced them that pair ended up being misplaced for about a week only showing up when I requested them to pay for replacements (If you end up stuck here, be sure to make a list with the nurse of everything your loved one has). We were frantically trying to arrange to move her, but she passed before we were able to.

Positive experience: I am not sure about all the negative comments towards Teresa and the social worker department. The team introduced their selves in a professional manner, put a plan together for my mom and covered all information clearly and concisely.

Fantastic experience: It’s a clean facility, cute court yard. They have a day room that can be used for private family gatherings. I Would suggest touring in person

Negative experience: My father was placed at this horror movie, mental hospital for physical therapy. They lost his street clothes even though I was told they made it with him from the hospital he was transported from. He fell the first nite, no one called me til morning! They were also giving him adavan and norco. Couldn't talk to him on Friday because he was "asleep" everytime I called. When I finally spoke with him, he was so out of it and kept falling asleep! The nite nurse, you know who you are, lied and was rude telling me that "maybe he was given the adavan so he wouldn't get out if bed!!" Seriously!? COULDN'T tell me what was administered. I was completely horrified when I walked in there today. My father was in a diaper!!! Any time he moved he would scream in pain!!! I immediately signed him out AMA (his primary doctor recommended I do this) and i took him immediately back to the er in the hospital he had left a few days prior. The staff at Haven that assisted me today were wonderful, I have to say. . The PT guy and the sweet nurse that helped me get him in the car, and the ladies at the reception that helped me to finalize the paperwork quickly.. Please heed my warnings, do NOT leave your loved ones here. They may not make it out!

Negative experience: Do not send your family members here. This place is terrible.

Fantastic experience: I have had a wonderful Therapist I wouldn't be where I am today without 1 person that helped me A great deal I would recommend Paris for anyone that needs theray. If it wasn't for the push I would be laying in a bed all day. I recommend this facility to anyone who wants to get back on there feet. The food is grate, and the nurses are attentive. Fantastic experience: I'm Victoria I had a stroke cane here 1/2 paralyzed scared-confused -worried nurse's are so understanding caring most do a great job my nurse's do any way but my therapist OMG they didn't let me feel sorry for myself or I keeped saying I can't do it my therapist let me know it's going to b okay every time I said I can't they let me know I could such caring understanding they worked hard with me but not just that talked with me let me know why my body hurt or what part need the most help that day always asking what I think I need or how I'm feeling it's not just work for them I'm a person in need of help and they are here for me really pushing me when I say I can't look at me now I feel better with myself walking-moving I might not caring about life again I'm not going to ever fèl sorry for my self they let me know really can do this thank you so much for really being here for me my family and friends thank all of you so much and let's not for get the the Activity workers they do so much to help your stay here always making sure I'm okay they work so hard helping me get my mind off all that's going on with me even if it is just a short time the art talking being understanding always coming up with new things bingo TV art and crafts yes they do get my mind off this what's going on with me thank you

Negative experience: It’s more like a 51/50 ward, the place is very unclean. There was two staff members that were actually nice. My fathers feeding tube was on the floor nurse was going to connect it. I told her to stop and she needed to sanitize that the social worker Teresa could care less she’s very unprofessional. After my father passed I tried to get a hold of them because I was getting billed, and I wanted somebody to explain to how or why. After a week I finally talk to her (Teresa )and she was extremely rude and even hung up. I’m sorry, but you’re a social worker. Your job is to help people not to be rude not to be condescending or not to be talking down to people. Shame on you. When I finally got my father into my home on hospice care he had what was called sludged on his right foot basically looking like maggots he had bedsores and he was extremely dirty. That tells me the quality of care that place gives their residence.

Fantastic experience: The food was well balanced healthy and excellent, all the nurses were good and caring the rec room girls were all very helpful to the patients no problems, the only thing that bothered me was the disorganization of the timing of food delivery, but not too bad most of the time, I injoyed the stay

Fantastic experience: Staff are amazing, my dad was not mobile after being at Community Hospital of San Bernardino for 7 days. 2 days at Haven post my dad was up and walking around and is doing much better. Food is not all that but other than that I really appreciate all the staff and their hard work.

Negative experience: Do not send anyone that you love or remotely care about here. They don’t know what they’re doing. When they’re wrong they will lie to you. My father who is a healthy 91-year-old, got Covid and had to go to one of two places. I had no reviews on either of them and made the grave error of choosing this place. It has been eight days of hell. He fell three different times. Had to call us twice on two different falls for us to call the nurse because no one would come to his aid. So he called us on his cell phone and we had to call the nurses desk to help him. The third time he fell he was on the floor in the middle the night for over two hours until he dragged himself to the bathroom and pulled the emergency help cord. The first day they didn’t feed him. Because he had Covid even though he was on day eight when he arrived they treated him as if he had the plague. But they were supposed to take care of them. No one gave him any physical therapy. So his legs became weak. We’ve had to go every single day and harp on them. Mind you we live in Joshua tree. That is about an hour and a half to 2 Hour Dr. one way. We’ve been trying to get them out since day one. Today they transported him by ambulance to San Bernardino hospital because his stomach was distended. Pray to God he will be OK. Do not, ever send anyone to this place. We will be writing the medical board of California, we will be writing Medicare, we will be writing Eisenhower hospital that use them as a referral, and we will be writing the administration of Haven postacute as to the absolute horrific care they gave my father. Angry gorillas would have taken care of him better than they could. The fact that they lie constantly is absolutely unbelievable. They’ll tell you one thing and then they don’t do it. Awful awful awful place. I guess if you wanted to send someone to hell while they were still on earth this would be a good choice. Thank God my father had My husband and myself as his advocates. God forbid somebody who gets checked in there that has no one.

Negative experience: I'm an "Inmate" at this facility. There just so many injustices here it would take hours to explain. But anything you ever thought could be wrong with a nursing home, it happens here. I'm trying to get transfered. Wish me luck. Those positive reviews have to be fake.That is the total opposite of my experience. The nurses are lazy vindictive and hateful. A lot of them come to this place and hide them spend the rest of their shift on their cell phones. If you like misery, come here.

Negative experience: I put my father in there care from Fresno ca. to San bernardino they stop me and my father from calling each other. the staff will talk back to you over the phone. and threaten you with them calling adult protective services if my father was to leave and he was able to leave voluntarily like they said to us. they took the phone out the room and every time my dad tries to walk outside he says they give him a shot. they have been violating his civil rights and paitents rights.

Negative experience: All reviews are fake they posted on IG that they would give 15$ Starbucks gift card to everyone that reviews this with 5 stars they are a scam

Fantastic experience: Very sweet, knowledgeable, and helpful staff. This place was clean when I visited. All CNAs and staff went above and beyond to make my grandma feel at home. Would recommend 10/10

Negative experience: Absolutely awful, my mom is currently trapped here, and is receiving piss poor care. Nobody there will ever pick up the goddamn phone.

Negative experience: I have my family member that has been wanting to leave and they keep avoiding his request, he is Paraplegic. Today I came to find out what happening as his health deteriorated physically and mentally. This man came to the door kicked me out told me that how do I plan to take him when he cannot walk, I replied that they are supposed to escort him in a wheelchair anyway the staff unprofessional they cannot force someone to stay there at there will, especially if they want to go somewhere else for medical help.

Negative experience: The place smells bad all the time. Like URINE. They'd leave my bedridden family member in his wheelchair for long periods of time after he'd get a visit. It was very painful for him to sit in the chair for more than a few minutes. They kept saying someone would be in to help but took over an hour. They'd leave the patients urine soaked for hours regularly. They don't care.

Negative experience: They treated my grandmother horribly. She only stayes 6days and only that long because it was a holiday weekend. They also lost her things. Please look elsewhere to keep your love one

Negative experience: Horrible place to have a loved one at…. All they care about is getting paid there’s not one ounce of humanitarian in here . They had a trash bag instead of a bed pad ! Cheap ,ghetto ,rude ,loud would not recommend this place to at all.

Fantastic experience: Our beloved mother spent her last 2 months at Country Villa. Naturally we were very scared to send her to Country Villa or any other nursing home. I would visit nightly after 8pm. What my family and I saw was they were very compassionate with mom and with us. I would love to give names of all the amazing RN's LVN's and CNA's but they were all so very caring and compassionate. Thank you, thank everyone of you.

Fantastic experience: I love the cute courtyard and sun room! For my Uncle. During Covid-19 we can’t visit but the staff made arrangements for my aunt to see her husband.. the tears were flowing but it’s nice that people try to accommodate especially in these crazy times. Super thankful and he should be going home soon bc his strength is up from rehab.

Negative experience: Patient refused request to go to hospital for leg infection and possible seperated. kneecap and/or broken bones in his only remaining leg. As Power of Attorney

Negative experience: Family member was there and passed away they did not want to give us his personal belongings claiming that he got there only with his clothes after we called the hospital where he was transported from and confirmed he got there with his things, 6 minutes later they found everything except his wallet this place should not be in business !!

Negative experience: I'm only giving them one star because most of the Nurses and MA's were nice and helpful. As far as the Director and other people that run the place "HORRIBLE HORRIBLE HORRIBLE" is all I can say. My mom was there for 20 days, not by my choice. There were no other facilities available for her with an empty bed local to her dialysis. The case worker from her insurance told me if I remove her and take her home her insurance would not cover her hospital stay. My mom has several health issues: Diabetes, Dialysis, weak heart, gut etc. I was at the rehabilitation before they brought mom there from the hospital, I information them of all her issues and it being vital to follow her diet: no salt, no sugar, no carbs, no dairy? fluid intake being 24 oz. per a 24 hour period. Nobody followed anything!! They gave her what everyone else got: applesauce, apple juice, fruit, peanut butter and jelly, cheese sandwiche in the evening for a snack, milk, tea, apple juice for breakfast. One night she opened her dinner plate it was a banana, a severing strawberries and a severing of pineapple!!! DINNER!! I had a meeting with everyone: Director, her nurse, dietitian and even after that no change. My mom was there two weeks and nobody gave her a sponge bath as asked. My mom gained 35lbs of liquid being there and I had to take her out in a wheelchair. They didn't pick her up for dialysis several times and they didnt pick her up from dialysis at times, they dropped the ball too too much. Everyone always Blames someone else. There is too much too include in this review. If you want good care for your loved one, please look elsewhere.

Negative experience: This place is nasty is int even worth that one star I put they don't even do their job right I had my grandma in their and a man was yelling for help for a whole hour and nobody did nothing and they didnt can the state should close it down...!

Negative experience: This place is the reason that me and my family are now soon loosing my grandma because they failed to care for her after her surgery and caused a bed sore that got badly infected. Don't send your loved ones if you don't want to loose them to.

Negative experience: Fist time visiting someone at Country Villa Hacienda Health Center in San Berdo was horrible. Walking through doors I instantly noticed how dirty the place was. Also reeked of urine throughout the entire building. Lazy employees. November 5, 2018 , my dad finally comes home after stroke last February. Very sad place to visit. Too many staff and not enough help. It stinks and very dirty. Place in Corona was just as bad. Clothes were stolen along with family photos. Mentally getting worse as if he was not taking medication. And would not give us any information regarding doctor name or number.

Negative experience: I would not recommend this place. We are trying to get him out as soon as we can. He is ignored and keeps being transferred. His clothes keep ending up missing as well as the liquids we bring for him and we tell the nurses and they don't seem to care. There is one doctor who visits all the patients. He is a nice man but they need more. There are loud persons who are there who make it even worse for the others. More than once he has been given a bed that doesn't work. There are some caring nurses there but there are others who should be reported.

Negative experience: The staff was okay to my mom but let her roommate holler at all hours. Food was okay apparently. I always got a bad feeling visiting, but that might just be bias. My mom was sent for rehab but rarely saw anyone about it. I still drive by the area and try to shut the memory out because it was not nice to visit due to the smell and lack of staff. Would not recommend though I know good nursing homes are hard to come by, especially for patients on Medicare.

Negative experience: So apparently my review was deleted off this site but as i stated before, i would NEVER recommend anyone to go there. My family member witnessed a worker putting hands on a patient in a abusive way. They never take care of there patients the way they should. They leave them sitting in a diaper that is dirty, their entire place is disgusted, and there employees are rude and don't care about anything but a stupid paycheck. NEVER take a family member there. They are a disgrace to resident care. Another thing is the nurses do not care about the residents. They complain about working and attending to the patients. All they do is sit around and do nothing. The employees even mentioned to us that this place is bad and that we should take our family members home or somewhere that will care for them the way they should be cared for. And the only reason i rated it one star was because they did not let me post it without putting a star.

Negative experience: I will NEVER go back there and do not recommend it to anyone. Let’s just say that the experienced I had while there were horrendous. So bad that I’m less than 48 hours I was taken out by paramedics. At a new facility now and much happier
