Fantastic experience: We have had experience with Mission skilled nursing in the past and I have noticed that it has improved lately in terms of quality of and number of staff. Would definitely recommend them if you need their services. They are now under the name of Win post acute
Negative experience: I visited my aún yesterday after she not wanting to be there for poor assistant, she just got surgery and needs professional care; so thing she has not receive in this facility, today she waited for over 1 hour and half for assisting. Finally someone from the staff arrive and agree that in this facility workers really don’t care about patients. Is very unfortunate that facilities like this have this types of problems, my family will moved our family member from here. Don’t choose this facility.
Fantastic experience: Nursing staff, kitchen and therapy staff was fantastic! They delivered every request I had made. Everyone here was friendly, cordial and giving, I really mean it. Dr Trinh and Dr. Park were excellent and explained everything to a lay person. I was able to have a conversation at length and they gave me their time. Everybody's bedside manner was excellent. That added to half the healing. I was able to speak on a personal level to everyone. I felt like I was more than just a patient. In conclusion I would recommend this to any doctor who wants to recommend a patient to come here for recovery. I have made a few friendships that I wish to have for the rest of my life. Fantastic experience: My mother, who was 95 years old when she died in March 2024, was a resident at Mission Skilled for the last 18 months of her life. She broke her hip and went to Mission for rehab, which was very successful. She made a rapid recovery and that’s because the staff really worked with and helped her. Unfortunately, she also had progressive dementia; the fracture and hospital stay resulted in a significant cognitive decline, such that she couldn’t live at home anymore. While it broke my heart to have to place her in any facility, I feel blessed — truly, blessed — that she ended up at Mission, which was just the luck of the draw based on availability. But it was a very lucky draw for sure. The staff treated my mother like family. They had endless patience with her (which wasn’t easy when her agitation and other dementia symptoms were in high gear). They supported her ability to take care of her own daily needs for as long as possible; they really cared about prolonging her sense of autonomy. As she lost various abilities and strength, they stepped in to help. They cleaned her and bathed her with dignity and respect. They helped her to eat towards the end. The aides were consistently kind to her and performed even the (literally) crappy jobs without complaint. The nursing director, Myrtle, is outstanding. She has built a good and caring staff. She is tireless in her attention to her job, and the needs of not just the residents overall but also individual residents. She truly knows and cares about the needs of each patient, and it shows. (And listen, my mother was a longtime registered nurse, she was a tough customer and so am I. Myrtle and team are awesome.) The same is true of Kim, the activities director, and her whole team (Luz, Sophie, and others). They cared for my mother’s wellbeing and let her know she mattered, every single day. And sometimes that wasn’t easy, either: she spent a lot of time hanging out in their office, even when she was having a “crunchy” day. It was her safe space and there was a reason for that: these women MADE it a safe and caring space. They also run programs to engage long-term residents on a daily basis, and provide extras you won’t get at other facilities (snacks, treats, parties that include both staff and residents, and more). They even welcome visits with companion animals. As much as I would have liked to be able to support my mom at home, the fact is that she got more social engagement and stimulation from this staff (and other residents) than she ever would have at home. While the physical plant could use a refresh in some ways, remodeling is not high on their list with limited resources, and that’s okay. Instead they place their emphasis on human interaction and patient care. Also: it’s the cleanest nursing home I’ve ever seen (or smelled) and I’ve been in quite a few over the years visiting community elders and such. They work hard at everything they do. Are they perfect? No. They’re human beings, that would be impossible. But, as for my and my mom’s experience, the imperfections were vastly outweighed by the positives. I base my assessment on lots of direct experience and observation at Mission itself. I visited my mother at Mission every day with the exception of maybe 10 days scattered here and there across the roughly 545 days of her residency, from the evening she was admitted to the night she died peacefully in her sleep (which, btw, the staff alerted me to probably within 5 minutes of her passing). So I’m not making quick assessments based on casual observation. I saw how these folks treated my mom, and I also saw how they treated other residents. (And I came at different times of the day, without warning.) They genuinely care about the work they do and the people they do it for. I am deeply appreciative of their role in the last leg of my mother’s journey in this world. (And, btw, to the person below who says these are fake reviews: nope, I’m a real-life person who lost his real-life mother five months ago; please take your assumptions elsewhere.)
Negative experience: Take your loved ones out of this hell hole
Negative experience: All those good reviews about this place are false . I'm 100 % sure that they where made by someone from the staff or by the owners. This place is 100% opposite from what they advertise on the pictures. My mom had to go there because her doctor at Kaiser Permanente recommend her to go there for physical therapist training, she was there for only 5 minutes when she had to call me, and ask me to please come back and pick her up from that crazy place . Trust me I'm not trying to talk bad about this place , but I hate when people are not honest and they advertise something completely opposite from the service they have. To begin with that picture on the advertisement on the website it looks like a really nice place for a patient I agree with that. But the way they actually have patients it's crazy . No privacy at all people screaming and yelling all the time people running like a crazy on the hallways nurses all over the place playing and making jokes all loud to each other, and the place it's a complete chaos they don't have telephones on the rooms for the patients like they do on the advertisement picture, the rooms are not private, there is no one room that is for a single patient, they do not have any chairs for family members for visitors to sit down while they visit, the food is not served in those beautiful tables they sit up for the advertising. To be honest with you this is a complete scam. I do not recommend you to take your loved one there, there's more options to look into it trust me you will not want your family member to experience this craziness

Both names have Mission but we're definitely not the same and not part of the same company. Is that possible? Fantastic experience: When it comes to making tough choices about the nursing facility, it is easy to make a mistake. But our choice was absolutely right! In Mission, they helped my brother to recover completely. It is hard to believe that two months ago he couldn’t walk and put his clothes on. Today he is leaving this place that was his home for more than two months. He can continue his life independently! Myrtle (DON) was able to build great team of great professionals, really skilled and with golden hearts There are honestly no words for how much I appreciate all they did for my brother and for all family. The words Thank you don’t seem like enough but I hope you accept them and appreciate them from the bottom of my heart.
Fantastic experience: Mission Skilled Nursing Center is 5 stars in my opinion. Every Doctor, Nurse, and staff were of the highest level in taking care of my wife. They worked hard to improve her health, with physical therapy, all Medications she required, and 24 hour care, every day. Their response to her needs were immediate And compassionate. I personally saw the hard work they perform. I spent a lot of time there so was able to observe their dedication. A few names stand out, Nicole., Rhady, Sandra, Richelle ( Nurse ) , Carolina, Silvia ,and Carla. Also , Daniel Nurse , I must add Vy, the Case Manager, she is excellent, and on top of everything that is required, keeps all concerned up to date , and prepares what is needed after going home. There are many more , whose names I can’t remember. But, everyone Was exceptional. Another surprise, was the quality of the food Three delicious and nutritious meals every day , well balanced and tailored to each patients need . I highly recommend Mission Skilled Nursing Center. Fantastic experience: My father has been a resident at Mission Skilled Nursing for almost three years now. Unfortunately, he has progressive dementia and is not manageable at home but at Mission they take such good care of him. The facility is clean, food is good, staff is highly professional, kind, communicative and compassionate. A special thanks to Vy Hoang – case manager, and Myrtle Estanilla – head of nursing, who are both so caring and helpful in not only taking care of my Dad but of all their patients. I recommend Mission Skilled to anyone looking for high quality skilled nursing needs. Ary C.

Fantastic experience: My Mom was in this facility for 4 weeks and had a wonderful experience. Her nurses, PTs, OTs, and doctors were caring and compassionate and upbeat. Her case Manager Vy was absolutely fantastic, keeping me informed (as I live out of town), making sure all of my Mom's needs were met in facility as well as going above and beyond with her assistance with all of the forms required when she was ready for discharge and admission to a new Assisted Living Facility. Vy even followed up after discharge to be sure all referrals and prescriptions were taken care of. Yes, the building is very old, but the staff is absolutely top-notch!
Fantastic experience: Dr. Is GREAT! Case Manager is Fabulous! Meds are usually given on time but I can’t bring in the supplements I usually take. Sometimes it takes a while to get someone to help me get to the bathroom. OT&PT are very attentive.and helpful. Overall very good care!
Fantastic experience: My husband was at this rehab for about 2 weeks ..they were so kind and reasureing I pray for the nurses and all the staff.

Fantastic experience: Overall my experience at this facility was great. The morning team was super helpful and very attentive. I am very thankful for their receptionists such as Vicki, Arsenia as they were super helpful when catering to my small needs. I am also thankful for Vy as she was helpful with the discharge and other needs. Overall I would not mind coming back to the facility, but l hope I don't because I feel so much better. The therapy team I am also thankful for as they were very adamant about making sure I progress. Overall I had a great experience and would not mind coming back, but I do recommend this facility.

Fantastic experience: This facility is very clean and well kept. The DON is always on the floor connecting with patients who all seem to know her. The staff members seem genuinely happy to be there along with the patients active in their therapy.
Positive experience: i want țo congratulate Ms Estanilla for having so skilled Teresa Ramirez CNA. You for not have to tell her what to do! She will act immediately and with Devotion. Her smiling face is again a proof of all consideration she is having for the work, position and instituțion she works for.
Fantastic experience: I love MSN. Unlike most other facilities, who put profits over care. MSN is a very clean facility, well operated. Great service from CNA, Therapy depth and RN's. Food service is very good and the best overall all care.

Fantastic experience: This is Dan O'Brien I'm very pleased to give a review of mission skilled nursing I've stayed here over a month and I enjoy all the staff and the people included in physical therapy the activities room everyone that that take care of the people that are living here and once again I've been very happy I have to have another knee surgery in 3 months in Palo Alto and I plan to come back here it's such a positive attitude I'm a very outgoing person and there's a lot of great people here that I've been blessed to get to know
Fantastic experience: We had a great experience at Mission recently. My 90 year old Mom fell and broke her hip and had to stay here for 3 weeks. I can’t say enough about that he nurses and staff here. They were all amazing and caring people who treated my Mom like she was family. My Mom was very confused and scared at night so they would sit by her door and watch her, making sure she didn’t get out of bed. They made her feel safe and loved all of the time. The facility was very clean and the food was really good! They have activities every day and a nice courtyard to get some fresh air. I would recommend this place to anyone looking for quality care and compassion.
Positive experience: My grandpa was here for about a month or so and I will honest, yes they are understaffed but they do the best they can for what they have. Nurses Keyshia and Annie helped take care of my grandfather and as much as i felt that he could’ve gotten more attention, they were working so hard to tend to many other patients on their own. Luckily my grandpa was doing a lot better than most patients that come here so i understand. But at the end of the day it is a rehabilitation center. Their goal is to get their patients back to good health if possible. With that being said if he were in a worse condition I feel that their lack of staff could hinder necessary care. But the staff did take time to listen to my family and tend to my grandpa’s needs. Front desk was always very sweet and greeted us with kindness.
Fantastic experience: Mission skills to me is the greatest nursing home I have ever been in out of all the nursing homes I have stayed in. All the staff have been so good to me. They have spoiled me so much!! The staff here is the greatest! They kept me up my COVID shots. Now I”m getting close to leaving and Angelà and Vy are helping me with my discharge planning. I would definitely recommend Mission Skilled facility home.
Fantastic experience: The CNA were generally cheerful the most compassionate could ever hope to meet and the therapist were excellent at explaining what we needed to know. We just love every one they are so encouraging. I just feel we revover sooner than we expected thanks to their wonderful help.

Fantastic experience: Mission is the second Skilled nursing home that my mom has been in. I think Mission is far better in every way. My mom even says she likes Mission more. So if my mom says that she likes Mission, that is good enough for me. Thank you for taking such good care of my mom.

Negative experience: Understaffed and rude assistants neglected my mother because she was bedridden, she died in their care they failed to check on her sooner when they found her unresponsive… do not put your loved one here this place will kill them.
Fantastic experience: I was fortunate enough to have Vy Hoang as the case manager for my daughter - a patient in Mission for 2-1/2 months (just recently discharged). I would give Vy and her staff 10 stars if it was an option ... her entire staff is exceptional - but this review is about my experience with her. Vy is a dedicated healthcare professional and very knowledgeable in patient care... it is commendable that she often steps outside the box to assist and help meet the needs of her patients and their family - she also listens. Vy has provided a valuable representation of her skills along with caring and compassion for what she does at the Mission Skilled Nursing and Rehabilitation facility. I don’t know what I would have done without her - especially during these times of COVID. She was always there to assist (very responsive) my daughter and me and continues to do so though my daughter has been discharged. Commendable is her helpful advice regarding additional financial and other services that we may qualify for. Vy - thanks for your support and care!!

Negative experience: Extremely terrible. Staff are kind but no actual medical care was happening. Dirty, broken windows, not enough patient care and poorly run. Staff apparently did not know how to do CPR on a patient they thought wasn’t breathing. But they were breathing, so I guess it was a positive they couldn’t get it done.

Negative experience: I am an RN case manager- my client was sent home with very poor discharge instructions on a Friday night - I spent most of Saturday calling the facility to clarify the discharge orders - no one could answer my questions or get me to the appropriate staff member- - she was also sent home with no apparent plan for a large facility acquired wound - this would not be a facility that I would ever feel comfortable caring for one of my clients or a loved one

Fantastic experience: Excellent staff and accommodations. My mom spent close to four months in this facility. At one point she was very ill and, after a procedure at the hospital, could not raise her head or move her extremities on her return to this rehab center. She was very vulnerable and needed care for her very basic needs. The staff was kind and skilled. I live out of state and traveled often to see her and help out. I got to know the staff and the routine and it helped make my mom's recovery more complete. My mom loved the food and on a couple of occasions, I had a meal myself. The facility is clean and we both were very happy with the physical therapy center. The doctor made efforts to keep us informed and approachable. I truly feel this place is worth looking into if your loved one needs to be at a rehab facility.

Fantastic experience: Thank you Tim Luc and your staff for taking care of my mother and helping her recover from her surgery. My mother under went surgery and had to stay here for two weeks and at first she was nervous due to past experience at other such facilities. I took a tour of the place and it looked like a good place. However my mom had nothing but praise for the staff. They were always there for her, attended to her needs and treated her more like family than another patient. Also she had an amazing room that faced the court yard. If I ever need to recover from a surgery, I will definitely want to stay here.

Negative experience: My grandma is here the nurses are friendly how ever there a rude unprofessional ladie that works in front she was rude and mean to my grandma I will be calling corporate office just incase this escalates because my grandma is in there you can tell she hates her job. Negative experience: My mom was at this facility during COVID. The administration was completely unprofessional and would not listen to family. My mom suffered from extreme pain and anxiety. She had two strokes and went here for rehab. The doctor took some of her pain medication away to try to make her more alert to do PT. This backfired because a person in pain can not do PT. We decided to take our mother out AMA, again administrators very unprofessional, didn’t even want to give list of medications and times. The Ombudsman was called. My mom went into hospice care at home 4 days after discharge. Her hair and body was filthy upon discharge as well. She was also severely impacted upon discharge. Shame on Mission!!! Beware do not trust this place with your loved one especially during COVID they do what they want and do not listen to families.
Fantastic experience: Just arrived here today at 2:30 p.m. worst place I have ever been I have yet to receive my pain medicine my call button does not work... I have no light switch...there is somebody in the hallway always screaming shut up and screaming and yelling and clapping... I just received my water that I asked for 2 hours ago . i really don't know what kind of place this is this a nursing facility Rehabilitation or is this a psych ward. I think this place is understaffed and I don't know anything good to say about this place I really wish that the owners would call me up. I do not recommend this place for not even my worst enemy

Negative experience: Don't bring your loved ones to this awful place!!! My father was admitted to this awful rehabilitation center following a fall wlthat caused head injury and stroke. The administration and so called health care professionals are all smiles and present themselves as caring, etc. Here is the worst of it. Due to negligence by their so called , "seasoned and one of their best health care nurse assistant, " my father fell in the bathroom. He was suppose to have 24 hour observation and assistance, he was unable to stand and dependant at the time. This nurse left my father alone in the bathroom because her excuse, instead of calling another assistant in to help she left my father alone in loose broken bathroom rails unable to balance himself, he fell hit his head against the sink. They did not call me. They tried to cover it up with smiles when I got there to check in and telling me how proud they where that they were placed top 10 facilities in the state. I went into my dad's room and saw bandaid, my dad was out of it I went to front asked what happen their response was oh he fell he is ok. I asked how. They lied. They said 2 nurses were their helping him but he accidently rolled off to floor. They said their doctor checked his eyes and nurse put bandaid on him and was fine. HE WAS NOT FINE!! I had him taken to ER, due to fall, blood developed in his brain again. He was back in hospital again. Mission skilled nursing is not skilled they are horrible.
Positive experience: I really liked this place. I was here for about 3 weeks. Staff was helpful got me back on my feet in about a week, with physical therapy staff. Let my husband stay as long as we needed and sometimes over night. Help me to get rides to and from Dr. Appt. When it was time to leave. They took me back home. I had the care I needed in and out of there. Thank you staff.
