Fantastic experience: Very well taken care of

Fantastic experience: Mom had great compassionate care and support while recovering from a broken hip. OT and PT helped a 100 year old body to safely move again and get to return home. Many thanks to all!!

Negative experience: Was given misinformation and had a bait and switch on my daughter's room at this facility. Was lied to by Jason Miszuk, director of marketing and admissions who told us they had a private room at the facility for her and her admission was all set, when we arrived was put into a semi-private room with an unsuitable room mate and when trying to get this rectified Alexis- the on site admissions person said she would be right back to speak to us got in her car and left! Jason Miszuk is not responding to our phone calls. The facility is not as described and smells like rotting meat. So here we sit. Please vet a facility before sending a loved one to it, do your research and do NOT send someone you care for to this facility. *Post script: my daughter was not at this 'non smoking ' facility for even 2 hours before we ventured out into the courtyard to be offered weed by 2 different patients getting high and the front doors are left open and unlocked, unmonitered 24/7. Absolutely anyone can come in and wander the facility anytime freely and unquestioned. She was transferred here to receive iv antibiotic therapy for a drug resistant bug picked up in the hospital. Iv to be administered every 8 hours. Checked in around 5 pm, 10:30 next morning still no meds given, no antibiotics administered until I arrived and confronted staff and charge nurse. Who, by the way, related the admissions director Jason Miszuk routinely ghosts the staff as well. I still have not received a call back from Jason. I have no idea how this facility can claim a 5 star rating and I highly recommend this facility be looked into by state board.

Negative experience: They're rude for using the wrong door they can put up more signs for food drivers instead. I delivered to the emergency room, firestation, police station all the time no problem. Going forward there probably going to have the same problem with other new drivers from their reaction, because they're too good to do anything about it.

Negative experience: This place is the worst. Never had anything to drink. What they called food was disgusting. I get there every morning always sitting in poop.not enough staff to care for the patients. Do not take your love one to that place

Fantastic experience: From the moment we walked in we were met with nice friendly faces. Great care and a really good experience for my family!

Fantastic experience: During my parents’ stay at Osborn Rehab, all the staff including the doctors, nurses, CNAs, physical and occupational therapists, social workers, receptionists, housekeepers and administrative team, went above and beyond in their care. They were dedicated, compassionate, and exceptionally patient with the residents. They acted with compassion and care for their patients. Without their help, I can’t imagine how my parents would have managed, especially since they don’t speak English. The facility is also always very clean, well-equipped, and safe, it is a supportive and well maintained environment for recovery.

Negative experience: Do not take the person you love to this place; the food is just tomato soup, green beans (though he can eat some beans), and a piece of bread. I called yesterday, left a message, and the receptionist said she would call back, but she didn't. The pictures are not the same; there are two people in the same room sharing a dresser. I will share the right picture so people know what this place looks like. It's unbelievable! I tried calling when I was at the front door, but they couldn't help me. She was so rude over the phone; she wouldn't give me any instructions. I went to the back, answered questions, and she got mad at me. It sounded like she was on another phone call; she wouldn't answer my questions. I asked who was talking, and she hung up. No customer service. The nurse opened the door; she was nice, opened the door, and brought me to the room, and I found all of this.

Fantastic experience: This place is awesome and full of work to do. There are never enough people anywhere you go but our team really pulls through here at Osborn!!

Negative experience: At Osborn Health and Rehabilitation these were the utensils that were given to my mother this morning, as you can see the spoon is filthy. On Tuesday, April 15, 2025. Breakfast was served at 8:20 AM this morning, my mother asked the nurse for new/ clean utensils. 12 PM came around and she still did not receive clean utensils. If any of you have love one’s in this facility please keep an eye on them and visit more often, there’s a lot of nurses/ people out there that do not care about your love one’s.

Fantastic experience: Osborn Health and Rehabilitation has been like a 2nd home to me. When I first left the hospital knowing that I was going to Osborn Health I was very skeptical because I have never been a rehabilitation facility and I didn’t know what life would be like in there. It was the best decision me and my family made. I lived and recovered at Osborn Health for about 3 months and by the time I left the facility everyone there was like family. I can’t say enough about the care and service I got from the staff at Osborn Health. From the gentlemen who run the facility, to the nurses and CNA’s, reception, kitchen staff, hospitality, even the cleaning personnel, I was on a first name basis with all of them and they are all so kind, courteous, caring, and professional. If I’m ever in a situation again where I’m required to have post operative care, I would choose Osborn Health again in a heartbeat. Thank you to everyone there you guys are the best! Update: March 24, 2025 I just recently was released from Osborn Health & Rehabilitation for a surgical procedure I had back in December. I have been there since January of 2025 and I can tell you that my stay here was just as great as my first time being here last year. I think they've gotten even better than my first stay with them last year because the response time when you click the call button has been a lot quicker. I've been reading these negative reviews about this place and from my experience of living here three months last year to another three months this year after two different surgical procedures, their experiences were very different from my own. Both times living here I got along great with just about every single staff member there. From Courtney, Megan, Ashley, Alexis, and Connie, to Gabby, Shy, Dede, Dave, Tiana, TT, Stella, Tess, Shayna, Winter, Freddie, Mae, Tyrell, Caleb, Katie, Christina,, Mikey (kitchen) Asia, Dr. Josh and the wound care team, Dr. Maskin, Dr. Shah, Dana, Eli and everyone else who made my stay there as comfortable as can be, I just wanted to extend my sincere gratitude for doing everything you did for me. So from a patient who has actually lived at this facility for three months after two different surgical procedures for my wound care, I would absolutely recommend this place for your post hospital after care needs.

Negative experience: My grandpa is staying here and it is horrible. They are mistreating him. They only take showers twice a week. He has something on his hand and they are doing NOTHING about it. They are so rude one nurse one came in and was yelling at my grandpa and said what what what like she was annoyed. He even said the food is disgusting. NEVER PUT YOUR GRANDMA , GRANDPA , DAD, MOM , AUNT OR UNCLE IN HERE !

Negative experience: Please, if you care about your loved one’s, do not send them here!! My mother had been denied water, medication, and even sat in her waste for hours on in. The first night my mother had fallen to the ground while I was on the phone with her. It took time, but they were able to help her up. She had fallen 4 more times after that, and they left her on the ground all night until 5am. One nurse would mock her and would tell her to make herself comfortable cause that’s where she’s gonna be for the night. Another night she tried to help herself up and a nurse walked in, instead of helping her, she mocked her and said “since you don’t wanna listen, shimmy your way back up in bed.” My mom tried plenty of times to help herself up, in a hospital gown, exposing her entire body, while nurses walked by and did absolutely nothing, not even cover her. One night she had even fallen face first, resulting in a huge bruise on her face. And again no one came to help her up, but left her there till 5am. My mom has neuro pain and needs her lyrica 3 times a day, or the left side of her body will feel like it’s burning. She begged for this medication one day (I was on the phone so I know) I can hear the nurse say “okay I’ll go get that for you.” and never once did they bring her it to her during her entire stay at this facility. It seems as if they would only give her water once a day. She would beg for water daily. And they wouldn’t even clean the water cup. She had mixed a crystal light in her cup, asked for water the next day, and they would just but new water in with the existing liquid for DAYS. She had not had fresh water. My mother was unable to get to the toilet without supervision, so they placed a diaper on her cause “they didn’t have time to watch her walk to the toilet.” My mother says the man changing her would rub his body up against her, making her feel uncomfortable. And they were so aggressive with wiping her, that she became extremely raw, so sitting in a diaper was extremely painful. She would beg to be changed. My mother would be so upset that she would refuse to listen to a specific nurses, they would get upset and hit my mother and my mother would hit them back. My mother may be sick, but she is not weak minded. This is why I think her nurse decided to neglect my mother. And to put the cherry on top of this horrific experience, while sitting/laying on the floor, waiting to be rescued at 5am, my mom could smell marijuana. Which was only during the night shift. Her room was literally right next to the nurses station. So it’s not like they couldn’t hear or see her. I believe some nurses truly only work for a check. There’s no love or care for the patient, just do what they need to do and leave. My mom could not stay another day and checked herself out. I will be looking up every way I can to report this place. We trust these people with our lives. And if you can treat a human being that depends on you like this, then you deserve to be reported.

Negative experience: Looking for a place to totally neglect your love one? This is the place! It may look clean and smell nice but they leave patients in soaked diapers rather than taking them to the bathroom and leave patients for hours in their own excrement! Rehab? More like severe neglect and lack of dignity. Admissions will lie to get your business ( yes folks, its a business and they collect from your insurance) then the patient will be treated worse than a stray dog at the pound! This place will be reported to their accrediting body as well as Department of Health Service and Adult Protective Services. I only wished that I could have rescued all the patients there and not just my mother. The staff that say this is good care, deserve to experience this when they are elderly and vulnerable!

Fantastic experience: Amazing people good service

Fantastic experience: I am a 74 year old Cirrhosis Patient with complicated issues and multiple needs. Osborn Rehab was the second such facility I had entered in a year and I was reticent to go based upon my prior experience. After my friend and MPOA did research on multiple facilities in the Valley, he received a call from Jason. That conversation was the deciding factor and from there it was a great experience as far as hospitalization can be great. Some staff were better than others, but none were unhelpful, impolite or unwilling to help. Room calls were answered and my guests were treated politely. Rehab and hospital food is OK at best and Osborn is no different. Some days better than others, but that is not why you come here. I hope this review helps someone decide.

Fantastic experience: My papa fractured his hip and was very weak and a terrible experience at another place. The therapy and nurses at Osborn did a wonderful job with the whole process. We are so happy for him to be in such a clean well run environment ! He made such great progress.

Negative experience: If I could rate this place a negative 100, I would!! Unfortunately, my brother passed away in this facility. He stated that he was neglected, not being helped right away when needed. Wait time was at least an hour. Unfortunately, the nurse didn't give much information on how he passed nor did the police. Listen to your loved ones ❤️

Negative experience: I had a fall 9/26 on 9/27 they decided to do an impromptu discharge. They also refused to take me to my Dr. appointment. I woke with chest pain and shortness of breath; the nurse gave me a breathing treatment and said the Dr. will be in to assess me and order a chest xray. That changed because they told Dr. Erik Maskin they wanted me discharge. It was Dr. Maskin and Megan Ponder who approached me. Told me I was medically cleared snd cleared by therapy; although on Wednesday at my care plan meeting they said I had another week. The Dr. also charted on 0/36 I had another week. I called 911 to go to ER they tried to send AMA papers with me. Fast forward after 6pm that evening I went to nurse station because I have not received my dinner yet. At they time I saw the nurse supposedly assessed me after fall; I protested that she didn’t do a body assessment on me and it was approx two hours after fall a nurse came to see me. Another nurse decided to antagonize me; at which time a lady in 122 was on floor. As a nurse for which I am I proceeded to tell them the order of operations things should happen with the incident. They didn’t like that to well. The four nurses went in the admission nurse office and came up with a story. Scottsdale police arrived did not get my side of story. They yanked me out my wheelchair and continued with excessive force. Fast forward I was taken to Honor Healthcare where I currently am from 9/27. I was said to have pneumonia; elevated troponin over 300 had a cardiac the following day. The hospital wanted to discharge me to another SNF when I’m medically cleared. I was accepted at Mission Palms until Osborn called to tell them don’t accept me. Megan Ponder, Asya Currie, Martezia, DeDe Darnell, Darna De Guzman, Frfrivk Abuan, and Gabriella Thomas all falsified documents under tge direction of Courtnee Scrivener; reportable to State Board of Nursing and legal action for negligence and defamation

Negative experience: This place is HORRIBLE! My love one was admitted her for therapy, which she really loved and enjoyed! She was promised a lot of things she never received. The care was disgusting, her call light was unanswered for 30-45 minutes, the nurses administered ALL her meds at once without waiting until she had taken them and EVERYONE there falsify documentation ( her medical records are a legal document). Nurses falsify documentation without even accessing her, listening to her bowels or ask her questions about her well being. She had a fall after a CNA emptied her foley bag and didn’t clamp the tube, so urine was all over the floor. She had her call light on, no one answered so she tried transferring herself from the bed to her wheelchair and slipped on urine hitting her back and head. I received a call from her telling me what occurred, I sent s text to Jason, Admission, Courtney, DON and the Social Worker Assistant informing them of the fall and letting them know the neglect was unacceptable. On Friday, September 27th the Social Worker called informing me that she was being discharged. I asked how when I attended a Care Plan meeting on Wednesday, September 25th that she had one more week. They decided that morning that they no longer wanted her in the building and refused to transport her to a doctor’s appointment.There was other occurrence leading up to this neglect. The facility ended up calling the police on my love one saying she was threatening staff and was trespassing! How was she trespassing when she was a patient. The Scottsdale police department came, threw her to the floor and put her in a 4 point restraint. My love one was there for therapy due to a Stroke, she couldn’t walk alone without assistance and balance was unsteady. Not understanding why both Osborn and Scottsdale would use that type of excessive force

Negative experience: Please, if you want your loved one to get better do not bring them to this facility. They did not care about my family member, frequently left them unattended when they truly needed assistance, and the staff is not proactive. They want to extend their patients stay as much as possible it seems like, probably for financial gain. Please look elsewhere as my family member wasted precious recovery time here!! Half of the 5 star reviews you see are from employees and not truthful. Stay far away!!!

But sooo helpful JP Day Nurse Chase, who procured a ride to my 2 week post- op appointment on 7/1, even though he no longer works in that department. He even double checked on appointment Day 7/2! Gia PT Annna Day Housekeeping. I would practice my Spanish with her, while she cleaned she would answer me back very fast in Spanish, thinking that I knew more than I had learned in the A-1 class Curtis CNA Steven, very nice young man. He Delivered the meals to patients regularly. He also seemed to help just about everywhere. Shayna Night Nurse PT Mark Gabby Spanish class A-1 and Kayla (Who is Caleb's fiancé) They were both so very wonderfully helpful organizing the 4th of July party for the patients. Fantastic experience: My family and I had exceptional service and care with Osborn in Scottsdale. My grandfather was taken care of every step of the way. The nurses and staff were outstanding, from being friendly and thorough they provided the best care. They truly put his needs in the forefront. Thank you Osborne for being the best for my loved one.

Negative experience: Worst food ever. Everything is frozen or out of a can. Hopefully you don't have a wait problem. DON'T GO THERE!

Positive experience: My mother was admitted into the Skilled nursing facility for 21 days and received excellent care. All of the staff was very informative and responsive when I had questions. The nursing staff and therapists were awesome and really treated her with the utmost care. I was very thankful for the doctors who kept me informed and then found out about Ascension hospice through this facility. Although nobody ever wants to consider hospice I wouldn't even have known about this if they wouldn't have come into her room and spoken with me. I would highly recommend this facility as I looked at five different ones before choosing Osbourne. I am happy that I made the decision to have her there. The only complaint I would say is that the nurses hotline after hours never gets answered. But otherwise when you call during the day you always get transferred to somebody.

Negative experience: I don’t know about the care given here, because my mom received none. They sent her home in one hour of arriving saying they couldn’t guarantee her safety as my mom has dementia and they don’t have a locked unit. Now I work in a hospital doing DC planning in another state and the way it works is Osborn’s Admission person reviews charts and extends a bed offer when they feel they can adequately care for someone. There was nothing hidden in my mom charts about what she would need in a rehab center. I had asked my dad several times if the facility could care for people with dementia and he said they told him they could. Not only did they send her home within like an hour but they sent my dad home with ZERO paperwork regarding her medication’s and future appointments. Three days later and he still hasn’t gotten anything from anyone there. Supposedly, the person who has access to the records has been out sick every day and nobody else can get this for him when it was their screw up. I am beyond furious with them. Stay away!!

Fantastic experience: What a wonderful facility! My mother fractured her hip and unfortunately her first rehab facility experience post surgery was a nightmare. Fortunately we found Osborn and they had availability, they did a wonderful job with the transfer process. We were so happy for her to be in such a clean well run facility with such excellent caring staff! She's making such great progress. Thanks again to everyone who helped us through this experience!!

Fantastic experience: Although I didn’t want to go to a rehab center, I truly enjoyed my stay at Osborn Health and Rehab. The nurses, therapists and all the other stafff were friendly and genuinely showed an interest in me as a person. I highly recommend this place!

Negative experience: September 6, 2023 my Elderly Aunt (80) was admitted after 6 pm for weakness in her legs and arms. When she arrived she was not offered any food or meal. She called us to tell us she felt kinda dumped in the room and they left. I was told that I had to wait till the next morning at 8 am to come see her since she was being admitted after they closed. I could not come with her and make sure she was settled in properly. The next morning I came in at 8:15 to find her trying to communicate and out of sorts. It appeared she was having a stroke or seizure. I asked for the nurse and to have her vitals taken. My aunt was out of sorts and the staff said she was fine 1 hour before. I found her placed in a diaper? She had no call button and was on a small bed with no side rails. After them struggling to get a line in her the nurse tried to get her to drink water out of a straw. When that did not working she poured water into her mouth 3 times. You DO NOT EVER try to pour liquids in a person mouth that is showing signs of stroke or seizure. They can aspirate, choke or later have what's called a dry drowning. The doctor was called to come see and didn't show up until after 11. It was a possible blood clot and called for a ambulance. (her records showed she had blood clots in her lungs when admitted. After being transferred the test were run she was found to ongoing seizures. Within hours was on life support and found to have fluid in her lungs. She passed away Sept 14th. It's not the facility's fault that she passed away. However I think the actions of the nurse in-charge and the lack of response for a dr on site to evaluate her condition should be brought to other people's attention. She had no way to call for help and having a nurse force water into her mouth while unable to respond is inexcusable. OVER 2 hours to have a doctor call an ambulance?

Negative experience: They allow you to lay in your feces and they allow you to scream and disturb others there are two people to a room there is no couch

Negative experience: I was so looking forward to going to this facility from The hospital after being in for 3 weeks. I cried myself to sleep the first night. I’m transferring to somewhere else tomorrow. I was forgotten for 3 meals , it’s noisy, I was told I couldn’t get out of bed until I saw a PT, loud noisy and some guy screaming for a nurse on and off for hours.The nutrition is non existent . When you need to heal it requires a positive Village. I then read the Yelp and not the Medicare reviews. I was terrified I was going to be forgotten in here. A ray of sunshine at 3 am when a worker came and asked if I needed anything. How can such a facility exist?

Fantastic experience: I was omitted to Osborne health and rehabilitation center Dec 17th to Dec 29th. I was terrified to go because of a bad experience with another company. I rated it 5 stars all because of one CNA name Libby. Libby made my stay so comfortable it almost felt like I was at home. Libby changed my whole mindset on care facilities. Libby is truly a gift from God. The care she gives her patience and not just me was absolutely amazing! Libby changed my life during my short stay there giving me hope and the strength to get my health back to where it needs to be. I just want to Thank Libby so very much, she blessed me and she touched my heart and I can't thank her enough....She the best at what she does. THANK YOU Libby! Sincerely, Andy

Negative experience: The receptionist that worked on 8/26 at 10am was very rude.Customer service was nasty and unpleasant,I will never come to this facility. If I could give 0 stars, potential customer.

Negative experience: I’ve been here for a little over two hours. As my blood sugar has been sitting over 300 for the last hour. I was here for an hour before anyone of the staff even said hello and that was because I went looking for someone. In the very little time I’ve been here, I can confirm every negative review except nursing staff. I’m going to give them a couple days before I write a another review. No a/c, smells like piss, the photos that have been posted are of the couple rooms in the front. The rest of this establishment is run down and smelly. I’m not sure how anyone is suppose to heal here. This is where people come to die. We’ll see what physical therapy is like this week, as long as I don’t die first. Hopefully my insulin is given to me soon

Fantastic experience: The staff was wonderful, especially Mysti! The facility took great care of me for the two weeks I was there. Thank you for everything.

Negative experience: The people were great However I was released to soon . Management told insurance I could walk up three steps and could go home. Sitting on the edge of tub slipped off the edge and fell into tub hitting my back on the edge of tub . Cracked a vertebrate. Never ever would go to a physical rehabilitation place again . Feel if I went straight home and did physical therapy myself and been more involved in my rehabilitation . I would have not gotten so weak as I did at Osborne. They only exercise you get is for 30 minutes a day . I realize every case is different but my care was not able to help regain my strength . It was at home and going back to work in the oil fields that got me back in health and strength .. Not Osborne.

Fantastic experience: I have been in Osborn now going on two year's. The reason I love Osborn because they have great friendly staff here. The CNA are all some people to work. They also have wonderful therapist here. I do recommend this rehab center to your friends and family. ❤️❤️❤️

Negative experience: The inside looks like a good place right, no. It has the nursing room two rooms down from a patient yelling for help and no Answer. Plus the fact my mom had bariatric surgery on the week where she can’t have solid food and what did they server her, solid food tortilla chips some bread and honestly I can’t remember what else was on that plate but she couldn’t eat it and to think that they openly said that they have one diet ONE. There was someone hacking up a storm in the other room which I don’t know how long it was if they got treated IF. And it was supposed t be private rooms two beds two beds and her roommate left as she got in and she said for breakfast was French fries and I think pancakes and she said even said she heard someone that needed help and it took TWO HOURS for them to get help in her words she said “This is where people go to die”. If you have anybody in your family that needs rehabilitation help don’t send them here this review is to expose them and tel you that this place is not what advertised.

Negative experience: Horrible, negligent, made a life threatening mistake that will alter the patient in question's quality of life and has led to sepsis. Left patient in excruciating pain for more than 8 hours. Nothing but problems since arrival. No communication to family. NEVER go here if there is another option.

Negative experience: My first comment to anyone seeking a skilled nursing facility is to not trust the website for the facility nor Medicare ratings. Please call and talk to as many of the staff as you can. Go check out the facility without announcing your arrival. You might be surprised by what you find. As with many personal reviews, you mostly find either 1 star or 5 star ratings with not much in between; meaning those with unpleasant experiences are going to write a review, as well as, those that loved it are going to write their review. I chose Osborn Health and Rehabilitation because of Medicare's 4 star rating, and because of the location. I called and spoke to the director of admissions, but I did not have time to physically check out the facility. That was my mistake. The website looked very nice, photos showed updated facility with "spacious private rooms," clean and inviting, dining room and family areas. Very, very misleading. My elderly mother had fallen and spent a few days getting checked out at the hospital, but because of fibromyalgia condition, her body was frozen and painful and she could no longer stand nor walk on her own. Her first night at Osborn Health, no one had given her any of the meds she required, nor would they answer her call button. She didn't have a light. She was in a dark semi-private room that could fit inside a shoe box. When she awoke, she found an aide going through her purse. She called me in hysteria. I tried calling the facility for 2 hours and no one answered any of the phones. I left a message with head of nursing, but never received a call back. When I arrived, I couldn't believe what I saw. Nothing like the pictures on the web site. The food was awful; nearly raw sausage and unhealthy foods while she was there for 5 days. I spoke to nursing and physical therapy. They really didn't do much to help her for two days, but I already was looking at other facilities to have her moved. However, that process took 4 days to accomplish. Hopefully where we moved her will be much better and deserving its 5 star rating. I'm afraid I cannot recommend this facility for anyone.

Negative experience: Dude don’t even waste your time going here. The staff is rude and they don’t help patients at all. They work on their own time, and they act like it’s killing them to do anything. I’m not even exaggerating. Go somewhere else. The place is dirty, and damaged. You’re welcome.

Fantastic experience: Went in for a tour and this place is amazing, it’s clean and the employees are very kind and nice. I wouldn’t wanna be going anywhere else than here!

Fantastic experience: Osborn has some of the greatest rehab staff and I’ve always enjoyed working with them when helping and assisting my patients l. EVERYONE ON THIS TEAM, Is amazing and I don’t know what I would do without them some days. My nursing team has become almost like a family and days when I’m not here, I always look forward to returning to run our weekly routines together. #GO-OSBORN****!!!!

Negative experience: This was a terrible experience. I ended up leaving. Part of recovery is being comfortable and this was far from comfortable. Food was not edible. Would not recommend.

Negative experience: My mother was admitted to this facility on 09/02 and she passed away on 09/12. During her stay my mother stated she did not get the immediate requests required for her to be comfortable such as a proper bed for her size, extra blankets, warm food and on one occasion she did not eat for an entire day due to scheduling issues with her PT and Dialysis because of the nursing staff not making proper follow up phone calls in advance. My mother was sent out to a location for her Dialysis and was not seen due to a cancelation that took place the same day that morning she was supposed to be scheduled for and she stated she waited hours before getting picked up and made it back to this facility after 11:30 that night. Another occasion she was scheduled for Dialysis and they left without properly checking to see if anything further was needed and the doctor advised they could not see her for now a 2nd time for her appointment since this facility who was to provide the lifter failed to leave her one. She was alone for hours until finally getting picked up. The lack of communication and care is atrocious and the nurse staff needs a refresher on common decency and patience and I would never recommend this facility to anyone. This facility deserves No Stars.

Negative experience: My sig other was placed their last night, visited today,in my 40 years of health care,this place is totally disgusting, staff not friendly,every room I looked in was disgusting and dirty ! Should be shut down immediately !!!

Negative experience: Had a tour today. Gross. I’d rather die than put my mom in this place. The remodel is all in the front and it’s mullet all the way in the back. Go to the terraces to see what a nice place looks like. If I was put in here I’d escape. And no I’m not contacting anyone. You know damn well what’s going on. Don’t act like your weak reply means anything. I’ve seen your yelp page too.

Negative experience: This place is Dirty, Dirty , Dirty cockroaches near dining room. There is no team work . No communication . If you CARE at all about your loved one . You definitely don’t want to be here .

Negative experience: My dad was a resident here for at least 5 years he went through a time when they were going through new management the old management was awesome I loved it here once the new management was in place this place was the worst. I don't recommend this place for anyone's loved ones especially if your loved ones are on Arizona long term care insurance they definitely treat paying residents differently from those who are on government assistance. I took my dad out asap found him a place to live and chose the caregiver option through his insurance now he is happy and so am I

Negative experience: Don't send your loved one here unless you can be there with them every day to ensure they are being cared for properly. My mother was here for several weeks and she developed many bed sores. They removed her bed rails she would use to reposition herself and wouldn't provide a copy of their bed rail policy to determine if an assessment needed to occur to remove or replace them. So when she was no longer able to reposition herself with the bed rails and they were not repositioning her every 2 hours, she developed many Decubitus stage 3 ulcers. She was left in the hallway for over an hour after her dialysis treatment asking for help to get back in her bed with no response. They are extremely short staffed with no communication between shifts. When discharged she didn't have any of her durable medical equipment that was needed for a safe discharge. They also discharged her with a low grade fever which later we discovered she was septic with a blood infection that led to her being hospitalized again. Unfortunately, she soon passed from this infection as it spread to her heart and caused a stroke. We are contemplating contacting an attorney.

Negative experience: Some cnas try to get to everyone as quickly as possible. Some cnas are not good at their job.. wouldn't recommend a short or long term stay, especially if patient is very sick or supposed to recover, the patient might not make it here because certain people don't do their jobs.

Negative experience: My father was admitted last evening and had no contact with any skilled nurses. They did not give him access to a walker. This morning he and his roommate was not fed. The physical therapist ( a wonderful and compassionate person) got him a yogurt to hold him over. A wound care specialist was also very helpful. His bed is broken and so is the television. There is no charge nurse on duty on the weekends so there is no one to oversee the derelict duties occurring. My brother is physically on site and is witness to the improper care that is occurring.

Fantastic experience: Love working here I’ve been here for about 4 months. The staff are good people. It’s family oriented here we love each other, help out a lot if you need it which is also amazing about the job.

Fantastic experience: This is the 5th facility in the past 3 months my husband has been at, and all the other facilities were not anything compared to this one!! All the staff here are so kind and helpful to me all the time. Lots of the employees go above and beyond to make sure my husband is happy. Everything is perfect! Their employee Katie has a very kind heart, and was always very professional. My husband was taken very good care of and the management always called me if anything happened with him. We need more facilities like this one !!!

Negative experience: Trouble getting pain meds on time took up to 7.5hrs instead of every 4hrs. Had great therapy tho. All in all had a quick rexocery. Some of the staff nurses and Cna's aren't very nice or helpful. After 1 of my 2 showers allowed a week my wound and ostomy got soaked the nurse told me she didn't have time to help rebandage it. That I had to wait until she could. I waited from 4 pm to 11pm for the next nurse to just cover the dirty gauze packing with a couple of big banaides. This should be unexceptable!

Negative experience: If I could choose a zero … I would. My dad arrived there and hadn’t received dinner at the hospital and they would not provide any dinner to my father. I had to call and DEMAND that they give him a peanut butter sandwich!!!! This is a truly horrific place. We pulled my Dad out of there as soon as we could. The smell of urine hits you when you enter. This was incredibly depressing, old broken furniture, apathetic staff, broken windows in bedrooms, hallway had all help lights lit up with no one addressing them… even had a nurse hand me a spoon by holding the part that goes in your mouth. Do not go there This is no place for someone you love It is a way station for people that have no where else to go… even the transport people refer to it that way

Negative experience: My Mom was sent here after surgery on June 30th of this year. She checked herself out on July 1st. I drove over to a horrible looking facility next to them and saw how nice the front of Osborn looked. I called to see if I could have a tour and was told yes. I drove right over there. I got there and spoke with Jason. He seemed nice and even told me his Aunt stayed in this facility. However, I couldn’t tour due to Covid. I was a bit apprehensive since I just tried to tour 5 other facilities and was able to see 2. They were terrible looking. I was told this was a 5 star facility and it was given by Medicare. Well, my Mom arrived by stretcher on the 30th in the afternoon. She was bed bound. She was using a bed pan at this time. She asked to use it and had to wait 45 mins to get one, so she soiled herself. This happened 5 times in a short time. Then when she was able to use it she had to wait 45 minutes to get off it. Causing her to get a very bad bed sore. Then my Mom said she saw black mold all over the walls. I had asked Jason if they had any when I spoke with him in person the day before, he said no! After my Mom complained about the bed pan waits and the mold, she had to wait longer . Then I spoke to a Troy about the issues. I told him I see negative comments on your page. He told me that not everyone will be happy with everything. He told me they would look into the matter and take care of it. My Mom insisted on leaving and was able to be discharged the next day. I spoke with Corrina she was my Mom’s case manager. I was told her medications would come home with her and that she would take care of the home care arrangements. She did take care of the home care arrangements. However, my Mom’s medication never made it home with her and the brand new wire and plug I bought her the day before was stolen. I was not happy! I left a voicemail that was probably not in a happy voice. The nurse from the home healthcare had to keep calling, because Corrina wouldn’t return my calls for 2 days about my Mom’s medications. Then finally she called me and had an attitude in his voice when she did. She then tells me I have to drive out there to get my Mom’s medication. My Mom lives 30- 40 mins away and is bed bound. No, not doing it. The home health nurse must of said something, because that night my Mom finally got her medication. Then when reading her release records this place lied as said my Mom finished her plan of action and didn’t need anymore help. Really?? She was and still is bed bound! Don’t let your loved ones go here! Horrible liars and not a good facility!
