Negative experience: As a caseworker, I was completely taken aback during my first visit to this facility a few days ago. Upon arrival, I was greeted by the front desk receptionist, who was kind, professional, and helpful. After being directed to proceed through a secured door, I observed a client in a wheelchair struggling to open the door for several minutes while staff members looked on. I stepped in to assist by holding the door open. Shortly thereafter, we were approached by Chuck DeWitt, who identified himself as the head of Human Resources. His demeanor was immediately unprofessional, demeaning, and condescending. He raised his voice at the client, stating, “Didn’t I tell you to wait up front?” despite the fact that we had followed the receptionist’s directions. Mr. DeWitt then proceeded to speak to both the client and myself in a dismissive and disrespectful manner and explicitly stated that he was not acknowledging me, treating me as though I were not present. This conduct was shocking, inappropriate, and unacceptable under any circumstances—particularly in a setting intended to serve vulnerable individuals. What makes this interaction especially concerning is that the client was simply requesting documentation to assist with obtaining food for their children. No individual seeking basic support services should be subjected to hostility, humiliation, or disregard.

Negative experience: If I could give a zero or below I would. Please don't take your loved ones here. I will say the physical therapy staff were kind and helpful. No complaints there but the rest of the staff are awful! From leaving my client on the commode or in a wheelchair way to long to not answering the call button, lack of any kind of care, not administering needed medication, no shower for over 30 days!!! I've walked up the sidewalk to the building smelling marijuana and smelled in the building. My client had a broken bed and wheelchair, they ran out of clean linens and sheets. This place is horrible and should be inspected or shut down. I witnessed extreme neglect and lack of professionalism.

Fantastic experience: Spring valley Rehabilitation center is one of the best places to go to have rehabilitation done. The facility is clean and employees are very helpful. If I had to go back to a facility l would give them my first choice.

Fantastic experience: My mom was at Spring Valley for therapy and they treated her wonderfully. She had a bit of a rough start, but she can be kind of a curmudgeon at times and wasn't excited about going to rehab to begin with, but she really warmed up to the staff and the other people there and they ended up really bonding with her too. The place is clean, very bright, and well decorated. They had several activities daily and happy hour every Friday and she REALLY enjoyed that and getting to know everyone there. And I cannot say enough about the therapy that she received. They worked with her for several hours a day and told me that they were getting her strong enough so that she would be successful and independent when she returned home, and that she was. It is amazing to see her thriving back at home and I know this would not have been possible without all of the hard work that she and the staff put in towards her rehabilitation. No place is perfect, but everyone there was really working to benefit her and I am so grateful for them. I would recommend this place to anyone looking for positive results.

Negative experience: Staff is bad I’ve called state on them these residents are not getting proper care don’t bring your loved ones here

Negative experience: Physical therapy is great They are short on cna’s but director Troy denies it , staff confirmes shortage Emily the new social worker is horrible truly does not send referrals like she says she does Richie previous social worker was on the ball Emily messed up our stay at the end due to not being on top of the days mom had been there and ins cut her and Emily was saying mom had more time and then at last minute back stroked and Ed had to find another place with a private room and could not take any time to do so , still probably be moving mom again due to social worker Emily’s big mistake ….

Fantastic experience: I visited the building, it is a very nice, newly remodeled, clean establishment. Staff is super friendly, and helpful. Highly recommend.

Negative experience: This place really needs to check into who they hire for their driving needs! The driver of their bus today was weaving in and out of traffic, slamming on his breaks, screaming out the window, got out of his bus in the middle of the road and swerved at my car multiples times. The amount of hostility that he had should def not be used in a work environment. I truly hope no one was on the bus. I called him in and they stated they would, "Take care of it!" In a tone that sounded like they would indeed do nothing about it.

Fantastic experience: I came here a year ago with preconceived notions about Nursing Facilities in general. I had nobody to care for me and had no home. They took me in and contrary to all my flawed beliefs I was treated with the utmost respect. There was not anything that I needed that that they did not provide. Most everyone was of a cheery and helpful personality. The food is on par with most institutional cooking. Some was good some not so good. But for the most part it is good. I just thought everyone looking for a place for mom or dad or whoever the case warrants should know the truth.

Negative experience: Absolutely the worst facility in town. I’m not sure how the state hasn’t shut this place down.

Fantastic experience: I had a stroke back in April and was in the hospital for a week. After returning home, about 3 days later i experienced severe pan in my groin area which paralyzed me so I couldn’t use my legs. My wife had to call an ambulance and transport me to the hospital. After spending the night in the ER the therapy department came in and assessed me and said I would benefit from some rehab. I chose Spring Valley. My first night was rough but I was in a strange place, strange bed, and not knowing any of the staff. However the nurse was great and then next day my therapy began. They had me back on my feet in no time. They were all awesome people. The staff was great, and the therapy department was great. I was there for 2 weeks and I am great now

Negative experience: If I could leave zero stars I would. The staff are rude and have caused more harm to my loved one than any good. One of the staff BROKE my loved one’s leg by putting his legs back in the bed so rough . They do not know how to care for patients. They have no respect. There has been problems since the moment he got there. Not feeding him. Letting him get dehydrated. Leaving him in his wheelchair for way too long and not coming in to his room for hours even after he has asked for help. Do NOT bring your loved ones here to stay. They should be shut down. It’s not just one nurse or CNA who doesn’t care it’s the whole facility. They have put him in a more worse situation than he was before he even went.

Negative experience: I just want to say how disappointed I am how they took care of my dad in his last days. What hurt the most is how can you not call my mother on the morning that they took him to the hospital. She had to get a call from the Hospital saying that he passed away. We all knew he was in bad health but still atleast show some compassion and call my mother first thing. I also don't like that all his personal belongings were taken but the tv that my brother was able to grab. But the blanket with his grandkids on it is gone, an afghan that someone made for him was gone too. He wasn't even gone a whole day before they got taken somehow. Just please do better that's all I am asking.

Fantastic experience: I had total hip replacement surgery and was transfered by a couple of transporters who were so very nice and made my trip to Spring Valley Health and Rehabilitation Center comfortable, the nurses and CNAs are very caring, and the Med Tecs make sure you get your medicine. My room was cleaned every day, and three meals a day plus snacks. The therapists are very caring also, I had to learn to do things a little different I highly recremind staying at Spring Valley Heath and Rehabilitation Center.

Fantastic experience: This facility cared for my Grandfather after a stroke and did brilliantly. They were kind, caring, and proactive with all his needs and preferences. They took the time to learn about him, listen to him, and talk with him. He loved the staff and my family will always be grateful.

Fantastic experience: I just returned home after staying 22 days as an in patient. I was there for rehab after amputation of my left leg. The staff were the absolute best, Rehab included Physical therapy, occupational therapy and spee h therapy. I cannot stress enough how wonderful everyone was, I had the best treatment from all of the staff. Kindness and professionalism from all. If you are looking for a rehab center, this one is the best. Thank you all for taking such wonderful care of me.

Fantastic experience: Even with all the staffing problems nursing homes have thanks to staff like Marnie and Elizabeth my mom was well taken care of I didn't have to worry about her. Ryan in the physical therapy room is a rock star and built my mom's confidence after her leg amputation up to where she was ready to come home in just a few weeks. I was surprised that Mitchell the social worker stayed with her and even got her inside the house. Everyone was so thoughtful and made sure we were in the know. I would recommend this place to anyone needing physical therapy or care God Bless you all.

Fantastic experience: My mother in law went through physical therapy after her amputee surgery. They did an amazing job the staff and the physical therapy department were truly life saving. Just so everyone knows I am so very Happy with Moms outcome. God really did watch over my mother.

Negative experience: If you care for your family, don't send them to this facility. My neighbor was sent here after a stay in the hospital and has been there for over a month. I have found a couple of workers that seem to care but he has reported he has not been taken care of. He was supposed to be here for rehab and he has received very little. I personally noted the conditions as I visit him every weekend. The floors have not been swept or mopped since he's been in that room. I had to clean up trash in the floor more than once and there is trash behind his bed that's been there over 3 weeks. He told me that the night workers are mean to him and lazy. They let him lay in his own waste until the next shift. I found a sticky note in the floor saying he needed to be changed. He has not even had but maybe 2 sponge baths since he's been there since confined to his bed. He is diabetic and needs food at regular times. He isn't served breakfast until 10 or 11 am, lunch at 2, and supper at 7:30. Supplies are in boxes stacked all in the furniture which means you have to clean out a chair to sit down. He now is in Cox hospital since he has had diarrhea for 3 weeks, severely dehydrated and has colitis from overdose of antibiotics. He is needing kidney dialysis for the damage caused from the overdose. He is refusing treatment and is not going to be with us much longer. I am so frustrated with the neglect. I hope this helps save someone's family.

Negative experience: Horrible communication with the family. Having been calling 2 days in a row in regards to closing out accounts and gathering my mother's belongings after her passing. The staff is/was rude, inconsiderate, and unwilling to provide any type of assistance during this difficult period for our family and continue to hang up on us and leave us in the dark. Is it even legal to not return her private property? We'd really appreciate a call back. I've been calling some much that they recognize my voice and I no longer have to identify myself. So if you'd kindly play ball, we'd love to get these losses ends tied up. - Nate P. (The Gray Family) Update 5/16/24 Finally received a phone call from the social worker "Mitch". This guy was lost in the sauce. What an embarrassing moment. He had no idea why he was instructed to call. Had no details of the situation and offered zero solutions as to how he intended to help. My advice is if you are going to take four days before reaching out to the family, it's highly recommended that you have some open communication between your staff. Because calling when you're that uneducated on the situation is only making this process worse. 0/10 stars I hope anyone reading would reconsider if you have a loved one here. And to "Mr. Owner-person" in the comments section. How is it you have time to reply on Google but can't oversee a timely response via phone to close out an account? If you actually cared you wouldn't have a 2-star rating. You are a parasite on the health care system. What a predatory system.

Negative experience: There are a few people that work there that actually care about the residents. The upper management appears to care more about their bonuses than the care of the residents. The building has some major construction needs. There has been mold in the showers for months. Many complaints have been made and many inspections have been done by the state. The residents themselves have called the state to make complaints. Somehow it is like they know the inspectors are coming and will make things look okay for the time that they are in the building. It is sad how things are overlooked. The residents deserve better care than what they receive at this facility. The little money that they get monthly they spend on food delivery/microwave meals due to the meals that they are served.

Negative experience: Have a friend in there right now and I've called and left messages for the nurse's to call me with no call back. Ive left messages for the director and assistant director Holly and Ann and still no call back. He hits the call button with no answer for hrs. He got there on Friday night and has wounds that wasn't cleaned until the wound nurse came on Monday. Said there was no orders to do it but yet when i asked the nurse on Sat when i was there she said she was going to do it later. Which didn't happen.

Negative experience: I do not recommend sending a family member here. There are a few good people, but there are more that do not care about the wellbeing of the residents. Most were heard complaining in the halls about pay and unreasonable expectations with limited staff.

Negative experience: Do not send your family members here! My mother was here for a full month and they neglected her and mistreated her on several occasions. She didn’t want to tell me while she was there because she was afraid if I said something it would make it worse for her. She witnessed others being mistreated as well. They were understaffed so that caused a lot of them to take their frustrations out on her. I had a bad feeling by walking in there and smelling the smell of cigarettes and weed inside of their facility. The nurses smelled like cigarettes a whole pack. My mother was yelled at but will not release the name of who. They do not treat your family members as you would treat them with respect and love. I used to work in a rehabilitation center and I always treated the patients with respect and love as their own family members would whether we were understaffed or not. Very unprofessional facility!

Negative experience: This place is short on staff .my wife didn't get changed for hours . They said they would check on her every hour.the therapy staff didn't know my wife alzhiemers

Negative experience: This nursing home has been consistently cited for abuse and failure to procure and distribute food from proper sources. Why is this being cited over and over again and again Need a a real house cleaning to stay in business. Don’t bring family here!

Negative experience: Each time my loved one needs help she calls me so I can call the front desk so the nurse can be forced to answer the call button. Their Blood preasure was 90/50 after dialysis and falling. They are understaffed and continue taking more patience than they can assist. And the nurses are spread thin. Zero

Negative experience: My mom, who had dementia, was abused and neglected during her stay before she passed. The staff members don’t care. Often times there was no- one on the floor because the nurses, cnas and med tech would all go smoking at same time. Food was awful. I would bring in food, but they would set food on table that was across the room from my mom. Cold showers; cold food, cold coffee. The staff was not pro-family. They would say they welcome family to ask any questions or help in some way. This is a fallacy. They made an attempt to go the opposite way. Inhumane treat. My moms ears were blocked. She had a life long battle with excessive ear wax build up. So bad she couldn’t hear a clap of thunder. Tried to get the staff to give her ear drops. They wouldn’t. I said ask doctor. My mom couldn’t hear the television, the phone ring. For a month of not hearing they finally did something about it. If the person you are going to put in thier is your worst enemy, that’s unfair to your enemy. If you want your person to experience abuse, uncaring staff, uncaring nursing supervisor, housekeeping. My mom wanted the heavy blue throw blanket kept her really warm. However, if she spilled something on it, off to housekeeping. Housekeeping would never return it to her. They gave her two small blankets in the meantime. If you love your love one DO NOT BRING THEM HERE. ITS A HOUSE OF TORTURE AND LIES. BEWARE!

Positive experience: My mom has been in their long term care unit for a little less than a month (the reason for only 4 stars) but I couldn't be happier with the care she's received so far. The nurses and CNA's work their tails off and there's always someone around who is happy to answer questions. The administrators made her transition easy and the caregivers are very friendly. And did I mention that the nurses and CNA's work extremely hard here to care for the residents. I am extremely pleased with how my mom is acclimating and I'm thankful that we've found such a good fit for her.

Positive experience: I give this facility spring valley health and rehab, four stars, only because they're a little understaffed, but every facility I've been in the last 2 years have been the same. The physical therapy is one of the best that I've had in the last 2 yearsThey have made all my requests that I have dealt with all my request, especially handling Medicare. The food is as good as it is suspected, the nursing staff has always been there when needed, the transportation department takes care of all my appointments, schedules them for me and make sure I'm on time to see the outside doctor's appointment, I also want to say that business office manager Teresa Gilpin has been extremely helpful in taking my welfare first and business second I have been here 60 days Plus , And scheduled to leave this evening

Negative experience: So far this Facility had not impressed me one bit. My dad went for rehab, the Nurses are extremely rude. I walked into my dad covered in vomit head to toe, even all over the floor. Instead of changing his sheets they moved him to the foot of his bed, still covered in Vomit! The Nurses are rude when you call to get a report. Thwy dont even deserve the 1 star. If you put a loved on here, MAKE sure you are frequent visitors, to make sure they are getting taken care of

Positive experience: My brother just finished a 100 day stay at this facility. I had read the previous reviews and must disagree with the low ratings. He has really enjoyed his stay at this facility. He came in after surgery for a broken hip and his rehab therapists were very good at getting him backing moving and progressing in a short time. He found the nurses to be very caring and helpful. Like most organizations post-COVID, they are short staffed and maybe it takes a while to answer a call button. But they are working hard. When he first got there, the food was just okay. They hired a new cook and he's really bragged on the food. The nurses went out of their way to make sure he had what he needed. He wanted me to give it five stars but I compromised at 4. Rose

Negative experience: My FIL went to the hospital August 24th from Spring Valley, he passed away August 29th. My husband and I went to clean out his room at Spring Valley September 3rd which was ten days he had been gone from Spring Valley. His food tray was still in his room. There was old food all over the bedside table. His floor was so sticky that our feet stuck to the floor. Sad that no staff could find time in ten days to clean a tray and old food. Couldn't imagine ever putting anyone there or how they are still giving cares.

Negative experience: Horrible place, done me wrong called the cops on this place first night here and i left, wont answer call lights, mine was unplug from wall, my woman had to leave work to come and get me they don't care here , WHAT A JOKE!!! DONT COME HERE

Came in the room and said what the hell.do you want I said I need to go to the bathroom Will I have 70 patients here to take care of They need to hire more help I was there for 10 days with covid. Negative experience: Never ever put your loved ones in this place. The way they treat people is awful! They forget to deliver food to your loved ones. When they remember the photo below is what gets delivered, on top of that the care " or lack there of" is horrible . They advise people to deficate them selfs because they don't have enough staff and they will come later to clean them up. They leave dirty used gloves laying all around the room, wrapers and pieces of trash lay around on the floor for days on end. They didn't deliver pain meds to patient for multiple days putting them through withdrawl. Please never put someone here.

Negative experience: I was there 2 weeks for therapy. They wont let you in or out without a coworker!!! Its a prison!!! Never gave me my medicine. Dont go there. The therapy people are the only good people there. Went from 195 lbs to 165 in 4 weeks if I include the 2 week hospital stay before being transferred there.

Negative experience: My sister is here now. We want her out. No call light answered after hour and a half. Took us over 20 minutes for someone to answer the phone line. We were told we were not allowed to visit whenever we wanted. And a CNA told her she is not to call her family when she has problems. What? If it wasn't for residents in there no paycheck ks.

Negative experience: My sister went into this rehab facility after her hospital stay. Tonight she was moved back to the hospital after being in this facility with Covid. She would wait hours to be checked on, she’s not mobile so she relied on them. She could NEVER get help after 10pm. She was admitted as triage in the hospital tonight so she went out in worse condition than she went in. The food was terrible!! I’m not how this facility is even open considering all the one star reviews.

Negative experience: I had a close relative in this facility. They provided little to no care. The medication was not consistent in distribution. They did not change them when needed. The room was not cleaned as it should have been. The food was terrible and pretty much inedible. Their health deteriorated and they were re admitted to the hospital They also lied when asked questions and made it the patients fault and not their own inadequate training and care. One star is a stretch.

Fantastic experience: I, like many others, had zero experience with care centers. How things work, how my Grams will be cared for, what to expect. Being the only family member left to make these decisions; Spring Valley made sure I was NEVER ALONE through this process. They stepped me through EVERYTHING They regularly keep me updated and make sure I am able to understand what the world looks like through my Grams eyes. How it feels from her point of view. They are able to help me better support her through their support of me, not just her. They have been amazing! No one ever wants to be faced with this issue. But, should you ever be faced with this decision I would highly recommend to anyone and everyone. Go to Spring Valley. They are DEDICATED, NOT JUST TO THE RESIDENTS BUT TO THEIR FAMILY! I know my Grams isn't in her home of 28 years, where of course, she wants to be. But, I also know, she has found a new family and that group truly cares. They always respond to my calls, questions &/or concerns promptly. Same goes for my Grams. They make sure she is not just as healthy as she possibly can be but also as happy as she possibly can be. Thank you Spring Valley, for helping to ease some of the burden I carried alone before. Forever in your debt!

Fantastic experience: Appearance is always the first thing that you look at when you walk into a company. My first impression when I walked in the staff was really kind and caring what's going on. The cleanest as well was just more of a bonus. I am very happy with this place!!.

Fantastic experience: I have worked at this facility for awhile now and one thing I legitimately can say, is that the staff truly do care for the residents. We all work really hard to try to make a family like environment for our residents because quality of life is the number one priority for all of us.

Negative experience: I'm Rodger Roller and I have too many bad experiences to mention. However, my last experience was they misdiagnosed a blood blockage for water on the knee. I left in my wheelchair and ended up in ER they put me on blood thinners as a result of Spring Valley neglect. I had a an infection in my big toe on my left foot and they amputated my left big toe as a result if Spring Valleys negligence. The food was cold most of the time.I will say most of the staff were decent. I do not recomend this facility to no one. Some people have good experiences and I didn't, to say the least. I'm seeking legal remedies as a result of their contumacious actions. It is my utmost desire to to seek compensatory n punitive damages.

Negative experience: This place offers sub-standard "care". Unfortunately I have to find out about the care being stuck here. Take my advice, don't bring any loved ones here.

Negative experience: Refused to properly care and provide for their residents and workers are basically only there for the money and take no head that these are people's loved ones Their negligence killed my grandmother

Negative experience: The food is cold, meals are at 10:30 am 2:30 pm and 8:30pm. The social workers do not have the residents concerns in mind, if they want to go home to their home and pets the do zero to help them towards improving towards a dismissal plan, pretty much they check in and are proved incompetent so they are not allowed to be discharged they keep the patients social security check so the patient is stripped of everything they have worked their entire life for. When I finally tried to get a friend back into her home, They told me I was lying that i didn’t have a attorney, which I do , I have one on retainer who said it should be a simple matter and told me how to handle it. He said there was no reason to pay a attorney $500 to write a letter when I had access to the form needed. They discredited me, I am a business owner and have no reason to be disrespected, my friend has set for more than one day cold because the won’t turn up the thermostat to make the residents comfortable. Think twice before using this Free nursing home, they give minimal care and give no respect to their residents.

Negative experience: My was staying there , their phone service is terrible. Call there you would be on hold for at least 15 minute's, after letting phone ring for 5 minutes before but anyone would answer it.

Negative experience: I had my father in law there and they treated him very poorly and did not give him the right medication and treated the patient rude and never change the bedding at all and the food is nasty

Fantastic experience: The pt is great but don't need a required pill the aids will tell u no and ignore you

Positive experience: In the past I was a resident there when it was Manor Care and things have very much improved. The rooms appearances have improved, better care concerning food and snacks and over all putting forth extra effort to making me as comfortable as possible.

Fantastic experience: I love the caring people that make up the staff at Spring Valley Health and Rehabilitation. My mother has been a resident for 6 years and the staff have become like family. My mother says she is very satisfied with her care. I would highly recommend this facility for anyone looking for high quality care for their loved ones. Thank you to all the employees that take care of my mother as they would take care of their own! You are all SO appreciated!

Fantastic experience: Thank you so much for your wonderful care of my good friend Carl Davis during his time there! He loved living there and told me it was one of the best decisions of his life to move from living alone to living at Spring Valley. Though he passed away much quicker than we expected, he often told me he planned to stay there for the rest of his life. He fulfilled his plan. Thanks!!

Negative experience: I have been here for nearly a month, I have never had to be in a nursing facility in my life.This has got to be the worst Springfield Mo. Has to offer. The people here with the exception of one or two (one nurse and one c.n.a.) should be ashamed of themselves! And the administration should be prosecuted for the treatment that goes on here! I have open bed sores on my backside and was allowed to lay in filthy sheets , that had'nt been changed for three weeks! The food is served ice cold, and much of it is unidentifiable and unfit to eat.(Burned cooked to mush ect.) I'm in a wheelchair and have fallen in my room and after pressing the call button have laid on the floor for over50 min. On more than one occasion.meds are always late and wrong. For god's sake please don't put anyone you care about in this shameful place.

Fantastic experience: I frequently visit my family member at this facility, the care the staff provide to the residents there is absolutely ABOVE and BEYOND!!!! The nursing staff is phenomenal and I would reccomend this facility to anyone who needs long term or rehabilitative care. All of the staff are friendly and go out of their way to make sure their employees, residents, and family members are completely satisfied! Thank you Springvalley for being an exceptional member of our community!!

Fantastic experience: I have worked here for almost 4 years. I have enjoyed being here, I have worked my way up into a position that I love. We moved our mother here last July. She loves it here. And everyone loves her. The staff have all learned my mom’s quirks and are real quick to see to her needs

Positive experience: Spring Valley was formerly Manor Care. It was sold to another company a few short months before my husband was admitted there. When my husband stayed there it was undergoing reconstruction on the rehab side. When the reconstruction is completed the rehab side will look great. My husband stayed in one of the new rooms that had been reconstructed. It was a private room but was very plain. I picked this facility for its rehab dept which is excellent. They did wonderful with my husband and helped him to recover. I can't say enough about all the rehab staff but especially want to thank Holly and Cindy. Now, for some of the things that I observed while he was there. First off the staff was always very nice, pleasant and particularly helpful to my husband and other patients. The CNAs were good, but there was never enough of them. I feel the nursing staff needs to go to some seminars and or short clinical updates to improve their nursing skills. Every nurse gets rusty and even hospital nurses go to nursing updates. There too, there is not enough nursing staff, and the facility would benefit from more med techs to pass medicines, freeing nurses to do nursing duties, versus just passing meds. . The worse thing that we encountered and heard from others, is that the food is terrible, rarely warm enough to eat when served, no condiments like salt, pepper served with the food to make it more palatable. I observed several meals at different times and there were multiple times that it was not able to be identified what it was. The rooms were not cleaned regularly, nor was the bathroom. I know for a fact that the linens on my husband's bed was not changed for almost 4 weeks, even though it had blood on it and after requesting for it to be changed. They really need a bath schedule for patients, there was none, and you had to ask multiple times to get bathed. I think if the facility gets enough credible staff in all capacities, this will be a good facility to go to. Especially for rehabbing based on the great rehab staff presently there.

Fantastic experience: This is a wonderful care center, whom takes wonderful care of my mom! Mom wasnt happy at her last place and she is beyond happy now. Staff is friendly, very helpful and tentative. Ky children love coming here as the staff is friendly with the kids when visiting! I would put more of my family here if need be!

Positive experience: I came here after surgery to recover and eventually go back home. So far there has not been one person who wouldn't bend over backwards to help me out here or get me what I need. I really miss home but I think that as rehab goes this has been a very positive experience and will get me home and heal faster. The only downside I have found is that sometimes the staff seem a little over worked or understaffed occasionally so sometimes there's a little bit of a wait, but I think you would find that in any rehabilitation center or really any place that has a service such as this. You have to remember that emergencies will happen

Negative experience: Please do not use this facility for your loved ones unless you can visit at least twice a week. This facility lacks compassion for clients with minimal outside support. Perhaps they are understaffed for the number of clients or they are not skilled in all areas of care. This is not an excuse for knowing your clients and treating them respectfully and helping retain their individual dignity while they are in your care. this aides in recovery. This is what happened to my loved one. All relatives lived out of state. My loved one served his country in WWII and actively supported his community through his church. Unfortunate for us; he sustained brain injuries and was taken in for rehabilitation by the good folks of Spring Valley. The first week was rough but things appeared to heading in the right direction. He complained of being ignored as he had no visitors. 1 week later, the doctors gave him 4 to 6 months due to his downward spiral. 2 weeks later; a cousin visited, found him unshaven, uncleaned and non-communicative. He passed the next day. I understand that the end of life is not always predictable but dignity and respect for our fellow human beings reveals the core commitment of persons and the organizations they represent. A person deserves to be bathed, groomed and shaven up through their last breathe. Perhaps if the individual names of the staff were published; they would care a bit more. They too may find themselves in this situation someday.

Negative experience: I only been here for 4 days now and the service here is lousy. On the first day of arriving, the room wasn’t ready and they had to fixed the bed. I sat in the hall for almost 2 hours. I didn’t receive any pain medicine Friday night and finally got it on Saturday night. I was in pain so bad. I watched the staffs standing around in the hall doing nothing but chatting, looking at the cell phone and doing nothing. Some of the staffs have long fingernails. I’m thinking about looking for another place that have a bed available for me. Don’t come here!

Negative experience: This place is DISGUSTING!! The rooms are NEVER cleaned, they would drop dirty needle caps, gloves etc in the rooms, and it would sit on the floors for weeks! Call lights would go unanswered for an hour at times, and when you would go out to look at the deserted hallways, You would almost always find all of the Staff outside in the Courtyard area smoking, while patients were not being attended to. If you were to ask questions to one of the so called head nurses, you would get a blank stare and told they have no idea. How this place is not shut down is a mystery to me. I am going to continue to report them, and hope no one else puts ANYONE that they love in here. These People should be ashamed!!!

Negative experience: This facility is poorly managed, poorly staffed, and does NOT care about its residents!! If you have a loved one that needs long term care, please, please, please choose a different facility!! Neglect, abuse, lack of cleanliness are just some of the problems in this place. The lack of basic human decency in this place is completely disgusting. I can only hope with continued visits from the state office, and multiple investigations that this place is shut down for good. These residents deserve so much more than they're recieving. Do not put your family member in here!!! Please choose a place that is warm, kind and caring. Not this poor excuse of a nursing home.

Negative experience: The rehab portion of the facility might be nice, so I can't speak to it, but the nursing home is decidedly not. 1 star is too generous. Gross negligence of care is an understatement. To their credit, I haven't heard of any physical abuse. My mother was left with an infection until it became septic and she went into renal and liver failure. Montala, her doctor, is a little weasel. He would frequently skip making rounds on her, when I went in to discuss it with whatever nurse was head of the hall for that week, because they can never keep people, they would try to deflect from the question and then say he would see her the next day; which he would not. He also works at Mercy, where she was hospitalized for nearly two weeks; he visited her 0 times. Any time she voiced concern, whether it was pain, or confusion, or an infection, asking if the doctor knew and if she should go to the hospital she would be told "If you go to the hospital we don't have to let you come back". When they have a good nurse, they usually quit in short order. The list of issues I've had with this place in a year could fill the pages of a small novel, this is just the most recent issue. Now, after nearly two weeks in the hospital, taking her in myself rather than letting her go back to that place, they are refusing to release her medications that her insurance has already paid for. IE they are stealing her medications. "The doctor says he's not comfortable releasing them.". That's cute, but it's also illegal. Keep digging the hole for yourselves, I guess. If you want to send your loved one somewhere to let them die, I guess it's fine. If you care about them at all, go elsewhere.

Negative experience: Do Not recommend this facility for the care of your loved one!! Staff is not professional and management doesn't enforce any type of structure. The care the residence recieve is very minimal care. The building is very old, dirty, and falling apart in so many aspects.

Negative experience: This place is disgusting! They do NOT clean the rooms, bathrooms, and the Nurses never wear gloves. I personally witnessed two CNA's, one was a black lady, and the other was Ashley, talking bad about Patients' and how "gross" they were while they were outside smoking. To say this place is low class and hire only bottom of the barrel employees is an understatement. This place really needs to be investigated!

Negative experience: My mom was there for two weeks. She was left alone in the dining room. Few times alone in her wheelchair, it was awful to ask for help. I begged for help, just to put her to bed to help her to feel comfortable! For 5 mins for help? It doesn’t take long to put her to bed. Well, I lifted her to bed. I fixed her hair. The best I can to keep her comfortable! Sadly, she passed away a month ago. But before that, I cAnt pass that! The nurses overnights were awful!!!!!!! Days nurse were so awesome!!! Things has been happened this nurse home, it’s sucked in my head. I can’t even imagine for others to go thur like I did! Horrible!!!!!! If I was the charge of that place, I make sure, those clients would be damn happy& comfortable as their lasting days! I pray hard for karma to get those nurses that's was so mean & awful! I even have pics too. Planning to report his place. I have to keep asking for her clothes, for her to be changed. Even my aunt had my mom’s file to be “not to leave her alone”. Well you did violated that.. so sickening!!!! I have seen some were left alone in the dining room& and some taht needs to changed so bad. My god. I pray for the karma to get you nurses for not doing thier job& fakes their behaviors.

Negative experience: I cannot believe that this place has even one 5 star review. Horrific. This was, by far, the worst nursing home I have ever seen. Disgusting. It's not the rehab part of a nursing home that matters...that's only 90- 100 days. Look at the long term care side of the building. It is cruel. Heartless. Barbaric. Upper management have easily already earned their tickets to hell...and make no mistake, they're getting front row seats. PATIENT BEWARE

Negative experience: I was here multiple times for lymphedema therapy when it was Manorcare. It wasn't great, but I seen worse. These new people, MGM Spring Valley? I cannot see how they are in business. Understaffed. Pt and Ot now is just there looking for new jobs. There lymph specialist was basically like he didnt wanna be bothered. Housekeeping? Lmao yeah ok. The CNAs are decent, but the nursing staff seemed confused along with management.

Negative experience: Most disrespectful facitlity i have ever had a family member stay at. I Purpously left 3 mouth swobs out one night for My gpa. To see if the nurses would use them.. He was there 2 days and not one time did the nuses use a mouth swob to help the dry cracked mouth. My gma had to do it herself the next day. Seemes like people goofing off more than work getting done. . This is not a place i would ever suggest taking your loved ones to, Or to seek employment at.

Negative experience: The nursing staff is pretty good, but understaffed. My relative was in there for about 11 days & received no shower or other hygiene treatments. His doctor refused to come in for an issue & he had to be taken to the ER to get treatment. Facility is pretty outdated & the patients only get a very small TV.

Fantastic experience: During my stay at Manor Care due to a broken hip, I was certainly treated with respect and compassion by everyone. They were always helpful and efficient, and I appreciated the good meals and the excellent rehab that helped me return home. I hope to remain home in the future but, if need be, I would certainly return to Manor Care. Helen Vinson

Fantastic experience: My uncle was a patient at Manor Care for 2 weeks. He received amazing care. My aunt was extremely pleased after a terrible experience at another facility. He received PT therapy every day as well as round the clock nursing care. He was able to return home without needing any home assistance.

Positive experience: Rehab and therapy very good. Nurses listen. Aides are doing what they can. Wifi is 5star. Food 3star.

Negative experience: The nastiest place in Springfield. Having been a patient here and treated like cattle being taken to slaughter, this is the worst place anyone could send their loved one. The fancy furniture, pretty lights and bright colors do not cover the truth of what happens when family leaves for the day or evening. The patients are made to wait hours on call lights to be answered. Clothing gets lost in the laundry. Staffing is never enough for the number of patients. Mold grows in the showers. Reports have been made but the state is lacking enough staff themselves to proceed for the awful way patients are treated here. The aides are not trained on the use of oxygen and are for the most part, untrained and inexperienced. Please do not take your loved one to Manor Care

Negative experience: My uncle has been in this facility for a couple months now and I must say I personally am not very pleased with the experience. I am a CNA so I understand the demand of the job and everything that comes with it, but I also realize how the job should be done. I feel like every time I come to visit his room is always a mess, (trash on the ground, his dirty dishes, etc) even when the housekeeper "cleaned in there this morning." It's not just the housekeepers job to keep his room presentable, so I feel like whoever is taking care of him does not take pride in their job. He is a messy eater, but his face never gets washed, or his shirts changed. His room and the facility as a whole is just bleak. He's had 3 different rooms and they've all had something wrong with them. The one he is cureenrly in has a light socket cover that's busted. Which should be considered a hazard. I realize that's all just surface stuff. But, I also have to comment on the staff, aside from a few exceptions, I feel for the most part the staff are very rude. Anytime I voice a concern, it seems like they just shrug it off. They never come back for follow up, or even follow through with what they say they're going to do in the first place. There have been instances where I'm here for 3 or 4 hours visiting and no one comes to check on him to see if he needs changed or anything else. This place has just left a very bad taste in my mouth and if I was in charge of the placement of anyone, this is not the place to send them. There are far better facilities in this town and I would suggest looking elsewhere.
