Fantastic experience: Everyone has been wonderful, the facility has been very clean and the staffing has been great here. I would recommend the facility overall. Therapy has been great as well. Overall a great experience!!

Negative experience: If you can not stay with your loved one all day until visiting hours end this is their fate and reality. It's so sad and disheartening. Tens of thousands of dollars the health insurance companies pay and this is the type of care- or lack there of they receive! Pay close attention to the urine soaked sheets & pillowcase as well as diapers my mother was left to lay in! Every day I arrived in the morning this is how I would find her! Beyond neglectful! Broken phone plug leaving zero access to the phone!

Fantastic experience: My overall experience has been wonderful staff takes good care of me and treats me well I feel safe and comfortable here the l therapy team is very comprehensive and well- focused so if anyone who need therapy would be well served coming here. Also I wanted to give some recognition to some amazing staff shelley the housekeeping aide inspiring and joyful which promotes hope without which faith and love would be hard to come by and Keisha concierge is so encouraging, energetic, joyful when she to visit really makes be included in activities and makes the resident feels welcomed.

Positive experience: This is my second time being here the care and therapy is still great the care tremendously improved like 50% the only concern would be at times the wait time from call light assistants is a little longer then expected but when aides do come the care needs meets expectations. Overall I would still recommend anyone who needs treatment and care.

Fantastic experience: From the moment I rolled in I noticed it was very clean and welcoming!! The other rehab facility I was in does not even remotely compare to Optalis. The staff here is absolutely amazing!! Big shout out to all my care people. You all truly care about your patients and I thank you from the bottom of my heart for that. The second time around is exactly the same!!!!

Positive experience: Overall the care is great no complaints I would recommend for therapy.

Negative experience: My Mother was just in there last week 12/10. The food was terrible it was all basically the same color, even the vegetables. We brought in most of her food. The staff was hit or miss. She was in over a holiday so staff was gone for family time and therapy going on was spotty.

Negative experience: My friend is turning them into the state for dirty rooms for the longer term senior patients. Also, the nurse staff gives 0 zero care for the seniors! Go somewhere else!

Negative experience: My mother is at Optalis Sterling Heights for rehab. Yesterday she was brought back to her room by a physical therapist who asked the nursing staff if they would get her into bed from her wheelchair. The nursing staff said they would get her into bed. My mother sat in the wheelchair in pain for several hours in spite of her and her roommate yelling for help. My mother cried herself to sleep that night. Disgusting “care” here. Don’t go!!!!

Positive experience: I like the staff everyone is very friendly the only issue I have is the kitchen needs some improvement but overall the kitchen staff is nice I feel I have been improving day by day just looking forward to going home before the holidays.

Fantastic experience: It's tough finding a rehab facility for a loved one, our family went through the process. You definitely need to do research before picking what the hospital says is available. Due to rooms availablity this facility is typically full, but we were lucky enough to get our dad into an available room. I told the hospital this facility had room, they said oh that's a good one. I was comfortable with the care from start to finish. Everyone was real nice, and helpful. Nurse Aid Shawn was great, I still own her a Black Cherry Vernors, my dad’s favorite flavor and soda, she had one and donated one from her lunch for my dad. That was very kind! The Nurse Admin greeted me with a welcome package and ethuasium. I found it easy to talk with her. I thought the facility was clean, nurses were informative, rehab personnel did a good job and entering the facility after-hours was no problem.Parking is good, check-in process at front desk is good. I liked how patients mobile and non- mobile on their own would hangout by the nurses station for some community social time.

Negative experience: Well, we are approaching the one year mark of my family member passing away due to the egregious negligence of Optalis "Health and Rehabilitation" of Sterling Heights. I still feel the same way I did as the day I wrote this. Frankly, we should have sued this place. I'm hoping that updating my review will place it back up at the top where it belongs, to make any and everyone aware that this place is incredibly unsafe, to say the very least, and should not be trusted. *Edit* “We are disheartened to hear that many elements of your stay were disappointing to say the least.” —- this tone deaf response only adds insult to injury. Disappointing?? A life has been LOST. There is no going back in time and nothing further to discuss. ———————- My loved one was transferred to the rehab here after a hospital stay. Rehab, not nursing home. This is a place where he was supposed to regain some strength and mobility. He had developed a manageable bed sore in the hospital. This bed sore grew exponentially in size and severity during the approx. 2 weeks he was here and looked infected. He was not receiving proper wound care or meaningful repositioning. The standard for bed-bound patients is to reposition them every 2 hours at minimum - particularly if a bed sore is already present. This was NOT DONE. He often waited very long periods of time in his own urine and/or excrement before being changed, greatly increasing the risk of bed sore infection in this sensitive area. He went into septic shock due to an infection and passed away. DO NOT entrust this place with your loved ones who may require any sort of advanced care or wound care. The purpose of my review is to warn others. STAY FAR AWAY FROM THIS PLACE.

Positive experience: The physical therapy here is phenomenal. The nurses are timely, caring and very professional. They are constantly present and very attentive . The housekeeping staff was present daily and kept my room clean and neat. I enjoyed their conversations. The entire staff was warm and caring as well. I would definitely give this a 5 * but the food was relatively cold and sometimes left much to be desired. Hopefully, they will rectify this problem soon.

Negative experience: I truly would NOT choose to send my loved one to this place. I now understand why the name changes. We had my father there and got him out as fast as we could for rehabilitation due to lack of care. The staff here is nothing short of lazy! Except for one male staff member. They would never come to the room when my father pushed his help button. After an hour of waiting for someone to help assistance my father to go to the bathroom, keep in mind that this happensed multiple times, I had to go to beg at the nurse station for someone to help. I pitty anyone in here without anyone looking out for them! Often, As I walked through the hallways to my father's room, I would hear elderly crying for help and nobody would come. I ask the staff once to help a person thay had been crying hard for a long time, they told me to handle my own business and rolled their eyes at me. I wouldn't allow my pet to be in the care of the staff here! Warning to anyone who has a loved one no to bring them there!

Negative experience: Beware!! This facility is horrible and unfit to care for any family member. The staff is truely mean to the patients. The evilness happens in front of the family members so I cant imagine how worse the treatment to the patient gets when no one is watching them. I wouldn't leave a animal with them to care for. The rooms are very dirty. The patients are not bathed or clean properly. The therapy appointments are often billed for and not received. I advise to take your loved ones elsewhere. They leave the urianals full on there food table right next to there apple juice. Look at proof in attached photo!!

Fantastic experience: my aunt is a resident here and when asked about the facility, she said she loves it here, and her absolute favorite in the facility is her cna Malik and nurse Lambra. All the staff is great and we would recommend anyone to stay at this facility.

Positive experience: The first three days were a rough, but since then the care and staff have improved. Nikki/Patrice the cnas was wonderful and very compassionate. Keisha the concierge is so friendly and made me feel good. Nurse Kenya/Irene was my number 1 nurses, Kenya being a excellent advocate for the patients. Carrie the DON was great and I was very impressed, the social worker was compassionate. Mynesha was compassionate as well and made me very comfortable . Overall 90% of everyone was good!!

Positive experience: We've had mostly 100% positive things of this rehab facility. When you enter, you are buzzed in and warmly greeted by Nicole (when she's working.) She promptly called for the concierge and I started my tour with Keisha. We saw about every department, noticing things like cameras, staff interacting with patients. We needed therapy so I was able to talk with several pt, ot staff. There is a wide variety of equipment to aid and challenge most people's needs. We ended up choosing this specific facility among other reasons because it wasn't the largest facilities. Dining rooms were larger elsewhere, but for my husband, it felt like the right place. We've had great aides, nurses, therapists. They end up being part of your extended family.

Fantastic experience: They try to make sure things are going good; seems if you have issue and bring to their attention the problems are fixed in reasonable time. Keshia is very out going & great attitude, definitely brightens the room & hall way everytime you see her.

Negative experience: The only way I could leave a review was if I put 1 star. This facility deserves zero stars. They almost killed my brother in a 2 week timeframe. He had no use of his legs and minimal use of hands. Little to no physical therapy there. It’s labeled a nursing home rehab. Incorrect. Their nurses are CNA’s no RNs that we saw. PT is an asst. Only 3 individuals qualified were head nurse, nutritionist and respiratory therapist. Food shouldn’t be served to humans. Night staff incapable of caring for patients. 2 weeks in Dr. in charge claimed my brother was fine (without even examining him). We had to call ambulance to take my brother to hospital. He ended up in ICU with pneumonia. I recommend not sending a family member to this facility.

Fantastic experience: The concierge Keisha made me feel welcomed upon arrival she was very informative, knowledgeable, very, friendly, helpful and understanding she is such a wonderful gem and a great asset to your facility. She really makes this experience better then what i expected always uplifting and cheerful also brandy the aide is amazing as well and a host of other staff. I am so ready to go but I know I gotta do the work but thanks to all that have made such a difference for my improvement and recovery.

Fantastic experience: My mother was here for her rehabilitation. The care and people were fantastic. I would recommend. The food is not desirable at all. This is the only downfall

Positive experience: The occupational therapist Marci she is very concerned while working with me it lets me know she wants me to succeed through this experience. Physical therapist Carrie she is also concerned and caring I would like for her to be my home therapist when I leave here and the housekeeping aide Pam is very sociable, very friendly she comes and talk with me and keeps me company some of the nurses and aides were better than others but overall I can say it is pleasant being here if your looking for a facility to get therapy and recovery.

Positive experience: There was confusion when I arrived around 4 pm on a Saturday. The concierge explained on Monday that it was her off day and apologized for the confusion. As the days went on, I learned how to ask for what I needed. I found the staff cooperative in meeting my needs due to my injuries. The nursing staff provided the pain medication which was authorized whenever asked for. The aides also helped in a kind manner when changing the briefs and helping when getting used to using the toilet and getting up. The PT and OT were very helpful in getting me stronger and back on my feet. Their listening and understanding of my needs definitely lent to my feeling more positive about going forward on my healing journey. The kitchen staff did follow the instructions provided on what items I could eat. However there is too much gravy on food, and some food was too spicy or mushy. The medical staff did respond to most of my concerns. However getting a follow-up on tests is slow. I realize that they have limited time to see the residents so that could be why. Some did explain in greater detail what they were checking. Overall my experience was pleasant and helpful.

Negative experience: My father has been there 17 days. I have repeatedly asked them to give him a shower and they have not. They always say they are too busy. They need to hire more staff as these girls are overworked. The menu will state what is on for the meal that day and then they bring something different and my father is looking forward to something and is disappointed when he does not get what he was expecting and the food is not at all very good. He goes to dialysis 5 days a week and I know for a fact at least one day when I was there they were waiting for someone to transport him back to his room and left him in that room for an hour. Just waiting to be transported and he says it's happened more than once. They tell you a doctor will come in. The doctors don't show. This is Very frustrating.

Positive experience: The nurses, aides and therapists are great! The 4 stars are strictly for the care staff. Negative stars would br given for the kitchen staff. Majority of the time the excuse is they ran out of salad, bread, veggies and the list could go on and on. Kitchen definitely needs a experienced staff or staff that takes pride in what they serve! Food is cold and tasteless 80% of the time. When I said something about cold food, Kitchen staff said ring for your nurse or aide to warm it up in the microwave??? How about you do your job correctly° Your only given half of what is on the meal card if you're lucky. Some of the food is stale and inedible, calls to the kitchen are rarely answered.

Fantastic experience: My encounter of this facility the nursing care has been caring, focused and well- being in my care. Therapy care staff has met my needs and expectations the goals to strengthen my physical and occupational needs to help me outlandish get better these departments have met five star goals. The only concern i have coming from a Food service management background i would definitely say the food service needs much attention and focused on providing meals that are acceptable and meet expectations for the residents so unfortunately I can't give a perfect rating to this department satisfaction rating would be a one but I would say my experience is going well and I would recommend for therapy and care.

Positive experience: The staff has been great as far as the nurses and aides but the mgmt. of the facility can work on a better process when the facility is short of staffing but I will say the staff has been doing there best to meet expectations. I would especially like to thank the Physician Assistant Beth for her care and the time she took to explain my medications and what to expect moving forward. Positive experience: Based on my experience on being here for a week I feel this place has been good therapy and also the nursing care is good the staff that I have dealt with thus far have been caring and my overall satisfaction of this place is this facility is good.

Negative experience: Care was terrible, husband wanted to be taken to restroom for a bm they told him to go in his depends. Then waited 3 hours to be changed. Most aides have an attitude. Food is terrible.

Positive experience: Optalis Sterling Heights is a good facility, but the furniture is outdated and the facility needs to be refurbished. The nurses are good. The CNA are good therapy is amazing but updating the facility will give the residence much more confidence and happiness. Overall I would recommend for care and therapy if your looking for rehab and care.

Fantastic experience: The staff is fine everyone has been great the staff could be better on the weekends but during the week the staff is on one accord and gets the job done I gave a five star rating because there are truly good people here who really cares for me my experience is great and I would definitely recommend.

Fantastic experience: At first I was a little hesitant about coming here because I didn't know what to expect this is my first time in a rehab facility but I'm glad I made the right choice by coming the people here are nice, caring knowledgeable and respectful the only small complaint I do have is the food come cold at times but it's delicious but when this happens the kitchen staff corrects the issue my overall observation of this place is they all made this experience great I would definitely recommend.

Fantastic experience: I chose this facility for rehab because my wife is a resident here and now the tables turned and I am now a resident here with that being said this place is great for therapy and care the staff here is very pleasant and very attentive and truly supportive, these generous people who care and does there jobs very well and I would definitely Optalis to anyone.

Negative experience: This place gives a great first impression — a warm greeting, a little welcome bag with a stress ball, even a "thank you." But don’t let that fool you. Once you’re in, the reality is cold, neglectful, and downright infuriating. The nurses here blatantly ignore patients. You have to practically beg for the most basic needs — water, food, or for them to change your loved one. There’s a call light button that’s supposed to alert them for help, but they don’t respond. Instead, they sit around at the nurses’ station glued to their phones or gossiping and laughing with each other while people are in visible distress — lost, confused, and in pain. My papa is here for hospice care — a place that’s supposed to keep people comfortable and out of pain at the end of life. But they failed him. They only gave him pain medication twice a day. We had to fight and plead for more, just so he wouldn’t suffer. Imagine having to beg a hospice nurse to stop your loved one from being in pain. This facility looks nice on the surface, but underneath the polished exterior is a staff that is cold, careless, and indifferent. If you care about your loved one’s dignity and comfort, think twice before trusting them to this place.

Negative experience: These nurses dont care and are rude! They do not sit the patients up or do the required about of rehab! Just lazy! Patients pee is dripping off the bed because they dont check on theit patients for 6+ hours!

Fantastic experience: The Concierge Keisha has been a blessing with my family. My mom is in rehab and she checks up everyday and makes sure things are getting done the way they are supposed to. My family and I are thankful for her being there. Thanks Keisha!!

Fantastic experience: The concierge Keisha is amazing she is very helpful what ever I need she does for my mom she goes up and beyond for my family she always comes in my moms room and checks on my mom if she needs anything.

Fantastic experience: The staff is really nice here. I love the physical therapy, occupational therapy, and speech therapy. They really encourage you!

Positive experience: My mom is a resident here at optalis she is going through rehab the therapist here are great especially Armando. The nursing staff is caring and concerned Malik the aide is the best. Keisha the concierge is very professional, knowledgeable she really makes a difference from a customers service aspect the people here makes a difference here everyday for my mom just overall so far my mom experience has been going well.
