Positive experience: Great area/neighborhood. Safe facility. Staff is generally helpful and friendly. All therapists are knowledgeable and skilled, most are professional, as well. All leadership and administrative staff are approachable and accessible. Due to being highly involved in the treatment plan and communicative with the healthcare team, we got fairly good results with this facility and had a fairly positive experience with the staff.

Positive experience: Recently, my mom underwent hip surgery, and after being discharged from the hospital, she spent some time at Sunnyvale Garden Rehab. Despite the natural challenges of recovery, the experience was genuinely positive. The facility is bright, clean, and thoughtfully designed with lovely gardens where both patients and families can relax. The staff were consistently professional and kind, and the management took the time to reach out to us as family members to ensure we were comfortable and heard. Overall, I’d give the experience a solid 4.5 stars. The only small area for improvement would be the food options. My mom is Asian, and we noticed there weren’t many Asian-flavored meals available. With more and more Asian patients enrolling, adding a bit more variety in that area could make the experience even better. Thank you again to everyone at Sunnyvale Garden for making my mom’s rehab stay as pleasant and supportive as possible.

Fantastic experience: My brother was admitted to Sunnyvale Gardens end of April 2025, for rehab from a stroke. He was bed ridden, unable to get out of bed, paralysed on his left arm & legs, severe wound on his left calf, with bed sores & very weak. During his 7-weeks stay there, he recovered miraculously, he was walking on his own using a cane, wounds all clear & was discharged to his home where he was mostly independent. We owe his dramatic healing journey from the excellent care he received from PT/OT, nurses, wound care, CNAs, nurse practitioners! They are empathetic, compassionate, and selfless medical team. Our family is ever grateful!

Fantastic experience: After 2 major back surgeries, I chose to begin my rehabilitation at Sunnyvale Gardens. The lobby is bright with large windows opening to a beautiful garden. The single and double rooms are large, newly painted and have new smooth wood floors. The food is excellent and the nursing staff is certified, capable and compassionate. Above and beyond these standards are the Physical and Occupational Therapist. They are excellent and they are the reason I am staying here beyond my insurance coverage. My 2 therapist for occupational and physical therapy are Deepak and Emi. Both of these therapist present therapy in a step by step , progressive manner. Every time, I see them they challenge me with a skill that used to be an everyday activity and is now a new learning curve. We don’t repeat the same skill everyday. They push me to my full ability in a thoughtful, progressive and compassionate manner. I feel absolutely safe with them! I’d like to take Deepak and Emi home with me! Jinal Shah is the Director of Rehab and she has compiled a stellar team of therapist. She is understanding and takes the time to explain things to her patients! If you need serious, methodical, compassionate therapy, Sunnyvale Gardens is the place to BE!

Fantastic experience: Very clean facility. Excellent services round the clock by Admission staff, Doctor & Medical staff, Nurse Staff, CNA Staff, Food services, Social services Staff. Physical Therapy Staff and Occupational Therapy Staff provides outstanding services,

Positive experience: My parents had stayed at this facility years ago before management change so I visited them when mom needed help again and was pleased to find some of the people that really mattered like Nurse Meenu who has a golden human interaction and management style and is on top of things, and also Nurse Wendy who is a hard worker who tries to make time for everyone as best as she can. Also CNAs Moon, Rebecca and May who show great compassion. Most importantly Dr. Jamali is a super physician with amazing bedside manners and knowledge. I'd give them 5 stars but there is some room for improvements. The PT department was instructed to send 2 people for my mom's issues and repeatedly failed to do so. A few times, they made up stories about my mom refusing treatment when she had only done so once when she had a bad day. Also, food could improve at this place; especially, the sandwiches had barely anything in them. Normally the place is very clean and nicely renovated but passing by some rooms it seemed like some patients had been changed (diapers) but needed an air freshener (bad small passing those rooms). In the end the medical staff which is the most important part was amazing but other departments need improvement. Overall I am very happy that we chose for mom to stay here again.

Fantastic experience: Facility is amazing! rayleen from admissions helped me adjust in my first 72 hrs, by changing my bed, helping understand my medication & overall I am having a good experience. I am very appreciative

Fantastic experience: Great staff, very professional and caring facilities for rehab of loved ones. The staff goes extra mile for the patients and their families. Very open space, great PT/OT facilities for recovery from fractures for seniors. Firetv and windows in every room so guests have their own open spaces. Highly recommend

Negative experience: Horrible horrible care! Entered facility to provide hospice care. They were unable to keep up with pain management. No dr or head nurse available on weekend to manage doses

Fantastic experience: I was here for about one month and a half. The staff was great. The nurses were great very helpful. The place is very clean. My room was cleaned every day. The physical therapy was great. Thank you Terese and Ryan. Sorry if I spelled her name wrong. The food was good. Special thanks to NURSE Cruz and CNA Anthony. Fantastic experience: I had a great experience when I went to visit this facility. The place is clean, rooms are neat and it has no smell (unlike other facilities I toured). Staff are friendly and welcoming. Alana from Admission is very accommodating & answered all our questions (since this is our first time having to look for rehab facility) and that gave us comfort. They also offer private rooms which is a big plus. 5 star for costumer service & clean nice facility!!

Fantastic experience: Great facility! Everyone is really nice and the facility is super clean! My grandfather had a great experience here after his total knee replacement. Would definitely recommend and come back if we ever need short term rehabilitation.

Fantastic experience: Highly recommended skilled nursing facility. Very clean, rehab staff are great, nursing staff are nice. You can tour the facility anytime and you can see the difference of this facility from other facilities. They have NP and doctors rounding 3-4 times a week to see patients.

Fantastic experience: My father in law was recently discharged from Sunnyvale Gardens. Our stay there was better than previous nursing homes, but there were a couple of shining stars that made being there just that much more comfortable. We really enjoyed the doctor while we were there, Shway. She was very to the point and direct. It was very helpful to have someone be very forthcoming with information, but also compassionate and confident. It was nice to have a competent doctor taking care of my father in law while at Sunnyvale Gardens. She gets five stars from us! While at Sunnyvale Garden’s, my father in law needed a tube feeding, for the first time ever, to give him his nutrition. From day one, their dietician, Diana, was there at his bedside to explain everything to us, discuss his formula and why he would benefit from this one or that one, always updated us on his progress, or any changes in his rate or formula or labs, and even gave us her private cell number so that we could message her anytime with our concerns or questions, even encouraging us to contact her if anything came up on evenings or weekends. My husband and I were both incredibly impressed by her knowledge (recalling so much information from memory, or doing math calculations in her head) and my entire family found her to be so kind and compassionate. What a perfect combination of qualities to have in a dietician, and what an asset to Sunnyvale Gardens she is. Another 5-star employee for Sunnyvale Gardens! Should we need another rehab facility in the future, we hope they will be there to care for our family members.

Fantastic experience: Facility is nice and clean. Staff are friendly and they offer private rooms. New management is awesome and I’ve seen lots of good changes but number 1 focus is patient care.

Fantastic experience: It is very difficult to even consider placing a family member in a skilled nursing home. Especially when they are normally very independent, but Ms. Alana of Sunnyvale Gardens went out of her way to accommodate our last minute visit, answered all our questions and gave us a tour of the facility. We left the complex feeling relieved and in good hands.

Fantastic experience: I toured the facility to find a nursing home for my grandma and the receptionist is very welcoming. Alanna toured me around and I gotta say it felt right. The facility is nice and clean and they offer private rooms. I chose this facility for those facts.

Fantastic experience: My mom was here for about two weeks post op total left knee surgery in June. Nurses and house keeper and even DON are very nice and welcoming. The place is clean, meal serve two times a day. My mom does Physical Therapist and RN changes her wound bandage everyday.

Fantastic experience: I visited Sunnyvale Gardens Post Acute today and was very impressed. The facility was very clean and the employees are seemed very happy and excited to be there. Overall a great experience!!

Negative experience: Very unprofessional service. Very bad food - better to starve. Staff ignores your needs. I had to scream Help me from the room and they would still ignore you. Didn’t give pain meds on time. Which is essential for best care and out come.

Positive experience: Both of my parents have gone through the hospitalization-to-post-hospital-physicaltherapy-rehab-skilled-nursing care protocol. ManorCare (now Promedica) Sunnyvale has been a relief for our whole family. From front desk to admissions to billing to p.t. to nursing to nutrition&kitchen to activities to physician liaising and the relatively newer (2.5yrs) managing social worker who brought a breath of fresh air in our opinion, Manorcare Promedica Sunnyvale is now and has been for a few years now, terrific. Perfect? No. But ManorCare Promedica Sunnyvale has been terrific. The facility went through a refresh and the halls look nice. The rest of the facility needs improvement. It's the staff that really bump up Manorcare Promedica Sunnyvale to Terrific for us. The individuals work so hard and are so caring in an industry that is so hard, they are amazing to us. Years ago, I didn't feel this way because of a few staffers who were simply not good. But since they were replaced, terrific! Hopefully Corporate will complete facilities improvements (which would earn the fifth star rating), but overall, because of the staff, my parents wouldn't go anywhere else. ~Daughter of a several years long-term skilled nursing patient and three times post-hospital pt rehab skilled nursing patients.

Negative experience: Do not go to this place. I would give it a zero star. At first, the marketing tour was good - not many patients (for many reasons) & looked cleaner floors but facility is old & Internet is bad. Then we found out the staff were unfriendly w/attitude, were lazy & asked the family to do the task like cleaning including housekeeping, didn’t give shower as ordered until family chased it. PT & OT were lazy and didn’t do as billed as a rehab center. Family had to do own therapies & then had to argue with their poor assessment when patient was capable. Speech therapy just came and went in half a min. Also in wet diaper with urine & poops for hours. Always forgot to put anti fungal power/cream until asked. Meals were bad. If u left your family member there alone, there wouldn’t be any knowing what was missed.

Positive experience: Mediocre service, mediocre food., annoying rules.

Fantastic experience: Having been a nursing home patient (broken back), I have experience with them and Manor care is one of the good ones. The administration is professional, kind, thoughtful, and wonderfully accommodating to each patient. My 98-year-old mother came to Manor Care from El Camino hospital following multiple pelvic fractures and a heart attack, and they went way out of their way to make sure everything was working for her and our family. Every request we made was met with kindness and a sincere effort to accommodate. Those same values extend to the nursing staff who are high caliber. They too were warm, helpful, professional, and responsive to my mother’s many physical and emotional needs. They also collaborated seamlessly with our Hospice of the Valley Team, and the admission and billing offices resolved a misunderstanding with great integrity and customer service. I would feel comfortable checking myself into Manor Care and recommend them to others. They are highly rated by US News and World Report and at the top of hospital recommendation lists. We are incredibly grateful for the 5+ weeks mom spent there, and even as a hospice patient she improved under their care.

Positive experience: My father has stayed at Manor Care twice, once after having his defibrillator replaced and getting excellent PT and OT therapy to help with his arm strength. Just this month he was at Manor Care after a heart value replacement fo addition rehab and therapy to help with his strength and balance for Parkinsons. The Nurses were very attentive and assisted him through out his time there. I do recommend Manor Care for rehab and therapy.

Negative experience: DO NOT send your loved one to Manor Care if you have any choice in the matter. If the person is not fully ambulatory or unless they can completely care for themselves and they are just getting therapy- this is NOT the place for someone you care about. No-one answers the phone, staff is rude, they do not keep primary doctors informed, they are solely focused on collecting insurance payments for the stay, and people barely do their jobs. You will wait for your meds while your nurse is on break and no-one will cover, they do not follow hospital discharge plans, and they do not care. If you don't have the staff to properly take care of people then STOP accepting patients.

Fantastic experience: Great facility. Staffs always try their best to help you out. You can tell it is not only a job for them. They love their job- helping people. Special thanks go to Meenu, Moon, Dianna ... who have worked with our family very closely. Definitely highly recommend this facility.

Negative experience: My father is there. They never answer the phone.

Fantastic experience: I came here after a bad experience at another skilled nursing facility, so I was skeptical. Everyone here has been kind, considerate, encouraging and helpful. Yes, you may have to wait 15 to 20 minutes after you press your call button to have someone come in, but everyone is busy taking care of so many needy patients! Plan on it! PT and OT have been working hard to get me to be able to take care of myself at home. The food is excellent. The facility is clean. Everyone introduces themselves and is kind, friendly and helpful. As the night CNA, Mila, said, I need to treat this as a vacation where I'm not at home doing the cooking, cleaning and all the chores. I will miss the encouragement that I find here when I am able to go home.

Positive experience: Therapy workers are the best! I have never been helped like this in years or ever! They are knowledgeable, and really work with you! Nursing staff is excellent, no one has a attitude, always a kind word and a smile! And then we come to the CNA’s! They are always there 24-7 for you , and again never a complaint, I Was there 3 weeks and it went fast! I have never been in any hospital except here without a issue! I highly Recommend Manor Care for anyone who needs the help!

Negative experience: Do not send your loved ones to this nursing home. They will be neglected, isolated and god knows what else. The only thing they do for them was many covid tests. Something very diabolic is going on in these nursing homes. My mother went in for wound care and died mysteriously a week later. Is there anyone left who doesn’t know what goes on with these nursing homes yet? God help us all.

Fantastic experience: I went her in April of 2020 staff from Nurses to nursing aids to PT & OT and staff in front office all was great. Only issue I had was the bed I’m a big guy and had a total knee replacement surgery and have bad hips and the mattress was way to thin they need to invest in a bariatric air mattress which adjusts so pressure points move so lower risks of so patches. Having other knee done on 9/10 and they hope to send me to Manor care on 9/12.

Negative experience: Extremely Under Staffed ...1 RN per 24 Patients with 1 LVN/CNA per 12-14 patients.

Negative experience: I should have learned my lesson the first time I actually really like the PT and OT but the rest of this place is a joke! I was transferred here from Stanford last night having a total knee replacement- it’s 1:00pm the next day and I just got my 1st dose of pain meds!

Negative experience: No one awnsers the phone, I walk in 4 people at the desk. Walk into my wife's room light on, shes been without care in the restroom for more than 10 minutes.if this wasn't a choice from the hospital for post care, I would have just brought my wife home! Will never recomend Manor care ever. Just check the BBB and with the advocates of this home.

Negative experience: My wife who was sent to manor care in Sunnyvale from Stanford university . Was there one night the next day she had bad fall was sent back to Stanford university. She is fall risk and the staff lift her standing by herself and she fell. Ended with a goose size knot on her forehead and a fractured knee. After 4 months at Stanford university she was sent back to manor care, at about 6:30, my wife sent me a message that she told the nurse she needed to help to use the bathroom. Instead of helping her, the nurse left to go and get a cna, the nurse came back 15 minutes later, to make a long story short, my had lay in her own urine for almost a hour before she got. I wonder what outh that nurse took and what part the word care she dosen't understand. ( UNACCEPTABLE)

Negative experience: my mom’s passed away in this rehab Long story, if she be alive again , never will not take her there again day by day everything changes. My mom's stop eating / talking in last 2 months ( up and down) she live in this rehab for 7 years with many up and down here we totally was satisfied but this days i see many issues that need Management care. specially Doctors. my mom's last doctor Dr Mery Manavalan was very good with her for many years, also her last American Doctor was the best ( i forgot his name) unfortunately this days her Dr is Mr Wood! He is most negative Dr ever i see, over 20 times i called him in emergency and leave messaged ( in 20 days) .. nothing! after some arguments with Nursing station, they called him and same day he called me, without any excuse, he said i cannot visit or do anything for your mom's except if you want order for hospital! i said Dr excuse me, but in last 3 months your her dr even one time 1 min you did not meet her!.... can i switch you to Dr Ayati ( another Dr here) and he shut down the phone to me!!!!!!.... I talk Dr Ayati directly he said after Christmas i come to visit her, by the law but just for one time as i cannot be her Dr!.. time passed and over 10 time Ayati told me i will come next week... but near 3 months passed and nothing happened by them!!!.... i talk two time with Dr Hussain , she was so polite but refused to visit or talk for her!!.... even for a mins.... really they need to go to first class to learn about meaning and act of responsibility.

Negative experience: So far they are lucky to get one star. The Dietitian thinks he can diagnose illness and ignore instructions from the hospital doctor for dietary restrictions. There is no clear chain of command after 5 Pm on Friday until 8 AM on Monday, and staff are not empowered to find solutions to problems. The contracted pharmacy Omnicare supplying patient medications is located in Lodi California, at least a 3 hour drive away, and ManorCare is not allowed to use any of the 14 pharmacies within 5 miles for urgent medications. The Omnicare pharmacist also thinks he or she can disregard a doctors orders and withhold medication.

Negative experience: My Father was a patient at this nursing facility in1992.I sure hope it has changed since then.I had many problems with the staff and Doctor there then. I would not recommend this place to anyone .The poor dying patients are not given any respect and the families were not considered at all.Very shameful and very unprofessional!

Fantastic experience: The best place my dad has ever been too. They took great care of him and every patient there. My dad's mobility improved gradually after he got his surgery. Many thanks to all the staff.
