Negative experience: My brother would have died there if we didn't visit and see how sick he was. His catheter was neglected and he had a very bad infection. Not what he needed on top of recovering from surgery.

Fantastic experience: Staffing was excellent on each unit, the residents are happy, coworkers are getting along on Unit 4….makes for a great day!! We can strive to keep this energy up so our facility can be rated #1 of course weeding out the bad apples.

Fantastic experience: We went to visit my brother in law who is having rehab. It was beautiful driving in and we were pleasantly surprised by the cleanliness and the check in was fast and the front desk was very friendly and helpful. Rooms were clean and spacious.

Positive experience: Third floor staff is neglectful and have plenty of attitude when asked to do something like put on clean sheets or clean up a mess. Everything wrong is blamed on the patient. "It's their disease " is the common refrain for why clothes are inside out and backwards, why clothes and bedding are soiled with feces. The smell of feces hits you in the face as soon as the elevator doors open. Staff is unresponsive when help is requested. They seem resentful for being asked to get off their phones and do their jobs. Staff smokes pot outside on their breaks. If you aren't allowed to drink while on your shift why are you allowed to smoke pot? They don't care about the patients and resent the fact that you do. I hope someday they are cared for in the same manner they're caring for others. HOWEVER the 4th floor is a completely different world, so I'm revising my rating. On the 4th floor they really care about the patients. The attitude is cheerful and caring. The patients are clean, the rooms are clean, it's 180 degrees different. Why the same standards aren't enforced on the 3rd floor is a mystery. I'm just glad I no longer have to be stressed about my loved one being abused and neglected.

Negative experience: Should not be considered a skilled nursing facility. The skill, compassion and leadership needed to care for those who cannot adequately care for themselves is definitely lacking in this facility. There are some dedicated and professional employees but they are compromised by ineffective coworkers and poor/absent leadership.

Fantastic experience: This was the first time I have visited Central Park. I was there to accommodate a resident with his wheelchair, I was impressed by the courtesy of the front desk and the ease of getting the information I needed to complete my job. Than the staff on the floor was excellent at accommodating me to complete my task. Central Park is definitely a great facility.

Positive experience: My experience entering this facility was pretty ok.. excellent treatment from the few staff members that I had contact with.. overseeing and caring for our elders and yes, some young adults that cannot care for themselves is extremely tough and even exhausting.. so hats off to the folks working in this Nursing Home industry.

Fantastic experience: A close friend was transferred to this facility and I was amazed upon entering of the size. It is set up to be so wheelchair, walker , hospital bed friendly and enough space to breathe ! The rooms are big and the hall space is fantastic. Friendly nurse staff and outgoing helpful front door man! Parking was convenient and love the self check in ! Scale of 1 to 10 , 10 for certain! Elizabeth Nagy

Fantastic experience: Upon arrival I'm always greeted by the workers and they are always helpful if you have any questions I have noticed upon arrival each visit the place has been clean and patience in the lunch room are taking care of I find Central Park has a great atmosphere on the outside as well as the inside I've noticed the nurses are very caring and friendly with a patients

Fantastic experience: My 94-year-old mother had a stroke and was sent from the hospital to Central Park 15 days ago. She has always had an aversion to nursing homes, so she was skeptical of the care she'd receive there. I've visited her there a few times and I've found the place is very clean, and the staff is always friendly and helpful. My mother tells me she really likes the food, and she says the care she's receiving is excellent. She has been undergoing physical therapy since she got there, and it's been so successful, after just 15 days of it she has improved markedly and she's ready to be released back to independent living in two days. All in all, her experience at Central Park has been a big positive with no issues. Five bright stars for Central Park!

Negative experience: My brother was recommended for rehab at this facility and a few days before he was to come home he died. I have nightmares because of his death. I know the care there was terrible. Staff never responded to the call bells, staff was rude and unhelpful. If call bells were answered it was always after waiting for over 45 minutes or more. I think that’s what happened the night I lost him. He called for help and no one responded. I will always blame them for his death. I despise that place. As far as the administration helping don’t bother. She never responded to my calls and is useless. my brothers personal phone was never returned. Someone stole it and is now using it. This place is a joke.

Negative experience: My father spent a little over a year on the dementia floor. Mostly left alone facing a wall. Soiled bedding as well as himself. Other residents walking around soiled. Feces on the floor. While the staff sits around on their phones. Constant concerns with management went no where. While leaving one day after visiting one of the employees commented on the elevator she wouldn’t even leave her dog there. My father had numerous bed sores which one became septic and he passed away. Unfortunately we did not have a choice where my father was placed due to financial situation. This institution is now trying to bring my fathers estate to court to pay his bill. My father had nothing or we would have put him somewhere that would have taken care of him.

Fantastic experience: My mother has been a resident at Central Park for roughly 10 months. There have been many positive changes over that time and I know she is in good hands. If I ever have any questions or concerns, I can always ask staff or call the desk and everyone has been more than willing to help and answer questions.

Negative experience: My mom was moved to the 3rd floor yesterday I came today to find her unpacked with stinking clothes on and the few that were in the boxes were not hers and they stank and had body fluids there’s no chairs in the room for them to sit down the were no chairs in the dining hall as well! Had to go out of the unit and grab one from the hallway, she was handed a plate of grounded fish sandwich it looked disgusting like dog food of course she didn’t eat it. Had to get the nurse to get her a plain turkey sandwich cause it seemed that they were ok leaving her without eating for the night, she was in the second floor they told me the move was better for her that was not my first impression.

Negative experience: I was moved her from another less reputable facility. I've come to find out, both are owned by the same company. This makes me feel less confident.. Some on staff have horrible attitudes , I've been dropped once, and fell out of my chair once. The food is horrible as well, o can't scream loud enough, they don't listen. Update- Staff attitudes ( weekend) is horrible. The refuse to do their jobs, anabuse d te patients. This place needs a serious state investigation.

Fantastic experience: Had the pleasure to coordinate with the team at Central Park Rehabilitation and Nursing. Customer service and communication: 5 stars!!! While visiting, the staff were so welcoming and assisted with whatever I needed, with kind hearts. Facility was very clean and coordinating with Social Services was a breeze! You could tell staff truly cared for their residents and continued that care with follow ups post discharge. Kudos to the entire team!

Fantastic experience: The facility allows folks to come in and go to the room , visit , and enjoy the betterment of the patient. That's trusting n understanding privacy of the patient. The staff is very friendly as well.

Fantastic experience: I felt good about the people there .it was clean and caring environment.the staff is amazing and very helpful and understanding.My mother seems very happy when I talk with her and Taylor cooper keeps in touch.i am happy to say I am very pleased with them. If I felt any different about this place or my mother felt uncomfortable there she would not be there.And I thank them for their hard work because that job is not easy. Negative experience: My parent received care there transfered from the hospital for rehab, and ended up back at the hospital. There's no staff. The staff that is there is short with you when family is visiting. They were very neglectful hence why my parent ended up back in the hospital. Never again.

Fantastic experience: I can say this in high regard that everyone from the staff to the case workers and managers have been helpful regarding my families situation. This is the second time our father has been sent here for rehabilitation and it goes without notice how hard they are going to guide and my back to health. I wasn’t familiar with rehabilitation/nursing homes in the past but with this experience I can say that I am glad my father is at a place where he is taken care of. I have no complaints.

Positive experience: My friend is there getting therapy for various reasons. I’m just upset that whatever is brought to her or what she and her family have brought with them as far as phone chargers and little gifts are being “lifted” from her room. That does not bode well with her family and friends. Needless to say management has an idea who is lifting these things and they should have cameras. We realize good help is hard to come by. Too bad they don’t have a conscious

Positive experience: I came to this rehab facility when I got released from Upstate Hospital because of no rooms were available . I have MS and had a UTI. First of all the staff(nurses, aides, p/t and o/t and cleaners) were extremely nice and friendly!! With that said nights and weekends are EXTREMELY different! Still the aides are very nice but EXTREMELY short staffed! I feel really bad for the nurses and aides for that reason. Long story short it's not easy for me 2 walk with ms but a UTI makes it Impossible to move, lift my legs much less walk.

Positive experience: The one thing that I do worry about is the nails and the aids. My mother has been scratched several times by nails of certain aids, trying to clean her or dresser. Their nails are too long. There are other nursing homes that demand the aids to have their nails short. I would like to know what your rules are.

Fantastic experience: Both my Aunt and Uncle are there and the staff is kind and compassionate. Taylor, the social worker, is fierce and passionate in her roll as a true patient advocate. She handled a problem I experienced with expediency and professionalism. I would recommend this home for anyone wanting peace of mind.

Fantastic experience: My father has been in Central Park rehab for about a month and we can not be happier with his care. Everyone we have come in contact with has been absolutely wonderful, respectful, and genuinely care about what is best for my father. His case manager has been absolutely wonderful the best case manager I have ever had the pleasure to work with, she is caring, compassionate and on top of her game. She has been upfront and honest about where we stand and what is best for my father, it has been so much easier to navigate this whole process and I can’t thank her and the entire Central Park rehab staff.

Negative experience: I would rather die than be trapped in that place. Employees don't pay attention. Smells. Dirty. Broken wheelchairs for tenants. Go to a real rehab. This place is a joke

Negative experience: I’d give 0 stars if possible. Neglected my grandmother & let her oxygen run out for unknown period of time. Sending her back to the hospital & no sympathy. One nurse was in the way of ent , when asked to move gave attitude. She was left in a chair for hours when she’s a fall risk. Hunched over in a chair leaving a bruise on her forehead from leaning against something for a long period of time. Send your love ones here only if you don’t care about them.

Positive experience: I am with United Healthcare, we partner with Central Park, and I am very grateful working with Asst Director of Social Work, Taylor Cooper. Taylor goes above and beyond for her residents, making sure they they have access to all that is available to them. She takes the time to talk with her residents and listens to their needs. She is a very compassionate and empathetic woman and Central Park is lucky to have her advocating for their patients and families

Fantastic experience: Went to visit my mother for the first time since she arrived there. The security woman filled us in on the procedure and called the girl we needed to meet with. Everyone we came in contact with was very nice and answered all our questions. My mother didn't have a complaint or any worries since she had been there. Update: it was hard to put this in words, I can't say enough about Taylor this womani is AMAZING !! She has worked with us in a way no one else has. Not even the nursing home I took my mother out of came close. The staff is right up there with Taylor, they are on top of things like nothing I have seen before. Rehab dept is great and getting my mother to do things I couldn't at home and that is so awesome. Derrick has pushed her and not let her slack at all. I wish he could come home with us to keep her excited about getting stronger.

Fantastic experience: My mother in law is a current rehab resident and I can not say enough good things about this place. From the first phone call with her social worker Taylor who we love, she goes above and beyond making sure that our family is happy and taken care of,and at least 1 call a week for updates, to the rehab staff Derrick, Tammy and Andrew they are all amazing. We couldn't have asked for a better place for our mom to get better. Thank you so very much...

Fantastic experience: As a housing facility, we recently had an interaction with the Asst. Director of social work at the facility. She was not only kind, but she cared about the patient. She took the time to call us and make sure things were going the way they SHOULD be going. She asked about the tenant's apartment and contacted his family (that he hadn't been in contact with in over a year). She called us back several times and kept us up-to-date on what was happening. It's one of the only times as a Property Manager that we have had a social worker call us and spend so much time making sure the best options are happening for their new patient.

She is always cheerful and welcoming and makes the mandatory Covid testing quick and painless. I look forward to seeing her on visit to my friend at Central Park. Negative experience: I as well as my family members visit the facility several times during the week. It's all good until you get on the floors. Lack of staffing is clear as we often have to bathe mom..make sure mom has decent food... Make sure she is clean and is in bed at a proper time...make sure no one is in bed with her! No..we are not there every night and we worry.

Fantastic experience: I became the Administrator at Central Park Rehabilitation and Nursing Center in the beginning of June. As a new face in the facility I was welcomed with smiling faces and open arms. There is so much longevity in the staff allowing for consistency and reliability in caring for our residents. I am so happy and proud to be apart of this team. If you are looking for employment with a tight-knit group of people and many exciting events for both residents and staff, come join our team!

Fantastic experience: I have been employed at this facility for over 20 years. I have had many different roles, CNA, receptionist and recently medical records. Throughout these different roles, I have enjoyed the many different resident here and past and have enjoyed the team work displayed from the employees. This facility has been my life away from home and I would go above and beyond for Central Park.

Negative experience: Not impressed,need to update staff on their duties. They are there to take care of the residents not ignore or verbally abuse. They need to realize where their paycheck comes from. I could go on forever.

Negative experience: This place is disgusting. My mother is there. Stained pillows. Sheets have holes. 23 minutes I waited yesterday for someone to take her to the bathroom. By the time they came I had her to the bathroom. Changed. Her sheets changed. Her depends was so soiled it smelled like Amonia. I can go on and on. I will be reporting to the authorites. God help the patients with no advocates that are there. It is sickening. I have photos . I hope the owners profiting are enjoying their miney. Karma is waiting.

Negative experience: If I could give no stars I would gladly. My mother was just there recently. She had brain surgery and needed PT and OT after this surgery. She has been a caregiver to the elderly for over 20 years. My mother laid in her dirty clothes for days. One day it was noon and no one had come to help her out of bed. Just laid there. She called my aunt after 4 days and told her to come get her out of there and take her home. The facility was not cooperative with this request and my aunt had to sign her out. Thank god she is now home and getting the care she needs. My mother is generally a very positive person and is always dedicated to doing what she needs to recover. She has been battling cancer since October. Now she is not the same person she once was. I am beyond upset and disgusted that this facility is operating so terribly. I feel awful for the remaining patients under their care. They should be absolutely ashamed of themselves. Also all of these five star reviews by the staff members are ridiculous and misleading. What a joke to do that!

Negative experience: Was a resident there in Rehab. Rehab staff was great. Most of the nurses were very good. My surgeon told me I had some of the best wound care he had seen. Thank you Maria!! Hands on care was not always great. Sometimes required long wait times. Rooms were not kept clean. Hopeful with the new director things will improve.

Fantastic experience: I am the director of activities and I love coordinating events for our residents, families and staff to enjoy! Today we enjoyed live entertainment, popcorn and refreshments at our first happy hour in our new space! I've never seen so many smooth moves in one room! Im so happy for the opportunity and I'm looking forward to creating a lot more moments to enjoy!

Negative experience: My grandmother was just in there and NO HUMAN BEING should be treated the way she was!!!! Staff left her on several occasions and was not bathed or changed in the FOUR DAYS that she was in there and the room was DISGUSTING! Wouldn’t even give it one star but that was the only option. Please if you are trying to find a place for your loved one to go, PLEASE DO NOT SEND THEM HERE!!!!

Negative experience: The worst facility you could ever send a loved one to. If you are thinking of it, run. So many issues, concerns, lack of staffing, filthy conditions, etc. Want to talk to someone at this facility? Think again! You can leave voice messages for days, send emails over and over again and receive not a single response or call back. But….let them want some money from their residents and I promise you you’ll be contacted! Will forever be disgusted at the care my father has received

Negative experience: My great grandmother stayed here for almost 3 months, over that time she had fallen with no true story behind what happened. My mother couldn’t get a hold of anyone about concerns with them all of a sudden cutting her pt/ot although the insurance company said she was still covered. At the end of her stay we had to quickly remove her, as they all of a sudden told us she was not covered for almost two months for room and board. This was not told to my grandmother, nor my mother who talked to many people working in the facility. My grandmother just received a bill for nearly $30,000! You can’t just house someone and bill them $400 a day without telling them! There’s a bunch of fraud going on in this place and they’re scrambling to get money from helpless elderly.

Negative experience: I was a resident there and I wouldn't send a loved one to be in their care. This place is horrible! I laid in soiled diaper and bedding for hours and developed severe diaper rash. The staff is mean and cruel to patients and management is the same. I have pictures to prove it. They were giving me medications that I didn't know what they were. I was there to get rehabilitated because I got sick and couldn't walk. Rehab was for 2 weeks then you are left in a bed. Patients (men) were coming in the room all night and they were understaffed. They were not wearing masks during covid. If you complained about anything you payed dearly for it. It was 30 outside and they took my blankets every time I had to leave the room for a procedure. When my daughter was finally able to release me they prolonged it as long as they could and even argued with my daughter saying I had a pace-maker which I do not. Please do not send your loved ones here.

Negative experience: This facility is owned by the same owners of Van Duyn in Syracuse. The are on the list to be focus facility, it’s all in management but no one is talking about that.

Negative experience: I WOULD GIVE ZERO STARS IF I COULD. Please do not send your loved ones here! My grandmother has been severely neglected, and showed up to a doctor appointment this morning covered in food, urine, and filth. The doctors were stunned!! They said she has not been bathed or properly cared for in a very long time. We are taking matters up with a social worker and hopefully getting her out of this disgusting place. Also, my grandmothers belongings have consistently been stolen from the so called “caregivers.” I could go on for days, but please do not even consider this place for a minute!! Edit: I had two other family members that I know for sure left a negative review. Very interesting that those aren’t being shown.

Negative experience: If you are having trouble contacting the facility and/or having them take any action, please search for the support group on Facebook to get in contact with other families facing the same problem. Separately, I would recommend contacting the NY Long Term Care Ombudsman Program (LTCOP) for Onondaga and speaking with the coordinator.

Negative experience: My father went in to Central Park after a hospital stay in March 2021. After rehab, the rehab facility sets up aftercare with a nurse and/or therapy. Central Park called Kindred At Home, and Central Park was told that they were not able to take on my father as a client. My father was released back to his independent living facility by Central Park. Since then, 9 days later, there has been no contact with Central Park for help with a nurse or therapy. I"ve called, my sister has called, his insurance company has called Central Park. There has been no response from Central Park. My father is 86 years old and aftercare is very important. Unless you have tons of time to track down care and continuously follow up with Central Park then it's a good option. I expected follow through and there wasn't any. I will never work with this facility again.

Negative experience: Do not send family members to this nursing home if you hope to see them again. The covid virus is rampant in this place and every week someone becomes infected. This means that your loved one must stay at least 14 days before the can be released! If someone else is found to be positive in the 14 days, the clock goes back to zero, they are held for another 14 days. Over and over and over for the last year, residents who are well enough to go home have been held prisoner without any way of going back leaveing !! I have tried numerous times to remove my loved one from this place and bring her home but the management continues to hold her hostage. By the way, she has never been found to be positive for the covid virus. Even though she gets tested twice a week . Can you imagine what's it like to be held prisoner without any visitor s

Negative experience: My mom is there and I can never get ahold of anyone there. Phone just rings and rings and If I get a answering machine no one ever returns the call. Its like moms in jail. Its funny they only respond to 5 star reviews on here. They are on top of getting payment.

Fantastic experience: I'm glad I found this place. I can't say enough about Central Park Rehab and Nursing. Upon learning that my mom would not be able to return home with me, the search began. I visited the three facilities that accepted my mom, and the difference between the two larger nursing homes and the smaller Central Park could not be more clear. First, Central Park had the better Medicare rating.. but it didn't stop there. The initial tour by Kevin at Central Park was genuine and sincere. He listened attentively and paid attention to our questions. Central Park is a smaller, four-floor facility, and everyone seemed to know everyone... I mean patients to patients, staff to staff, and staff to patients. I could see as a business owner that a smaller facility would be easier to manage, easier to navigate, and just simply more friendly and personal. At the larger facilities, the initial tour seemed to be just another distraction from their busy work day. The intake social worker Renee was fantastic too. Almost everything that could be needed is on-site... nurse practitioner, meds, physical therapy, social workers, entertainment, etc. Visitors are treated as friends, not a nuisance. The staff in general is really, really excellent, friendly, and helpful. My mom had dementia, and often is was difficult to leave her... that is, she wanted to come with me. All I had to do was ask a staff member for help,and they would know what to do. They would gently and lovingly divert my mom's attention from me and, for example, say something loving like, "Come on Rose... time for dinner soon"... and I could be on my way. I can't say enough about Central Park Rehab and Nursing. My Rotary Club friends recommended it to me, and that was more than enough of a recommendation for me. I brought my mom's priest in to see her, and as we left on his first visit he looked at me and said, "THIS IS THE CLEANEST NURSING HOME I HAVE EVER BEEN IN". And coming from a 65-year old priest that, is saying a lot. Accidents cannot be avoided, but they are tended to. And if it was busy, once the staff was made aware of a situation, it was always addressed immediately. If an issue arose, we were contacted immediately by the charge nurse, which was usually Sue. She is also fantastic, with excellent communication skills. She always kept us informed, but in a compassionate way. The finance department was SUPER to work with, which makes life so much easier. Central Park was the right place for my mom. We were so grateful for all they did, we brought in a catered dinner from Dinosaur BBQ for entire staff. I made a certificate of appreciation for the staff and presented it at the dinner. It is still hanging on doors there. I am very, very glad I decided to place my mom there. Lastly, as for the location... please don't worry about it. Central Park Rehab is a secure and respected facility, close to downtown and convenient to the highway. You will not have trouble there. There is an ample-sized parking lot, and I never felt unsafe there.
