Elmwood Nursing and Rehabilitation Center is a skilled nursing and rehabilitation facility with 77 beds, located in the beautiful community of Berkeley, California.
We are located near Alta Bates Summit Medical Center and Kaiser Oakland. Our staff has been providing a combination of specialized therapy services and skilled nursing care in a calming and quiet environment for over 45 years.

Fantastic experience: I'll bet there are plenty of people that find themselves in my predicament. I'm middle aged plus, a couch potato, & too casual about activity & health. In October a condition rendered me bed ridden for 2 weeks. To my surprise I had muscle atrophy & found myself unable to stand. I called the Berkeley Fire Department who took me to Alta Bates. After a few days of steroids etc the doctor advised me to get help from a skilled nursing facility & gave me a long list of places. After consulting hospital staff, friends, & and attorney friend I decided on Elmwood Nursing & Rehabilitation Center in Berkeley. That afternoon I was medically transported to Elmwood & placed in a room. I spent the night wondering "what has happened to me? why am I unable to stand up?" I was assessed within 12 hours by 2 very knowledgeable & positive staff members. The next day I met the full staff: Shanni, Charlie, Regina, Melanie & Jyoti AKA "The Fab Five Of Physical Therapy". I immediately began a daily routine of upper & lower body exercises, strength training, stretching, sit to stand, & walking. Under the staffs care my condition, confidence & outlook have greatly improved. I'm also impressed with their no guilt attitude, their team approach & their willingness to give opinions based on their own experiences. Also despite their heavy caseload, they find time to give extra attention to persons like myself that are nearing the end of their therapy sessions. In conclusion, I came to Elmwood Rehabilitation on a stretcher unable to stand or walk & l'm leaving on my own 2 feet.
Negative experience: This place is as bad as the reviews. My Aunt was admitted here a couple of days ago. It’s a severe problem getting assistance to the point that we have to call staff on the phone. I’ve witnessed limited staff, it’s a shortage or the staff are hiding. Not a place for a full care patient if no one is available to provide care. I will be filling complaints across the board starting with Kaiser.

Negative experience: Place smells like lingering raw sewage 24/7. After 20 days of being there, your PT is automatically revoked no matter how bad your condition requires it and the PT is lackluster aslt best. When a bed is changed from a big bed to a small and uncomfortable bed, they will lie and tell you it's the same bed you were in. Visiting hours end after 6:30. Getting a day pass takes a long time. Food is horrible. Patients go crying for 15min to a half hour at a time. Outside agencies won't hold the facility accountable. I'm working on finding a new facility tomorrow and wasn't expecting to hear about people dying from UTIs. It wouldn't surprise me if the place got shut down. Even the fire department medics told me the place was horrible and to not send her back when she gets out. I'm on my way looking for a new nursing home with the best reviews in the bay area. Elmwood is absolutely disgusting.
Negative experience: They killed my mother. They didn't tell me she was vomiting and not drinking. I realized she was vomiting when I lift her up by her lower back. I called the nurse, she didn't come for about 15mins. I had yell at them to come now. I had her sent to the hospital. All her veins had collapsed from severe dehydration. Alta bates called me that night and told me she passed.

Negative experience: My dad was here after a hip surgery. Appalling lack of care. Rude staff. Filthy.

Negative experience: I was there for almost a year, it was the most horrible hell of my life, it needs to be shut down, read the reviews they speak for themselves and that is the truth,, first example is my first roommate there had Alzheimer's and needed oxygen and they didnt check his tank and he was struggling to breath pretty bad, I had to call the CNA to check his tank,,also when he passed away they were supposed to remove him or me before that happened which they did not, the next thing I know they are in my room with a body bag taking him away, no kidding, it sucked watching him being removed in a bag, I truly believe that this place made his passing a lot faster, I have so many pictures of things there that should not be going on, I called the state and county a few times, they didnt like me, they wanted me out of there, the administrator there is a liar and she is fake, I caught her in several lies, if you want to loose weight don't eat there horrible food, I stopped eating the food there after finding weird stuff in the food, I took pictures, do your homework before you even think about sending someone there or if you choose to go,, please believe me,,,
Negative experience: If I could give 0 stars I will!!! This facility is horrible and the nurse to patient ratio at night time is ridiculous!! The management is horrible at communicating and does not care at all. The name of the facility does not live up to reality. I feel bad for the patients.
Negative experience: This place is dangerous. The staff are very nice but there must be a top down lack of organization and continuity of care issue because they nearly killed my mom by not giving her the blood pressure medicine she has been on for years and needs to prevent stroke and a-fib. They transferred her to the ER because her pulse and blood pressure were dangerously high. I’m so glad she is out of there and we will never go back. The care at Kaiser hospitals is excellent, and somehow when it gets transferred to offsite skilled nursing it all falls apart. Kaiser, this is a major oversight, and creates an extreme lack of confidence in your care.
Fantastic experience: Everyone here is friendly, courteous, and caring. They been very kind and caring with my mother. The bad reviews do not reflect their nature here at this rehab facility. Thank you for all that you do!
Negative experience: Horrible place, so unorganized, food terrible and care even worse. Couldnt wait to get out of there.
Fantastic experience: I was a patient here for a few months. They helped me rehab and found me new housing. If it was not for the therapy and social workers I may have been on the street.
Negative experience: Still have nightmares about this place. I was 12 years old visiting my dying grandmother/ best friend who was being "cared for" at this "nursing home" my grandmother was the most giving sweetheart until the day she passed.. never wanted to feel like a burden on anyone. Every single time I would come to Elmwood to visit her after school she would be in tears saying how the nurses in this facility treated her horribly.. money would go missing from her purse.. they would spray her down in the showers like she was a dog.. no care or softness was shown to her here. I remember feeling so helpless because I was so young and there was nothing I could do. I was finally able to beg someone in our family to take her in to their home so she wouldn't have to endure the abuse at this place any longer. If you love your family member please never leave them here. The nurses do not care about your loved ones.
Negative experience: This facility is dirty, noisy and unsafe. No patient should ever be placed here for care. The top edges of the walkers are crudy and have not been cleaned. The beds have little cushioning on the mattress; the side rails are only 1/5 th the way down the length of the mattress. So it’s easier to slip off the bed. The staff yells down the hallway at one another and the residents are conversing in 4 letter invectives. I was so physically scared for my life, I called friends to come and get me and I checked myself out. I was safer unattended in my own home.
Negative experience: I’ve been in the Facility admitted for healthcare I’m Kaiser member. Record 3252356 I’ve been here. My experience here. Regarding meal are so inappropriate. I’m diabetic I’m presently on dialysis hypertension. Heart failure condition. Kaiser just performed hip surgery. My left leg I have a lot complications. So far I ve my diet is so horrible I’m inappropriate. All they have feed me rice beans piled on top of each other I’m presently in room 208 A my sugar has dropped to numbers below 50 and I’ve complained to Staff with out any regard or Care when you ask questions the staff gets. Angry then they leave you in wheelchair. They won’t touch You because is not their job. The lady in the room with 208B. She got diarrhea with blood. If this is read by A person in Politics. That cares about seniors that have no one to take care. Y
Negative experience: My friend has been in this facility for two weeks. It might be that the facility is short-staffed, but my impression is that the work is very poorly coordinated. There seems to be a lot of people working, but whenever my friend makes a request, and he's made the same request to four or five different staff people over two days, the person invariably says, 'ok, yes, i'll take care of it' and then does nothing. if my friend (by the way, he's a patient guy not given to complaining or being demanding) identifies the person and later asks for them by name, the reply is always, 'oh, s/he's gone home.' and no one ever responds. this situation makes the patient and the visitor lack confidence that people there know what they are doing. at last, my friend's daughter complained for him and now someone appears in his cubicle now and then to ask, 'is everything ok?' but still the feeling we get at this facility is that it can't be trusted to even make sure that he gets his antibiotics or is made ready for the kaiser ambulance to pick him up to take him for his kaiser appointment. also, the food is awful. my friend has several relatives who are good advocates for him. what happens with a patient who is too ill or too infirm to make a request? does that patient just languish unattended? that's what it seems.
Negative experience: This place was awful. The staff didn’t seem caring at all. They lied to Kaiser about my aunt being able to walk on her own without help just so they could discharge her quickly. They told me she wouldn’t get discharged without a meeting between the nurses and doctors but that was false. I also witnessed a woman being left alone in the bathroom for a long time. She had to scream for help and had me flag someone down for her. I wouldn’t recommend this place to anyone.
Negative experience: This place never returns phone calls, they have constant turnover with management, the hr person is rude, and it's filthy. This place should be closed down permanently.
Negative experience: Watch out for Augusta! He is really rude, arrogant and very unprofessional! I'm filing an official complaint against him.
Positive experience: My Brother is a permanent resident at Elmwood Facility of 9 years the staff has changed over the years and still they can maintain good quality staff my brothers quality of care with this staff has made it possible for me to feel good that he’s getting the best care. Negative experience: I came there is supplemental help for nursing staff to see the roof leaking and black water falling I however stated my shift at about 9am I was Approached by a very threatening individual who Introduce yourself as customer service she said she was in charge of the building which I found bizarre because she was a CNA Who had worked in the building well anyway she presents me with a brand new schedule after I did three showers her explanation was that if medley or Clipboard was to come in there at a higher pay rate than what she gets they would have to do the fast majority of the work One of the other CNA’s who is two hours late wanted to come in on my run And as a result she threaten to beat me up if I came outside the building all the agencies ban together and said we would all leave if she was asked to change her assignment however she wasn’t ultimately asked to leave she was asked to be a shower aid they didn’t ask her to leave because she was on a double when she left from the a.m. shift to start her p.m. shift she was placed on a run were none of the patients was touched by the previous aid the customer service said you get what you get coming in at agency with a higher rate I wonder if management understands the impact this could possibly have on their Facility there are many shifts for Berkeley on both my registry dashboards just sitting there all they would have to do is remove one person and their building would be filled with supplemental help
Negative experience: This nursing home killed my mother they let my mother sit there didn’t give her no medicine didn’t feed her nothing and my mom passed away. I see my mother day before she was perfectly fine. The next day I went back my mother was dying been sent to the hospital in the ambulance. My mom didn’t make it through the night. I know they have new staff members there because my mother passed away 2011 but I can say one thing please don’t put your parents in the nursing home they’re not gonna love them and take care of them like we will my mother used to call us crying every day telling us how bad they treated her up there
Negative experience: Applied & offered a position here, which I turned down an offer at another employer to accept. What did this place do? Not return any of my calls or emails, essentially "Ghosting" me. It was a cowardly, low-class thing to do to someone, so all of the horrible & scary reviews and low ratings seem appropriate. Looks like I dodged a huge bullet! Find another place for your loved ones. You can do better: I did!
Negative experience: My friend, who was a recent patient there, had his bank card stolen from his wallet in his nightstand. The last week he was there, someone entered his room and stole clothes from his travel bag. This really bothers me because he is a disabled person and no one should be taken advantage of. He did say the staff were kind to him.
Negative experience: My grandma stayed here years ago after a surgery. The staff was careless and cold. The people directly assigned to my grandma were always so annoyed and didn't fake it. They didnt help her to the bathroom and hated cleaning her up. One time I opened up her meal tray and there was a huge fuzzy spider walking out. I should have taken a photo but I was in such shock. You can hear residents in distress and the staff not bothering at all or joking around resident that passed away throughout the night. They denied for a family member to stay overnight with grandma, we couldn't trust these people. A particular charge nurse prohibited visits after 8 PM however they stayed calling my phone number numerous times throughout the night because they couldn't calm my grandma down. They later arranged a meeting with me stating my family was interfering with the way staff worked and visiting too much, this meeting was managed under a filipina woman. I hated them and this place so much. I gave thanks to God the day my gmom was discharged from that awful sad place, a CNA commented they were discharging the family too. Well F u too.
Negative experience: Kaiser should be ashamed of sending their patients to this facility. I've had kaiser employees tell me they wouldn't send their loved ones to this facility. I was a patient there and left after 5 days. Its horrible
Negative experience: I worked "on call" here and this nursing home is absolute trash. Everyone who worked here two years ago when I did was super over worked and stressed out. Most of the nurses here were so rude to the patients and the cnas had way too many residents to take care of which made it more stressful as needed. The nurses never helped with any team work with the cnas. Other nursing homes I've worked at were team players besides this one. Horrible environment. Don't take your love one here.
Fantastic experience: I was a resident of Elmwood for about seven weeks this past spring. I found the staff to be kind, caring and efficient. My room was kept clean and when I n3ded help it was readily available. I found those in the P. T. department to be knowledgeable, supportive and encouraging. While I hope never again to be in need of a rehabilitation center, should that hope be dashed, I would hope that space at Elmwood would be available.
Positive experience: The level of care is far from adequate, however, the staff is overworked, underpaid and unappreciated. There is a significant lack of attention to the patient's needs. There really is no trust between the patient and the caregivers, and even less respect. But I will say it is a small step up from Princeton Manor where I was initially.
Negative experience: My grandma had a hip surgery and was sent to Elmwood, she compliant to me and begged me to move her out of that horrible facility, she said she wasn’t fed or cleaned, and most of all, she caught Covid in that facility, they had/have an outbreak but did not inform us or ask us to move my grandma out of the facility before she caught it. My grandma died on Monday because of that, she survived the fall, the hip surgery, the stroke, but she didn’t make it due to Elmwood’s lack of care and protocol to prevent the Covid outbreak.

Fantastic experience: They took good care of me after my accident. The Nursing staff were very capable and friendly. I was there from Feb. 2017 to Aug. 2017.
Negative experience: Residents are not regularly taken care of or monitored, really upsetting, unhygienic, and unsafe. Would not send my relatives here.
Negative experience: Angelie or Angelee is the inept staff member who seems incapable of allowing someone the opportunity to complete a sentence before jumping in to highlight her lack of knowledge and inability to tell the truth. Nothing she said was true, and, in fact, most of her asinine statements defied basic logic. As icing on the pathetic cake, this Angelie person hung up once she had her say. It's no wonder nursing homes are as poorly spoken of as they are. Unqualified slow thinking individuals lacking in common decency will continue to ruin what little reputation may exist. Thank you for the rudeness and disgusting, disrespectful treatment. I hope Angelee or whatever, gets an opportunity to experience what I did because of her. Zero stars is what I give this place.
Negative experience: If I was able to give zero stars then I would. My grandfather needed extended care past what we as a family could provide after a hospital stay. We left him here for less than 24 hours because of how completely incompetent this facility is. If you care for your loved ones at all you won't bring them here. Staff left my grandfather sitting in his own waste from 5pm to after 9am this morning without checking on him. He received no care during the night and was unable to access a phone to call us for help while they left him languishing. After complaining to the facility they brought in supervisor after supervisor asking us to give them another chance and try one more night with them. We refused. If you are charged with caring for the elderly you need to care for the elderly. Whatever staff they have here is being paid to ignore some of the most vulnerable members of our society. If these staff members don't wish to provide care then they shouldn't be paid for it. My poor grandfather shouldn't have had to suffer the discomfort and the indignity that they put him through and he should have had adequate access to a phone to call for us if he needed. I beg of anyone please do not allow your loved ones to enter this facility. It is a sham practice and I wish we had never considered them for "care." Someone should check into their business practices because I feel there are some glaring violations. I will post this review on every forum I can find until this “care facility” is held accountable for their substandard version of elder care. Their “care” towards my grandfather for the rough 18 hours that they had him bordered on abuse and I will not be silent about this experience.
Negative experience: Negative five stars. Phone line not working. RNs don't check on patient, leave meds at patient's bed, leave patient in wet bed for hours. Call light out of reach, when patient called out for help, they just closed the window. RN, social worker, and director don't respond to family's request to call for questioning. No where to file grievance. Patient was supposed to be transported in the afternoon, PM shift RN reported that patient's been picked up around 9PM, but night shift RN called passed midnight asking why the patient was still at their facility. Patient's relative had to help coordinate the care for transfer to Kaiser hospital. This is a Kaiser contracted hospital that should be closed. PM shift RN didn't get the patient ready for transport, the entire facility is full of problems, even their own staff admitted that. Nightmarish experience, unsafe facility, don't go there.
Fantastic experience: Was there 2 months everyone is very caring. If I need to go back I’d be happy too to
Negative experience: my mom is a high risk fall patient every time she needs to get up to use the toilet no one ver comes to help her, one time she couldn't get up on her own she pied in her bed she was wet and cold and asked the nurse to change her blankets, the nurse told my mom to wait until the morning. my mom stayed cold and wet all night. The evening staff gather in one area most of them were just sitting around, nobody is watching both entrances any one can come in or any patient can leave and anyone noticing
Negative experience: Incompetently run place. Nearly killed a friend due to overdosing on Coumadin. DO NOT let anyone you care about stay in this place.
Negative experience: Negligence. Understaffed. Unprofessional. Tended to ignore hospital orders. Was sent there for PT and gradual activity, but was left in bed and ignored for two days. "We don't do physical therapy on weekends". Our patient declined while there. Be sure to be there to supervise or your patient could be in terrible situation.
Negative experience: Negligence. Physical therapists not showing up, leaving patients alone in the gym and not returning. Insufficient care. This place needs its license revoked. Take your loved ones here if you want them to lose hope and die. Avoid at All Cost!
Negative experience: During winter, Freezing at night, cold water in am sometimes. Power went out - two hours + to come on. Was on shutdown once because some disease went viral (suppose happens) but some employee had me walk (on walker) on THAT side of facility which was supposed to be off limits. Think she was a temp or something. Staff response time slow (overworked). My bed broke 2x. I would never return. Did not tell me my time was up then was billed for additional days; I missed deadline to file with State.) Had to “pull teeth” to get documents signed for my employer. Social worker unaware of SDI rules, regulations; administrator - she unaware who signs SDI forms - it was them because they my interim care! Staff hardworking, caring; therapists outstanding, dietian worked with me. May be OK for some, definitely not for me.

Negative experience: I had a very bad ten-day stay here following knee replacement surgery. I was unable to sleep because of 24-7 lights and noise, they were unable deliver my pain medication in a timely fashion, there were festoons of dust over my bed and my asthma kicked in. Never again.
Negative experience: I spent two weeks here in 1998 because of serious sciatica. I just put in my health directive that I don't ever want to be sent there, because it was hell on earth. Don't ever go there or have anyone you know sent there. I was neglected, abused and lied to even though I had a group of three female friends who tried to help.
Negative experience: My Mother arrived on a Friday in stable condition. By Tuesday, she was sent to an ER and died soon after, apparently from a UTI that had spread. Why Elmwood let her situation escalate to the point of near death over a weekend is beyond me. My family's grief is unsurmountable. Do not bring your family here. Elmwood Death Center is a more appropriate name. "Care" doesn't live there.
