Signature HealthCARE at Jefferson Place Rehab & Wellness Center

Fantastic experience: Jefferson Place is the most efficient facility I’ve been involved with ever! Their staff are knowledgeable and dedicated immeasurably. My loved one was immobile when she was admitted and in no time walked unassisted. I can’t say enough about Jefferson Place and their compassionate care! Thank you Jefferson Place! You are highly recommended and appreciated in my book!

Fantastic experience: I absolutely love this facility! The staff are amazing and work diligently to care for the residents. Amy the administrator and Erin the DON are incredibly knowledgeable and are wonderful leaders! The facility is one of the most beautiful and clean of any of the many nursing homes I've worked in. I would recommend this facility to my friends and family because I trust the care they will receive will be top notch!

Fantastic experience: The place to be, a loving and caring homey facility. My uncle was here a little over 2 weeks for rehab, the nursing staff took great care of him as we would. He Received great therapy from an amazing therapy department. & has recorded great. He has transitioned perfectly back home with his dog who missed him very much. Positive experience: I was admitted from the hospital into Signature Care at Jefferson Place Rehab & Wellness Center for Rehab and Therapy. The facility is a very nice and cozy place. I was there for 28 days. While there, the staff welcomed me with open arms and made me feel like I was a part of their family. They took care of me when I needed it, however, I had to wait for a little longer on most occasions when I pressed the call button and ended up having some issues that needed to be taken care of right away from that, but the staff took care of me. The Physical and Occupational Therapists were all really nice, however they did get me up and moving when I really needed it. But sometimes they pushed me to the limit and I ended up overdoing myself to where I ended up getting sick, but I was okay after a days rest without any therapy. I was also welcomed by the Activities Director to be able to participate in the daily activities that they had with the residents, and other patients. I was able to meet some very nice people that live there permanently. They made me feel like a part of their family too. And all of the activities were so much fun to do. I did have issues with the food that was served there. I was not able to see menus on a daily basis like I had requested. When the head person from the kitchen came to talk to me about foods that I like and don't like or can't eat. I specifically told them what I can and cannot eat, etc. but they ended up serving the food to me anyways. I wasn't able to eat any of it. So I missed out on some meals at times because of this issue and was not able to get anything else which was not fair. I hope that this situation can be resolved really soon before it happens to someone else. Thank you Signature Healthcare at Jefferson Place for your hospitality. It was greatly appreciated. I will be back there for rehab and therapy after I have my surgeries. Thanks again.

Fantastic experience: We admitted our Mom after she suffered a stroke and we were unable to take care of her at home. Jefferson Place was great. They were able to get her eating again and did a great job of getting her comfortable.

Fantastic experience: Four months ago, my wife nearly died from staph infections resulting in MRSA and a retropharmengeal abscess which required a 4.5 hour surgery. In and out of consciousness, she was intubated and eventually received a G Tube. It was horrific and many times the medical team offered little encouragement. However, thanks to the efforts of the University of Louisville Hospital ICU , the Infectious Disease, and Surgical teams, she was released into the caring arms of Jefferson Place where she recuperated under the supervision of competent and well-equipped Occupational Therapists, Physical Therapists, Speech Pathologists (for her swallowing issues), and caring and dedicated nursing staff, she made a miraculous recovery. I brought her home last week where she is now convalescing. I take this opportunity to demonstrate my sincere appreciation to all who aided in her recovery. Jefferson Place is clean, modern, and well equipped. Clare and I are grateful to all who helped, but especially the nursing staff at Jefferson Place. Thank You!!! William Mefford Penrod, Ed.D. Fantastic experience: My father went to Jefferson Place after his hip replacement. I was so impressed with the care and rehabilitation services. The nurses were very attentive and caring. I highly recommend this facility for post surgery rehabilitation. Hopefully we won’t need to return anytime soon, but I wouldn’t think twice in the future!
