HealthBridge Arlington Heights is now offering Complete Decongestive Therapy (CDT) for lymphedema. While there is no cure for lymphedema, CDT is considered the gold standard for treating lymphedema and reducing any resulting swelling and possible infection. CDT treatments can include targeted exercise, manual lymphatic drainage, compression bandaging, skin care, and lymphedema-specific education to help provide patients with the necessary tools to properly manage this chronic condition. Our therapists are trained and certified in this highly successful therapy regimen.

Fantastic experience: I stayed at Transitional Care after leaving the hospital to recuperate from an motor vehicle accident. The facility is spotless, brand new. Each suite is private unless you request a two bed room for you and a family member or friend. Staff was attentive and friendly. OT, PT and Speech therapist were very knowledgeable and compassionate. I feel my recovery got off to a good start at Transitional Care. I would definitely go back if the need ever the need arose again. I have felt confident recommending Transitional Care to several families since my stay.
Fantastic experience: I had back surgery and my two weeks at Transitional Care contributed greatly to my recovery. The most positive experience was the physical and occupational therapy. Except for their days off, the same two young ladies worked with me for my entire stay. They were most thoughtful and professional. They knew just what exercises I needed with the result being to prepare me for independence at home. I could not have done as well with in-home or out-patient therapy. The entire staff, especially the nursing, was very professional, friendly, concerned, responsive and compassionate. This too contributed significantly to my recovery. The level of staffing was more than adequate. Another important aspect was the food. It was fresh, healthy and in appropriate portions. The facility was kept spotless and cheery, as you expect of a new building. Lighting and corridors were generous allowing for easy walking. The overall environment was very pleasant. Many of the patients were very welcoming to newcomers and friendly throughout my stay. The bottom line is that I recommend Transitional Care of Arlington Heights most highly.
Fantastic experience: EVERY room is private (only one bed per room) and nurses, OT/PT staff are all friendly and efficient. Construction was completed in January '16 so no nasty c. diff nursing home odors. Also, their dining area looks similar to IKEA's restaurant: electric fireplace, brushed steel furniture accents, black framed artwork on walls. Plus state-of-the-art equipment in their therapy rooms.
Fantastic experience: I believe this facility is one of the best facilities that I have seen very clean. Very respectable so clean you can eat off the floor. You can’t get no better than that. Patience is well taken care of the staff is great.
Fantastic experience: My sister was at HealthBridge following a fall that left her with a ruptured pelvis. When she arrived she couldn't get out of bed or take even one step. She left yesterday using just a cane for assistance. The nurses were kind and helpful and they were very attentive to her needs. The PT and OT was excellent - understanding when that was needed and pushing when that was required as well. I can't say enough good things about the social worker, Beau. This was a new experience for us and he provided so much help and support and was my day-to-day resource in coordinating care. Highly recommend HealthBridge if you need skilled care and rehabilitation.
Fantastic experience: Highly recommend this place for skilled nursing and short-term rehab! This was by far the cleanest facility I visited of 5 others in the area. Because they don’t have permanent residents they had a great nurse to patient ratio. My dad came here after having both feet amputated and a ruptured aneurysm. Their private rooms w/ mini-fridges made it easy to bring home cooked meals, and their wound care and nursing staff did a great job of tending to my dads complex needs. Their open feedback and no-retaliation policy make for a healthy culture where we didn’t feel unsafe asking for anything. They were also transparent about what our insurance covered from the start. Beau, their social worker did a wonderful job of handling my dads complex, urgent needs and my onslaught of questions. Our transition home was flawless as a result. Set up a tour with him and you won’t regret it!
Positive experience: My mother was at the facility primarily for regaining her strength which was lost during a recent illness and hospitalization. The excellent PT and OT staff exceeded our expectations. She seems stronger than her original baseline. The nursing staff was friendly although seemed to be a little stretched thin at times. The meals were very good and provided a lot of variety. The kitchen was very accommodating with dietary restrictions and requests. The social worker was very helpful in coordinating additional home care and discharge instructions. I would recommend this facility.

Fantastic experience: My mother, age close to 90, was sent to Health Bridge following a few weeks at the hospital and hospital rehab dealing with various infections. I thought her next step would have been to a nursing home Health Bridge went above and beyond expectations she is now in assisted living with only a moderate level of care needed. Entire staff was very competent in their jobs, and communication was very good. Was a flawless transition from Health Bridge to Assisted Living yesterday.
Fantastic experience: Quality people. Compassionate. Food is great it really is. Great therapist, Debbie, Nicole. Nursing treated me well. Social worker Beau was great. Excellent place for therapy and to get better.
Fantastic experience: HealthBridge provided excellent care, PT, and OT for our 94 y.o. mom following a fall. She was at Healthbridge in Arlington Heights for 13 days and, during that time, she received optimal care and rehabilitation. Nicole was wonderful with helping our mom rebuild her strength and Ray helped ensure she learned how to best perform her activities for daily living given her age and physical abilities. Beau, our Social Worker, was very helpful and connected us with Assisted Living resources early on so we could begin researching and visiting Assisted Living facilities. With everyone’s help, our mom regained her strength and was able to smoothly transition to her preferred Assisted Living facility directly from Healthbridge. Thanks for a great job all around!

Fantastic experience: I cannot say enough about the help I got from the administration in my brother's complicated case (shoutout to the social worker Beau who went above and beyond to help!). The communication was excellent, such a different story from the last facility. My brother made excellent progress in learning to walk again and I received regular phone calls from therapists and administration (even the doctor!). I only wish he could stay there! He has to move due to insurance reasons but I will move him back as soon as possible when the insurance paperwork gets worked out. A very nice facility and my brother reports all the nursing staff have been great as well. Highly recommend!

Fantastic experience: My husband had very good care at HealthBridge in Arlington Heights. He had a lot of PT, OT, and Speech Therapy. PT and OT showed a lot of knowledge and they were respectful of his limits as well as knowing how much to challenge him. They were friendly and personable, too. The Speech Therapist was also smart and knowledgable. She was very nice. You also did not go hungry with three meals a day and offered choices if you needed. My husband kept telling me how incredibly smart the staff was.

Fantastic experience: I saw an extremely inaccurate review by a Jeri Gunther, so I want to set the record straight. My name is Jeri Gunther and my mother is in the HealthBridge facility. Yes it's true the music's too loud when you are on hold, and Pablo was an exceptional nurse, but the review doesn't mention that they have treated my mother with the utmost respect and concern. She has had the best experience possible considering the circumstances. The Social Worker Linda Kinsella went above and beyond to ensure that my mother would have what she needed during her stay and have a smooth transition when she goes home. Linda also assisted in my out of state work FMLA paperwork. Dr. Abdo was an incredible comfort and helped us to navigate our mother's end of life and also provide her with comfort. There are many people I am sure who were on the front lines with my mom regardless of her situation. I thank all of you and offer this accurate review. HealthBridge has been a great comfort to our family.
Fantastic experience: I want to start this review by saying that I have personally met the Owner and Administrator of this facility. They both have Nursing back grounds and it shows. They love their employees and it shows. Now on to the review. My Mother was admitted here for rehab of 2 things. The first was removal of a knee replacement ( she has temp. Plastic in her leg at this time), the second being an ongoing wound that was half the size of a tennis ball. From the very beginning I met Christine (wound care nurse). She was the primary nurse for my Mom's wound treatment. I want to personally acknowledge how happy we are with her. She knows what she is doing, should be trusted to always do what is best for her patients recovery. Christine not only performed her job well, she helped my Mom heal the wound almost completely in 5 weeks. I should add that my mother was under the wound care of a very specific high-ranking individual at a nearby hospital that had three and a half months to attempt to help to heal the wound. Not only did that person not help heal the wound but in fact made it worse, so bad that my mother was admitted to the hospital with a severe knee infection which caused her to have her knee replacement completely removed. Christine is a tremendous asset to not only TCA , but to everybody she will ever come in contact with during her professional career. Thank you Christine from the bottom of my heart. Now onto the others who helped, hugged, treated my Mom like family during her 5 1/2 week stay. Emily (PT), You are awesome as well. Patient, attentive, courteous and also a tremendous credit to your trade. Ray and Lauren (OT's) were also awesome OT's. Rayna, Ashley, Zenona, Aweda, Cynthia, Caleb (RN's) were terrific, attentive fast responding Nurses. Andi, Donna, Angelica (CNA's) were fantastic as well. In general I can not count the number of times one of your staff hugged my Mom, wished her well or generally treated her as a family member. For those reading this, this is a facility that is under new ownership since July of 2019. I'm sorry if your loved one had a poor experience prior to this. I know with complete certainly that my Mother will be back here at TCA when the time comes for her next knee rehab. I cannot say enough positive things about this Facility. Fantastic experience: My wife was recently discharged from your facility after almost 3 week of rehabilitation as a result of a turn for the worse with her fight against cancer. Her goal was to get back home again and get back some of her independence. We found your facilities to be modern, clean and patient friendly. The food way exceeded our expectations. The nursing staff, CNA's and OT & PT therapist were very positive and supportive of my wife's needs. She left in a much better place then when we started. I wanted to acknowledge one of your CNA's, Seon Duquesnay. She went above and beyond with her level of patient care. She always came to work with a smile on her face and a positive attitude that shined. My wife felt very special every time Seon was with her. I trust that you have some form of employee recognition system to reward those who make a difference for the patients they serve. We would not hesitate to recommend your facility to others in need. Thanks to everyone at your Arlington Heights facility who had a hand in helping my wife, Dorothy get back home again.

Fantastic experience: My Mom had a terrible experience at Manor Care in Northbrook in 50 minutes Transitional made room for her and had nurses and dinner waiting for her. She had heart surgery and stress was not what she needed. Thank you so much

Fantastic experience: This place is a great find! Not sure why the negative reviews but our experience was exceptional. My 78 year old father was admitted pre and post OP before and after a surgical procedure. Upon admission he had a difficult time walking and performing every day functions. It wasn't safe for him to be home alone with his cardiac and thyroid issue. TC's staff was very accommodating making sure things were just right and his medications which was quite extensive were properly dosed. Rehab with the PT occurred on the floor facility just a few doors down and was very effective. Dining hall is also at the end of the hallway which was a very short walk away and the staff is great treating diners more like hotel guests than patients with each of the courses brought out after detailing out what was on the menu. Food was healthy, portions appropriate, and my picky father approved of its taste. We were also very impressed with the Nursing staff. All were very responsive and friendly. Most notable was their willingness to work together to help out. We had the utmost confidence in their ability. Father was discharged when he got to the point of being independent again. He only had good things to say about this place which is gold considering he's not an easy guy to please! I would not hesitate bringing anyone here.

My sister-in-law received the best care and is now home after a major back surgery and successful rehab. My family is grateful for the wonderful care she received. We will recommend this facility to everyone.
Fantastic experience: I Spent over a month at Transitional Care with my brother needing very specific care and quality control. The facility, nurses, and especially the management were simply the best I have ever experienced. My brother came into the facility a broken man and left with great hope for the future and his health was dramatically improved. I would give Transitional Care 10 stars if possible. Positive experience: The physical therapy and occupational therapy was priceless! Everyone of these professionals are hardworking and compassionate. They know when to push you and when to back off. I had total knee replacement and because these professionals were so wonderful I actually LOOKED FORWARD to my sessions with them. Hands down, I would have never gotten on the long road to recovery without them. The food I thought was pretty awful. But then, I'm picky. This is easily solved by having visitors bring food. Each room has a mini fridge.
Fantastic experience: I highly recommend this Rehab center! It’s clean, and pleasant! Everyone is so wonderful! It’s by far not a nursing home. Not long term, so everyone is welcome to do a quick transition back to life again. Love, Love, this place!!!!
Fantastic experience: While licensed as a skilled nursing facility, this center only focuses on the short term rehab patients. This is not your typical ‘nursing home’. There are patients of all ages. The concept is a hospitality (think hotel) focus, with highly skilled medical team and patients that are here for just a few days. It’s an opportunity to more fully recover from surgery, devastating illness or injury before you go home. While this concept is not unique, it is new to our area.
Positive experience: A mixed bag. The physical and occupational staff were thorough and highly skilled. That was my reason for being here, so I have been basically satisfied. On the negative side, you get little information about basics like when you will see your MD (I have now been here 30 hrs and have not been seen by the MD), when you will see your nurse, and the basics of the process here. You are told that certain things will happen, but they get forgotten. They direct you to an app, which is vague. Many staff have been very helpful and cheery. Some appear to be passive-aggressive. Food is very good for a rehab facility. TV is outstanding but Wi-Fi very weak. Facility is relatively new. Mercifully NO smell of urine. Nicely informal atmosphere.
