Our Wellness Initiative will focus on Community Wellness. Through this type of care, community members are able to take preventative actions towards their health as they age through services such as complimentary 6 week wellness assessments , in-Home therapy options, group exercise programs, personal exercise training for seniors, complimentary home safety assessments and restorative services. We believe that a happy patient has a better chance of reaching their goals.

Our primary goal is to provide a continuum of  patient care that is designed specifically for the residents in the communities that we serve. We fully understand that we are STRONGER WORKING TOGETHER AS A TEAM. Our Senior Wellness Initiative is designed around family values. Members of our continuum of care team, care for patients as if they are family.  Our Wellness Alliance decided not to spend time trying to recreate the rehab wheel, our focus is on patient care.  Every partner brings a unique service that is critical to our wellness model. Bonnie McClure, President of Town Park Rehab. Bonnie, her husband Mark and their 5 children reside in Green, Ohio. Bonnie has over 30 years of experience as a physical therapist and has worked in a variety of settings including hospital, outpatient, and home health. Bonnie enjoys treating patients of all ages ranging from pediatric to geriatric, and appreciates the uniqueness of each person.
