Ascension Holy Family

Ascension Holy Family is a long-term acute care hospital (LTACH) caring for medically complex patients, the only LTACH in Northwest Chicagoland. It is the first long-term acute care hospital in Illinois to be certified in Disease Specific Care for Respiratory Failure by The Joint Commission.

Our care team at Ascension Holy Family specializes in caring for patients who are critically ill with complex conditions and who need to be hospitalized for an extended period of time. Most patients here are transferred from critical care units at other hospitals.

Your care team includes intensive skilled nurses who provide follow-up care after you or your loved one has been hospitalized for a critical illness. Our long-term acute care facility focuses on those who stay an average of 25 days or more. Your care team may include doctors, nurses, wound care specialists, respiratory therapists, physical and occupational therapists and social workers. We focus on care for your physical and emotional health.

Long-term care services include daily care, including assistance with feedings and getting dressed. Your care team at Ascension Holy Family offers a much higher level of inpatient care, including:

Medicare benefits apply if you are transferred directly from an acute-care hospital or admitted to the long-term acute care hospital within 60 days of being discharged from an inpatient hospital.

We provide specialized care for patients with acute or chronic respiratory disorders who may have tracheostomies or require extensive respiratory treatments to maintain normal breathing without mechanical support.

Physical, speech, and occupational therapy are offered to all patients with a doctor's referral. Therapy is provided bedside in your private room. We also have a rehabilitation activity room and assistive devices for walking and self-care training.

Ventilation is used as a life-saving measure while the patient recovers from serious illness. Weaning occurs during continued medical treatment and rehabilitation. As weaning progresses, our care team works with you to improve strength, cognition, nutrition and communication.

Wounds that are slow to heal or chronic can occur because of diabetes, lack of circulation or other health concerns. With intensive treatment and advanced care, our care team can help you and your wounds heal.

Ascension Holy Family was the recipient of the 2017 Goldberg Innovation Award from the National Association of Long-Term Hospitals for the Dedicated Education Unit in training the next generation of nurse professionals as well as the 2018 National Guardian of Excellence for Patient Safety by Press Ganey. It has been recognized by the Illinois Hospital Association Award for Patient Safety.
