Our memory care is a structured, research-based memory care program. We utilize a person-centered, wellness-based model of care that incorporates stimulating daily activities to sustain independence and promote a healthy spirit.

Long-Term Care is a great option for those who struggle with daily tasks and can no longer be cared for at home due to illness, injury, frailty or other limitations. Under the guidance of an attending physician, Long-Term Care is provided in our communities by licensed nurses, specially-trained health care aides and a full interdisciplinary care team.

Long-Term Care includes a variety of services, including social services and activities as well as physical, occupational and speech therapies to assist our residents in maintaining the highest level of independence possible. Our settings are home-like, cheerful, soothing and secure to ensure the highest quality of life for our residents.

Along with private pay, ASC is proud to accept a wide variety of payment options including Medicaid for qualified applicants. Our Medicaid specialists can assist you and your family through the application process.

Short-Term Rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.

Our person-centered program is tailored to the individual. Short-Term Rehabilitation includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.

Exclusively at American Senior Communities, the Road to Recovery program offers the opportunity for our interdisciplinary team, the patient and their family to meet within the first 72 hours after admission to map out their Road to Recovery by identifying discharge goals.

Returning home as quickly as possible is always the desire. However, returning home without the proper rehabilitation can greatly increase the chance of seniors experiencing a setback resulting in rehospitalization. Prior to returning home, our team, in partnership with the patient and family, will conduct a comprehensive in-home safety evaluation to ensure patients can successfully navigate their home with confidence.

No matter what your age or health level, our clinicians are skillfully trained and committed to your success. They take time to get to know and understand what motivates you. Based on proven methodologies, you can count on our clinical team to provide the latest treatments, with respect and compassion. Your health and happiness are always our priority.

The goal of Speech/Language Therapy is to increase functional communication, cognitive skills and teach safe swallowing by introducing diet modifications and special feeding techniques. Therapy may include muscle retraining, compensatory strategies and use of communication devices.

Services include wound care, rehabilitation screenings, fall prevention, nursing, contracture management, wheelchair position, mobility training and walking ability programs. Add this to our highly successful strength-building wellness and aerobic conditioning program, and you have an incredible combination that will truly help you move forward!

Fantastic experience: This the best place for someone with Dementia and Alzheimer's. My special friend is here and the staff are really good with the patient's. When I go there they are very friendly and know me by name. They make sure that people are clean and well taken care of. I would recommend this facility. And give them a 5 star rating. Thank you all for your kind and thoughtful things you do.

Fantastic experience: I did a tour of Elkhart Medows, they have really made some great improvements! And the are doing even more!!! The staff is very friendly and the Admission Dir. is very personable & professional.

Fantastic experience: My husband, who has early on set Alzheimer's at 59 years old, receives great care. The whole staff is kind, understanding, knowledgeable, caring . I'm so glad I found Elkhart meadows to help me and my husband through this very difficult and trying time for us. They are a great help.

Fantastic experience: My mother has been a resident for 3 years, they made the transition as easy as possible. I have never had to worry about my mom being cared for. They have always been in constant communication with me. The staff goes above and beyond, in all aspects. We will be forever grateful for the care & love, thank you!
