Our goal is to provide residents and patients at NHC HealthCare Charleston with a great quality of life by making every moment count. We believe that life should be celebrated at all stages.
NHC HealthCare Charleston is a private, spacious campus with a 132-licensed bed post-acute 24-hour skilled nursing Health Care Center. We see many individuals who need skilled nursing care after a stroke, joint replacement surgery, a cardiac procedure or a serious illness.
At NHC HealthCare Charleston, we are dedicated to offering the best possible experience for our patients. Every morning, all of our partners participate in a "Better Way" stand-up meeting, during which we focus on one of our 20 promises. This helps us to continuously improve the care we provide.
Since the kitchen staff started preparing meals, my husband has been eating three times as much as he used to. I'm really happy that he's finally getting the nutrition he needs.
This is a great program for physical therapy and occupational therapy. The staff are very considerate of your feelings and very friendly. I felt very comfortable with all of the staff.

Negative experience: I would not put my worst friend in this place! It needs to be shut down after what they did to a family member. It’s bad when you walk down the hall and can smell poop. Our family member laid in her own urine for hours. I will be contacting the state.

Fantastic experience: NHC has provided exceptional care for my Mom during several admissions for rehab. We were so impressed with the nursing care and results from their physical therapists after her first stay about a year ago that we request to return there when she has needed subsequent rehab care. The physical therapists at NHC are the best. Nursing care and continuity of medicine/care provide stability for Mom during her stay. Food service/meals are tailored for her diet and preferences and she enjoys her meals. Our family is thankful for the care Mom receives during her stays at NHC.

Negative experience: I would give this place a zero because of the conditions that they left my mother in on daily basis and other residents that I have witnessed. My mother was there from July to October 2023. I stayed with my alot of the time and I attended to her not the staff.When I was there with my mother all day,no one came to check and see if she needed to go to the bathroom . .I confronted Meme about the lack of care in which she a sure me that would be fixed. Nothing was ever fixed in there and management is non-existing in this facility..Her behavior really,, really begun to bother me in September when I went back to work two weeks and told my employer that I needed to go on fml and not because my mother was being discharged . I let physical therapy and thethis facility.m know that I trying to move management doesn't need to respond to this post because they didn't when I was there.I have a lot more to say but for anyone looking at this post it is true about the place and people who are there and running this facility

Negative experience: My problem is with getting my pain medication. I had hip replacement surgery in Roper downtown where I was given 10 MG every four hours. I came here late Monday afternoon. I did not get my pain dose. I was given my dinner and meds, no pain meds. I was told that my "narcotics " did not get here. Much later, I was given, after asking, 5 mgs from their box. It's been hit and miss on getting anything at all for my pain. Understand that I take NOTHING for pain at home, never. I am not confused. I do not have an additive personality at all. I am being told lies, having my medication stolen, being treated with an attitude when I ask politely for something for my pain. Being questioned about my need for the narcotics. Being told I have gotten my pain medication every four hours since I first got here! And being told I am confused! I thought this was fix and finished yesterday, but obviously, it is not! Friday morning I got my med at 8:30am. I got my second dose of oxy at 4:15pm. That is not every four hours, and every time I have asked for it, I have to wait, because the nurse has gone on lunch break! This is absolutely absurd! There is more to this, but this should be enough for someone to get some help to me, or should I justleave? I am in a place where this particularwomandoes not care for me. Rather, it'squitetheopposite! I do notfeelsafeat this time.?
Negative experience: Comes to find out this place does not care about their employees and family. Was told my wife can not join me in the ER with my daughter under 2 who has been sick for 2 days. Sad to hear this. Hope my daughter is pulls through.

Negative experience: DO NOT put any family member or loved one in this place. I would give less stars if possible. Nurses are there for a paycheck and do not care about the patients. The food is horrid, so my father refuses to eat any of it. Nobody seems to care or even ask why the patients aren't eating. If you have no ther options and this place is the only one with availability, please stay there with your person as long as possible to make sure they get the care they need. I found medication on the floor that anyone could've picked up and taken. Nurses sleep with their heads down on the desk during their shift while patients are yelling for help. There are major issues here that I don't see getting fixed any time soon. Avoid this place! My father is currently in there and he will be going to another facility as soon as possible.

Fantastic experience: We had to wait a little while but the nurses and doctor were very accommodating and helpful and took care of our 3 year old wonderfully and answered all our questions and fixed anything that was an issue afterwards. Thank you so much ladies.
Negative experience: Nursing home illegally blocked our family from seeing our loved one; they (the nursing home) stopped doing this after we reported what they were doing.
Negative experience: Needs to work on serving it's community better. Very lazy staff and less informative of their patients status. No physician is ever available.
Negative experience: The facility was nice; however there was no one at the desk to greet us or provide directions. Staff members in a nearby room looked out but no one assisted. We moved to the next area where a pleasant employee assisted us.
Negative experience: If you are a patient, they make you sit up until you have therapy! I was there for 2 hrs. and was told they would not put my family member back to bed until he had therapy, regardless if he had been up for several hours!

Negative experience: The facility is the total mess. After ringing the nurses call bell, I spent 50 minutes waiting for some to come to the room. When I asked to see the medical chart I was told to wait on a nurse. After another 30 min wait and ringing the call bell another CNA appeared whim I made aware that I am still waiting on a nurse. Long story short the patient was given medication for which a Dr.s Order was written over a week ago to immediately discontinue and yet the Meds were administered anyway. The patient has now been hallucinating for one week, has no knowledge or Recollection of where they are. The attached pictures show pulls that I found on the floor next to the patients bed as well as under the covers of the bed. I would rate this facility a zero if I could due to negligence and irresponsible conduct.
Negative experience: My husband was recently there from Jan 30th until yesterday Feb 9th 2023. He started throwing up blood n bile about 830pm on Feb 8th but the cna who was employed through a staffing agency did not report it until 630 on Feb 9th. They waited until 830 am to transport him to the VA hospital. He was admitted through the er. He ended up with a ng tube which is a tube in his nose to his stomach for 24 hrs. He's still currently at the VA getting care. After reviewing his chart of messages he should have been on the dr stopped several needed life savings men's he should have been on for unknown reasons. His vitals were only done 1 time on the 8th in the morning and not done again until the 9th. On Feb 2nd his wbc was high with nothing being done to recheck it. They did not call me until 930am on Feb 9th to let me know he was on way to er. Today when I went there to get his personal stuff they wouldn't give me name of the cna who neglected his care. I spoke with Tyler Bennett who is the director of nhc about everything. My last statement to him was to advise him that I have spoken to my attorney about everything and will be filing charges against the cna even through I was told she was dnr which is do not return to the facility. If you are going to place a family member here I would think twice.
Negative experience: This place was recommended to us believe it or not. Long story short my grandmother was here for 2-3 days. In that time she fell and was laying on the floor with no one finding her until she had been laying there for some time. They put a ice pack on her the next day to help with the pain. She can't move really because she just had hip surgery. The ice pack was left on so long it completely melted and she was soaking wet shivering with the ac blasting in the room with no blanket or anything. If anyone would have even peaked in the room or checked on her they would have seen what was going on. She couldn't hit the call button because it had fallen where she couldn't reach it. You don't know how upset I was when I heard this! Do not recommend this place if you care about your loved ones.
Negative experience: A few of the many reasons to NOT BRING ANYONE HERE! The only reason I give this place two stars is because of the four out of five star service received by the physical and occupational therapists. A select few of the nursing staff are good at their job but the majority are not. Management at this facility is abysmal at best and should be known as mismanagement. I hold them directly responsible for the extreme levels of neglect and staff incompetence which contributed to my mom's near demise. More than once a staff member was to take blood pressure readings and simply did it wrong because they were unqualified, uncertified and or simply didn't possess the capacity to perform the task at hand. At least three times my mother did not receive her meds at all and more often did not receive them in a timely manner. Stafford very often leave pertinent things such as the EMERGENCY BUTTON, pillow, water, trash can, tray with all her necessities, light cord etc. out of reach. Much of the staff is unhappy and condescending to the patients. Some of the staff are comfortable enough to throw racial slurs around like they're at home and it's 1950. The facility is old, dark and full of dilapidated equipment as well as people who should not be in the position they are. The people that are in the right positions and doing the right things are special people indeed! Much gratitude to you all and I applaud you because you are very special people indeed! There just aren't enough of you there.
Negative experience: Worst experience I wish someone had warned me prior to my mother going here after the hospital from pneumonia. This place used to be one of the best now so far from what it once was. In three weeks here she lost 15 lbs and was slowly losing her memory as if she was being dusted. Everyone who works here should be ashamed and embarrassed at the level of care. If you end up with a loved one here I would suggest dropping by randomly at different times day and night. Or better yet have your loved one transferred to anyplace other than NHC.
Positive experience: Staff is extremely nice and always along to help. Having problems getting medications as ordered. Most nurses try to be accomodating, but a few are argumentative Therapists are awesome amazing and experienced! Ancillary staff very nice.

Fantastic experience: Words cannot express how grateful my siblings and I are to the staff at NHC Healthcare Charleston for taking such excellent care our sweet mother. Upon arrival, we are warmly greeted at the front desk as well as by all staff members in the hallway on the way to our mother’s room. The nurses are so kind, caring, and compassionate, and would go out of their way to ensure that the residents, as well as their families, feel right at home. Just watching Nurses Leslie M. and Sharita C. take such loving care in assisting mother in preparation for the day brought tears to our eyes. Never have we been in the presence of such kindhearted people outside of our own family. Thank you all for making our mother’s stay at NCH Healthcare Charleston not only a positive experience for her, but her entire family as well.

Fantastic experience: My dear father has been a resident for many years and I visit twice a week…I have always seen excellent care and respect for him and they have helped keep him healthy and comfortable even through the pandemic. During those lonely months for him being quarantined and away from his family, the staff came up with many creative ways to help us stay connected and see each other! The nurses and Cna’s, and all the other staff there should be commended for their jobs of love.

Negative experience: Was visiting my father in law just now. Walked by a patients room and he was on the floor. I went and told Nurse that was sitting at the desk, she did not move but said, Yes I know, we are waiting on the CNA. But yet no one was in their checking on him. How long do they leave them laying on floor before they check on them?

Negative experience: Needs to shut it doors!! Where is the owner!! Management a big joke you point out problems and they talk about you to the staff!! The food is awful, Mom’s allergic to pineapples and they have her pineapple with the ticket on the tray that states.. allergic to pineapple I’m RED! Patients meds found on the floor in a bag!’ I handed it the the CNA after picking it up off the floor! Faucet in bathroom corroded! I had to call the so call maintenance guy and left him a message of how discussing it looks and it needed to be changed now! Nurse couldn’t be found by staff I had to go find her in the parking lot!!
Negative experience: I was admitted to NHC for rehab after having sepsis and a seven day hospital stay. I was very weak and was there for physical therapy and to regain my strength. I found no evidence of a rehab facility, it was a nursing home. I suffered through one night of no sleep, the staff laughed and yelled and seem to be having a party at the nurses station all night long. Patients with mental issues cried out all night long. Patient room doors were all left open with TVs blasting and I was told the doors could not be closed. Pressing the call button yielded need little to no results. After one night, I left. The charge nurse became very angry, very rude and belligerent. She told me that I could not leave, that they use a skeleton staff on the weekends and there was no one there to take care of this and I would have to wait until Monday. I remarked that I was not paying a skeleton price on the weekends and that I was going home. I asked to take my medications with me and she refused until I signed some sort of a consent form.Then she said that she would give me my medications, but that I would have to wait one hour for them. As she made that statement, she was standing in front of the cabinet that contained the medications. She also refused to give them to me until she spoke to my personal representative. I said that I was my own personal representative. She insisted on calling my emergency contact, who is not a personal representative. She took my telephone for the conversation into another patient’s room and closed the door so I could not hear what she was saying.My emergency contact said she is not responsible for making my decisions and if I want to leave, then I am leaving. I was able to finally get the medication after speaking with the social worker. The woman followed me out to the car to confront me again. I have never had such a terrifying and horrible experience in my life. There was a total lack of empathy for patients. I caution everyone to avoid the NHC facility. As we were leaving, I could see that all the hallways were full of the same noise and confusion. There was no possibility for any rehabilitation there.
Negative experience: Don't send your loved one here! I had to repeatedly complain about their lack of care there. Don't be surprised if your loved one starts losing weight as the dietitian prescribes a 2600 calorie intake per day Btw 2000-3000 is needed to maintain weight for the elderly . Besides that they exposed him to Covid ... the facility has had it for months .They put him in the day room with no mask!! And yes he contracted it the next day! I discharged him and he is home with me now... I am convinced he can live longer here.!! My husband is a Vietnam Veteran and certainly deserves to be treated with more dignity. The 1st time I visited they had left in bed, not done his laundry so he was wearing a gown . I had to complain as they didn't do his nails and they were also filthy. They were giving him the wrong dosage of his Parkinson's meds for a month. Also the weekend staff is especially poor. No compassion ! Believe me I had meetings with everyone including the administrator!
Negative experience: This is a place we're I wouldn't recommend family or friends too. To many issues and first hand I sit back and watch. Nurses not responding to patients as needed(change patients in the morning and let them sit all day and never answer when my mom asks to be change. Meds not given as how patients are to have them or you can not find them. It's something every Friday and and through out the week. Staffing is horrible at the place. Foods that some patients are not to have is still given. If its not the nurses or the kitchen or meds cart or maintenance. I just dont understand. When corporate comes everyone is on pins and needles and certain things get done. Corporate needs to come in unknown and and see patients them selves. Ask them and get a full report and see were things need to be corrected across the board. Carpet in the halls is nasty and urine smell over take the hall that make me sick to my stomach. If this place is part rehab and part nursing home then it needs to be separated from nursing home patients. I'm in the process of filling a full report of everything with this facility since my mom arrived.
Negative experience: I was trapped in that hole for almost 2 months, thanks to a car accident. The guy that ran the kitchen and my PT and OT representatives were really nice... But most of the nurses and their assistants were the worst people I have ever met. They rarely cleaned me and often left me un-wiped. At one point they tried to force me to wear diapers so they would not have to come back as often? As if they intended to leave me with a filled diaper? When you click the call button, there is no intercom and no one answers. In most cases they did not get to my room for 30 minutes and sometimes even longer... And in many cases they would just come in the room, turn off the call light... then say they will be back. If they do that, they never come back, so you have to click the call button again. After all of that, the worst part was the man that was supposedly running the place. he was a horrible person with a terrible attitude. He actually had the nerve to talk to me like they were doing me a favor, as if they were not being paid high dollars to attend to me. Finally, they have a dangerous practice of just leaving the front doors unlocked, far into the night. On more than one occasion, a delivery driver or just a random person looking for someone else, just barged into my room, unannounced and without any form of apology. If all of that is not enough to make you seek other accommodations, then just go find out for yourself... Good luck.

From the front check in desks welcoming young lady to the nurses we encountered, Everyone was very polite and helpful! My mother seems happy with her care and how she's treated. Fantastic experience: Of all the facilities I have ever been to this facility is by far the best. All the staff here has been very friendly and accommodating. They have specialists in every area that have helped my client to return to his former self! Anytime I have had an issue, I would bring it to their attention it was resolved quickly! I would highly recommend this place. Keep in mind, everyone is short staffed at this point and time, but NHC has dealt with it gracefully.
Negative experience: Our mother has been in this rehab for weeks ... no one can visit due to Covid and they took her phone away. No one has spoken to her in weeks! The call back from nurses or staff is not consistent and this is extremely difficult and scary for our mother needs here daughters this is absurd. I would not ever let ur loved ones go through what we are currently experiencing.
Negative experience: My mother was there for two weeks and thank God it wasn't longer. She is in her nineties, has dementia, and was there for inpatient physical therapy after surviving COVID-19 pneumonia, which left her too weak to walk. Obviously, she required assistance with everything. The staff was very nice to us and therapists may be OK, but overall the care was poor. When she was in the activities room, she enjoyed the games and the other patients, but most of the time she was left sitting in front of a TV that was turned off or lying in bed with the blinds closed. The kicker was that no one would help her go to the bathroom. They told her to go in her adult diaper. The result of all this was that she now has a fully-developed pressure ulcer (bed sore) on one heel and the beginnings of one over her sacrum. This place may be OK for semi-mobile patients who just need PT and can look after themselves otherwise, but for patients who need more than that, look elsewhere.
Positive experience: Clean place they have some private rooms which I’ve been blessed Doctors and most CNA and Nurses friendly helpful work hard to care for patients. My room kept cleans and supplies provided. Communication needs improvement. Computer software needs updated to bring to attention medication that need be provided with meals as in dialysis patients and binders . Appts missed . Needs improvement for therapy to utilize equipment. Therapist and Occupational Therapist have been great. Food most times for breakfast and lunch good but serving size needs be a little larger especially for patients working out in therapy and on the move. Dinner food is a nightmare staff don’t prepare food to menu they throw together anything to get out. Mostly soup and cold sandwiches, corn dog, stale chard grilled e. Items missing from trays I believe they are rejects fired from McDonald’s working food prep and tray service. Portions are extremely small enough feed a 3 year old . Sweet teal like syrup. Really sucks patients can’t go out get themselves good food they need prepare something they themselves would want eat.
Negative experience: This facility I will never recommend anyone to take their family. My mother was there for three weeks and have bed soars. On her private parts on her right side. She was rushed to the emergency room while she was there to find out that she was dehydrated. They said she would sleep all the time. I guess if you medicate a 95 year old with certain medications don’t you think she would sleep a lot. Not recommended.
Fantastic experience: Very nice staff. Room cleaned daily and thoroughly. Bathroom cleaned daily.
Negative experience: Where do I start? With the fact that they left my mother in a wheelchair four hours unattended and never checked on. They assisted her to the potty and left her sitting there for an hour. The first and only Sunday she was there, she was left in the bed all day. When she said she had to pee - they said “let it rip”. Where is the dignity for our elderly? Needless to say we removed her from their care. Respiratory, Physical, and Occupational therapies were the only positive aspect to this place.
Negative experience: Daytime care is good. After hours and weekend care is terrible. I would suggest that you place a camera in your loved ones room. For after hrs.

Negative experience: My father in law was treated horribly and then they didn’t want to deal with him and had him shipped back to the VA hospital. How are you gonna treat a veteran like that?? Shame on you worst experience ever

Negative experience: This place is a JOKE! My grandmother is getting the worst care there. I see the “owner” keeps commenting on these reviews and that’s funny because NHC says there is no owner when you call to complain. They have talked to her like she is trash and she is 80 with a broken bone. Been left alone, and could of fallen. Her room is never cleaned and she had roaches in the bathroom. They also never give her her medication on time and ran out of it so that left her in pain for hours!! This place is beyond awful, look hard before choosing this place.
The facility Administrator is the Google page "owner" and he would gladly meet with you and your grandmother in person to discuss your concerns.
Positive experience: I spent three weeks at NHC, rehabilitating after spinal cord surgery. The staff was excellent across all functions, day and night, weekdays and weekends. The nurses and CNAs were knowledgeable and responsive regarding medical procedures, pain management, and patient cleanliness. The physical and occupational therapists pushed me, but not excessively, such that I was surprised by my progress. The staff administrators didn’t let anything slip through the cracks. Food service was responsive to my requests. Overall, the professionalism and kindness of the staff were very touching. These folks work here out of love. The facility is well-equipped, clean, and cheerful. ———————————————————— UPDATE, AUGUST 10, 2019. I’m going to downgrade my initial five-star review of NHC to four stars. During my stay at the facility, I was told in no uncertain terms by several staff members that the doctor in charge had signed off on a power wheelchair that I would receive permanently, reliably, and immediately, paid for by my Medicare and Medicare supplement insurance (except for an optional feature that I agreed to pay for). After I arrived at Brooks Rehabilitation in Jacksonville, Florida, I was told by NHC that it had merely arranged to get me a loaner wheelchair (which was helpful) and that Brooks would have to start the power wheelchair application all over again. Brooks added that Medicare and Medicare supplement typically fight a patient over the payment for a power wheelchair; and, even if a patient is successful, delivery of a power wheelchair typically takes about two months. NHC needs to tighten up the information it passes on to discharging patients. It’s thrown a monkey wrench into my plans for rehabilitation.

Fantastic experience: The nursing staff and administrators are so amazing. Kind, sweet, and sympathetic. From Anna at the front desk to the admissions personnel, they couldn't have been more accommodating. The rooms are large clean and very welcoming. My mother is so happy to be there with such wonderful people. Such a great piece of mind knowing how well taken care of she is.

By friends I don't mean the other patients, however they were very nice to socialize as well, I am referring to the wonderful staff who worked with her daily. Management at NHC are wonderful. In the beginning we had a meeting to discuss what to expect. This meeting went over every possible question I could think of and then some. Throughout the stay I was educated on her progress. When it became discharge time, the lady who met with us in the beginning met with us again to go over the plan. In addition to her expertise- I was also encouraged by the rehab team who not only had my grandmother at full potential but also eased my mind by taking the time to also meet with me to tell me exactly what to do when we get home. In addition to the overall experience I was also given the option to have my grandmother readmitted within 30 days of discharge if I felt there was a decline. All of that info became handy when I was able to send her right back and avoid sending my grandma to the hospital when she had a slight decline at home about three weeks out. She is already back at home and is seen by home health three times a week. I suggest looking at other places before making a decision for short stay rehab patients so you become appreciative of what NHC is. You will joy find a better team of healthcare workers, a cleaner building, and the amount of communication anywhere else. I can assure you that!

Fantastic experience: Thank you NHC Charleston for caring for my Mother-n-law. I appreciate your attention to detail. The staff has comforted Mama as well as my fears. Physical therapy is going great. I feel welcome to help Mama and myself to juice, fruit, coffee and even ice cream. Staff have introduced themselves and recognized that we are a large family. Negative experience: This place is awful my mother could've been dead if I hadn't arrive within minutes to check on her. Some staff member are helpful(about 10%) and 90% are useless. Call bells are going off and staff members just sit and play on their cellphone.......STAY AWAY...if I could give them a 1/2 star, i would.

Fantastic experience: Could not ask for a better experience. Most helpful staff and family like atmosphere. Patients always look healthy and happy. No smells or problems. Simply the best care I have ever experienced.
Negative experience: Got roaches? If not, you can pick up a gross of dead ones in my mom's room. Room is dirty, unswept in days, unfinished drywall patches, a staff that will respond in no less than an hour! Just like paradise. Want pictures? I got 'em. Disgusting.
I sincerely apologize for the state of your mother's room. I have contacted our pest control to come out, housekeeping to do a deep clean of the room, and maintenance to paint. That is not the standard for NHC and I will work to ensure we correct all issues.
