Fantastic experience: I had the opportunity to visit the facility on Christmas to see my family, and I was genuinely impressed and amazed by the Christmas party held for the residents. The staff went above and beyond to ensure every single resident felt included and received a gift. It was heartwarming to see such dedication and care. Truly a memorable experience!

Fantastic experience: I recently visited my aunt and was able to enjoy a holiday meal with her. The food was surprisingly good! I wasn't expecting it to taste the way it did! I feel at ease knowing she is being served good food and that they are also looking out for her health needs as well. I was even able to meet a few of the employees the food director Mrs. JJ! She was so nice and caring and the activity aid Ms. Maria. They both brought smiles to our faces and helped us enjoy the day even more!

Negative experience: My brother can't walk he is shaking a lot his legs are swollen but they want to kick him out in the cold he's in very bad shape he needs constant care it's a shame when the facility kick handicapped people out the door he came home once before but had to call ambulance cuz he fell down he has a very bad back but they don't address that issue.

Negative experience: They're trying to push my dad out of the nursing home and he's not ready to be sent home :( his going to fall and end up in the hospital and the nursing home and I hope it's not this nursing home all they care about is money they don't care about the people there I don't recommend this place

Negative experience: This is the worst nursing home facility I have ever experienced with my parents and grandparents. The manager refuses to return even a first request for a call back after months of repeated requests. Their business office has lost check Payment, does not bother billing you or sending you notification until months after due date, their is turnover in the business office. There is no communication about billing, and the corporate office personnel repeatedly apologize for incompetence.

Negative experience: The staff here is very incompetent beginning with the Director,Steve to the rest of the admin personnel such as Jenny. My Dad came into this facility for rehab which he did not receive but rather just drugged so they didn't have to bother with him. Once he was discharged, he was transported to VA hospital in Nashville where the Drs advised us that he was very malnutritioned along with a severe UTI that led to Sepsis. He died a week later. VA is preparing all the needed evidence to present to the state for legal matters against this facility. Don't dare respond to me by saying I can call you anonymously. I'm telling you now!!!!!

Negative experience: Signature would get zero stars from me if this was an option. This place is horrible. They neglected my grandmother. She ended up getting bed sores and ended up passing away last week. My grandmother had ants crawling all in her room on her table and dresser with her clothes. She also had ants crawling all up her wall. There was a tarantula in her room and it was huge and if we hadn’t been there it would have gotten on her which she wouldn’t be able to call for help in the condition she was in. Her phone and emergency call button stayed unreachable to her. They didn’t support her needs by positioning her in and moving her around. She had no legs to get around for her own needs. My grandmother was transported by a van instead of an ambulance to Tennova for an emergency surgery where she was bounced around due to lack of restraints and support. She was screaming and in tears asking the driver to pull over because she was falling out the seat. Her iv came out of her arm and there was blood all over the transportation van causing the driver to call 911 for assistance to get her transported to Tennova. The staff were never attentive and always on their personal cellphones in the hallways instead of tending to their patients. They had her so pissy that after they changed her you could smell the urine in the room.I can't stomach to see another family go through what our family has just endured. I would never recommend this place to anyone.

Negative experience: If it possible I would give negative stars! Do not send your love one to this facility! My dad went there after a fall which resulted in a broken fermur for rehab. Rehab was repeatedly skipped. He was left dirty and in pain despite repeated calls to staff. He developed a sore on his bottom which was ignored which resulted in it becoming infected. After being at the facility for barely two weeks he was sent hospital because his fistula for dialysis wasn't working. Thank God this happened because the hospital determined his system was septic. The facility ignored my repeated questions about the wound, his mental state, and the lack of response. If that fistula had not stopped working he would have died from sepsis. He has been in Vanderbilt hospital for over a month, still there dealing with the infection. Horrible patient care! Do not send your love one to this facility!

Negative experience: Would have rated zero stars if I could. My grandpa was admitted here and at first my family thought it was just what he needed but we were quickly shown wrong. Every time we visited him he got worse and worse. His mental state declined so quickly that we couldn’t hold a conversation with him a month after he got to signature. The pain in his leg (which they knew about before his admission) seemed to either be ignored or given very little effort to ease. We finally got him out of there after it was discovered he had pneumonia and from what the hospital told us he’d had been there for a long time. We got him into a much better facility not even a month ago and he is better than he has been in years.

Negative experience: Treating my step father like he isn’t even there. One lady hurt him and got suspended and yet they still don’t check on him in a timely manner. My family and I are not happy. Check one ur patients they need oxygen or to pee or cleaned. Also meds…… and when called in don’t say be back and never come after being called in 3 times.. if u love ur family I recommend not letting this place care for them.

Negative experience: I have never been so upset with a place in my life. My husband is in there for rehab. I love the rehab department they are wonderful but as far. As the nurses and cna' s I don't recommend it. They don't care they do as little as possible...

Negative experience: My grandmother has been down there for a week and hasn't had a bath at all!! The food they serve his cold so we have been taking her lunch and dinner so she has something good to eat. I asked A nurse about her shower situation and she acted like it wasn't a big deal that she hasn't had one

Negative experience: I’ve seen more AMAs here than any other place in 1 whole week. ‍♂️ Had a nurse named CARLA hold keys & refused to pass CARLA meds. After I got done passing mine, this same nurse wanted me to do CARLA’s job & pass CARLA’s meds. I was then told to leave the facility. This nurse CARLA went around asking what folks wanted for meals but never passed their medications. They had 3 techs passing trays. They didn’t need a nurse to ask people what they want on the menu. Had techs witness & tell the administrator & still no accountability. This same nurse still works there in admissions. What a joke. Had family complain of the nurse not passing her medications for 3 hours since shift started. Signature of Bordeaux closed down. Signature of Madison closed down. What’s next? The cost of the nursing home community at Signature Healthcare Of Clarksville starts at a monthly rate of $1,133 to $6,996. Update: I did try to reach your administrator several times & through your schedulers & DON. I was ignored. I was told she would call me back but she did not. How are you going to claim you want to talk now?! Carla in the Business Office. For a reason.

Fantastic experience: My sister was there with signature for rehab after having an amputation and receiving her prothesis. The care there was excellent and the physical therapy team superb. She walked out a stronger more confident person.

Negative experience: FOR FUTURE PATIENTS LOOKING AT RATINGS…MOST OF THE 5 STAR RATINGS ARE FROM EMPLOYEES. THAT IS A PROBLEM AND SHOULD BE ILLEGAL. My mother has been in Signature HealthCARE of Clarksville for 3 weeks. She’s a diabetic and didn’t have her blood tested for the first two weeks. Every time I visit her, the person changing her bedding complains about the nurses “sitting at the desk on their phones and doing nothing”. While her bedding was changed, I stood next to the door and listen to an older lady in the next room scream and beg to God, to be put in a sitting up position. The nurses were still nowhere to be found. Now Covid is running through this place and my mom is sick, once again. I wouldn’t recommend this place to my worst enemy and my mom is getting moved tomorrow. As for the “Owner” saying message or call you so you can make it better….you’ve obviously done nothing so don’t wast my time.

Fantastic experience: My mom was placed here after a severe fall for rehab and I can say I felt very comfortable being able to leave her at the end of the day to go home. Their staff went far above my expectations for what I consider to be great patient care and after being in health care myself for over 20 years thats saying a lot. Honestly it is a wonderful place with wonderful staff and I will always be greatful for all they did to help my mom.

Fantastic experience: The staff at Signature Healthcare could not have been kinder or more attentive to my mother. She was a patient twice in a short period of time and passed away at the facility recently. Each time she was there, she was given wonderful care and the staff and administration were caring and accessible. While she was in hospice care they provided us with everything possible to make things more comfortable for her and for us. The facility is in need of a bit of a facelift, but I understand they are doing some updating as space availability allows. It's a very homey sort of place and not institutional or cold feeling. It's obvious the staff cares about their patients and it doesn't seem to me like a high turnover facility. Of special note is the food, which was amazing. It's very unusual to see full homecooked meals, served hot, fresh and in very generous portions on actual dinnerware in a nursing home. I couldn't get over the great quality and it was wonderful to see my Mom receive such beautifully presented, appealing food. I can't say enough good things about Signature and I'd recommend them over any other nursing facility in the area.

Fantastic experience: My husband has been receiving outpatient therapy and They are amazing. Shane has been helping him so much to correct his balance issues and to walk without falling. We are so appreciative of the help. I would highly recommend the Therapy department. They are awesome.

Positive experience: My father spent the last few years of his life in this facility. All in all, Signature took amazingly good care of him. He beat Covid, had urinary tract infections which were treated properly and suffered a list of other maladies due simply to his advanced age. All were dealt with properly and professionally. The administration also kept us in the loop very well. I would say that anyone considering long term care for an elderly loved one, could do very well here. But here is the down side: Someone here is a horrendous thief. Over the years, my father lost several iPads, candies, bluetooth headphones, his false teeth (which took months to replace) and cash. In fact, any cash, even coins, that might be left in the drawer on his night stand or out in the open, would disappear very quickly. I suppose there is no way to completely prevent this but nursing homes must be based on complete trust and this sort of thing completely destroys credibility.

Negative experience: Absolutely HORRIBLE! My mother-in-law was discharged from the hospital to Signature for rehab therapy. She arrived on a Friday evening. I had stopped by 4 hours earlier to see if I needed to bring certain things and to find out her room number. When she arrived by ambulance, the bed was not even made. No one bothered to greet us. The nurse who finally walked by was new and had no information. The following day I stopped by and the oxygen machine was not working properly. No one knows how long she had laid there without oxygen. I left and brought her oxygen machine from home. On Sunday, the lunch she was served was sloppy Joe with fries and a glob of mashed potatoes. She is a heart patient on a low sodium diet. The following Monday I was asked to remove her oxygen machine. I said I would when one was provided that actually worked. Then we were told she could not have her CPAP machine in their facility. On Tuesday, their administrator called 911 and claimed my mother-in-law was in respiratory distress. She was not in respiratory distress. She was sitting on the due of the bed talking to the fireman when I arrived. They were simply trying to get her transported out because we pointed out the awful care she was receiving and then blamed it all on her particular CPAP machine. She left with me in my car because there was no medical need for her to go to the hospital in an ambulance. We went back to get her medications and were refused. Signature would not release her medicines nor would they forward her prescriptions to her pharmacy. The administrator claimed we had left AMA. They were crossing T's and dotting I's to make their paperwork say what was necessary to get her out. This was all because their mistakes and terrible care were being pointed out.

The Rehabilitation Department (Physical Therapy, Occupational Therapy, Restorative and Speech Therapy) embraced my Mother at her lowest point (physically and emotionally). Mom’s diagnosis was spinal stenosis mimicking symptoms of paralysis (pre and post surgery). The Signature HealthCARE Rehabilitation Team set goals and a detailed course of action. The plan was aggressively followed and supported by all of the Signature Staff Members (our family)! Fantastic experience: I have worked for Signature for four years now and can honestly say you can feel the warmth and love that is spread throughout this facility the moment you walk in the door. Our residents are our family!! We love each and every one of them! We are all thankful, grateful, and blessed to have Brandi Cox and Misty Patterson lead us daily! This is the best decision you will ever make for your family. Our rehab department is outstanding with over 100 years experience between them led by the one and only Katie Beth Hankins. Nursing team cannot be beat, you will be incredibly thankful if you get to experience how they give their all every day. We have a chaplain on staff Bill Smith to nourish your spiritual needs and for a good laugh when you need one. This teams works hard and we deliver!

Negative experience: TERRIBLE CARE for my 86 year old Mother who was there for 8 days following a accident. The staff and nurses just did not want to be there in there jobs and it showed. My Mom needed help going to the bathroom and there was little to no help as she was told she needed to do it herself that they were to busy. When checking her out of the center the nurse came in and did not want to answer any question and in fact lied to us about the checkout procedures and stormed out of the room talking to herself out loud leaving us thinking she must be on drugs. Some of the staff and nurses were very kind and caring but the other half was horrible.

Fantastic experience: I love working at signature.! My co-workers are AMAZING.! They really care for their residents. The Therapy team is extraordinary they work so hard to get these residents back in shape. The management team is also amazing. Fantastic experience: My mom was here and i have to say the dietary staff was amazing. I never had to worry about leaving mom to go get food. I was offered a tray every meal. Therapy worked great with mom and understood her limits. I'm thankful we chose Signature Healthcare of Clarksville

Fantastic experience: I had a massive stroke, and after my three-week stay in the hospital, l was transferred to signature to complete my stroke rehabilitation. When I got there I literally couldn't sit up on my own.They all worked with me endlessly. They encouraged me daily and challenged me in my recovery and gave me some quality of life to get me to a functional state to go home.They made me feel comfortable.A very family-oriented environment;which was a plus in my recovery in my opinion!Just knowing someone genuinely care about how well you recover speak volumes.It's not just a job to them.They have a truly remarkable Rehab Department.I am forever grateful for all the hard work put in on behalf of me.

Fantastic experience: Before working for Signature of Clarksville, I worked for another company that worked closely with them. I have always adored this facility and its stakeholders and now I work for Signature myself! I would send any of my family members or friends to this facility. The staff have ALWAYS felt like family and I truly appreciate the spirituality behind this amazing place.

Negative experience: One of the worst experiences of my life. My grandmother was admitted to the rehab wing. Dried blood on the floor in her room when we arrived. Constant, horrible smell from residents not being cleaned. One nurse for an entire wing. 30 minute wait for bed pan. That's when I was there, so no telling how long when I wasn't. Physical therapy had no time to speak with me. Every time I came to visit she was sitting in her own urine/feces. Was verbally attacked by another rehab patient for screaming out in fear. She could not walk due to surgery and experiencing mild to severe dimentia. They did not provide her with bed rails. She got up and fell, on her bad leg. I had to ask for an x-ray to be done because nurse thought "vital signs" being stable was appropriate. After she fell I told them I would be by in the morning to get her and take her home. Only reason I sent her there was for her to be kept safe and it had become obvious they were incapable of doing this. They told me it was against doctors orders for her to leave and I had to pay $200 just to get a ride service to get her home. AFTER struggling in parking lot for over an hour. Medicare/Medicaid seriously needs to start a program where patients can be safely transferred from the hospital into their home! If this was possible I NEVER would have sent her to rehab at a nursing home! She starts home health tomorrow and is so happy to be home. Since she has been home her mental state has improved dramatically.

Negative experience: It was one of the most horrifying experiences of my life. It was understaffed at night time and during the day. In the beginning, my mother was unable to use her arms or legs after being on life support for 13 days, . She would wait all morning for someone to come by and feed her. By the time anyone had paid attention to the fact that she couldn't feed herself , her food was cold and old This apparently fell on deaf ears as I complained for two weeks about it... I once went out to the store while visiting her and an aide left a bed pan under my mother. When I got back 45 minutes later, the bedpan was still there and she was begging me to get someone to remove it. They often made her wait 45 minutes if she asked for a bedpan. I also complained about this. My dog doesn't wait 45 minutes. One night, another patient came into her roomt and was feeling along the walls. This scared my mother so I informed the staff about it. The nurses could be abusive at times bending down over my mother and telling her "lay off the call button"..again, my mother couldn't walk. A nurse also told my mother that her " daughter, "thought she knew everything." They obviously do not like you bringing up serious issues regarding substandard care.. At no time did I ever try to impart medical knowledge on anyone, I kept my eye on my mother. One time I had a bad case of pneumonia and had to run out late in the cold night to help my mother because someone had verbally abused her. These were significant and serious issues that need to be investigated. I would complain about these important matters to the director. . My mother was unable to walk at all in the beginning, nurses told her to "stop using the call button" When my mother was finally able to come home, not much had been done as far as physical therapy. The physical therapists blamed my mother and said medicare would not pay for my mother because she wasn't making progress..This was not true, My mother had been there 36 days and medicare gives their patients 100 days. It was their fault she made no progress.. There was absolutely no encouragement or motivating behavior on the staff's part. I came to the conclusion most of them were just didn't care. It wasn't their mother or father or family members. By the time I told them I wanted her discharged after being there over a month, she was still unable to walk so I asked for medical transport home. I got a van instead and the driver who would not help her into the home. We almost dropped her twice. He stood on the porch with his arms crossed doing nothing. I would have transferred her to another rehab facility but the therapists were writing notes my mother wouldn't participate. This is obviously untrue, as she works hard with her therapists from Gateway. I would not admit my mother to this facility if I had it to do over again. Take heed...watch carefully and be involved in your loved ones care at all times. I only hope no one from this facility or any other experiences what I have. As much as I disliked their treatment of my mother and me, I wouldn't wish this treatment on anyone.
