Fantastic experience: WOW WOW WOW!!!!!!!! I want to start by saying that when the time comes as caregivers that you feel like you have no other options and what you’re doing isn’t working that making the decision to put your loved one in crisis in a short term facility is not easy. We struggled for weeks on end before finally making the call to Unity to get the process started. My FIL is only 63 years old and has suffered from Parkinson’s disease since 2016 and the past 2/3 years he’s began to develop dementia. He has faced SO many life altering changes just this year and was starting to do better but then rapidly declined. From June to August 19 (the day he was admitted) every day seemed to be worse than the last, no medication combo was working and his medical team was no help. We placed him at Unity on 8/19 and he completed a 20 day stay there. I was so scared of him coming home and being angry at us for our decision but we genuinely didn’t know what else to do and we were worried about his quality of life and safety. His medical team, social worker and discharge coordinator held a weekly phone meeting with us to address progress, med changes, concerns and his day to day life. We were allowed to visit him 3 days of the week and we were allowed to call him 3 other days of the week. The first couple of times we spoke to him on the phone and when my husband went to visit we hadn’t seen much progress he was still very confused and angry while still suffering with some very scary hallucinations. On day 16 we got the call that he would be discharged at the end of the week. I will be honest with you both my husband and I were very nervous and skeptical but we shouldn’t of been as my FIL came out of Unity his old self! He’s now able to hold a conversation, he’s calm and not in a constant state of anxiety/panic and most importantly he’s safe now. We were so scared of how angry he would be that we sent him somewhere and that he wouldn’t understand why but he has only been home 45 minutes and he was telling us how thankful he was, how he learned so much there, how useful this place was to him and that he learned a lot of new coping skills to manage conflict or change in the future. THANK YOU to the staff at Unity…from the admissions coordinator, to the front desk staff, the nurses, social workers, doctors, nurses and discharge staff THANK YOU!!!!! Thank you for giving my FIL his life back!!!! We are so beyond happy to have him home and very thankful this resource is here in our community!!

Fantastic experience: When it comes to worst case scenarios these guys are the best

Fantastic experience: I have read a lot of negative reviews on Unity in Clarksville. However they have been great with my Father. He suffers from dementia and when we visit him at Unity he is looking much better than when he arrived. A lot of the swelling in his legs has improved, his demeanor has improved, they appear to have given him excellent treatment. The only improvement I could recommend is verbiage some of the staff use when engaging with guest and themselves. It at times can come across as unprofessional. However as stated their actions with the patients seem to be very professional.

Negative experience: The staff at this facility were disgusting and so unprofessional with my grandmother’s care. Having nurses and techs be visibly bothered while asking about her wellbeing and making comments like “Yes. your mother/grandmother has a mouth on her” made me livid. Working in a mental health facility would lead one to believe that there would be more compassion and empathy for not just the patients, but their families as well. This facility is not just a disappointment but an embarrassment. Their palpable indifference and disregard for quality of care is infuriating. Please do not send your loved one here for care.

Negative experience: Whatever you do, do NOT let your loved one enter this facility. At admission, they ensured that my grandmother was going to receive quality care- she got the exact opposite of that. They over medicated her to the point where she could not even feed herself or speak properly. Visiting her was pitiful. They also claimed she was “putting herself on the floor” even though they did not ACTIVELY see her doing this- they would just walk in the room and see her on the floor. How do they know that she didn’t just fall When asked how she was eating, they claimed she did not have any trouble feeding herself which was a LIE. My family started asking more questions about the shady behavior going on and they sent her out for a “possible stroke” which turned out to just be over medication. When leaving the facility, a staff member told my mom “she’ll be back.. maybe not this place but she will be back in a facility like this.” Who says that to someone?!?? The hospital let her detox from all of the medicine and she is improving GREATLY. NO BEHAVIOR ISSUES. SPEAKING NORMAL AGAIN. This facility disgusts me and it should be shut down. I cannot believe this place is even up and running.

Negative experience: MY FATHER N LAW WAS SENT TO THIS FACILITY SO THAT HIS MEDICINES COULD BE ADJUSTED DUE TO THE SEVERITY OF THE AGGRESSION HE HAS FROM VASCULAR DEMENTIA. HE HAD BEEN THERE FOR THE PAST 3 WEEKS. HE WALKED IN THE FACILITY, BECAUSE THE FIRST TIME WE VISITED HE WAS UP WALKING AROUND AND LAUGHING. AND WHEN ME AND MY HUSBAND WENT TO VISIT HIM RECENTLY HE WAS DOPED UP IN A CHAIR WITH A BED SORE UNABLE TO WALK! WE DEMANDED ANSWERS RIGHT THEN AND THERE SEEING AS THE STAFF LIED TO US THE WHOLE TIME TELLING US THAT THEY WERE DOING PHYSICAL THERAPY WHEN THE CONDITION THAT WE FOUND HIM IN SAID OTHER WISE! ALSO KEEPING A PATIENT IN A CHAIR AND JUST BENDING HIS LEGS IS NOT PHYSICAL THERAPY! WE WERE ALSO TOLD THAT MY FATHER N LAW REFUSED PHYSICAL THERAPY, WHICH I KNOW WAS A LIE BECAUSE WHEN SOMEONE IS DOPED UP ON MEDS LIKE HE WAS THERE WAS NO WAY HE COULD HAVE REFUSED ANYTHING BECAUSE HE WAS OUT OF IT! 3 PEOPLE FROM THEIR MANAGEMENT STAFF CAME THAT SATURDAY THAT WE VISITED, AND IT LASTED 21/2 HOURS AND THE ONLY THING THAT WAS ACCOMPLISHED FROM THAT MEETING WAS A DECREASE IN HIS MEDICINES! THIS MEETING WOULD NOT HAVE HAD TO TAKE PLACE HAD THE STAFF DONE WHAT THEY WERE SUPPOSED TO DO IN THE FIRST PLACE! ME AND MY HUSBAND CALLED EVERYDAY THE WHOLE 3 WEEKS AND WE KEPT GETTING TOLD THAT WE WOULD GET A PHONE CALL BACK AND THOSE PHONE CALLS NEVER HAPPENED. THE STAFF DID NOT DO WHAT THEY WERE SUPPOSED TO DO FOR MY FATHER N LAW. THEY JUST DOPED HIM UP SO THAT THEY WOULD NOT HAVE TO DEAL WITH HIM. ALSO HE CONTRACTED COVID WHILE HE WAS IN THIS FACILITY! HE IS NOW OUT OF THAT FACILITY COVID FREE AND IS NOW GOING INTO REHAB TO REGAIN HIS STRENGTH TO DO THINGS FOR HIMSELF AGAIN, INCLUDING WALKING AND FEEDING HIMSELF, NO THANKS TO UNITY! NOW THIS PROLONGS HIS RETURN TO THE ASSISTED LIVING FACILITY THAT WE HAVE HIM IN. I AM VERY DISGUSTED WITH THIS FACILITY AND THE WAY THEY TREATED MY FATHER N LAW WHO IS ALSO A VETERAN! I HIGHLY URGE THOSE OF YOU LOOKING FOR A FACILITY FOR YOUR LOVED ONE, PLEASE DO NOT PUT THEM IN UNITY! ALL THEY WILL DO IS DOPE THEM UP ON MEDICINES, SLAP THEM IN A CHAIR, PUT AN ALARM ON THEM AND LEAVE THEM SO THAT THEY WILL NOT HAVE TO PUT UP WITH THEM! I DID NOT EVEN WANT TO GIVE THEM A ONE STAR RATING BUT IT DID NOT GIVE ME THE OPTION OF NO STARRING THE RATING!

Negative experience: I am extremely disappointed in what I experienced at this facility. Today I spoke with Dr. Reagan and the care team for my dad. Last week I had the social worker threaten to have my dad picked up by the state if I did not make arrangements for him. I shared with the facility that I have been in touch with my dads primary care provider for over twenty years and asked them to give him a call for guidance. Background; I am an only child, I am a single mom, I do have a college student and a very premature baby who requires medical attention. While these are stressors for anyone, they used this against me. I shared with them my personal journey to include I was working on a PhD and I am an attorney and Dr. Reagan asked over five times what law school I attended in the middle of our discharge conversation, They shared that if I did not come up with a plan, they will turn my dad over to the state in the next two days. I have attempted to provide them with information, but they constantly ignore my requests and today was extremely exhausting as Dr. Reagan made no attempt to help but to continue being loud and disrespectful. They hear nothing that I have to say as the patients family member. Additionally, I sent my dad items and they went missing. I have called and called to FaceTime him and they shared at the beginning I can do this anytime, but the technology has been broke (tech is always fixing the iPads, they don't know passwords), so I have had to go days. The communication is off. One weekend, two nurses shared my dad had a very bad weekend and week we had our weekly convo he was all of a sudden much better. They have made a point to say my dad is able to do things that I know he can't. My dad lived with me and was unable to do much, so I know what he is capable of, but they have truly disappointed me because of their lack of respect. This has to be one of the worst places to place your loved ones. I was initially told I couldn't visit only today to hear that I could. I do not trust anyone at this facility and cannot wait until my dad discharges this week. I recorded the conversation to protect me and to share with proper authorities because they are unable to put a person out or lie and state that I am unable to take care of my dad because I am researching and advocating. If anyone desires to listen to the conversation, I am listed as public so feel free to contact me and I can share the conversation to show how disrespectful and unethical this staff is. I recorded the call last week with the nurse staff as well saying this. This place is HORRIBLE!

Fantastic experience: I couldn't ask for a better staff. Mr. Paul truly cares about his patient's never for the glory...though I wish he would take credit but he will not. I am so happy with the way they have treated my Mom. Her Dr is amazing and I'm so blessed to have them...I am blessed to have staff to care her! They are amazing! I know that my Mom is okay and I can sleep at night!

Fantastic experience: I'm new to Clarksville. It's stressful finding any type of physician, especially one available for continuing mental health. I lucked out finding CANDISE BELMONT! She's fantastic, truly. Candise exhibits a reliable and comforting bedside manner. She's grounded, continues to broaden her field knowledge and certainly has a knack working together to find what her patients need to feel their best. She's NOT the type to write a few scripts then send you on your way to get to her next patient. She's a phone call away. That is a huge relief. I respect her diligence, curiosity and kindness. She's also flexible with your schedule. She's a keeper!

Negative experience: I recently went to see the facility and happen to run into a mental healthcare worker that worked there (i saw her badge at the store) I asked her about the facility. She says shes been there a. 5/6 months and cant stand the new management. I was really disturbed because she smelled like marijuana. She said the staff there doesn't care about their clients they are there for a check. I will not be sending my mother there if these are the kind of employees they hire. My mom is a 30 veteran retired and should be treated well. This facility should be ashamed having employees work there who are high and management who can't manage their staff.

Negative experience: My uncle was there recently and he had to go to the hospital because he was severely dehydrated. Tennova medical center ER said alot of the patients from Unity come in to the Er for dehydration they aren't cared for well at all. They are severely short staffed and the patients sit inside.a dining room area for hours without being toileted. My uncle didn't have skin tears when he went in and now he has them all over. They need to be investigated. Dr Ragen is horrible he doesn't even speak to the family at all. Don't send your loved one here. Its just a simple nursing home with a different title.

Fantastic experience: The staff at Unity are some of the most compassionate medical professionals I have ever met. They treat their patients with dignity and respect. They go above and beyond for the patients as well as for the family. I would highly recommend this facility.

Negative experience: A nurse here, specifically Lorna, was very unkind My grandfather was awaiting hospice due to kidney failure and writhing in pain when I asked the nurses if he could be given any pain medication. Lorna responded very sharply that he had all the tyelenol that he could get and that she'd already told another family member this. I, having just arrived, was unaware that he'd already been given tylenol or that she'd already informed another family member. As a nurse, I found her response very unprofessional and unsympathetic. I also understand that nurses are often overworked and understaffed. I can sympathize but want to remind that these are people's family members that they've loved their entire lives that are suffering. There was a lack of empathy today and Im saddened by that.

Fantastic experience: We could not have asked for better care. At a time of uncertainty and emotion, the staff showed sincere kindness, respect, empathy, and love. We are forever grateful to them for the exceptional care given.

Positive experience: we received individual treatment plan for my mom with a great staff and very clean facility.
