Cognitive based therapy is provided by appointment only for patients that have been referred through our primary care services. The clinic provides group cooking classes and a fitness empowerment program for patients.

Come join us in our kitchen and we will provide you with the food to cook a healthy meal at home in your kitchen. Changing nutritional habits can change health outcomes for generations to come.

A social worker is available to provide patient advocacy services including prescription assistance, coordinating additional health services with area providers, and referrals for area social services programs.

Our patient navigator services provide a source of support and guidance; helping you navigate the health care system and resources available to help you optimize your overall health and well-being.

The Community Clinic began in 1993 when several doctors and community members wanted to help those less fortunate receive general medical care for their immediate needs.

Fantastic experience: The resource I wish I knew about sooner! If you need medical care, and don't have many options, call them. The front desk staff are all very kind and helpful. I feel a little more at ease with every visit. I appreciate the supportive environment.
