Fantastic experience: Very customer oriented practice. It is professionally run from the front desk, billing, ease of scheduling, cleanliness, and the overall care is white glove. Annie is an excellent practitioner, I feel my treatment is very patient centered, taking into account my areas of concern each visit. Annie has helped my fibromyalgia symptoms exponentially. I highly recommend this office.

Fantastic experience: My Problem: Partial tears of Labrum and Bicep Tendon per two Ortho Surgeons who reviewed my contrast MRI. Had my right shoulder problem for close to three years. I had 18 Physical Therapy Sessions, and two PRP injections (those out of pocket) but my shoulder did not improve. I wanted to avoid surgery at almost all costs. I'm an avid aerobic and strength training gent who is now 62 but always wanting to improve my physical condition. I discovered Peak Form on YouTube as I was interested in Shockwave therapy. I ended up having 5 Shockwave treatments and 8 extended Physical Therapy sessions (twice per week). My insurance did not pay for these treatments but I was focused on fixing my problem and the cost was reasonable to me. Travis, before each session, would write on the grease board what we were going to do that day. The sessions were intense and felt like I was back in school competing at the Collegiate Level. The pain in my shoulder during these exercises was "good" pain in that most of it was fatigue. Each session I felt more and more confident that my shoulder was improving (about 90% today, was 60% prior to Peak Form). I liked getting schooled by Travis. He reminded me of how outstanding coaches that know how to motivate and get the most out of their players. I spent many many hours reading and listening to patients with chronic shoulder pain and everything Travis was telling me made sense. I'm on a better road to full recovery which is a GREAT thing! A shout out to Daysha at the front desk for making all customers coming and going feel very welcome. Also, putting on the right music to help motivate me:>I strongly recommend Peak Form to get you back to your old self!! It's nice to know who can I can go to in the future as I'm sure I'll hurt something else:>

Fantastic experience: Amazing experience all-around. My 14 year old kiddo has arm stiffness most likely from pitching and was uncomfortable enough to ask for help. First time for everything. Dr. Rose gave a thorough assessment and worked on his arm a bit today, and my kid says the relief was immediate, and palpable. We left with a follow-up treatment plan, and will be headed back soon to continue with it. The whole experience was top-notch. From the initial intake, scheduling, insurance stuff, and payment, it couldn't have been easier. The staff is super friendly, and the facility is clean. Very happy with Peak Form.

Fantastic experience: This summer, Dr. Kevin Rose helped my daughter through tendonitis due to ballet. His treatment got her back to dancing her usual schedule without pain. She just sprained her toe yesterday, right before two weekends of shows, and we felt very confident to ask his advice on whether to have her perform, or hold off. He saw her last minute, and gave her a treatment plan to offer the best chance to perform, while also recommending that she skip some parts of it to keep her safe. We value his expertise and experience in ballet, and sports overall.
