Negative experience: They reported a wrong diagnosed to me. They reported I have a TB, As a result I was discriminately handled by all medical staff to all hospital they brought me. The ambulance staff even dragged my two heavy bags in my legs, and did not event want to put my bag in the ambulance chair, it was put between my two legs. However, after a long series of medical examination to me for 6 days I was cleared and don’t have any TB as presumed by Golden Gate Pavilion

Negative experience: If I could give this place a zero I would. This place does not take care of their patients AT ALL. No one from the social services team is helpful and the RN do not help the CNA's at all. They are always UNDERSTAFFED AND LYING ABOUT IT. The only helpful person I met is Jackie the head nurse. Front staff is so RUDE they hang up the phones and the CNA's all have their cliques and cover each other when they take long lunches. Patients who can not speak for themselves are ALWAYS dirty. IF A PATIENT HAS ANOTHER CHOICE FOR A FACILITY I CAUTION EVERYONE TO TAKE THE OTHER CHOICE. I have tried multiple times to discuss matters internally and NOTHING CHANGES. I am sure there are some staff that work hard but unfortunately there are more terrible ones. I thank all the CNA's who do their best with the amount of work they are doing. THIS PORTION IS AN UPDATE: Bay( the person that is supposedly in charge) is a very rude individual. When I rose concerns about my mother not only did he state “you can take your mom somewhere else” not once not twice but MULTIPLE TIMES, is not helpful when I rose concerns about retaliation against my mother with three nurses!! He said “I am tired of all your complaints”. But your JOB as a person in charge is to handle COMPLAINTS. Bay is not only uncaring but his mannerism leaves a bitter taste in your mouth. I even tried contacting the person above Bay and no one replied. After a horrible meeting(if you can call being insulted and talked over a meeting) I heard Bay call me CRAZY. Unacceptable behavior from everyone there. My mother has had patients fling FECES in her shared room and it took MONTHS for this to be taken care of. HORRIBLE PLACE ANY-OTHER OPTION TAKE THAT ONE!! People in administration need to do better not only for there employees but for the patients.COMMUNICATION WITH THE FAMILY MEMBERS IS NOT AN OPTION ITS A RIGHT. Fantastic experience: Nurse at Golden Pavilion Healthcare center have been providing an unconditional help and exception needed to many patients at the center. Especially my mom’s at 96+ year old, brook her knee cap and been here to rehab for about a month now, and she need lot of help, but all the staff at the Golden Pavilion has been work tirelessly day and night; that giving me the impression of their Professionalism and compassion of everything they do. Thanks to the medical director Miteshkumar Petel M.D, and nurse case manager Dinno Dimaacali , LVN lead the team successfully for many years to come…

Negative experience: Called the facility and the calls were forwarded to a patients room. Unacceptable! My Mother has had her food stolen, and clothes. The others that share the room don't have that happen to them. A CNA had very long sharp nails. The facility keeps allowing these individuals to come in. They also never say their name or have the badge visible. The water for the shower is cold, sewage has come up in the shower which is very unsanitary. They have also tried giving my Mother medication that is not in her chart. When the facility is out of bed pads and adult diapers and wipes, I bring them and they have been stolen too. A patient had access to the supply room.

Negative experience: This place is the opposite of 'good/wonderful/exceptional'. How they received 5 star reviews is laughable. Ask anyone who has worked in this dump. They will tell you the truth about management, corporate and the employees. Beware of this facility. They're trying to make a terrible facility shine.

Fantastic experience: Great patient care and staff support. May need a translator for non-English speakers. Solves the problems of the client's side and being there for them whenever the assistance is needed. There are multi-lingual staff (Spanish, Filipino, some Chinese, some South African) throughout the building.

Fantastic experience: Amazing grace : at first we were lost and finally found the right place. Our mother was sickened with aspiration pneumonia and was hospitalized. We were directed to Golden Pavilion for rehabilitation and was happy it was in close proximity to our house. Day one at the Pavilion was a getting to know you stage. Very different from a hospital. The patient was still incoherent. After a day had elevated heart rate and was feeling delusional. They had to call 911 and send her back to Mills Peninsula. Long story of it , she was discharged too early and prepared to be in a rehad facility. Had to stay another 5 days i. The hospital and finally got better. The caring and compassionate Ms. rachel of the admissions was very sympathetic explaining the procedure of admission and care for Medicare patients. Navigating the whole process was complicated but Rachel walked through us step by step. The care given the nurses and specially the CNA’s and other staff was excellent. They make you feel like family. From feeding to dressing up or just by just checking on them once jn a while. Shout out to Amanda and Erika for their dedication and unwavering care and services. Nurses Miriel, John and Mark were just commendable. The entire staff of the social services especially Dino and Joanne were our partners to get by our needs for our mother from a short stay care to finally deciding to be back hime. Everyone was professional and caring. The logistics and health system at Golden Pavilion is a gold standard top it with great compassion and care of the staff. You just have to be patient to understand the entire system of a rehab or long term care. Fantastic experience: My grandfather Mr.Gaw has been there for 5 months now. All the nurses, therapists, CNAs and staffs are friendly and nice. They are doing very well and helpful to my grandpa’s needs. Especially the awesome staff in the social service office, Dinno, Yana, Gian are very responsive and helpful to all my requests for my grandpa. Moreover, the facility is clean and feels safe. My grandpa is very happy living there. I really appreciate to all the staffs for taking care of my grandfather in Golden Pavilion.

Negative experience: Terrible front desk customer service the receptionist doesn't even look you in the eye when speaking and then interrupts you with " hold on " "hold on " when answering phone no hello no smile nothing .She looks miserable she should get another job. First impressions are everything.

Fantastic experience: This review is for the awesome staff in the social services office—Dinno, Yana, and Gian. Dinno was helpful with sorting through the logistics and options financially for my dad to access care here. Yana was helpful with quick outreach to make sure I had a line of communication open with their office. And Gian ensured reliable transport for my dad to his dialysis sessions. Fantastic experience: My mom was in rehab for a little over 2 months at this facility. The nursing staff, CNAs, rehab therapists and wound care nurses were very caring and attentive to my mom's needs. Furthermore, the admissions person was very responsive to my inquiries and requests for assistance, via texts (even during the weekends and late night hours). She got things done -- connected me to the appropriate staff when I'm unable to get a hold of them. Overall, the facility is clean and the staff is friendly and caring.

Negative experience: My mother has been here for 2 years, I have not gone a month without an issue! It’s been 3 months since they dropped my mother and chipped half her tooth they seem to have no urgency to fix or talk about it. Also not receiving medication due to the carless handling of it. In 2 years I’ve seen 4 administrators, 6 nursing directors and numerous nurses every week there’s a new nurses. They do have a few fantastic nurses that care and enjoy their job. I’ve been visiting 4 times a week for the past 2 years I have made friends with some patients and nurses. Everyone I have talked to from supervisors to administrators do not keep their word or follow up they just delegate and forget. Just got an automated response in 2 hours that recommends calling the administrator SHE IS THE PROBLEM! Funny how I’ve asking to talk to anyone of the owners or regional managers and nobody seems to know anything, even the internet is vague with any information. I would like to be contacted by “owner” or anyone that is competent enough to rid this place of the people filling positions for a paycheck and get competent employees that enjoy their job.

Fantastic experience: I love this place! My grandma was resident at this facility. She passed years ago but I just want to give a good review about this place. The place is very clean, staff are very nice and caring always smiling.. So if you have a family member that needs a rehabilitation this is the place.

Negative experience: They do not EVER pick up the phone at night. The patient is left without access to his call button (which is ignored). They're understaffed. The staff that's there is often hiding in the rooms just to sit around and chat/text on their cell phones. Infections/UTI's often go unnoticed. The only saving grace is that some of the Nurses, residents, and CNA's are good people. Zero is done for the quality of life of the residents. Nothing to entertain their minds, to keep them feeling like human beings. Very insensitive, uncaring atmosphere.

Negative experience: This place is terrible my grandfather was there after a surgery and developed a infection that spread through his body and was physically visible, meaning this was neglect by the nurses and healthcare workers, he is almost septic in a hospital right now. If I could give this place 0 stars I would. DONT LET YOUR LOVED ONE COME HERE UNLESS YOU WANT THEM DEAD

Fantastic experience: My dad stayed at this facility for 3 months and they really good care of him. His nurse's good at getting him to eat when he didn't want to eat even when we put him on Hospice they became even nicer.

Fantastic experience: My mom had to get physical therapy and rehab for a month here. The staff took very good care of her, and she made great progress. We especially appreciated case manager Dinno's efforts to keep us updated and get her the best care options!

Negative experience: Terrible customer service here. I worked for a doctor's office and lots of our patients stay at this facility. They only answer the phone at the front desk but nurses never picks up. Every time you asked to be transfer they do an after multiple times they will hang up the phone on you. Whenever they return your call they will barely pay attention to you and keep talking in Tagalog between them. This place should be report it to the county. Residents staying there complaint that they don't get the assistance they need or that they are not feed it correctly. Please someone needs to do SOMETHING!!!!!

Negative experience: My wife,had a bad expirience over there. She was neglected by a several nurses. When she arrived from Kaiser,she had a huge wound on her abdominal wall. That nurses,didn't even know how to change her wound dressing. They infected her even more. Stayed over there only 3 days. My family arrived to visit her at this day. She didn't feel well. She asked the nurse to call for on ambulance to go to Kaiser Emergency Room for 3 hours. So,she went back to Kaiser with temperature over 101. This is not a rehab. It's a morgue.

Negative experience: If I can give this a -10, I would. This place is a morgue. My brother was a resident. The service is HORRIBLE. They don't answer the phones or when they do, you transferred to another line that no picks up. I've asked to speak to several directors and they keep transferring to the same voicemail that no one checks. The wound care team is a joke. I took all my brother's medical records and per their records, they show they changed my brother's bandages everyday. My brother was admitted to the ER and the ER staff says no change in bandages, which caused major infections. Those lose orders. Majority of the staff are incompetent and can't speak English! When they are asked a question, they look like a deer in headlights and just stare at you. I left several messages for the director of the facility, but I get a return call from a social worker. What part of director do these people not understand? Majority of the staff are timid or scared of the patients. If you are timid, you shouldn't be working in this line of business! The staff tells the patient that they are going to die. My brother was able to speak, so he reported all this to me. Imagine what the staff says/does to the patients who are unable to speak for themselves? I've already reported this facility to the county for their work ethics and abuse of patients. They will be investigated. For those who are reading this review, and you love your family member and want the best care for them, contact/report to Medi-Cal. This facility is paid crazy amount of money by both Medi-Cal and Medicare to care for these patients. Someone needs to held accountable!

Fantastic experience: I have been at st.francis pavilion since the end of December 2018.the staff has been good to me..those I've been in contact with have treated me well. such as Lauren Bueno who was my social worker during my stay prior to her leaving. She has a cheery personality. And will be missed.. during my stay I've meet many staff members such as Anita, cipriano,stephannie, and the other physical therapy folks for there help... and many nurses who's names I don't recall but will remember especially Tim the administrator a really good man and very helpful. overall my stay has been excellent.

Fantastic experience: I have been going to St. Francis Pavilion for more than three years. During these times, I have found Ms. Nygel Pangilinan to be a super great nurse that takes care of my mother in law at this nurse home. She is always ready to take good care of her in any time. I found her an excellent nurse and very professionalisms. St. Francis Pavois lucky to have her as a worker.

Negative experience: I have lived at this place for almost two years. If it wasn't for CHYRIL DACLESON my (cna), she has made it bearable for me to be here. Updated version: This place is the most unprofessional, weak, non-carring, group of people I ever met. Like I said before thier are only a very view employees that are good that's left still working thier. Chris(cna)Loyda (cna).are the ones I trust. Even when I transfered to a nother place they lied about everything they were suppose to do for my discharge. If I was not coherent, ( I WOULD BE DEAD!!!!)

Fantastic experience: I have been here for less than a week so far, and yet my experience of the CNA’s here has been quite special! Since I first walked though the front doors of St. Francis Pavilion, I have been treated with care and loving kindness! The best care I have received is from Nujuma Abdulkidr. It does not seem to matter how important or menial the task is that I ask for help with, that task gets handled with care and competence. And a sense of compassion pervades their every action. It has been difficult for me to be here, and yet Juma has brought both wisdom and timelessness to her caring every single day. It has been a gift and blessing to be healing here!

Positive experience: My husband just got out after a 30 day stay. It started out poorly room was no great roommate was not a good fit. The next morning the Social Worker came in Lauren Porveno and got a new room for me a great roommate. The food was good, Fredericko was also the best very nice people that work their. They are caring

Fantastic experience: St. Francis Pavilion provides a unique kind of home care service for older adults in Daly City, California. My mother became a resident of St. Francis Pavilion early 2017 while my family & I moved to Bay Areas, California. Their staff/companions have helped my mother through an extremely difficult few years. My mother was diagnosed with a form of dementia and Parkinson that make it difficult for her to communicate and move. As my mother's disease progressed she needed more and more help. The service and cares that their staff/companions provided are amazing. I am very pleased with the care that they have and continue to receive. They have cared for my mother like she was family. Overall, I am very pleased with the nurses (RNs & LVNs) and CNAs. Most of them are professional, respectful, and kind. Especial, Evelyn Cabanatan, Matilde Baliwag, and Rina Fe Villena are bright, compassionate, and responsible caregivers (CNAs) who seek out ways to make a positive difference. They are dedicated to providing the highest level of long term care service for my mother and have been always there when we needed them. I don't know how my mother would have survived those few years without the amazing staff/companions at St. Francis Pavilion. I felt that management team wanted to maintain a high level of service and cares.

Fantastic experience: Our mother spent over 3 years at St. Francis Pavilion. Overall I can say that her time there has been pleasant. The nursing staff is good, some better than others, but overall very kind and caring. Kudos to CNA’s Nisa Sotto and Shirlyn Cometas as well as Nurse Nygel for always taking excellent care of my mother. They have gone above and beyond the necessary duties. They truly care for her as a family member. Thanks for all you guys are doing!

Positive experience: I was concerned that the nurses brought my pills already out of the wrappers. Are they sure and how can i be sure. I needed an ointment and it took three days to get its. i could have sent my husband to get it in half and hour but that wasn't allowed. other than that i had a good stay. The staff was caring, it was over Christmas and the staff and patients would sing together etc. I know they are all overworked ( I was a nurse for 40 years) but they just kept working and got things done!

Negative experience: My mom was admitted here and the care was so horrible we had our Mom moved out right away. My best friend's dad was also admitted here and died within a week. We also had a friend that was admitted here and died within 2 weeks. During our visit, we found our mom and thought she had died. It turned out she had UTI so severe we are the ones that called for an ambulance. This place is for dead people.

Negative experience: I am at this facility becaus I can not walk at this time. I hade a very bad accident that one knee and one Ankle are both broken and can not put any weight on them. The staff members are all some as well. For the physical therapy team is does a out standing job of getting the patients motivated and doing the pt side. I do not how long I am going to at this site. I can say that all team members and the regular patients that stay here on a regular basis are so nice to be around at this facility. Yes there are more senior citizens they are fun to be around with. The games night that they do with all senior citizens they can gard some times they may fool you thinking they can not play but they can and thats what makes the place fun to be at. Update on 7/31/18 I have been at this site for 2 weeks now and I do see big problems with some of there staff members. On two sundays in a row the pm shift does not want to answer the call button at the beds. There is a problem with the system and needs to be fixed. Now on Tuesday pm shift it looks like the same team that works on Sundays is working on Tuesday of this week. I can not get out of the bed and stand on my two feets yet. I need my brace for my knee and need some type of a walking ad to help me in a wheelchair. The team does not care about the call button and take there time. The cleaning team does not look at the lights at each rooms to see if any anyone need help or medical need as well. The day time and grave shift staff are good and willing to answer any calls buttons and give medical ad to the parents are on site. This is happening more on the pm shift team. The cna only wants there lunch and does not care about the patient needs or help they are too busy and eating there lunch. Where are all the reset of the CNA doing then and taking there lunch at the same time. This was going on all most 30 mints be for the cna did showe up and to give a add a toot with me and did not like it when I call for help to get change. It seems like some of there staff members does not like the hospital patients any more when you are under the age of 70 that can not care for them self any more. They will not say any thing back to them. This needs to stop with any cna that do not want to work with pride any more. They are looking at this a way get a fast pay check.

Positive experience: Very caring staff. Clean facility. If you ever find yourself in the position where a skilled nursing facility is required, St. Francis Pavilion is worth considering. From aides, to nurses to all other staff members, every one is empathic, kind and extremely hard working. The teams here do jobs that family members are unable or unwilling to do. They perform these tasks with care and tenderness. Thank you St. Francis!

Positive experience: I had the good fortune to get a single room at St. Francis Pavilion for my recovery from total knee replacement. I'm sure my experience would have been different if I'd been in a room with one or two other patients. I found the physical therapy and nursing staff professional and friendly. I have type 2 diabetes and someone from the kitchen came to see me about my food preferences, and did a credible job accommodating me. I had a good experience and was able to walk out within a week.

Negative experience: I renamed this place "Ramshackle Estates." I was there 1 week December 2015, following 3 weeks in Seton for Sepsis. I had to constantly remind the staff when it was time for my meds. The wait for a pitcher of water was an hour. It was difficult to explain medical issues to the nursing staff, who predominantly spoke English as a second language. I was grossly mismatched with a roommate. This is not a rehab facility, it is a nursing/hospice place. I am a vibrant, bordering on immature, 50 year old. My roommate (no disrespect intended) was in her 90's and near deaf. Her TV was always on SO loud. When the nurse wouldn't respond to her call button (which was frequently), she would consistently badger me until I too wrung for the nurse. i am being generous with 2 stars.
