Fantastic experience: I just wanted to give a review to Tania from social services she was very friendly and knowledgeable and gave me a run down of all the programs that they have at the facility she was very nice and very helpful and answered all my questions that I had to determine she was a big help very friendly staff thanks again it’s staff like her that make places stand out a lot she took pride in her job to go beyond answering all my concerns and questions thanks again ..

Fantastic experience: I have been working with the Downey Community staff for well over 10 years. In the most recent month's, I worked closely with Tania Sanchez in the social services department. Ms.Sanchez has proven herself to be Professional, Effective, and Efficient. I have found her to be one of the companies' greatest assets. Fantastic experience: The staff here is just wonderful! They are kind, courteous, caring, loving, & showed so much patience! They made sure I had everything I needed & then some. When I was feeling down on myself, not being able to care for myself, they encouraged me & let me know it was OK. All my concerns & needs were met! The food here is delicious & nice size portions are given for breakfast, lunch, & dinner. Coffee r tea is served with every meal. The facility is clean & neat. My favorite activities was arts & craft & jewellery making. Most of my socializing/interaction with the other patients was in physical therapy. We, the patients, looked forward to seeing one another in the morning! I was unable to bear weight on my left knee. The care I received from the employees of VERTEX (PT & OT) blew my mind away! I thought I was going to have to accept another limiting or debilitating condition. But VERTEX improved my strength, stamina, balance, & my confidence. They gave me a new song to sing & a better pep in my step. May God continue to bless the works, care, kindness, & encouragement of each staff member that not only changed my life, but the lives of many other patients. I will truly miss seeing them everyday. It was the highlight of my day! I'm gonna miss being around the staff & the friends I made while a patient here. I'm blessed to hav been sent here!! Even though I'll miss everyone at Downey Community Health Center & the staff of VERTEX, today is my discharge day on 3/15/24! In the words of Dorothy from the movie, The Wizard of Oz, "There's no place like home!" As an extension of my family, I will continue to pray for the patients & the staff. Fantastic experience: Aracely activities director is the best!!!

Negative experience: My mother was only there for one week and two days. This is the most horrible health care establishment to have a love one. Most of the CNAs are rude. A couple of LVNs and RN if you ask them for something, they will answer back, let me get the CNA to get it for you. And you will never get it. Won't go into more details of this place because it’s not worth my time.

Fantastic experience: Son atentos(as) con los pacientes tengo buenos recuerdos porque mi papá estuvo ahi y les pedía café ☕️ en cualquier momento y si se lo llevaba recuerdo que es un muchacho de la cocina gordito de nombre Jesús Barba siempre fue amable con toda mi familia cuando hibamos a visitar a mi papá.

Negative experience: Most worse nursing home in southern California I wonder how much money did they pay to print a sign that says best nursing home in America for 2022 and 2023 by Newsweek that is posted outside the complex . Went to visit my father I asked the nurse for the TV remote so my father and I can watch TV together. They never brought it to me . I was totally disappointed the patient resident are half the time yelling and screaming in need of something from food to water. I brought up my concerns to the head nurse of the facility who is a Filipina lady who is very rude to talk to. This place is horrible I don't recommended place at all for loved ones to live there.

Positive experience: Just want to start off saying that front desk receptionist patty is great. Makes you feel like your at home. She literally puts a smile on your face the minute you walk in. She is just great!

Positive experience: Downey Community Health Center may be difficult to find but it’s a well staffed facility and very sanitary restrooms and cleaning staff. A caring environment for a family member. Great Social Workers and nurses!

Positive experience: Kudos to the cook at the facility. My Dad’s food always has a great smell and he says the food is delicious. Grateful for the staff. Fantastic experience: Visited a family friend and the staff was exceptionally nice and very knowledgeable. Very clean nursing home.

Fantastic experience: The nurses and physical therapists were pleasant and skilled, when my niece stayed there for a week after her surgery. The janitors and housekeepers were busy cleaning every day, so the building was very clean. The employees were nice and worked hard. When she came in, the dietary supervisor met with her to discuss her food preferences. The activities and social services employees visited her regularly. We definitely recommend this nursing home.

Fantastic experience: Excellent place! CNA treats residents like family, kitchen have a long selection of food, even better than a restaurant. Clean nursing home with excellent staff. Residents live a outstanding life in the building, having excellent food, daily social activities and friendly environment.

Fantastic experience: This place has a excellent stuff with really exceptional workers. The kitchen gives a excellent performance and provides a lot list of food options. Juanita the cook gives perfect flavor to the food. Karla from activities makes a exceptional job with social activities for residents. There is nothing I can ask for this building, excellent place.

Fantastic experience: Downey Community is a clean, safe and caring health center. My mother stayed at this facility after she had surgery. The staff and environment where so GREAT to her! My siblings and father couldn't have been anymore happier seeing my mother be so well cared for. My father in law had surgery during the pandemic, this facility was our #1 choice. The girls in the office especially Cynthia Tinajero (admission coordinator) were so HELPFUL making sure everything was taken care of and running smooth. I cannot thank them enough. My family TRULY appreciates the staff and everyone who made everything better for both my mother and father in law.

Fantastic experience: It starts with a big smile e v eryday that i have been. No complaints from me. My mom is staying here and it would have to be 100% perfect or i would not leave her there been 10 days and im ok with leaving her there

Fantastic experience: This is Gayle Conway. I arrived here with a bad blood disorder and very sensitive skin. Every one was so careful and kind. The first person I met was Mercedes and had those guys and girls working like perfect machines. I had never been in this situation before and was humiliated. The entire team was so sweet and put me at ease. LVN Victor T is so kind and greets me everyday with a smile and good morning. He chases down my med changes or anything I may need. LVN Alex is also as great. There are a few CNA’s that I want to mention that really love their job and great at it. They all assess the situation of each patient and their needs. Their kindness and laughter brings me great hope during a bad situation. 1. Alexis Torres 2. Adrianna Rivera 3. Blanca M 4. Diane Last but not least my wound specialist Nora. My saving grace. I have those horrible sores that very painful. She works so hard and very late hours to make sure we are all taken care of. There is Andrew Salvador who’s training and very tender and caring as well. Maria and Marianne RN’s that handle all the other problems we have. They find answers and solutions.

Fantastic experience: The employees are all very helpful and caring. They explained daily on what they were doing to help me and the rehab team was great to work with. They always asked what I would like to work on for therapy and what I can do at home after I leave. I would highly recommend this facility.

Fantastic experience: Downey community center is an amazing place, the personal is very friendly and they care for the patients. Super clean and a lot of activities, they encourage patients to participate in activities, food is decent. Phisical Therapist Peter and Paola are the best and very eficient. Highly recomended.

Fantastic experience: As any new Rehab admit I was very skeptical and I am sure I appeared that way to the staff. I was there daily at 10:00 AM. and stayed till 4:00 PM. The 1st night he fell by bed. I received a phone call. That led to my anxiety I then had a wonderful meeting with CM, SW Head Nurse PeterRehab. I truly came out of meeting feeling much more comfortable. The daily staff were so helpful. The sitters introduced themselves and offered their help. My husband was initially in a lot of pain. The med nurses really tried to be on schedule with meds. And, he took a lot of meds. Please understand there were some hiccups but were quickly resolved. Larry was there for Rehab and that was a chore sometimes. He needed pain meds prior to his time in rehab. That got resolved. The facility was always very clean. The staff (all of them , housekeepers included), I wish I could remember all names. But if I mentioned one then I would have name all and I can’t do that. Please just know I would highly recommend Downey Community Health. Thank you all again. ❤️❤️❤️

Fantastic experience: I was so pleased to go back there to visit my dad without the extenuating covid restrictions and found he was exuberantly cantankerous which means he's been taken care of very well. His appetite and cognitive skills were just perfect. They do an excellent job of taking care of him even though he's a tough cookie..... This is not my first rodeo with this institution. They are wonderful

Positive experience: Facility is clean, they have the rooms divided for possible COVID-19 patients and the ones that are clear, temperature check at the front door, plenty of parking for visitors

Negative experience: They are a horrible place to get into see your loved ones. They are very unorganized. You can not schedule an appointment with anyone on the weekends because only one person schedules appointments. They are very rude.

Negative experience: Do not send your loved one here. Horrible run down place. Neglected our loved one. We brought our loved one home on hospice they let my father come home with a full diaper belongings thrown in a bag like they were trash. Also did not feed my father or listen to any of the request that we asked. Horrible place. Mittie is a horrible useless incompetent person who needs to learn how to do her job. Horrible raggedy place do not send your loved one here you will regret it 100 percent. They don't even speak proper English when you are talking to them just horrible.

Fantastic experience: This facility has excellent management, very good cleaning team the facility is always tidy and sanitary, and the kitchen serves great meals for patients. The RN LVN CNA nurses are very professional they address and meet your needs with your love ones. Receptionist at front desk is so polite and Karla from Activities is the best with visitations and arranging appointments.

Fantastic experience: I have worked at Downey Community for some time now, and only have amazing things to say! The staff is top notch, quality of care is excellent, and things run smoothly day-to-day. I have worked at other SNFs and none come close to Downey Community.

Positive experience: Not a bad place except the tenants there have no courtyard available to even get fresh air. They are stuck inside day after day. We have a brother there and he is stir crazy. No sunshine

Negative experience: Very unprofessional the way they handle the visitations , stories aren’t straight and they don’t communicate with one another . I understand Bc of covid they have different regulations . My grandma is in hospice alone in a room and they’re only allowing us to go one at a time to visit for 30 mins at a time yet when they have 3 people in one room they allow 2 people at a time . Don’t bring ur family members here

Negative experience: Absolutely the worst retirement home in existence, here patients are treated in unethical ways. They do not care if a patient takes medication or not. In certain occasions, they even allow their patients to miss their doctors appointments. Most times patients diapers are not checked, here they allow the elderly person to sit in their feces for hours to go. The negligence towards patients is unacceptable, patients faces are filled with grief and sorrow, as they are constantly ill-treated. Nurses manhandle their patients resulting in their patients to fall, exacerbating their health conditions. One nurse in particular; Olivia has shown the uttermost disrespect towards patients and their families. CHANGES NEED TO BE MADE FAST

Positive experience: My sisters staff was good to her. The SW. Bridgett needs to be fired. She has a personality conflict with my sister and let it get in between her and her job responsibilities. My sister is loud and she smokes and uses profanity, but the real problem is that she is Disabled and was transferred to this location where Bridgette was trying to get her moved from the first weeks she was there. Mrs. Bridgette is not mentally organized. On several occasions we had scheduled meetings with members of the unit on my sisters team. Bridgette always came to the meeting late and asking what is this meeting about then when she was briefed on the meeting She would stand up and say" oh ok I will work on that and i'll call you no Diana give me a call in a few days yes it will take a few days and I will have something for you. My sister had knee surgery and was always told that she has no pain pills and that she needs to get off the pain meds. Her therapy was a walk down the hallway of approximately 4 doors then the staff said Oh are you ready to walk back to your room. And then to bed or the wheel chair. Then most recently Bridgette transfer my sister to another facility and the facility dont take my sisters insurance program. My sister has a bill of 2000.00 as of june 15, 2019. I am going to report the facility to Ombudsman and then see how the facilty deals with Bridgette.

Fantastic experience: A realative stayed there, very clean!

Negative experience: They had ordered transportation for them to take my grandma to get her dialysis at 3:30 PM it is 1:12 AM and my grandma never got picked up for her dialysis. That the worst thing you can do to a 80 year old woman. Especially on a cold rainy day .

Negative experience: 1st time there visiting grandmother and hoping she is not there long. 1. Floor was dirty from another patient area that spread in to the other patients small area they are given. I saw no janitorial staff the time I was here. 2. I was there for 3 hours and saw a LVN twice. Once because we were checking my grandmother in and they were doing their run down of what will happen during her stay. 2nd time was to treat another patient in another bed. 3. Small areas patient areas, that can easily encroach on your privacy when another patients entire family decides to visit with screaming little children. And the family takes more then their share of the amenities of this shared room and blocks access to sinks and bathroom for the other patients in the room. 4. Food was horrible, if you can even call it food. Im not sure what was on the plate they brought, They also said that they would receive a snack around 7. I left close to 8:30 and they still had not brought any thing. 5. The CNA was nice, when i was there and took care of my grandmothers needs when she called for help. Will check in with staff and front desk concerning visiting policies concerning the amount of visitors and age limits required. Screaming children even if they are apart of the family should not be allowed in the patients room. maybe a common room. For someone just released from the hospital my fist experience was very lack luster at best. We placed her here because it was convenient for the family to visit, on a daily basis. We should have listened to our friend who told us to place her at a different facility.
