Negative experience: Update Mother left this facility IN AN AMBULANCE . AT THE HOSPITAL THEY FOUND A bed sore, a MRSA UTI, AND DANGEROUSLY LOW POTASSIUM, AND DEHYDRATION. THIS PLACE IS A DUMP. ALL THIS 3 WEEKS. DO NOT TAKE YOUR LOVED ONE HERE. They do not have any essential supplies or enough staff. Half of the staff are not permanent workers, so the other half is registry workers who are not trained, nor regular workers/on call temps. You never know who will be with your loved one one if they're here. No apointments have been set up for my mom. I asked for an appt to be set up, they just say, "Yeah" and no appt. Yet! Taking mom out of here asap. There is no doctor. The owner is trying make as much money as they/he can at the expense of these frail patients. Most 5 star reviews come from staff. Truth, is I have seen enough to confirm most elderly people die at nursing homes due to neglect. So the stats are verifiable, the state does not do enough to regulate this circus full of incompentence and profiteers. My mom almost died there. Calling 911 and having the hospital run test confirmed they were not giving her vital meds. I had to plead with them to give her a uti test..they told me they did not feel she needed one. Well..they finally gave her one with no result. The hospital uti test confirmed another infection. Never bringing here again!

Negative experience: I visited Southland for the second time today regarding the admission of a patient for Assisted Living. All went well and I was very pleased with Zach and the other people I spoke with. I had hopes that this would work out and that my old friend would find a place at Southland. However my friend just called and said that he had been rejected because he is level 4 care. Their website says: "At Southland Care Center, our talented and focused staff will work with you, your family, and your healthcare provider, to create a comprehensive and effective care and treatment plan. Our welcoming smiles will tell you that Southland of Norwalk is a place to regain your strength and mobility and a place you can call home." and "We are proud to offer a wide variety of residential and supportive services, all created to promote the well-being of our residents – physically, mentally, emotionally, spiritually, and socially." Perhaps this is not true and they simply have not updated their web page. Or perhaps there is some other reason that my friend has been rejected. Keep in mind that he does need physical help, but he is not a social or psychological problem. After years of working in hospitals myself, here in Downey and Long Beach as a member of hospital staff I know that there are some patients who are very difficult. My friend is not.

Fantastic experience: My father has been here for over a year now. And he has shown significant improvement over that span of time. He always have great stories to tell, sharing how he feels comfortable staying in SouthIand Care Center, and how he is being treated with care. Just a couple of weeks ago, I learned that my father got hospitalized again. I immediately booked my flight and flew all the way from the Philippines to the USA, just to see him. He stayed in the hospital for a while, but would always request to be brought back to Southland. With God's grace he was transferred back recently. I felt relieved because I know that it has always been his heart's desire to be back to Southland. With my own eyes I saw how the staff have been very friendly and helpful, how they treated their patients with care, and go extra mile, rooting for my father when he does his therapy. After being in the ICU for days, and hospital stay for weeks, now I can see that my father is starting to regain his strength, he is able to eat again, and sleep comfortably in his room. Leaving the USA soon makes me feel sad but knowing that my father is in good hands gives me peace, even when I am away. Thank you Southland Care Center. ❤️

Negative experience: My mom was in this facility because she fell during rehab time in the same facility. She had a surgery due to falling, and some of the technician and nurses were not friendly at all. I wonder what was the rehab technician doing? How did my mom fall under their care. My mom suffered a lot due to this fall and it wasn’t her fault. Technician: Veronica was lazy and passed body shaming comments for my mom. Food was not good at all in the facility.

Fantastic experience: Two years ago I realized my mom couldn't live on her own anymore and decided it was time to move her into an assisted living facility. My first impressions were that the Southland assisted living facility smelled clean, not like some others I had visited. The common areas were kept clean and the staff seemed to take pride in the appearance of the facility. You could always see somebody cleaning somewhere. I decided upon Southland because they were able to provide the services my mom required and we moved her in within a few days when she was released from the hospital. I was told later that was the quickest they had ever moved a resident in, but I think they handled it like true professionals. Mom’s first year at Southland showed an improvement in her overall health. The combination of regular meals and knowing staff was nearby to help when needed provided her comfort and security that she could not get living alone. She did not visit the hospital one time during her first year! Mom lived there for a total of 20 months. During that time, she experienced exceptional and consistent care from the staff. This included daily cleaning of her room, meal delivery, and assistance with restroom visits and other mobility issues. Staff would check on her regularly and sometimes would just visit for a few minutes. She especially loved bingo and the other activities at the facility. She had great neighbors too. Mom had some challenges during the last part of her stay, including a few hospital visits and a bout of Covid-19, for which she had to be confined to her room. Overall, her stay there was very positive. If I had to make recommendations, improved WiFi so her TV would work better. As with any institution, food is always an issue. There were some menu items she really looked forward to (grilled cheese sandwiches and Mexican food), and others not so much (too much fish). When mom’s health deteriorated, the facility was able to accommodate her in her own room during hospice care. When she ultimately passed away, many of the staff members asked if they could spend a moment with her to say their final goodbyes. The staff truly cared about my mom, and it showed in both their actions and their words. I credit Victoria, the facility administrator, for providing the leadership and vision for Southland assisted living and making it such a nice facility. Knowing my mom was in good hands brought both of us enormous peace of mind. Thanks again for everything!

Fantastic experience: This is a great facility! For being a nursing home it doesn’t really feel like a nursing home at all. Staff is super friendly. They have private rooms for rehab patients which is cool! They have great assisted living as well which I found out is different than just a nursing home. Overall a really good place.

Fantastic experience: Kaiser provided a list of care facilities that they interfaced with for postoperative care and rehab. My choice was Southland Care facility due to the ratings I read about. I also have a friend who also recommended it because of the care her mother received in the past. I was very satisfied with the facilities along with the staff and workers who took the extra steps to provide excellent care. The rehabilitation group was also top notch, five stars. I had some unexpected medical issues that affected my planned care and rehab, each of the groups adjusted the medications and the rehab constantly by monitoring my blood pressure to allow me a timely release from the care center.

Negative experience: My father was admitted to this facility in June 2022 for care after his spinal surgery. The facility DOES NOT read patient charts or adhere to doctors orders. A special bed was ordered the day he arrived to prevent wounds and although it came the next day, the staff let the bed sit along the wall for three days before they placed him on his bed. When he arrived he had one small blister. From June 30-July 3rd his blister turned into an unstageable wound. He was not turned every two hours, nor was he placed on the bed which would have prevented this. My father is now fighting for his life, having endless surgeries for a bedsore wound that could have been prevented. The nurses that were assigned to him said they were not told to turn him every two hours. When cnas did come into his room, they stated that someone else would need to turn him because he was too heavy. This facility, to date, has not accepted responsibility for this. It did not have to come to this and they need to be shut down. I would attach pictures but they are too graphic. We will be taking legal and media action against this facility.

Negative experience: I would of give this place a zero. First of all, do not put your parents/grandparents in this facility. All the nurse are very lazy (especially the 2nd floor). Nurses will talk back and not listen to protocols. We been asking the nurses to put a phone in my grandma room….. and have not done so….. it’s been a week. The nurse name Nancy, is rude. She asked me that she TOLERATED my grandma.. Never had I ever had a person talked to me about my grandma like that. And another time my mom went to see my grandmother and the door was closed. The nurse Nancy came out and told my mom that my grandmother requested the door close. When Nancy told my grandmother that she offered it, my grandmother said that she never requested the door closed.

Fantastic experience: At Southland facility there was great, attentive, and caring nurses and staff. The all over staff and administrators are the best you could have. Also. My mother had the best time at their physical therapy program. We will always recommend this facility to anyone that may need one. Thank you Southland Care Center.

My father has been under their care from September 2nd till October 24th, and we have been incredibly impressed from day one. The entire team at Southland Care Center, from the dedicated nurses to the skilled rehabilitation specialists and supporting staff, has demonstrated a level of care and compassion that is truly outstanding. They don't just provide medical care; they provide emotional support and go above and beyond to ensure the well-being and comfort of their patients. The rehabilitation specialists have played a crucial role in my father's recovery. Their expertise and patience have helped him regain his strength and mobility, making his journey to recovery much smoother and more positive. Moreover, the facility is always clean and well-maintained, thanks to the hardworking housekeeping and maintenance teams. The friendly receptionists and staff ensure that every visit is a pleasant experience. Southland Care Center is a shining example of what a rehabilitation center should be. Fantastic experience: I had craniotomy Aug. 18th. I had right sided weakness & some aphasia. I embraced the process of healing with the support of physical therapy & occupational therapy. Their OPD rehab place had competent & very nice therapists. My last day was on a Friday 9/29. They said it's my 'graduation day' I really will miss them. They guided my way to newfound strengths. Negative experience: If I can could rate this facility a Zero I would but you can't. My mother was in this facility shy of 2 years. September would of been 2 years that she would of been in this facility. Many things didn't go as planned, my mother's care. She was retaining water, they did not pay any attention to that even though they had 24 hour care in this facility. We had noticed that she was going through a bit of a memory issue Prior to being admitted. We asked the facility if they can help us in trying to figure out if she was in some stage of any memory issues. They did not , their response was that she was not there permanently so they couldn't help us out. I was confused because she resided there 7 days a week shy of 2 years ??? They will leave her unattended, this I know because when she came home with 8 boxes and 10 plastic bags, I found that she was hiding food in her belongings that she had someone help her pack. When my wife and I try to speak to anyone in charge that would be hard to do so. Because sometimes we couldn't find anyone or their mailbox would be full or even better wouldn't be active to leave a voicemail. We even asked to speak to the director of this facility and nothing happened. We even went to such lengths to file an aggrievance. She has been home for a little over a month we ahad to learn the hard way all in a day. Both of us are going to work and she was unable to do her Adul Daily Living needs. And without any hesitations we went from two income to just one. And we went from a two bedroom to a one bedroom. To provide 24 hours needs for my mother. No she's in her best primary care and the ball is rolling.

Negative experience: Would not recommend. Staff seems to be short or just not very caring. When items arrived there with patient, they were lost and misplaced, as well as new items that were brought after patient arrived for the stay there, - we’re also lost.

Negative experience: If for any perusal of my following comments as a recent (June 15, 2021) and previous Southland visitor, these may provide a means of avoidance for an otherwise disconcerting experience such as mine. On a practical level for visiting access, even in terms of a Covid-related transitory stage, both Southland telephone and website sources for information are either exceedingly limited or nonexistent. Such as would appear a Covid update (last produced for March 2020) is incomplete and seldom relevant to the current practice with change of visitation. Relying on telephone contact is also problematic, but with persistence(on and off for 2 hours starting at 6pm) and my sister trying on another line, we reached a nursing supervisor and were told a visit was possible after 8am in the morning. I arrived around 1039am and was turned away-receptionist said starts at 1pm for about 2 hours, nursing director said130

Positive experience: My mother is a patient at Southland Care Center. The facility is very clean and comfortable. The staff is helpful and caring. For example: Lamay, Patricia, Maria, Todd, and Ana in Nursing Department; Suresh, Rommel, and Trish in Therapy Department; Margie in Social Services; Isabel and Marge in Activities Department; and Jesse, Mizzy, and Max in Administration.

Fantastic experience: Came in to visit my aunt and this place has single private rooms. I have gone to other places and they either have 3 or even 5 patients to a room. It was clean and the screening process went smoothly. Receptionist, Michelle, was very warm and pleasant. Overall experience was nice.

Fantastic experience: As we all know there is not such thing as a perfect place, but the friendly kind and compassionate staff at Southland was impressive. It was clear that the staff that worked at Southland were there to care for the sick and afflicted. The lengths that they go to ensure cleanliness and disinfected facility stays clear from the nasties out there in the community, is commendable. Thank you for being there for our Seniors.

Fantastic experience: This is the only Skilled Nursing/Rehab facility that had private rooms available. They partner a lot with PIH Downey and Kaiser. The staff is very helpful and pleasant. The facility appears meticulously clean and modern.

Fantastic experience: All the staff are welcoming and attentive. The private rooms/bathrooms make it feel more homey. Perhaps the best part are the therapists and the therapy gym. It is large and lots of light pours in. They truly seem to care about getting the patients back on their feet and home.

Fantastic experience: My mother stayed here after having hip surgery and it was a pleasant experience. The physical therapy was great! The place is well kept and everyone is friendly. They also have numerous daily activities for their patients. I would recommend this place to friends, family, or anyone who wants their loved one to get the proper care in a clean environment.

Negative experience: My mom recently stayed at Southland. It was nice that she had her own room, but the care was mediocre, at best. The nurses were very nice, but the entire facility is understaffed. The ladies at the nurses stations don’t really pay much attention to the customers. I appreciate the help, but she won’t be coming back unless completely necessary.

Negative experience: Horrible place how they treat their own cleaning staff no respect from supervisor “Luis”I believe it’s the name of supervisor this person was screaming to staff in hallways no ethics no profecinalism no respect every employee should be treated with dignity and respect unbelievable that how treat their own staff I wounder how humans resources allow all this happening no body report this person if you call call him a person, I can’t imagine how he will treat residents that are not alert I was visiting my mom he make me feel bad just by listening I just imagine poor housekeepers I’ll invite you to report him to department of health and labor commission.
