Fantastic experience: My Brother is post stroke .. he had an extended stay at Mission and found the therapist s to be kind and overall very good.. he became very fond of staff and when he returned to ARU he seemed somewhat sad to leave.. Social services was very helpful with this transition..Gabby Harold Laramie Peter Queta Darrell daisy Pablo and many others Thank You

Negative experience: Patient calls family 2 hours after meals to say, my blankets are constricting and my meal tray is on table where I cannot reach it. WHY AS FAMILY DO I HAVE TO CALL FOR EACH MEAL TO GET TRAY WITHIN REACH?

Negative experience: I am writing this review to express my profound disappointment and frustration with the care my mother received at Mission Care & Rehabilitation Center. Our experience was nothing short of distressing and unacceptable. Upon admitting my mother to this facility, we expected a high standard of care, professionalism, and compassion. Unfortunately, what we encountered was the complete opposite. My mother developed severe bed sores, which is a clear indication of negligence and a lack of proper care. Bed sores are entirely preventable with the right attention and care, and it is disheartening to see that this facility failed in such a fundamental aspect of patient care. In addition to the bed sores, my mother also developed a rash. This further highlights the inadequate hygiene and attention given to her needs. It is absolutely unacceptable for a patient in a rehab home to suffer from such conditions, which could have been easily prevented with basic care and diligence. One of the most shocking and disappointing aspects of our experience was the facility's reliance on my father to change my mother's diaper. This is not only unprofessional but also goes against the very reason we entrusted my mother's care to this facility. We expected trained and compassionate staff to handle all aspects of her care, but instead, my father had to step in and perform tasks that the staff should have managed. This placed an unnecessary and emotional burden on our family during an already difficult time. The level of care my mother received at Mission Care & Rehabilitation Center was far below any reasonable standard. It is distressing to think that other families might be experiencing the same neglect and unprofessionalism. A rehab home should provide a safe and nurturing environment for patients, ensuring their well-being and comfort. Sadly, this was not our experience. I urge the management of Mission Care & Rehabilitation Center to take this feedback seriously and implement immediate changes to improve their services. Patients and their families deserve better. No one should have to endure the neglect and lack of care that we experienced. I hope this review serves as a wake-up call and leads to significant improvements in the quality of care provided by this facility.

Fantastic experience: The Mission Care Rehabilitation Center was the best. The Staff Wound Care service Daniel is a great person. and the PT/OT services are great. LVNs and CNAs.especially Mara was the best of all. Thanks so Much I Miss you all! Home up and walking.

Negative experience: My dad has been in his soiled diaper all day. Nor good

Negative experience: My mother was placed here in 2018 after hip surgery. Staff said she'd be ready in six weeks. She was discharged after TEN weeks in worse condition than when she was admitted. The very next day, I had to take her to the emergency room at San Gabriel Valley hospital, who called her discharge "improper". They then placed her at Monrovia Gardens, where she fully recovered in just four weeks. Wish she had been put in Monrovia Gardens in the first place. Mission Care nearly exhausted her Medical and then had the audacity to bill me.

Negative experience: Dreadful facility. My dad was placed there after surgery. He was put in a bed and left there. After my dad spent time there, he lost a lot of weight and was a shell of his former self. (He has since recuperated). When I called for updates or to check in no one would respond. When I tried to visit him I knocked on the door, pounded on the door really, someone would come and open the door say, "ok, ok" and close the door and then ignore me. I stood outside 45 minutes and this happened 4 times! Avoid this place. When I was there I met their 'social media person' and strongly suspect many of the 5-star reviews have been placed by people they pay for reviews. If you look at reviews, many are posted by people with 1-2 reviews only and are glowing reviews. Staff is insincere and act like they're put out if you ask for some help. They can and need to do better.

Negative experience: Food was not up to par for health building!

Negative experience: My mother just got discharged after a few weeks. She broke her pelvic bone. She was discharged without any pain medication. I just called and got a standard response that in summary was that there is nothing that can be done. In the meantime my mother will be in pain. Thanks Mission Care. You failed. Update It is almost 10pm. My mother is in pain because you failed to include her pain medication.. You offer no solution except to show up in the morning. This is unacceptable.

Positive experience: Food was terrible. They get one star, The highlight of being in this facility is the TLC and excellent service given by the physical/occupational team they have. They get 5 stars. The majority of the LVNs are great.

Positive experience: So far only a three star in my book. My mom was checked in yesterday and the lady at the receptionist desk kept telling us that they’re gonna call us back with updates on when we can visit no call back so far. I don’t need to tell you that my family is nervous and we could sure use a call back right about now

Fantastic experience: I was so impressed with the staff. They were very friendly and kind to me. The administrator was professional and knowledgeable and went if of his way to make me feel welcomed and comfortable. I highly recommend this place.

Fantastic experience: I'm a journalism student and was able to interview the director of Mission Care for a story. Everyone was very professional and there is a clear commitment to care there.

Negative experience: Very poor patient care and follow up one night nurse for 35 patients, totally not acceptable. I recently had a relative there who passed. I'm still waiting for a call from this butcher shop. You would have better luck sending your loved one to Auchwitz He was forced to lie in soiled linen for hours, before he was tended to.

Fantastic experience: My mother spent six weeks in the Mission Care after her hip replacement surgery since April 2021. The nurses, physical therapists, occupational therapists and staffs were all loving and caring. Nicole (the nurse) would hold my mom’s hands to encourage her to eat more. The therapists (Charlene) would encourage her during the therapy to walk again. One nurse set up my mom’s iPhone to have the tidal wave sound to help her go to sleep. With all these professional and emotional support, at the fourth week there my mother was able to walk on her own again with a walker. She is 86 years old and had a three hours surgery! Lee, the administration director had gone above and beyond his duties to support us at the most struggling stage when my mom just got admitted to the facility. At the end he also guided us how to get prepared at home when my mom was ready to be discharged. From admittance to discharge, the whole process was pleasant and smooth. My mom’s health had absolutely improved A LOT after spending 6 weeks there.

Fantastic experience: 5 STARS!!! The first time I came to the facility it was so clean and smelled great. I was greeted by a nurse who personally directed me to where I needed to go. The staff were so friendly and kind. I have been to several other nursing homes and this one was by far the cleanest and friendliest.

Fantastic experience: The staff here is awesome! I love the rehab team how they were with my mom. The cnas and nurses treated my mom like family.

Fantastic experience: Mission Care in my opinion is one of the finest managed, ultra clean Skilled Nursing Centers in San Gabriel Valley. In fact I Woolf rate it one of the top 5 SNFs in San Gabriel Valley. Staff is experienced and most have worked there for years. As an Eldercare consultant it is one of the SNFs I refer people too when ask me for my recommendation. One of the things that impress me the most is the staffs longevity. In the hall of the nursing center see the wall from an employee’s perspective...it pretty well says it all. Also being a professional in healthcare I have never heard of a SNF where the social worker has been there 23 years. Overall Excellent Five Stars!

Fantastic experience: My mother at 88 years of age fractured her pelvis and we were told she may never walk again but at Mission Care with their AMAZING physical therapists she proved the doctors wrong! The entire staff at Mission Care is wonderful. They are not only Health Heroes but they are truly people who care. The place is clean and very comfortable. I go to bed at night knowing that my mother is well cared for!

Fantastic experience: What a beautiful facility! I took my grandmother in following her surgery and as I wheeled her to the front I was met with a pleasant smell at the door. Very well maintained and clean facility. Any staff that we worked with over the phone were Always so friendly and positive. The administrator answered the phone a couple times and was nice enough to take my grandmother the phone himself! Just awesome service!!! I was a little skeptical about her improving given her age, but therapy really catered a rehab plan for her and grandma was home in just a few short weeks. We even got a follow up call to see how she was doing, that is something I did not expect at all! Service is really patient centric - very positive experience. Thank you MISSION CARE!

Negative experience: Let me say upfront that this review only applies to the Healthcare Partners part of this facility. My mother (in her 90's!) was recovering at this facility from a broken hip. I found the facility to be clean, well run and the people very friendly and helpful. If you are a Healthcare Partners patient, however, they will try to discharge you even if you are not capable of functioning at home. Both my mother and her roommate, who also had a hip fracture, were unable to even get out of bed without full assistance, yet Healthcare Partners tried to discharge them both to home. On a Friday afternoon, the coordinator told me that I would have to arrange 24 hour care for my mom at home as she would be discharged Tues morning. I'm very surprised there have been no legal cases brought for patient dumping.

Fantastic experience: Our dear friend was recently discharged from Mission Care, where we visited with him. He was one of those patients who wasn't always patient with the staff, because he was frustrated with his situation and unexpected immobility. But the staff took it in stride and always responded with patience and kindness. His wife wanted me to share that Sofia, Admissions and Discharge Coordination, was "outstanding"; Xavier (nursing staff), Peter (Physical Therapy) were similarly terrific; and Juan F went above and beyond when he intervened with the doctor to ensure treatment for the wounds was in place. The facility itself is clean, spacious, and pains are taken to ensure that there are no residual smells usually associated with these kinds of facilities, which I - as a visitor - also appreciated.
