Fantastic experience: When entering Palomar Vista, many times you can hear in the background the laughter of their team while they care for their many patients and residents. This assures me that the culture here is truly “family” driven. A lot of the team members have been there for many years. Their dedication to the residents and patients shines through. The PV team takes the time to know each and every patient/resident that enters through their doors. Excellent patient care is their number one priority. Bri, the activities director focuses on creating activities so their patient’s days aren’t so mundane. The facility is always clean when I visit. Kat, the admissions director is always quick to respond when I reach out to her. She even responds after hours. Kat will always try to accommodate getting a patient admitted when beds are available. The administrator is always very pleasant and welcoming. They have created an amazing care team at Palomar Vista!

Fantastic experience: I had family friend who was admitted here years ago and I had the privilege of meeting the staff in station 1. The nurses and the CNA’s are so warm and welcoming! They gave my aunt an exceptional care.I am so impressed by how the staff show their empathy, making sure my aunt had everything she needed! Upon my observation, I noticed that facility is well-maintained, clean, and has a welcoming atmosphere. The common areas and patient rooms are designed to promote a sense of comfort and security, contributing to a positive healing environment. Fantastic experience: Can’t say enough good things about this facility.! The most caring loving staff I’ve ever met. Mama and Papa both are here and have had wonderful nursing, physical, occupational, and speech therapy. Everyone here genuinely cares for ALL the residents. Everyone works together as a team.. everyone helps out wherever they may be needed. All the employees here whether it be nursing, CNA’s, office, therapy.. Kitchen,, housekeeping … all have shown great kindness to my parents . Gives me great comfort, knowing that my parents are being cared for by a group of highly passionate trustworthy knowledgeable and genuinely kind and truly good honest people ! I see it firsthand I have been here myself every single day spending time with my parents for nearly 8 months …I myself have been shown great kindness and care . All of these people have become family to us and will be forever grateful for everything they do and have done for Mama & Papa .

Negative experience: This place needs to be shut down they don't no how to take care of there clients state needs come shut it down my mother did not have a call bell for over a week and they kept telling us there was Noone to fixts it this place needs to take accountability for what's is going on I had to call state on this place so please before u bring your family members to this place do ur home work they don't care about Noone but there pay check I don't care if they see this the truth is going to come out ...

Fantastic experience: This is a great place I’ve been at other facilities where you feel like your not getting the medical attention that you need, but here they’re on top of things staff is very attentive and caring, always with smiles on there faces ready to help with whatever you may need they have smart tv’s in the rooms so you can watch literally anything you’d want they have activities so you aren’t just sitting/ laying in your room 24/7 from gardening to bingo and everything in between, exceed my expectations. If need be I’d come back and I recommend this place

Positive experience: My Dad was a resident 8 years ago. At that time Karen King was the Admission Coordinator. She was very helpful with the process of getting my Dad approved for medi-Cal. My Dad enjoyed the food and gained weight while staying at this facility. He enjoyed the Olive Tree in his backyard and even did some sunbathing. The only complaint he really had was that he felt like the bathroom could have been ventilated better. His bathroom did not have a fan. The staff made him feel very welcome when he arrived! I recommend doing a tour prior to deciding if this is the right facility for your loved one.

The CNAs, including Sera, Justin, Dulce, and Sam, have displayed unparalleled dedication and compassion. Nurses Krimson, Jay, and Daniel have consistently gone above and beyond, ensuring top-notch medical attention. The seamless coordination among the entire staff, from social workers to housekeeping, plays a vital role in maintaining the center's excellence. Negative experience: Absolutely no organization or any type of proper management at this facility. The OTs & Pts were very kind and friendly, the cleaning staff were very helpful but every one else is just all over the place. Always took way too long to get pain meds, help with hygiene or just out to socialize. The staff is very outgoing and friendly to family members when visiting but according to the patients, not with them. This place has potential to be absolutely amazing and beneficial with the right people in management and administration services. For now, it is your average what you see on "tv news," facility. FYI... there are companies that will pay/ compensate their employees to review with a 4-5 star... be careful of what you read out here. Be mindful of your loved ones who can no longer speak, defend or advocate for themselves regardless of where or with who you leave them or of the age of your loved one.

This is my third time here and I recommend it to my family and friends. I am always treated with respect and caring. The staff really listens to you and tries to accommodate when able.They make the experience of going to a SNF the best possible one can get.

Fantastic experience: My Grandmother was recently placed at Palomar Vista for rehabilitation. The staff was very friendly and accommodating . They exceeded our expectations and played a vital role in her fast recovery. My family and I were impressed by the staffs professionalism and the optimal care they provided my grandmother while she stayed there for a couple of weeks. I would definitely recommend this place to anyone that is seeking rehabilitation. The food is great and so are the staff!

Negative experience: Disgusting and unsanitary. Agency nurses are a Godsend, the staff nurse was rude, dismissive and completely unprofessional. There will be a conversation between myself and the nursing supervisor on Monday morning so that my voice will be heard

Negative experience: ATTN REGISTRY CNAs: don’t pick up shifts here. They’ll give you 15 patients and the old front desk lady, let’s call her Karen, will terrorize you then play victim.

Fantastic experience: My father stayed here for four months while he rehabbed for a hip replacement. I'd visit morning and afternoons, the staff treated him and myself well. He never told me of any issues and when it came time to leave people were sad to see him go.

Fantastic experience: I was here at the beginning of the pandemic, the staff were well trained and took every precaution to follow COVID-19 CDC guidelines. The physical therapists, Occupational therapists, and the SLP are amazing folks! Overall, the rehab department will go above and beyond to help their patients! (Not to mention the one-on-one treatment sessions) The management/front office staff will answer any questions that the patients or their families may have. Without any doubts that PV will go above and beyond to ensure quality healthcare is delivered.

Negative experience: I just received a phone call from my family member notifying me that he had literally been ignored for over 30 minuets, and in desperation called me as a last resort. He could have been gasping for air, he could have been having a heart attack, and NO ONE would have helped him. Finally, I called and asked for him to be assisted immediately, and I was told that they were all "in a meeting", and that was why they weren't assisting him. I am absolutely appalled, and should a family member or a loved one require this type of facility RUN away from this one as quickly as you can.

Negative experience: As a healthcare professional who works in a SNF under the same company (Ensign), I am absolutely appalled by this place. My grandma went in for PT after breaking her knee and having surgery. She was fine for a couple of days and then started to feel worse. Her ill feelings were dismissed as "she doesn't want to eat or take her medications." I constantly called telling them that something wasn't right and that she would never refuse her medications. I called over and over again asking for them to run stat blood work/tests because she wasn't eating/drinking, complaining of nausea/a sore throat, and declining everyday. I asked them to send her out so that she could receive her medications through an IV since she was having trouble taking them due to her sore throat, and they told me that the hospital would probably send her back because she wasn't in distress when clearly something was going on. She wasn't eating or able to speak. She had two blood transfusions within a three week time period, and for some reason they were only aware of the second one. They should have scheduled an appointment with her doctor (who seemed to never be concerned with her change in condition) to figure out why she was having so many blood transfusions within a short period of time (her hemoglobin was at 6 both times, when the normal for her age is 11 at least). They should have sent her out because her vital signs were all over the place and certainly not within normal limits. My concerns were put to ease for a brief moment when I spoke to the DON and the Administrator who promised to look into it and call me back that same day, but I never received a phone call. I called back that same day, but was transferred to a nurse who told me that she is now scheduled for a GI consult and a CBC panel. I told him to tell the doctor to make it stat because she was obviously not doing well. That never happened. Two days later, they decided to send her out when her BP was dangerously low and she was no longer able to speak or stay awake. She was clearly extremely weak. Once they sent her to the hospital we found out that my grandma was dying of sepsis and only had hours to live. I constantly called and begged the nurses to look into her change of condition but no one would and now she is dead. The Administrator (Dudley Fetzer), DON (Katherine Jersey), and every single nurse's (I spoke to every nurse sometime after shift change everyday) lack of care, response, and knowledge caused my grandmother's death. DO NOT SEND YOUR LOVED ONE(S) HERE.

Negative experience: I would leave no stars if I could. My grandmother went into this facility with normal health and now she is at the worst condition she’s ever been. We have been informed that she does not have long to live, a month ago she was at perfect health and now she can go any day. This place does not take care or help the patients, instead of giving the proper medication to their patients they will leave the pills on the table by the bed with no water. They have no sense of care as they literally leave people on their beds without blankets. On top of that when we went to visit had us waiting for so long because they were “eating”, once we went in we found my grandma in the conditions I stated. If you love your friends and family you would not put them here. There may seem to be a lot of positive reviews but it does not reflect the current status of this place.

Fantastic experience: This place is amazing!!! The rehab team is so kind and caring. They put in the extra work to getting you strong enough to go home and ensure you have all the resources you would need. The nurses are awesome and very helpful as is all the staff. I would highly recommend checking this place out and the services they may offer you or your loved ones. Shout out to Amy, Lorraine, Bernard, Dennis, and Sierra. You guys are great and make a big impact to everyone’s lives there.

Positive experience: Our mom was at Palomar Vista until her passing. I need to thank the following staff who cared for mom like she was family; CNA's Robin and Maurice. We knew mom was in good hands when these 2 were on shift. Nurses- Ada, Jonabell, Maritess, Marybeth. I also have a special shout out to Lulu! She went above and beyond to make sure we could go in and see mom once she went on Hospice care. These are the people we met and if I forgot someone, my apologies. Thank you all for making moms last days here better.

Negative experience: This place is dirty, nurses are rude and careless. They take things with out asking even if they are personal. What a poor place.

Negative experience: I called this facility on June 17, 2020 at 1:45 pm and talked with Ms. Karen. I explained to her that we were researching possible facilities for my Dad. Before, I could ask her anything she immediately went into how they do not have any beds and how she doesn't show any discharge orders to give vacancies. Mind you, I did not ask her ANYTHING YET. I felt her abruptness as a first line for this facility was rude and lacked any type of warmth. Regardless of NOT having vacancies, she did not allow for any questions regarding what the facility provides, nor did she exhibit any qualities reflecting if this facility did have vacancies that I would want my loved one to reside. Your office managers, receptionist, or front desk staff is the face of the company because they are the FIRST person a client, potential client, or simply someone who is doing research comes in contact with, those individuals role is so very important because they ultimately set the tone. For me, this facility needs to remind their front office staff of that and if this is the tone of the front, I am not sure what can be expected of the day to day staff. Covid-19, has made the healthcare profession and the world has a whole very difficult, however one should not forget they are taking care of someone's loved one, and one bad FIRST IMPRESSION, can cause potential clients or undercover reviewer to rate you poorly. Nevertheless those staff that are in this facility, we thank you and I wish you blessings and happiness especially on those tough days. Please remember your WHY, the reason you perused this profession and the reason your needed, but your needed at your best...matter of fact better than your best.

Negative experience: If this is a "five" star place (like they advertise inside) I would hate to see what a 3 star place would look like. I don't like pointing out a specific race or gender but some of the Filipino female employee's (yes nurses and other staff) treated myself and my father who stayed there callously. One nurse was constantly on personal calls ignoring patients. They spoke in Tagalog continuously, which made you feel out of place. We are happy my dads home now and in better care.
