This program is designed to help those who have suffered from fractures, dislocations, or joint replacements to return to their home and be reintegrated into the community. To do this, various goals are set for each patient based on their specific needs, such as teaching them how to walk, climb stairs, transfer, build up their strength and endurance, stay safe from falls, and complete activities like dressing, bathing, and toileting. Fantastic experience: I can't say enough good stuff about this place. When I arrived I was in bad shape and could barely move my arm or leg at all without intense pain. It's been less than two weeks and the amount of mobility I have now is great. I also have a lot less pain and have now have the tools to keep healing up faster and stronger. I can't think of a single thing to complain about everyone here was wonderful. From Dr Shah and the other Drs to all the people here that helped me get better. I would like to mention them by name they did such a great job. People like Sergio, Joanna, Lucy, Kyle, Andrew, Mirian, Rob, Ann, Rita, Kirti, Honey, Allison, Leslie, Joe, and Savannah all were amazing at helping me improve and get better. My only regret is that I can't remember everyone's name to give them all credit I'm sure I'm forgetting some. Again if you are in need of rehabilitation care you couldn't pick a better place than the Palomar Health rehabilitation institute. 5 Stars all the way these people are freaking rock stars of rehab!

Fantastic experience: My husband has been a patient at his facility twice. The level of care is outstanding. The facility is clean and well stocked with the type of equipment to assist rehabilitation for various injuries and health issues. The staff is wonderful. The doctors, (Shah and Dr. B) were caring, available and helped us feel we were always listened to answering all of our concerns. The nurses, and assistants were always there for us. Most of all, the therapists make all the difference. We worked with several, but three stand out in my mind, Gina, McKenna and Kyle. All were caring and trained to help my husband get back on track. Negative experience: The security guard is picking pumpkin seed shells out of his mouth and then writing our names on visitor badges and then handing us our badges with his spittle on them. GROSS! He was rude and aggressive about enforcing a two visitors only rule because we had several family members here to see my mom. But we saw other rooms with more than three visitors. The therapists however are top notch and did a fantastic job rehabilitating my mom!

Fantastic experience: I spent 5 days last week with the team at Palomar Health Rehabilitation Center and each part of their team did an incredible job helping me to get mobile again. After an ugly bike accident on Del Dios Highway and 7 days at Palomar Health Hospital -- I moved to Palomar Rehab. The Therapists were tremendous -- all working persistently and creatively to get me back on my feet. The nursing staff helped me through a very difficult and painful stretch and patiently took care of my every need or request. Finally, the team of doctors spent time with me answering all my questions and helping me understand my path to recovery. Amazing.

Fantastic experience: The entire staff at Palomar were very helpful and kind. My nurse Wendy was very nice! Dr Shah and Dr. B we’re able to answer all my questions and were great to work with. Jesse was my PT and he helped a lot with my exercises and pushed me to work hard in my recovery. I’m grateful to have had the opportunity to undergo rehab at this facility.

Fantastic experience: Following a catastrophic fall and needing neck surgery I was lucky enough to go here. I was blown away by the modern facilities, private room, etc. having spent some not so pleasant times in facilities with an elderly parent. The therapists, doctors and nurses were very kind and supportive as I struggled with something as simple as walking. 7 weeks later on January 1st I went on my first mini hike, all the while being reminded of what my therapists told me. Thank you Richard, Dr's Patel, B, Shah and the amazing nurses who were so supportive, April, Gerry, I cannot remember all their names and all the therapists names. The journey is ongoing and will take a while but so much progress from the start here following surgery.

Fantastic experience: This place is an amazing. They genuinely helped save my life. They put so much work into me, and gave me hope when I was at my most hopeless. Therapy staff, nursing staff, counseling staff, and administrative staff were great in so many ways. I can't complain at all, the food was even good. If you have a loved one with a severe injury, this is the best rehab center around!

Fantastic experience: We were surprised... We never expected such great service. My wife broke her leg just below the knee. She broke both bones and required surgery. We were expecting at least 4 weeks of intensive rehab but the staff, combined with my wife's stubbornness and determination, got her released in 9 days. The staff worked miracles. If you need intensive rehab then consider this place. Now for the staff, she had many people working with her and each and every one went above and beyond. For that my family and I are greatful. Her only complaint was they kept waking her up in the middle of the night to take her vitals. I told her to put a sock in it. But she knew the importance of these actions so she only complained to me. These people were fantastic and highly recommended but I pray we never have to use them again. For us once is enough. Fantastic experience: Excellent all around. Very clean, new, modern, high-end resort type of place. Dr. Patel is an excellent Dr. He was attentive to my needs and concerns and followed up with me daily. Was on top of my meds, and always treated me like a person of value not just a patient. Dr. Zuleta was also great and made sure I was on the right track to recovery. The staff is great, from the nurses, PT, OT, SPL, and COTA, shout out to Richard, MY PT, he is tough yet caring and efficient. Also Sam, Carlos, and Diane. They all got me walking again, and woke up my muscles after 40 days in the hospital on my back. Within a week I was walking again. There are more of them. The whole crew here at Palomar Health Rehabilitation Institute is incredibly awesome. It's very rare where I can say all the nurses here and CNAs were all my favorite but they all were fantastic with me. I highly recommend this place, they are new but let me say, they got their stuff together and excel over any other institute out there. Truly, and sincerely a great place to recover. Fantastic experience: Unbelievable care and rehab! Spent a month there after breaking my neck as Dr Shaw predicted, I walked out of there back to my split level home. What a blessing!

Negative experience: Even though I am happy with the medical staff at the facility, the so-called security detail (one person in particular) needs to be replaced. Had similar issues at the main hospital next door also. Biggest issue is that the medical team does not interact with these folks at all. They have their own entrances or just walk by. The regular 'Joe' at the main desk does not appear to like his job or just doesn't like people, doesn't pay attention, and likes walking around without a mask. Does Covid not like this guy either or is he just a Maskhole? Almost as bad as the folks patrolling the parking garage. I can't wait for my last day

Fantastic experience: I was dreading going to the Rehab Institute because I had recently visited a friend in a Skilled Nursing Facility. This place is absolutely not like them. The nurses and CNAs were quick to respond to call lights, the housekeeping was great, and even the food tasted good. Three hours of therapy a day prepared me for the long recovery period at home while my broken leg heals. I have no trouble showering or toileting or getting around because of what they taught me. Dr. Shah was attentive and solved the one problem that arose because of "protocol." When Dr. Shah wasn't available, Dr. B was there to answer my questions. I cannot say enough about the doctors, staff, or therapists who made my transition home very smooth.

Fantastic experience: I recently had a fall which required rehab. I had never heard of this facility.....but I am so very glad I came here! The staff are wonderful. Dr. Shah is kind with a twinkle in his eye. And my favorite PT - Gilbert and OT - Sam were wonderful. The nurses were attentive as well. The facility was comfortable with many areas (gym, outside, etc.) in which to receive therapy. While I sincerely hope no one needs to come here, I highly recommend this facility should you need rehab.

Fantastic experience: I have had the privilege to heal here twice. I’ve been to many “Rehab” places, PHRI is the most positive place to heal. They get you up and moving with 3 hours of therapy, 5 times a week. They want to heal so their food is good and plenty of it. Now about the staff, Dr. Shah is the Dr. who over sees the staff. He came in everyday to make sure my needs were being met. He’s a very caring Dr with a great sense of humor. The Nurses are outstanding, companionate and listen to your every word. The LVN’s are prompt and caring if you need them. If they need help, anyone on the staff will help until the task is completed. The Therapy Department is incredible, they have everything you can think of and more. I’ve had the pleasure of working with, Speech, Occupational and Physical therapy. With all their knowledge and help, I met my goals. This is the place I’ll be going to after my surgery, like I said, this is a very positive, compassionate place to heal.

Positive experience: My wife was there as a patient for 12 days and made great improvement over that time with the exceptional care from the Physical Therapists and Occupational Therapists. The nursing staff was also phenomenal. One of the pain management Dr's was less than helpful and didn't have much bedside manner. Now come the real issues. We were informed that she would continue in-home care following the stay and the referral would be submitted. we understand the time it takes to get things approved through our insurance company so a week before discharge, we asked repeatedly to make sure the request and referral was submitted and was told yes, multiple times. It has been five days and no home-care and her progress has come to a complete stop because of this. Furthermore, a referral or request was never submitted to UniHealth as promised. My insurance did not receive a referral, nor did UniHealth. Additionally, when we contacted them, they didn't have any record of her, and they are booked with no patient availability. Now we are left to start the process over through her primary care. The overall unkemptness of the facility was surprising. When we arrived, we were initially impressed with the facilities but came to realize that wasn't the case. We began to feel bad for the nurses and staff who we complained to but could nothing about it. The hallway floors were a mess daily, trash overflowing in bathrooms, AND her room was not cleaned or even swept for the entire duration of the stay. We asked multiple times and I even just asked for a broom to do it myself. Still nothing. My mother-in-law brought a Swiffer just to make it through. Even after sweeping ourselves and leaving a pile of dirt by the trash can, nobody came in to clean. The staff could not even change the toilet paper in her room or put paper towels in the dispenser. Instead, they placed the toilet paper on a makeshift table and gave her a large roll of paper towels. I've tried to reach out directly without any returned calls and could not find an email address to send this to. The care and people were great. The support staff and case managers are way below standards. I hope they find a new facilities company or hold them to a higher standard in the future. They also need to evaluate the case managers who failed to fulfill their responsibilities and repeatedly lied to us.

Fantastic experience: Palomar health rehab Institute is truly an amazing facility, the facility is immaculate and clean the private rooms are comfortable and clean and make you feel at home. The medical staff are truly amazing people they are so professional and courteous and respectful, they always greet you with a warm smile and words of encouragement their professionalism I have not seen in other facilities they genuinely make you feel like you’re cared about and that they’re going to do their best to get you back on your feet. All the staff members work as a team for your benefit and for your healing process I can’t say enough about all the personal moments I had with many members of the staff and after 10 days of being there I was able to get up and walk out of the facility on my own. When I left I felt like I was leaving family and that’s how the staff makes you feel amazing facility amazing people that are there to help you heal my thanks from the bottom of my heart for all of their help in genuine heartfelt caring.

Fantastic experience: Three weeks ago I had a fractured femur (unknown to me) which broke and I fell to the floor. I was admitted to Palomar Hospital for surgery to repair it. Then I was transfered to Palomar Health Rehabilitation Institute for rehabilition. The staff was professional, caring and compentent. Each day I participated in extensive OT and PT to rebuild my body and strength. The eleven days I spent there made it possible for me to move forward in my healing process and my life. I came in on a stretcher and walked out on my own. Now at home continuing to receive OT and PT from Palomar. I'm grateful for the exceptional care.

Fantastic experience: My mom went to PHRI after a stroke and I don’t have enough nice things to say about the facility and the whole process. From the very beginning Daniela was amazing- she is the social worker who arranged for her transport from the hospital to the facility and took care of all the paperwork, insurance, etc. Once my mom arrived there, so many people came to introduce themselves and welcome her to the facility. Even the cook came by to ask her what she likes to eat on her first day of admission and said she would keep the notes on what she likes. Then, the physical therapists were great. Gina in particular made sure she was up and walking every day and encouraged her to push herself. The facility was very clean, the nurses were attentive, and I believe she got the best possible care during a very scary time in her life. We hope no one else in the family needs this type of care, but if they do we will certainly be requesting to go to this center. The only thing I wish they would change is their visitation policy in regards to kids. We understand them not coming inside due to Covid restrictions, so they came to visit in the tables outside but were told they couldn’t be on the premises. Sometimes people seeing their grandkids is their best therapy so I hope down the line they can at least allow outdoor grandkid time!

Fantastic experience: My mother recently took a fall and first was admitted to Palomar hospital and treated for pneumonia and other issues. Everyone (it takes a village!) was amazing! After a week she was transferred to PHRI and stayed for a few weeks. I learned so much from the physical, occupational, and speech therapists. Different exercises to help my mom with to strengthen her arms and legs. Helpful suggestions to help my mom to get up from a wheelchair or bed. Many things to know that may seem obvious but little pointers were just what I need to learn! Thank you to all the nurses, therapists, doctors and coordinators. So much to learn!

Fantastic experience: My experience was much the same as Lawrence's recent visit at Palomar Rehab. The care was excellent; the food left a lot to be desired. I was there for 10 days and the difference it made for me was remarkable. I was able to return to my home and live independently because of the guidance from everyone at Palomar Rehab. Except for the food, the facility gets a 4.75/5 ☆s. Thank you to all who participated in my care.

Negative experience: Don’t be fooled by the new building. Rules changes by the day … They are not too accommodating to the patient and the families, no too friendly or welcoming when you walk in.. facilities like this one need to understand that the patient having family around is very important and it makes a difference in the recovery .. This place should be more friendly and accessible with family… don’t hold you breath on this facility they don’t really work with you look around and have more choices don’t make this place your first or second choice..

Fantastic experience: The facility is clean and very well maintained. The receptionist at the front desk greeted me immediately, I believe her name was Naomi. She was very friendly, courteous and professional and had so much information to questions I had about the facility. She was very well knowledgeable which made my family and I feel much more comfortable about leaving our loved one here. She remembered our names the next day we came back, she makes that place feel like home to everyone she truly does. The staff are amazing, the therapist are so quick to answer any questions we have, and they are so kind Mia and Justin are awesome at what they do, Rob is extremely helpful and very pleasant to talk to. The nursing staff are awesome too. The cna's are so quick when it comes to attention. The nurses really know what they're doing and I couldn't have asked for a better place. They went above and beyond and exceeded our expectations. I would highly recommend this place.

Fantastic experience: My Father spend 2 1/2 weeks at palomar Health Rehabilitation Institute recently and received phenomenal care. Fantastic experience: I had spinal reconstruction surgery. Upon my arrival at PRI I was able to walk with a walker but my legs were very weak and I could barely lift my feet. After 21 days at PRI I was able to walk with confidence with my walker and had more mobility. The staff was great- very supportive and friendly. The staff was well trained. The facility was very nice. Rooms were private and very large with private bathrooms designed to accommodate anyone’s needs. They had several outdoor spaces to enjoy to help me feel like I wasn’t stuck indoors. Many times the physical therapists and occupational therapists went above and beyond to help me with my recovery by helping me to do more . Dr Shah was essential in insuring I got the appropriate length of therapy as well as insuring my medical needs were addressed. If the food doesn’t quite tantalize your taste buds - acquire some of the Italian dressing packets in the cafeteria If your in the need of an acute rehab facility I would definitely recommend you consider this place.

Positive experience: **MY EXPERIENCE** I was a patient at Palomar Health Rehabilitation Institute (PHRI) from June 30th until July 9th, 2022. I was glad to be admitted for rehab following spinal fusion surgery at the Palomar Medical Center in Escondido. These notes are based on my memories of my stay at the facility **THE FACILITY** • New, modern, very clean. • Well-designed private room. • Personal TV with many channels. • Call buttons for nurse, medication, toilet, to control the TV, and to turn the overhead reading light on and off. • Individual thermostat control for room heating/cooling. • Large, well equipped private bathroom with shower. • Visitation rooms available for family visit (check for current visitor policy). • Visitors are not permitted to eat in the building, an outside area is available. • Masks are required outside of patient rooms. **MEDICAL SERVICES** • Skilled medical team. Doctors make daily rounds. • One RN and two assistants per 5 patients, very skilled and compassionate. • Response time to call button varies, depending on staff workload. **REHABILITATION SERVICES** • Excellent rehab therapists who are skilled and compassionate. • Large, well-equipped gym for physical therapy. • Demonstration kitchen, sample shower stalls, small auto used for occupational therapy. • Outside area for practice with walkers and other assistive devices. **THE PATIENT EXPERIENCE** • Three hours of therapy/day MON – FRI plus one Saturday or Sunday each week. • Physical therapy focused on gaining strength and control of muscles, returning to normal function. • Occupational therapy focused on learning to live independently with a disability. Therapy can address the “Activities of Daily Living.” List of ADLs vary but usually something like this, Bathing. Dressing. Transferring (ability to move in/out of bed/chairs). Toileting (controlling bladder and bowel function, getting on the toilet, and cleaning self). Feeding. • Patients who are judged to be a fall risk are kept in bed unless accompanied by staff. You may be allowed out of bed to sit in the reclining chair in your room or to roam the building in a wheelchair. An alarm will sound if you get out of the bed or the chair. • You may remove your facemask in your room, but it is always required when you leave your room. • Wi-Fi is available. Bring a charger for your devices. • Be prepared to entertain yourself for up to 20 hours a day. Bring books, puzzles, electronic devices, etc. • Because I was a fall risk and confined to my bed or my chair (I wasn’t interested in the wheelchair), the only time I saw another patient was in the therapy room. Everyone was busy then, so we didn’t have a chance to talk. • Depending on their workload, it may take the staff a long time to respond to a request to use the toilet. Plan ahead and don’t wait until the last minute to request assistance getting to the toilet. **FOOD SERVICES** • Food service is the bête noire of my experience. I did not enjoy most of the meals I was served. • There were good aspects of the meal. Food was served in my room, and it usually arrived promptly at mealtime. The meals were warm, if not hot when served, a real plus for enjoyment. • Portion sizes were reasonable. • After the meal, the food tray was usually removed in a timely manner. • I had problems with the taste and texture of many items on my tray. • Packets of Italian salad dressing (ISD) are available and made a huge difference in my ability to eat the food. They contain only 25 calories and no oil. They add flavor and moisture to food that ordinarily has none. Thank goodness for the ISD. **SUMMARY** I am very pleased with the care that I received at PHRI. Everyone I met was highly skilled, compassionate, and dedicated to helping me regain a normal life. I highly recommend PHRI. The biggest problem with my stay was the food. Should you choose to stay at PHRI, be sure to have a plan to help you cope with the flavorless, overcooked meals that they serve. The food probably won’t meet your expectations. I would rate them 4-3/4 stars if I could.

Fantastic experience: I can't say enough good things about Palomar Rehab. My sister was recently visiting from out of state and had a stroke. After 5 days in ICU she transferred to Palomar Rehab. She was treated like family by the staff and after a week of intense thereapy was able to travel home to continue her thereapy and receovery. Even after her discharge her therapist continued to check in and offer encouragement and support. I would highly recommend Palomar Rehab and will always be greateful for the care shown to my sister and our family. Special shout out to the outstanding therapy team, Mia, Justin and Brittany. They are amazing caring professionals and helped my sister to acheive her goals and get her home. Thank you again to all of you!

Fantastic experience: My dad was here for approximately 2 weeks after fracturing his hip. I can not say enough good things about this place. I was honestly blown away. The staff was amazing. Justin, a PT, stood out and Mia, an OT, went above and beyond. Even the staff at the front desk were exceptionally sweet. I was happy when my dad got to go home but a part of me misses getting to go and see everyone .

Fantastic experience: PHRI far exceeded my expectations regarding rehabilitation. Every staff member was personally invested in my improvement. They LISTENED! I was there for 10 days and could barely walk on day #1. By the seventh day I was climbing stairs to the second floor and without the pain I had prior to surgery. 3 hours per day of PT and OT improved my strength without wearing me out. Kudos to the entire team! I recommend PHRI highly!

Fantastic experience: My father had a brain tumor. Upon discharge from the hospital I was going to have him discharged home. It would’ve been the biggest mistake. I love the staff at Palomar Rehab. They were and still supportive to our needs. My father was discharged july16th 2022, I still get calls and messages checking on his progress. The facility is clean, the staff are nice & polite. Also, very attentive. Me and my siblings made the drive everyday to see him. About a 30-40 drive. It was well worth it. I’d like to thank everyone personally who had a hand in my father’s care. This is extremely difficult to see. My fathers progress is slowly getting better. Some days better than other’s. Be supportive of your family member going through any type of brain trauma. Appreciate the time you have with them and be kind. Most of all, be patient. Thanks everyone for not stripping my father of his dignity and treating him like a person. Thanks Palomar Rehab for everything you’ve done. You’re an awesome team I wish only continued success here on out! The Ellis Family! God bless

Fantastic experience: Palomar Health Rehab Institute Does wonderful work and getting me back to Independence from my brain surgery I had a left side weakness where I couldn’t move and then when I got transferred over to Palomar health rehab institute The occupational therapist and the physical therapist did miraculous work on me everyone here has been so wonderful in my treatment and being so positive I came back from left-sided weakness to independence I praise everyone here for all my hard work to get me back to where I can feel confident enough to where I can be independent And continue my healing through home health at this wonderful facility has Such a great staff I love that Dr. Shah the medical director as well the nurses CNA‘s here have given me such great attention this is a wonderful place if you need to get back your independence from any kind of surgery that you had at the hospital I highly recommend this being your first choice it has been a wonderful experience thank you to everyone here at the Palomar rehabilitation facility

Fantastic experience: I was fortunate to land here for rehabilitation after my hip surgery. Brand new facility! Entire staff has exceeded by far any expectations that I might have had! Fantastic nursing staff, physical therapy and occupational therapy staff, doctors on duty, right down to housekeeping!! Every single person is cheerful, friendly and encouraging.
