Fantastic experience: Physical therapist Lance wonderful! Shows genuine care able patients. Love this!

Negative experience: If I could give this facility less than a 1 if I could. My father was transferred here from a VHC for what was supposed to be a 2-3 week stay. A large cockroach greeted us when we arrived. The facility claimed not to have any information from VHC about my father's care needs (medications, dietary restrictions, etc). My father had been hospitalized for a contagious respiratory infection. The hospital told me that precautions still needed to be taken for a few days, but Alexandria rehab put him in a double room with no precautions to protect the other patient. When I asked about that, I was told "don't worry about it". Said double room also has a toilet that did not flush and was filled with excrement. When we asked if there was another restroom available, they brought a commode into the restroom and suggested he just use that. The room is so cramped that there isn't space for my father to get in and out of a walker from his bed. My father also arrived around dinner time. He is diabetic on a low carbohydrate diet. When we asked for a meal they brought him a massive bun with some sort of meat that was slathered in cheese and tater tots. When we said he shouldn't eat it, they returned with some sort of fried patty they claimed was chicken and mashed potatoes, not a vegetable in sight. We asked repeatedly if the facility has the medication my father needed to take this evening and were.told they were working on it. After 2 hours of waiting, we were told the supervisor was on break. Just after that I went to the lobby to let my husband in to bring my father a suitable food, and saw the supervisor just sitting there. Just a terrible experience overall. I did not feel comfortable leaving my father there in their "care" and would strongly recommend against anyone considering this location for anyone. A disgusting facility and a complete lack of care.

Fantastic experience: A good friend of mine has been in this rehab facility for about 10 days now and is being released today. I have been so pleased with the help and care that my friend has recieved here. Deborah the Social Worker is a wonderful person that cares deeply about the patients that she cares for... and has been present and helpful , and communicative EVERY step of the way. She evan had several phone conversations with me on her day off on a SUNDAY right after my friend was admited (who does that?). Also Justin the Rehab Director has been awesome to work with... and very available to discuss progress, concerns, and treatment plan moving forward. He even spent time discussing how to set up my friends house better to mitigate fall risks. And lastly Regina in the business office has been a tremendous help as well in coordinating extra days of care that were needed as well as refunding the family for pre-paid days not used. Keep up the great work caring for your patients guys... your team made a big difference in a tough situation for our friend and his family!

Positive experience: Since I came to this place everyone has been nice. I only had one problem when i couldn’t reach my call button but they fixed that where i could reach. This is a nice place and clean. I would come back but hopefully i stay well.

Fantastic experience: Decided to take a tour at Alexandria Rehabilitaiton and Healthcare Center before my mom was released from Alexandria Hospital. First impression when I walked in the door was great. As I walked the halls the facility was very clean, employees were very professional especially Ms. Giovanni. Made the decision to have my mom admitted for rehabilitation.

Fantastic experience: I want to express my heartfelt gratitude for the excellent care my sister is receiving. The nurses and all the staff have shown outstanding compassion, professionalism, and teamwork. They encourage her to participate in mobility and rehabilitation each day with kindness and patience, even when she is uncomfortable or in pain. She has cried out from leg and foot pain, particularly in her right leg, and I worry about neuropathy after her second stroke. Despite these challenges, the team remains supportive, explains each step, and provides reassurance, which means a great deal to us both. May God bless you all for the care you give.! I love you all!

Fantastic experience: Deborah was extremely helpful to my mother in helping secure a stay for my grandmother as well as writing an appeal for my grandmother to receive the physical therapy she needs to get back on her feet. Without Deborah, this process would have been incredibly stressful and difficult, and she made it possible.

Fantastic experience: This rehab/ nursing facility is the cleanest and most properly staffed out of 3 I have visited regularly in the area. To many are under staffed and therefore the residents/patients recieve sub standard care and that's being nice. Fantastic experience: Extremely good staff-everyone we met

Positive experience: I visited for the 1st time yesterday, Father's Day. I wasn't really impressed much. The room smelt of urine, the bathroom had dirty clothes hanging off the hamper, towel on the floor, all from the roommate. My ex husband has no mobility of his left side. He cannot do some of his daily hygiene by himself. I tried my best to get him cleaned up. Both his hands and feet needed the nails tended too, I only got the hands done. Is there anyone there that can help him do these things ? Also, the food portions were way too small ! The fruit bowl consisted of 5 pieces of fruit, 1 small piece of watermelon, cantaloupe, peach, pear and something else......Cole slaw wasn't even a quarter cup ! Men usually eat 2 hot dogs compared to the 1 he got. He needs to start PT asap on his left hand and foot. No mobility there at all ! He has a brace for his hand, which requires assistance from a nurse. He is unable to put it on with not having mobility of his left hand. I told him to start wearing it daily and call the nurse to help putting it on. Bedside manner of the nurses could be worked on also. Try to have some type of consideration that some of the individuals in there don't have many visitors or people to talk to. Sorry, but this review I cannot give 5 stars.

Fantastic experience: The staff was amazing!

Negative experience: Extremely Disappointed – Negligent Care The front entrance is currently under construction, and access is redirected to the back of the building. Upon entering, the facility has a strong and persistent odor of urine and feces, which was very concerning from the start. More importantly, the administrative and nursing staff showed a clear lack of attention and care. During my parent’s short stay, their health significantly declined, and despite obvious symptoms, staff failed to notify emergency services. I ultimately had to call 911 myself to get my parent the urgent medical attention they needed. This facility demonstrated serious neglect and poor hygiene, and I would not recommend it to anyone looking for safe, compassionate care for their loved ones.

What we initially anticipated as a challenging and uncertain chapter turned into a deeply comforting and healing journey thanks to the exceptional care provided at the facility. One of the most remarkable aspects of our experience was the diversity of the medical and administrative staff, which created a warm and welcoming environment. The staff’s cultural competency in healthcare delivery played a powerful role in my father's progress. From understanding our family's background to communicating with kindness and patience, every interaction made a difference in his recovery process. I would like to extend my heartfelt appreciation to the team of doctors, nurses, therapists, and administrative staff for their dedication and compassion. Negative experience: DO NOT BRING YOUR LOVED ONE TO THIS FACILITY. Our experience with Alexandria Health and Rehab was extremely disappointing, and we would strongly caution others against sending their loved ones here. The facility was unhygienic — there were bugs in the rooms, the air conditioning had not been “turned on for the season,” I found used diapers thrown in the corner of the room, and our loved one's room was never cleaned during the entire stay. The LTAC floor was severely understaffed, with just one nurse responsible for over 20 patients. As a result, nurses were only able to distribute medications and were not able to monitor or assess patients' conditions, meaning that signs of illness or decline could easily be missed. The floor supervisor, Amanda, was shockingly unprofessional and even yelled at family members who were simply trying to advocate for better care. The one bright spot was the physical therapy team, who were kind and hardworking, but they too were stretched thin. Therapy sessions lasted only 15-20 minutes despite being advertised as an hour a day for both PT and OT. This facility failed to provide basic cleanliness, dignity, and medical attentiveness. We are incredibly relieved we were able to move our loved one elsewhere. Families deserve far better than this.

Negative experience: Inadequate staff, not trained, many language barriers

Negative experience: Neglected Care at Alexandra Rehabilitation – Sepsis & E. coli Due to Filthy Conditions Review: I trusted Alexandra Rehabilitation with my mother’s care from and they completely failed her: • Unhygienic Conditions: Her sheets were rarely changed, and her wounds were left dirty and untreated. • Catheter Neglect: They didn’t clean or change her catheter properly—she was left lying in her own feces for hours. • Dirty Water & E. coli: Patients were given unfiltered, filthy water. My mother contracted E. coli from the water they served. • Dangerous Delays: When her fever spiked, staff said they’d “watch” her overnight—but by morning she was rushed to the hospital with full-blown sepsis directly caused by their poor wound and catheter care. • Inadequate Nursing & PT: Nurses often fell asleep at the station, and she received only about 10 minutes of physical therapy per day—far below what was ordered. • Family Had to Advocate: My relatives and I had to visit daily, demand they follow her care plan, and remind them to change dressings, bedding, and catheter supplies. Without us, her condition would’ve been catastrophic. • Poor Food & Atmosphere: Meals were barely edible, and it was obvious staff were overworked and underpaid—patients were being neglected. My mother is thankfully recovering elsewhere now, but please do not trust Alexandra Rehabilitation with your loved one’s health—look elsewhere for truly compassionate, competent care.

Fantastic experience: Great care and service

Negative experience: This facility doesn’t deserve 1 star! I’m not sure if the 5 star reviews are from real people because my mother was not getting 5 star treatment. I’m not sure where to begin. The food is horrible, canned food and frozen food. Consider yourself lucky if food was omitted from your tray which was consistent on a daily basis. The staff from the top down is beyond professional. They never respond to your questions even though they promise they will get back to you. The nurses and aids are something to be desired. Yelling to each other from the end of the hallway in their language continuously. Your wait time after you call for something is at the bare minimum is 30 minutes and if they don’t care for you they won’t answer you at all. Most of the time they disappear or sitting at the nurses station. Medication was never given at the same time and hands you a cup of your medication and gets upset if you question them. I’ve witnessed this several times. Since my mother was there I personally watched so many people decline and there is no urgency to do their work or answer patients calling out for help. I’ve seen staff walk past the room several times. Everything you ask for is an issue. Most of the staff are very aggressive and mean to the point it could be viewed as adult abuse. The nurse Amanda calls herself the nursing supervisor. She’s does not live up to her title and it reflects on the staff underneath her. I’ve reached out to the Administrator named Jen with no response and never in her office. OSHA/HIPPA would shut them down if they saw what I and other patients and families saw. The cross contamination is consistent and disgusting. They take it very nonchalantly and shrug their shoulders when they are questioned. Always remember you never know who is watching you and report this facility to several different agencies. While I was there several of us had conversations to find out we all witnessed many concerns. I am requesting for an itemized bill to go through line by line. If you have Medicare they do cover your bill except for the copay and they can’t force you to pay until they have been paid by your insurance company. If you are having to use a facility for your loved one please investigate yourself and not go by reviews on the internet. This is not the place to choose.

The facility is staffed with people that truly care about your loved ones and providing a safe environment with amazing nurses and clinical staff. We got PT, OT, and ST services in-house. Mom didn’t want to be here, but after a few days called the staff angels.

This facility is great, I know that they care as well for the residents and families. We are a fantastic team working together as one.

And had a very productive and positive conversation with HR Director, She was very attentive and answers all of our concerns. Thank you everyone for all your caring ways and very hard work. Negative experience: Not recommended for those with dementia and fall risks needing attentive, monitoring care. My mother went there for PT/OT after a hip surgery and ended up falling and fracturing her shoulder and bruising her forehead while trying to go to the bathroom on her own. She is hospitalized for a week with acute pain and trauma. When she was first admitted, communication about her care was disjointed. The standard message seemed to be a few weeks just to assess and that she should stay for a month! Most staff seemed friendly/OK but a few were quite rough and unkind in dealing with helpless patients who simply could not speak for and help themselves. Given my mother's weakened condition, she got minimal PT/OT while there for 5 days. My request for bed guard rails/restraints/alarm mats were resisted citing regulations and the need for the medical team to assess which didn't happen while she was there for 5 days. Food was pretty awful.

Fantastic experience: I give a 5 star rating only because of the 3 CNAs (first floor: Fatu S. and Frances. second floor: Jennifer) who have helped me past and present. I compliment them on their love of their profession. They are always courteous, organized, efficient, kind and good listeners. Though at times they are so focused on what they're doing (trying to make us comfortable) that they tune out distractions. They are so good at their careers, and it seems to me that others tend to abuse their willingness to be helpful. There are two part timers that also need recognition for the same traits - Allie and Michael. And most importantly they have command of the English language. Anyone who receives care from these stars are blessed.

Fantastic experience: The Alexandria Health and Rehab is a great facility, in a great neighborhood. Inside The Alexandria, you will find welcoming and supportive staff. The building is very clean and newly refreshed. I have worked with clients there in the past and this is the best I have ever seen the building overall, including the staff and residents care. I would recommend this to my other clients in the area who are looking for a skilled nursing facility. Thank you for all you do for the residents.

Fantastic experience: I own a home care agency and I called Alexandria Rehab to follow up on our client who recently had a hosptial stay after a fall and then ended up at rehab. I got connected to Devon the Social Worker and she was very responsive, friendly and helpful. My amazing Life Enrichment Director( Latosha) is full of new fresh ideas giving our residents that simple pleasure of life with meaningful events that spark joy laughter & build a sense of community. She’s there to lead and answer any questions I have in the department, Ayanna has been a great help showing me the ropes and introducing me to residents. I truly enjoy the atmosphere and I’m looking to work for Marquis years to come!

Negative experience: I have a room at Envoy.....the food is usually inedible, and the smell of human excrement is so strong you cannot leave the room door open. I'm told Medicare/Medicaid no longer pays my bill. No way I would use my Social Security to stay in this horrible place. I'll be leaving around May 15. Hooray!!!!

Fantastic experience: I've been observing this young lady for alittle over a month. She is always pleasant, when she comes on duty she peeps in the room to say hello to the patients. I've notice her soft spoken kind voice when she welcomes new patients if they has something to say or needs her assistance she listens and assist. An example of her kindness I was nervous about leaving my love one one evening be he seemed a little upset and was trying to get out of the bed. She said "you go head and leave I'll sit with him until he calms down " I called back and they said he was resting. There are several act of kindness I've seen in this young lady not only with my love one but with the roommates. So I want to say thank you Fatu Sankoh in unit LTC on the 3pm to 11pm shift. You have makes it a little easier leaving your love one while they recuperate. I give you 5 stars and more for showing that you cares. There are others who does a wonderful job of taking care of the patients and I appreciates you all

Fantastic experience: Alexandria Rehab is a wonderful facility that is tucked away in a great neighborhood in the City. Friendly staff, who are dedicated to taking care of their residents, and all while motivating residents to achieve their optimal level and their personal goals!

She has warm feeling and greets me every morning with a friendly hello, she attends to my small needs in the morning helping getting my coffee and my remote from under the bed. She listens to my concerns and stories and tries to find a solution, when I miss activities she brings me snacks and independent work. I enjoy seeing her every morning. (James Brown)

Fantastic experience: As my wife and I visited Alexandria Rehabilitation and Healthcare facility, we wanted to share how clean and orderly the facility is. In addition to this, we both noticed how committed and caring the Director Jennifer Patrick and her staff are with their patients. It’s very rewarding to see loved ones taken care of the way we all hope and expect.

Fantastic experience: My neighbors and I have quarterly meetings with Jennifer Patrick and Jane Hughes of Alexandria Rehabilitation and Health Care Center. My husband and I are so impressed with Jennifer Patrick, the Center Administrator. She is prepared for every meeting with notes on prior topics, current issues and asks for any new issues. If there is an issue, then she is quick to address it. She’s an absolute joy to team with!! Jane Hughes, Director of Community Relations, is also a wonderful teammate and has a thorough grasp of healthcare issues, facilities and related events in our community. Like Jennifer, see is quick to engage and assist across a broad spectrum of patient and family issues. We’re grateful to have Jennifer Patrick and Jane Hughes as our neighbors and managers of the Alexandria Rehabilitation and Healthcare Center.

Positive experience: In all honesty, the staff are not the best. Just from my personal experience, the night I brought my lady, the aides were giving me the impression that they did not want to do their job. It almost made me think that bringing my lady in was an inconvenience. There is this one aide. her name is Fatu, she did a great job! She is cheerful and is compassionate. I would recommend her if anything.

Fantastic experience: Nee is new to the staff members in activities. She knows how to listen and tries very hard to understand what I am trying to say. She's funny and easy going. And very helpful.

Positive experience: Great remodel job.. everything looks very nice...I did use a restroom on the second floor that could have used some maintenance..and the break room TV was really loud... But overall..great job on the new look etc.

Fantastic experience: my friend is a new resident at alexandria rehabilitation health center.the new recreational assistant came by doing her morning rounds with the daily chronicle and welcome her with open arms.nee robinson was so well mannered and attentive to moms needs ,also welcome her to the dailyactivities within the facillity she offered her independent work activities & an ear of support.she made her settle in with ease .i know this place to get my friend back on her feet

Fantastic experience: Regina in the business office is phenomenal, compassionate and kind. I could not have navigate through the Medicaid process without her. She is so much more than a Business Office Manager. Any organization would be proud to have her working for their company.

Negative experience: Not well run. 3 person curtain ward with inadequate attendance. Smelled like a portable John at times. Do not be mislead by the front desk and garden I felt sorry for the patients who remained when I left early for another facility R snider. Washington dc
