Fantastic experience: My first day there as a student CNA was fantastic! The staff they are all friendly and welcoming. Seems like a perfect environment for staff to work but more importantly a safe and caring place for their residents.
Positive experience: My 1st day there as a student. Overall a fairly organized and Clean facility. (No Pee smells) I One Observation was the center quad area which was nice and relaxing overall, Had semi permanent temp wet floor signs over several areas where water ran off the roof due to AC condenser run off. Causing potential slip hazards. All Preventable, due to leaf filled rain gutters that aren't able to direct water to designed down spouts.
Fantastic experience: Nice place. Everyone is friendly here. The only down-side is I had to wear a mask again which makes it hard for the residents to recognize their visitors... but I understand why.
Positive experience: I’m currently a patient here at Fairfield Pist Acute and have been here for a few eeeks so far. The medical staff for the most part are friendly and courteous . A few things that bother me about this Skilled Nursing Rehab facility. For starters each shift has th very understaffed especially the mid shift and more especially the graveyard shift. I’ve noticed there only one CNA per unit which I believe are more than 10 patients. In the unit I’m in I think there are 5 rooms wiyh 3 patients per room. I don’t think all z5 rooms in this unit are at the max of 3 patients but still with all the patients and one CNA it’s still not enough medical staff to attend to a patient. I think there should be 3 or more and at the very least 2 CNAs to cover the unit. My first night here did not impress me one bit because of lack staffing and secondly my roommate was watching TV so loud it kept me up most of the night and didn’t get to sleep until 4-5AM. And with only a minimal amount of sleep a nurse comes in to take vitals and give meds. I requested to return in a few hours but she was adamant about giving my meds so early in the morning. The exact thing happened for several nights with my roommate although I asked the staff to turn off or lower the volume on his TV. Same routine for the nurse too. MY Physical Therapist s showed up in my room for PT early, at least what early to me is snd explained to them I’m so exhausted and tired due to lack of sleep no thanks to my roommate. That gave some slack and came back at a late time.the PTs are friendly courteous and motivating and inspiring to get well. I understand what they’re telling me but I told them if I don’t have the energy to do PT to please let me rest more. With that being said one of the OTs came back to my room and showed me some bed exercises I can do which gave me motivation to do it because the extended TLC they showed me. After experiencing what I have the first few nights I requested to speak to the manager for staffing as that was my biggest concern. I was told a message will be sent to her and till this day she or he has not stopped by to introduce themselves or ask me about my concerns. REALLY UNPROFESSIONAL!! I hope it happens sft Redon Dr time is worse I’ve timed it ooner or later because I’m ready to seek a better Rehab Fscility that has the staffing for each shift and eagerness of staff to attend to patient in need. The average response time for a CNA especially during the late night graveyard shift is 10-25 minutes and the mid shift response time is worse. I’ve timed it a few times and waiting 15-25minutred another time made me very angry. Explanation of course was because they were with another patient. Or sometimes with another patient then attended to another who has been waiting too. I know changes won’t happen overnight but I hope they hire more in the very near future!! Another unpleasant experience is my TV volume is low and at night after 10PM OATIENTS ARE ENCOURSGED TO USE THE PROVIDDD HEADPHONE. I’ve tried the provided headphones and only one side has volume so I tried my personal head phone and the same results. I requested and asked the front desk to contact maintenance to take a look and troubleshoot. That particular day I had dialysis so I wasn’t sure if maintenance came to my room to check and troubleshoot. At night around 10PM I tried the headphones provided and my personal one and still didn’t work. Today i requested and asked again for maintenance to check and was told they’ll contacted and come to the room while im in it to troubleshoot but no show!! It’s ludicrous the things that should be in place already isn’t and no actions are being taken. That responsibility falls on the Manager or Director of Staffing. As far as Maintensnce goes it’s inexcusable to not show up after you suggested the patient to bee present while troubleshooting. Hopefully Fairfield Post Acute gets their things together!!!
Fantastic experience: We enjoyed our experience friendly management. Sarah and Julie were both very nice. I went to meet Sarah because she said she has a new opening for my husband Gerald and we desperately need to get him into a acute facility. She said she would as soon as possible. She has managed her facility for 21 years and I feel that this place will be a great place for my husband. It's clean but we did not get a full tour yet A man told me his sister has been there 3; years and he loves Post Acute Fairfield. This made me feel very good Diane Moreno-Bouvia Today is December 8 2023 The last review was made mid August before my husband came to live at Post Acute Fairfield. I have been asked to do a new review. The facility is very clean He gets his medicine regularly. When he had the need for two urgent trips to Kaiser it was taken care of right away. Jerry was welcomed back so sweetly, he told me it made him feel good and happy. He likes his P.T. session's, and it's VERY necessary for Jerry with Parkinsons the need for moving is crucial the CNAS are supposed to walk him regularly 3 times a week around the facility when P.T. ends. RNA is what they call this transformation. The staff are very friendly and most of the time answer any questions I have concerning my husband. We had a care meeting and staff informed me that they are trying to hire more CNAS because there isn't enough. So I hope they do because this is what I see needs improvement. Other than that things are going pretty well. Diane Moreno-Bouvia 6/2023 Things seem to be going fairly well Jerry usually gets 3 showers a week. On average he enjoys that because he is a clean person. He has a few CNAs that are new. There are a few he is very comfortable with there are some who put there heart into their job. Not all of them do that. I often help them by getting Jerry ready for bed before I leave. They work so hard and I figure getting a jump start helps them a bit. The areas are clear and clean. The nursing staff are pretty attentive and answer my questions when I have them The activities provided are excellent and I bring all sorts of lunches for Jerry and I from home and fastfood. They don't mind when I do. The kitchen provides good food. But I like to treat Jerry He lives there permanently and is still trying to adapt. It's hard for him. His handicap Parkinson's is very hard on him and that's why he needs EXTRA help and patience from the CNA team. He can't lift his legs up off the floor with out assistance. So I think it would be very good that all the CNA know the limitations that patients have. In fact most of the workers I need to explain what he can and can't do. Overall I think things are going pretty good August 6 2024 Post Acute Fairfield is always clean. I have always liked the nursing staff they show care for the patients and they get their meds and blood pressure done regularly A care meeting is scheduled every 4 months for residents. I have my list of things my husband and I will bring up at that time. Other than that the CNA staff are all pretty good. There aren't enough in the afternoon and early evening I think sometimes it's because the state only allows so many? That's what I was told one day when I asked higher staff. I help the CNA s when I am there too. They have a really hard job and because they have so many patients it's hard. I always thank them for taking care of my spouse because they need appreciation I wish Jerry had more roommates he could talk with. That would be a plus. He has never had one he could communicate with well due to their disabilities. I feel bad for him in this sense as I was told he would be paired up with someone he could talk to and it just hasn't happened Well otherwise things seem to be going okay. I still think the staff should be educated on the physical limitations each patient has so CNAs don't push them harder than need be.
Negative experience: Pathetic they can't even answer a phone the staff on the early evening shift are questionable

Fantastic experience: They were fantastic with my step father. He had PT, OT, speech therapy everyday! He could barely walk when he went in! He came out able to use a walker and get in and out of bed. They truly helped him physically but also mentally. They were loving and patient. Sure they don’t have a CNA for every person but who does? If he needed something, he got it. If we complained about something, they fixed it! Sarah and my step father’s case manager were amazing! Really listened and were empathetic! I had fears when he went in he wouldn’t come out! But he did and he is sooooo much more mobile and happy! Thank you guys so much for listening to us about our loved one!
Negative experience: Seeing my sister move around and being social was a good sign. But when she got home little did I realize she was not given her proper meds in the facility and she went right back into the hospital the same day she was released. Dr advised this is why she had relapsed.

Negative experience: It's sad how my Mom, on Hospice, has had her medical equipment "Misplaced." They "Sutter Hospice" ordered her a personal wheelchair and walker that was delivered, signed and disappeared. Her legs are extremely swollen from hanging instead of being prompted up properly....She wants to sit up, and they just let her....even when you can clearly see her legs are extremely swollen without supported elevation. So for three days......no one has been able to find the delivered medical equipment.....As nice of a place it appears to be, one relative of another patient told me they've lost 2 wheelchairs of her Mother's......don't they have serial numbers on them? It's just sad.

Fantastic experience: I only come on Saturdays to play religious music and read the Bible to the residents and pray for them. This is my ministry for over 34 years. The staff and the residence are courteous and hospitable and I’m well pleased visiting the Acute nursing home.
Positive experience: The rooms are small and cramped, but the staff is very good: caring, knowledgeable and helpful. PT is very good, OT and speech therapy are great too. Nursing staff has helped me so many times…..if you need to recover, this place will help you do it.

Fantastic experience: The staff at Fairfield Post-Acute are so friendly with the patients and the families. My mom has been in this facility since 2015 and she has not had a single bedsore, due to the great care that the nurses and especially the CNAs provide. I am so blessed to have my mom at Fairfield PostAcute because the staff treats my mom as if she is their own family member. They are always friendly, helpful, and amazing . Thanks you guys for all that you do for my mom and all the other patients

Fantastic experience: My mother was here after surgery early last year and she was impressed. Today I visited my young friend that says he hasn't had many complaints here. The meeting area was clean and the staff was friendly.

Fantastic experience: I went to visit my Aunt who is receiving rehab services. The staff were helpful and polite. The facility was clean and the nurses were extremely attentive to the patients, treating them with dignity and respect. My Aunt is improving and can walk much better than when she arrived. It has only been a little over a week so I was surprised at her improvement. She typically complains and is uncooperative. The staff were patient with her, explaining why she needed the rehab and how it would help her quality of life. She actually wants to use her gym membership to continue her rehabilitation when released. I would like to thank the staff for helping my Aunt.

Negative experience: My sister put my mom in here, after I told my other sister to not let her do that!! I knew it was a bad place! She did Anyway! My mom fell several times!! They gave her the COVID booster shot she didn't want!! They lost her teeth and clothes!! They wouldn't allow her to go to the hospital when her neck was EXTREMELY PAINFUL FROM FALLING SO I HAD TO GET THE OMBUDSMAN INVOLVED!!! Then someone caught COVID! They STILL haven't reimbursed her for her teeth!!

Fantastic experience: We visited today, my wife and I. We didn't go on a tour we didn't have a appointment set up for that. We dropped off some papers for Sarah. She was very nice person and said she may have a room for me. I'm excited and happy about that. The staff was very nice My time with Sarah was great. I'm Diane Jerry wife. I'm looking forward to a visit to see the rooms and eating area and outdoor area for the residents. Sarah was very supportive and told us a lot about her staff and the type of care Jerry would receive routine wise which was very helpful for me to know he will be well taken care of. I am so happy we got to go and meet with Sarah. I pray Jerry gets his room soon Praise God
Fantastic experience: My sister and I went to visit my auntie and were very happy with the care and attention my auntie was given. We went for our visit and to feed my auntie for 12pm lunch on Sunday 5/21 and met my aunties nurse’s name was Christina. She was very pleasant and thanks to Christina her nurse, my auntie was clean and groomed as well! The staff there at this home was very polite courteous and helpful and the home appeared clean. We felt our auntie was in good care while observing and visiting there . Thank you very much!

Fantastic experience: I love the all cna’s because they are embracing my students. They are open arms teaching the students, sharing their knowledge. I love HR josephine very nice, polite, helpful. Kitchen staff are awesome helping the cna pushing the food cart to their stations.
Positive experience: Very easy checking in & out. Place was clean everyone mask up. Only thing air conditioner set little to cool. Had to put on extra blankets on my love one. All was good

Positive experience: Your staff is very hard working and do a great job. Although the staff is amazing, you are so extremely understaffed and your employees under paid! I believe Burger King pays more than you pay your CNAs( shame on you).
Positive experience: It’s a tuff job and someone has to do it, My mom has been here 10 years and I love a lot the staff like family and it’s not fair how they are treated. and with that said the long term staff is OVERWHELMED, OVERWORKED, and UNDERPAID. Better training and a better trained-training coordinators, would be a good start to retaining some of the short term employees.

Fantastic experience: My grandmother whom is staying there seems really well and takin care of. She would absolutely say something if she was not comfortable there. She seemed to have great relationships with the people working there as well. My son and I were greeted and smiled at by the nurses/staff and overall had a great experience during our hour long visit.

Fantastic experience: Fairfield Post Acute Rehab is a 5 star facility in every way. From the workers both medical and non-medical to the environment which is clean and nicely decorated, the facility is a place that anyone would be glad to have their loved one in. There is a reason that they have won many regional awards for care; one visit will show you why. all the way!
Fantastic experience: One of the worst feelings in the world is having your loved one 2,500 miles away from you, unable to speak and tell you how she is doing. As a daughter who loves her mother more than anything in this world, my heart was broken when I was told years ago that she would not be able to live here. I prayed harder than I had ever prayed for anything in my life that she would be able to live in a place that was bright, clean, and comfortable. Fairfield Post Acute Rehab has been that place for me and my family, and for my mother, for several years. I am beyond grateful for their caring and competent staff. When covid hit I knew that I wouldn't be able to see mom in person for a while (I usually made the trip twice per year), but I was able to call at any time and talk to someone who would let me know how mom was doing. As soon as possible the staff updates families about the status of the facility after someone tests positive. Additionally, whether facilitating video calls or answering numerous questions about her care, I have never had an issue with the staff at FPAR. Whether in person or via Skype or telephone, meetings about my mom are conducted professionally yet with compassion. I have dropped in unannounced as early as 6am before an early flight, and as late as 10pm - I am ALWAYS able to see mom. Numerous friends are family members drop in when they are in the area, and they are always treated kindly. No rehab center is perfect, but my family is very happy with the care that my mom is receiving. While it is true that I would prefer that she be located closer to me, FPAR is the next best thing to having her next door. Day or night, even with the 3-hour time difference, if I want to know how things are progressing all I need to do is call. Someone always answers, and if they don't have an answer they call back quickly once they know what I need. I don't use the phrase "God sent" lightly, but that is exactly what FPAR has been for me. I would give them 10 stars if I could!
Fantastic experience: 8 years ago i had the opportunity to work with Fairfield Post Acute when I was a community Liaison with Sutter and PCL at Sutter Solano. I was amazed at the level of professionalism at every level and was received well when Palliative discussions as POLST, and care coordination with keeping the continuity of care. Every staff member part took in furthering knowledge to better care for their patients. Every patient felt as if they were the only patient in the whole facility. The staff held high standards for all entities providing care or partnering with providing care for patients; on transitioning home every patient was aware of their disease trajectory. On a personal note, I had Fairfield Post Acute in mind in the event an immediate member of my family needed care. I have returned back to the area my first stop is Fairfield Post Acute. Looking forward to partnering in molecular diagnostics. I would have rated with a higher number of stars but I am limited to 5 stars.
Fantastic experience: Fairfield Post Acute is a very nice facility and their nursing staff took such good care of my Mom. I will continue to recommend them to anyone needing to rehab or stay there long term.
Fantastic experience: I had a good experience while I was rehabbing from my gall Blatter surgery which I don’t recommend to anyone! Just say NO. Staff was great esp. the CNA’s who did all the heavy lifting on a daily basis. PT & OT were very good you can never have enough.

Negative experience: My 69 yr old mom had been there for not even 72 hours and the lack of care and horrendous treatment included a staff member allowing my mom to fall even though she was clearly wearing a bright yellow wrist band that said "Fall Risk" since she just had 2 strokes in the last week and has no strength on her right side. Staff member didn't even say sorry until I called to complain. Now they just called to tell me my mom went to Vacavalley Hospital ER by ambulance although they say it wasn't really a fall. This place does not care or have any compassion for your loved one and my mom was afreaid for her life!!! A shameful discrace!!!
Negative experience: This pace is filthy, inadequate and an unacceptable facility to place your loved one. I discharged myself after a few days, much too early for my major spinal cord surgery, I had to escape this totally unacceptable disgusting facility.

Fantastic experience: Great facility,superbly run by a wonderful staff. A staff of caring. Individuals that are there to help and aid those in need. I have been there for rehabilitation after two surgeries and have had some of the best care ever. Gym staff, entire operation is superb.
Fantastic experience: If you are looking for a safe Clean Well Care for your Loved Ones then Fairfield Post Acute is the place in Fairfield, CA. I had swear to my parent's that I will take care of them and I will not out them a nursing home. When I could not care for my mother at home Fairfield Post Acute become my extended family help me through this difficult time. I had just lost my Dad I was very difficult deal with. Joanne Van Dyke and her staff took GREAT care of my Mom despite my craziness. My mother was at this facility for 5 years. I have had several family member who have come through Fairfield Post Acute I Highly Recommend the place.!
Fantastic experience: Clean people well cared for. Always there is a helping hand. Patients leave there healthy and many who are of need of care again will come back where they were treated with loving care.
