When you arrive at Woodlands, you will be greeted by a medical team who will create a personalized care program to help you regain abilities and reduce the risk of being readmitted to the hospital. If you or someone you know needs specialized nursing care due to an illness or injury, Woodlands is the place to turn. Our experienced nursing staff works with your doctors to develop a comprehensive plan to help you reach your highest level of functioning. Our registered nurses (RNs), licensed practical nurses (LPNs), and certified nursing assistants (CNAs) are available 24 hours a day, 7 days a week for both short-term and long-term stays. Woodlands offers a range of services, such as wound care management, cardiopulmonary therapy, fall prevention, incontinence management, IV therapy, full-time physician coverage, geriatric psychiatry services, and podiatry services.

Fantastic experience: After major back surgery at Duke I was transported to Woodlands Rehab by ambulance. I have graduated from being flat on my back to a wheelchair, then walker & now a cane in just over 2 wks! I have been very pleased with the facility, the staff, the physical therapy, the entire recovery environment! I am blessed to have chosen such a great Rehab place! Will be going home in a couple of days!
Negative experience: Spare your loved ones from this facility. This is, by far, the WORST facility ever! Just today, they have left my 86 year old, Veteran, Grandfather at the emergency room and refuse to pick him up. They have hung up the phone on the transportation administration from the hospital and refuse to answer phone calls from the family. The lack of professionalism from this administration is absurd and they should all be fired and replaced.

Fantastic experience: The staff is good with their clients, Monday through Friday . The process in excepting my husband was great, I was given a tour twice and each time I came in everyone was very understanding! I highly recommend woodlands rehab!

Fantastic experience: My mother was placed in hospice care at Woodlands due to unexpected circumstances. Initially, we thought my mother would be going for rehab and suddenly things changed and we needed hospice care. I must say from the initial contact with them at the hospital to the day of her arrival (within 24 hours), to her last breath, the staff was nothing more than professional, accommodating, kind, and full of empathy towards our circumstance. Upon walking into the facility , the first thing I noticed was how wonderful it smelled - it smelled amazing - so fresh and so very clean! My mother was a placed in a private room that was large enough to accommodate visiting family and friends and the room was immaculate! We were provided an additional private Sunroom space where overflow family and friends could congregate and meander when the room was full. The amazing staff were so pleasant and gentle with my mother while providing comfort care and it gave me a sense of relief knowing that I could focus on being with her over the short remaining time we had. I will say again, everyone on the staff was amazing from the intake coordinator, to the billing /business ops staff, nurses and nursing aides. Just absolutely wonderful! I wouldn’t hesitate placing my loved one at Woodlands for hospice care or rehab services. You will not regret it!

Negative experience: Terrible management. They gave away her bed while she was at the hospital and lied saying I told them I didn't want her coming back there. Of course the administrator took their side even though I told her they lied. I will definitely be filing a complaint with the state.

Fantastic experience: If I could give 6 stars, I would. I have had my mother in 2 other facilities over as many months. Since arriving at Woodlands, my mother is finally receiving the care and rehab she needs and deserves. Her entire attitude has changed and she sees herself having a future outside of a facility because of the care she is being provided. The staff is caring and compassionate. They provide me with honest feedback, expectations and have set valid goals. I wish I would have found Woodlands sooner. If your loved one is in need of rehabilitation and PT/OT, I highly recommend here!

Negative experience: I don't know if I provided a review in 2016 but here it is others need to know ,I can't give u a 0 but u deserve it. U don't deserve a Star. Your staff , if they did their job back then have my father the medicine he was supposed to have he would probably be here today. We picked him up and took him to the hospital one day because he got sick woodland would not do anything and said nothing was wrong with him. , We called an ambulance and got him to the hospital the meds he was supposed to have were never in his system. Woodland Nursing home never gave it to him and they were provided all the meds. He developed congestion heart failure, and fluid buildup around his lungs because he didn't get the medicine. On New Year's Eve 2015 , we got news from the physical that my father only had 30 days give or take to live just because someone failed to give medication. ( My sister died in 89 her birthday was February 5 - we made it past it , my father passed on February 8, 2016 at home , we wouldn't allow for him to return to a nightmare on a place . Beware Think Twice, before putting your love ones , if you do. ⭐I suggest for you to do Surprise visit. Don't let them tell you Different !!!!!!!! TAKE PICTURES !!!!! Other Patients died when we were visiting our father. The workers did nothing , had no clue what to do at first. Surprise them. Check on your love ones if there. If not there , don't put there.
Negative experience: If we could provide 0 stars, I would. Other than under treating my aunt for her pain, the staff at this facility chooses to break legal protocols. I understand HIPAA and not providing information to appropriate parties. However, the Social Worker at this facility refused to provide information to me, even after reviewing legal documentation confirming I was the Healthcare Surrogate for my aunt. Her actions prohibited my aunt from receiving appropriate care and caused a much more traumatic experience for my aunt and our family.

Fantastic experience: It is a great facility for elderly
Negative experience: This place needs to be torn down. My sister was there and her gowns were stolen. The rooms are way too small for two people considering everyone so concerned with social distancing. They would not take her when I wanted to put her un respite because she didn't have that shot. They need to build a new building or upgrade, which could have been done whenever they added the rehab part. If you can put your relative somewhere else, do it!

Negative experience: If we could give a negative star we would!! We have been trying to work with the social worker to move my father in law since the beginning of December. He also had an incident where one of the nursing aides took his phone because he was going to call the front desk supervisor for help( he is barely able to be functional with his hands) and the nurse aid removed his phone and took it then somehow she is still employed and nothing came if the situation all while arguing with him. This place shouldn’t be open and the state of North Carolina will be notified!!

Negative experience: If anyone gets to read this about this nursing home and rehab center, please don’t go! I fell and broke my hip and I was at Cape Fear Hospital going through my rehab there and then I was taking by surprise that they were moving me to this nurse OH my gosh home and even though I told them no they took me any way, without a coat on or my shoes on. I didn’t get no meds until the next day when my husband came and took me out of there. I wouldn’t leave my dog there for them to take care of. I only got 1cup of water and the room wasn’t what I needed as for as my hip was concerned an I hope this gets out to anyone considering this Fasciluty

Fantastic experience: To express the love I have for the residents and staff. Is to say we're a family. From the long nights talking to the resident until they fall asleep. To spoiling them like my own grandparents. When I started it was just a job. Now this facility is home.

Negative experience: This place lies and steals and covers up their wrong doings for each other. Don't bother to bring your loved ones any snacks or letters because the staff will just put the things you bring, in your loved ones closet, and not tell them about it! Horrible communication among all levels of management.

Negative experience: Hello, I am leaving 0 stars! This facility is unprofessional, they will not answer the phones. & I quote one of the employees told me “I’m going to be honest with you things go missing here all the time.” Very unprofessional

Negative experience: At first this place was great, aside from the oder. The nurses and na were attentive and friendly. I could have gone on and on about how awesome they were with not only my father, but others as well. But then, THEY CHANGED MANAGEMENT. Once this happen, everything changed. Staff started leaving, seemingly inexperienced ones came. They were not engaging with families, very cold and robotic. They always seemed like they hated being there, I'd even heard several of them complaining. The residents went from social, happy and free to almost frightened. I visited my father at least twice every week. I was kept out of his room and not permitted to see him most of the time I was there. I was told they had to bathe him, or change him, or personal care. I always came at the same time every week so before, they would have him up and dressed in his chair. So I changed the times I would come. Still happened. One day he was able to get me alone, he whispered to me in a manner that was so helpless and desperate. He said, "You are the only one I trust." I didn't understand until it was too late. When he was at CFV hospital I was able to see what they had done to him. He came with all kinds of breakfast food in his mouth, despite the fact that it was evening and he was to be fed and have his mouth checked after eating. No one at the hospital could figure out why he wouldn't talk. I asked them to sit him up so he could open his mouth. Yep, he was pressing on grits and eggs to keep it from choking him. Then, the severity of the bedsores. The bedsores were actually so bad he had sepsis. Wound Care at CFV was so disgusted. He also developed pneumonia. While he was in the hospital, we met staff members that remembered him from Woodlands and we're shocked that he was in such horrible state. They also express that thier integrity and their love for people was the reason they left Woodlands. The hospital staff did everything they could to help us get him removed before he was discharged. We called SS but apparently you can't investigate a whole nursing home only individuals. My father was discharged to another facility. Night and Day! More of staff that fled Woodlands was there and they gave similar reactions to his state and reasons they left. Unfortunately, my father never recovered from what Woodlands did to him and transitioned in my arms in the hospital. I have pictures that too hard to look at. In my family's eyes, Woodlands killed my father.

Negative experience: If there was a way to give a negative rating, this facility would definitely receive it. My mother was in the facility and had it not been for the family constantly calling and checking in on her she would have never received adequate care. There are too many many complaints that I do not know where to begin. Unfortunately my mother passed 13 days ago and NOT once did the administrator call or send a card expressing condolences. Shame on the administrator and staff.

Fantastic experience: My father is cared for at this facility. I wanted to write a BIG thank you for all the wonderful caregivers. I am having pizza delivered Wednesday around 1 pm for the wonderful staff who care for my dad. He will be 90 years old this month. Since I can't visit him, I thought I should thank all the people that take such good care of him. Again THANK YOU ALL!!!! Susan Berry - grateful daughter.

Negative experience: Where do I begin? Let me start here — if you give a nickel about your loved one, get them the heck out of here. The rehab facility looks great, the nursing home department - horrendous. Mold/mildew in the closets, I was in the room for 30 minutes and got a headache from the mold smell. Floors sticky and no evidence of prior mopping. When I arrived in the room, a pile of food was just sitting in the middle of the floor from a dropped tray. I waited 30 minutes to see if someone was on their way to clean it up — nope. The urine smell in the hallway was stifling. How is your loved one supposed to eat inhaling piss fumes all day? The smell of the place could gag a maggot. I showed an employee the mold in the room, pile of food in the middle of the floor, etc and she helped me move my loved one to another room until we discharge him out of there. There is one wing in the nursing department that is halfway acceptable, so you better pray that hall isn’t full. I think it’s comprised of about 10 rooms. Another employee told me that someone had just died in the original room not too long ago as well. Oy vey... It breaks my heart that people’s mothers and fathers are spending their last good days in those conditions. Check out the photos of the room in my profile.

Negative experience: This place is a disgrace. They should be ashamed of themselves for the improper care they provide. Being short staffed is no excuse to leave anyone in fecal matter, not provide daily care routines and also malnourish anyone! I wouldn't bring my family to this facility ever again. I hope this review saves a family from ever having to deal with what my family has. Please if you love your family enough do not bring them here. There are plenty of facilities that will provide the care that is needed for your loved ones. And if you pay attention to detail there is not a good background on this facility. Hopefully the state steps in and sees how awful this facility is and shuts it down immediately and helps another family from the abuse and neglect that my family has had to endure. I ensure you that i will spread the word of this disgraceful facility. I've worked in a nursing home for 10 years and I've never experienced such heartbreak, neglect, abuse in one place. If you can't treat people you care for as your own family then get out of the healthcare field. It's not meant for everyone and I assure you that neglect and abuse for a bit happen like this in every facility. I beg of everyone who is planning on their loved one to come here that they take a better look at reviews, walk through the facility and pay attention to details, research anything you can before the ultimate decision on leaving your loved one here.
Negative experience: It was nasty and smelled bad when I took my mother there.
Negative experience: My dad has been here 3 years after a stroke. Use to be privately owned now is corporate owned by Liberty Health Care. Found several of the employees from other locations worked here as well as other care facilities. They are short staffed on weekends and holidays. Employees call out and no coverage If they can't get someone to come in on their day off. Management will say they are not short on staff when it is obvious they are. When you try to call no answer many times. If you leave a message they rarely call you back. If a problem on care and you complain they tell you they will get more help do more training whatever it takes for you to hear what you want. The phone system has messages from old administratior gone over 2 years , mail boxes have people no longer there name on message if you leave a message. They are suppose to call if any accident but that is rare or 2 days later. He isn't bathed regularly n his daily dental care is not done. They leave him sitting in wheelchair too long n do not change him often enough. I find it is the people working not doing the job as I have seen many good people leave n bad ones kept. What I have found is all the local places are pretty much the same. They get new folks out of school they get experience n leave. Depending on staff his meds are late I've only been called by on call Dr. 1 time n I tell them I want to be included on qtrly care discussion but they don't call me. Since new company took over they let the management go n brought in others. I have ask for names n phone only get the DON. He is not able to speak up or complain or ask for help n is dependent on 24 hr care from others so he is left in bed or his chair. Very sad to have him go through this n I have to grip every time I'm there. I feel bad for the ones that don't have family fighting for them. If there was a better choice I would move him. they are often short on supplies like linens adult briefs cream so I take in his own items. Don't get me wrong. There are some great folks there very caring take their time but too much time on 1 n they get behind with others n they have to go home sometime, have to take the bad with the few good. some of the workers are dear to our family for the consistent care they have given dad over the years. No place is like home

Negative experience: I had several problems with the "head nurse" at the facility. She was extremely arrogant, disrespectful and rude, Not only was she very condescending to my Mother and our family, she was also like this towards her own staff. I was trying to figure out the best hospital to send my Mother to when she fell ill again and it was decided between the Doctor on staff at this facility and myself that she would be transported to Duke where her illness began. Nurse Englund somehow thought it was her place to override this decision. She also thought it was her place to inform me that transporting my Mother to Duke at our cost was too expensive. Our financial situation was none of her business. I will NEVER recommended this facility to anyone and will make it a point to make a complaint to as many people as possible to help ensure that no other family has to deal with this woman.
