Form and Function Physical Therapy

Fantastic experience: Money well spent. I went from having problems walking involving surgery for my knee to being able to run. I’m thankful to be where I’m at right now.

Fantastic experience: Trevor is the best physical therapist that has ever worked on me. I’ve been going to chiropractors for almost 20 years almost every week. Going to Trevor for therapy provides improvements that chiropractors cannot do. My neck, shoulders, achilles and coccyx area has improved within 30 days. I don’t feel a need for chiropractic treatments anymore.

Fantastic experience: Shout out to Trevor and his team Form & Function (FF). What I really appreciate is that I reached out to them and I forgot to follow up for scheduling, however, Cassidy followed up with me the following day (Thur 20 Jun) and scheduled the Discovery (Eval) appointment for that Friday. FF admin is very easy and they provide you an online account with a welcome onboard folder. At time of Discovery, I was 5 weeks postop from my Achilles surgery… and I was very impressed how Trevor listened to my concerns and with the treatment provided within that short period of Eval time. I went home with my Achilles feeling really good. The following day I went to a soccer game and there was this Porta John that was left on the trailer-hitch which was maybe 2 to 3 feet off the ground. I really needed to use the restroom so bad that when I went to the front, my instincts kicked in and I mistakenly lifted off my surgical foot (supported with VACOped Boot of course) and I thought I will feel pain immediately but I did not, nor did I felt pain throughout the day…. and that’s what sold me to Trevor and FF. Today I am 8 weeks postop with having received three 1 hour sessions, and let me tell you my Range of Motion (ROM) has improved dramatically. After each session I go home feeling really good and most importantly in the morning getting out of bed my ROM improves, which gives me a whole lot of confidence of the treatment for a speedy QUALITY…. recovery. Oh by the way, when I arrived I couldn’t stand straight, as of this morning though I’m at 90 degrees standing flat footed… and WALKING out of my boot steadily with minor pain FF has reasonable treatment packages, and their flexibility regarding payments is an easy process. Lastly, at my Eval I conveyed to Trevor not to take it easy on me throughout my treatment plan. Be honest/patient with yourself and transparent regarding your current condition with Trevor because you’re in great hands… literally magical hands! Form & Function motto is…. STAY IN THE GAME! I would definitely recommend Trever FF to family and friends because I’m eager to get back to my normal life curricular activities and most importantly attend family functions. Fantastic experience: One of the best sport therapist I have been to in a very long time. Just a little back ground, I have played soccer for 21 years and still going. Even though it is located in a baseball training facility, do not let that stop you from seeking sports therapy for any sports related injuries. My experience from there has been nothing, but positive, insightful and informational. Not only is the staff, adaptive, but just easy going people. You will leaving there with some if not all the answers you need. I am currently working through a left knee pain and since starting there I know my knee has improved. I love the fact they give you a list of excises for you to continue the physical therapy after your session are done. Anyways they are the first place I am going with injuries. Just a side note, for these of you that have Anthem Blue Cross for medical insurance. They do not cover this location unfortunately. Well they didn’t for me. BUT it doesn’t hurt to always try.

Fantastic experience: If anyone need physical therapy I recommend you check out Dr Trevor such a great guy! Brought my father here after his knee surgery and he liked the way Trevor gave him his full attention it’s been almost 2 month and my dads getting better thanks to Trevor! Definitely a must check out place if you need any type of therapy.

Fantastic experience: Don’t think twice Form and Function is the right choice. The team at Form and Function physical therapy are amazing! Trevor spends the time to explain and walk you through what’s causing you pain and discomfort. I’ve had pain in my hip, knee, and ankle for quite some time. This pain has slowed me down and after just 4 visits I have had significant improvements. He teaches you exercises that you can do at home to continue to improve. I wish I found Form and Function sooner.

Fantastic experience: My brother suffered from severe tendonitis. Caused by bodybuilding. We tried many doctors & therapies to fix it. None worked until we found Form & Function. They were simple & straight forward. Everything was explained in the first appointment, during evaluation. The price per appointment never changed. Exercises were uploaded online, as you were trained to do them there. The schedules was very flexible, with being able to reschedule. All the personnel were professional & courteous. Never felt ignored, always answered questions. Best of all, the location being inside a gym helps tremendously.

Fantastic experience: I have seen Trevor for 2 months weekly and the progress I have made is amazing. He provides videos of your at home exercises which have helped me be more accountable.

Fantastic experience: I was looking for physical therapy after having wrist pain for almost 4 months from dancing. I was desperate because I thought I would not be able to dance again. After attending the initial session, I knew there was hope. We've identified three main areas that need fixing, including my wrist. I see great improvements from each session I attend. After the first two, my wrist pain was gone! Trevor identified the problem, showed me the exercise, and I was pain free two days later... After four months of wrist pain!! I'm so grateful. Highly recommend!

Negative experience: I went to Form and Function at Rancho Cucamonga, my mother had a condition and was not able to walk. She was wheel chair bound. I was paying Out of Pocket, 300$ /1 hr. We reached there and the owner who was also in many video advertisements, was the one who attended my mother. Without understanding her underlying conditions (to pretending that all will be well), he gladly said that if we continued therapy with them, she will probably start walking. He spent some time with her, and probably when the next client came in and was waiting next to us, he cut the session short, giving us less than an hour. The session was a waste of time, and he should have never made my mother go through that session, as it caused exacerbation in her symptoms and we had to hospitalize her next morning. Probably, she was ill, and probably she would have gone to the hospital in a matter of time, however, knowing her future/prognosis very well (she passed away three weeks later), the guy kept on giving us false hope just to take the money. They might have five stars, but my experience was horrible. It is a sham.

Fantastic experience: I started PT almost 2 months ago due to very limited range of motion in both shoulders as well as significant pain. Trevor has worked wonders with hands on treatment, cold laser therapy, cupping and other approaches. I especially like that he videos my "homework" exercises so I know exactly what to do. At this point, my range of motion is back and pain is greatly reduced. I'd recommend Trevor and F&F to anyone seeking quality, professional treatment.

Fantastic experience: Words aren't enough to express how pleased I am with Form and Function Physical Therapy. A shoulder surgery for a small torn rotator cuff, biceps resection and arthritis left me nervous about my physical therapy recovery. I've been treated by other physical therapy clinics for various sports injuries throughout the years and I wasn't pleased with their care and attention to my recovery. I needed to find a physical therapy clinic where the therapists truly cared for their patients. I found Form and Function through other reviews and the staff and therapists here have gone the extra mile to help me recover! The staff is very friendly -- I'm always greeted with a smile. The exercises are fully explained and the latest therapeutic technology is used to help the tissue in my shoulder heal. At the beginning of every appointment I'm assessed about my progress, my pain level and how the home exercises are working. The staff is encouraging and I look forward to upcoming appointments because I know the staff is truly working to help me. I've had amazing progress just after six appointments. Look no further --Form and Function knows what to do for you!

Fantastic experience: Dr. Trevor is an excellent Physical Therapist. When it comes to sport injuries he can get you back on the field right away. My two sons, my wife and My self go to him. Thanks to Trevor, my older son has been able to recover much faster helping him get back on the Soccer field without missing a beat. Trevor knows exactly what to do to get you back in the field ASAP. Fantastic experience: I have had a bruised heel bone for about 7 months. I started PT at Form & Function about 7 weeks ago. Trevor started with showing me stretches to strengthen my lower back, hips and core to address the root cause as its all connected along with videos of me doing the exercises to do at home. Adding Laser therapy, PEMF therapy and cupping, I feel I am well on my way to getting back to hiking. I am very impressed with the different modalities available at F&F which helps speed up recovery. I highly recommend Trevor and his team very professional, friendly and really care about their clients. I normally don't do reviews, but feel more people need to know about F&F. Thank you.

Fantastic experience: I broke my back and didn’t know it for 6 years. After having a disc replacement and fusion therapy of my spine I was in pain on top of pain for half a decade. Trevor was precise, worked directly yet gently on my worn and weary body. I was barely able to bend at all on my first visit. 1 hour later I could reach the center of my shins and calves. I’d say it was miraculous, but truly, it was Trevor’s skill and experience. Fantastic experience: Working here as a future DPT student was truly a gift. I started out simply observing, then was later given the employment opportunity. It was the best job I've had up to this point! I was able to learn so much from Trevor and his staff. I was welcomed with open arms, very well supported, and was adequately trained quickly. Fantastic experience: Trevor and his team were able to assess my body’s needs after a car accident injured my back, neck, and left shoulder. They designed a treatment plan for me that includes massage, stretching, strengthening, and laser therapy. Each physical therapy session helps lessen my pain and strengthen my upper body.

Fantastic experience: I started my physical therapy journey here for a broken foot after reading good reviews. I chose to go here and after 12 sessions we were able to get to where I needed to be in order to go back to work. Trevor and his staff are very friendly and great to work with. The facility is clean and has all the high end equipment. I highly recommend Form and Function and would refer any family and friends. Fantastic experience: I have known Trevor for over 8 years. He was my sons physical therapist during his High School and Club volleyball career. I recently had Stem Cell injections in my knee and shoulder and Trevor was the first person I called to assist me in my recovery and pain management. I had my first visit today walking in with a lot of fluid around my knee and a stiff painful shoulder with limited range of motion When I left the fluid around my knee was almost gone and my shoulder had more range of motion and much less stiffness. Not only dies Trevor have state if the art equipment but amazing knowledge and techniques only a top notch physical therapist would have. Fantastic experience: My wife tore her oblique almost a year ago and it never has fully healed because she lifts our 1 year old son. I knew Trevor from when I used to play professional baseball and worked out at EM. I’ve gone to Trevor in the past myself so I recommend my wife to him. Trevor did an amazing job at helping relieve my wife’s pain and used technology that is helping her fully heal.

Fantastic experience: Came in with a swollen, bruised and stiff knee from a recent surgery. After only 3 sessions the swelling is gone, bruising has almost disappeared and my range of motion has improved immensely. I look forward to my PT sessions with Trevor and all the knowledge he provides me with. Form and Function has great communication and staff are very professional.

Got a lot of help, a lot of hands on as far as examining where the pain is coming from and what they did to help relieve the pain. A lot of info on what I can continue to do on a everyday basis. I go in for maintenance on my elbows, back, etc. I work at a warehouse and the work is really demanding so I’ve gotten a lot of injuries over time. I do the one hour session once a month and I always leave that place feeling like a million bucks. Worth every penny!

Fantastic experience: I don’t know where I would be without Trevor. What started as a painful and extremely discouraging journey has become hopeful and practically pain free. This was done VIRTUALLY from another country, so I can only imagine what Trevor would be able to accomplish if I lived in the States. I owe everything to Trevor and I will be forever grateful for his knowledge and support.

Fantastic experience: I had a fall and twisted my ankle and injured ligaments, tendon and cuboid bone. I was treated with cam boot, ice pack and elevation and tons of pain killer for almost 3 months with Minimal improvement. I still couldn’t move ankle joint, Pain with any ankle joint movement and still unable to do full weight bearing. I started to have weak calf muscle and got very depressed. I looked up local physical therapy group and found it . The front desk is friendly and takes time to listen to meet your expectations. Their discovery assessment is only $35 but the service is excellent. May be i get lucky and it is a discount period . They scheduled me to be seen quickly and was seen by Trevor who can assess my problem quickly and fixed it more than 50% of my suffering in first visit. He pays attention to my problems and really take serious in helping me go back to normal like before and understand I need to return to work asap. His manual manipulation works amazingly and fixed like almost 90% of ankle joint stiffness and restricted range of motion. His Exercises are easy to follow and improved my calf muscle weakness in 2 days. I can sleep without pain waking me up at night after 2 days I was seen by him. I am more hopeful to recover back to normal like before. I have seen many PTs for my neck pain, shoulder pain but he is the best one I have ever seen. I will strongly recommend him and will refer my friends and family to him as well in the future.

Fantastic experience: I am not the type to write reviews or be on social media but I truly feel like this place is a hidden GEM that people with knee, hip pain need to explore and get natural healing benefits instead of taking those ibuprofen pills. I have been bringing my 14 y/o son since March after severe hip/glute pain he had been having from what we think was from a most recent back fall and improper stretching. When I brought my son in March he couldn’t even do a proper squat and he was out of shape and in pain. Keep in mind he had always been an active child and played soccer for a few years (age 5-9) and other recreational sport there after so one would think that he would be in good shape. My son’s initial onset of random pain began one morning years ago on the knee and transferred to the hip which was on and off pain but every time I took him to his pediatrician all he said was he had growing pain to give him ibuprofen only as needed. I never question this because I trusted the doctor but now I know some doctors don’t really care and you must take matters into your own hands. Since this most recent fall or pull my son said he felt, was when the severity of pain was more intense, I was worried and lost hope but God, Trevor and Stacy were there since day 1 and from the bottom of our hearts we would like to THANK them for being kind hearted, resilient and helping my son get moving so he can be a kid again and run.Thank you to Alyssa for booking our appointments as always and having a beautiful smile when arriving. That always brought happiness to my heart. In our case, we saw the pain diminishing around week eight. Keep in mind you DO need to do the exercises daily in order to get better which my son did. My son also takes there Joint Care supplements instead of ibuprofen since he does have inflammation. I highly recommend Form and function to your physical needs. God Bless You!

Fantastic experience: I had been dealing with tennis elbow for about 5 months, and was amazed by Trevor’s level of knowledge and the clarity he provided about how he planned to address the issue. He quickly assessed what was going on with me, and how it was impacting my body. He laid out a plan, and made the most of the hour, using every minute to intervene. I felt like he really wanted me to understand what was going on and why, and I’m excited to continue working with him.

Fantastic experience: This team is absolutely amazing!! I was having chronic back pain for over 2 years from a cross fit injury. Trevor and Stacie got me back to normal when I thought it would be impossible. I bore easily so Trevor adjusted many of the exercises that I had to do at home to keep me engaged. Everyone here is friendly and so easy to talk to. In the past at other PT locations, I have dreaded going in. Time dragged. I never felt this way going to Form and Function. I actually feel a little sad that they were so successful in treating my injury because it was such a pleasure working with everyone! I never write reviews but I felt compelled to make sure that anyone suffering from pain knows about this business. It really is one of a kind!

Fantastic experience: I went to Form and Function for a lower back injury. I only had 6 sessions but they videoed some of the exercises so I could do them at home. I felt so much better after my PT sessions and I continue to do the exercises. I must say that I’m pain free now but only because they were so helpful and tailored everything for my specific injury. I highly recommend them.

Fantastic experience: I was referred to Trevor by my Naturopathic dr. I was diagnosed with arthritis in my right big toe. I also have challenges with plantar fascistic. Trevor recommended Cold Laser therapy. After a couple of months, I had no more pain and the PF is 90% better. Who needs shots that don't necessary last? I plan to go back and work on some more areas with the Cold Laser treatment in the near future. I highly recommend Trevor, Jim and his staff!

Fantastic experience: Ever single day was extremely hopeless and stressful whilst I was injured for a year and a half. I looked on the internet for physical therapists near me and I noticed that this place had high ratings and GREAT, detailed reviews. I can see why, I went from thinking that I wouldn’t be able to live my life normally and exercise for a decade, to slowly being able to workout the way I used to. The team is helping me with my recovery every step of the way, they’re cooperative and friendly. The great reviews that people are leaving behind are legitimate. So anybody experiencing issues that a physical therapist can help out with, then you should ABSOLUTELY go to Form and Function. (Update) I’m able to exercise again, there is some pain at certain moments due to my muscles and tendons being somewhat weak, but as I grow stronger, the less pain there is. I’m extremely grateful for the physical therapists helping me. They’re the reason that I can help my family again, have fun again, and to finally get back to my workout routine.

Fantastic experience: I came in not knowing what I needed. Trevor was friendly, professional and effective. I was assessed quickly and we started work immediately. I came in heavily relying on a walker and left only using the walker as a fallback. I could walk! After the FIRST session. I can't tell you how excited and hopeful I am - after weeks of immobility I am beginning to gain my independence back!

Fantastic experience: My daughter dislocated her patella at dance rehearsal and needed physical therapy. My friend told me about Form and Function and said they are great so I thought I would give it a try especially since my daughter is a senior in high school and in the midsts of her competition season. Just after her first visit she could feel the difference. I knew right then that this place was special. She goes twice a week and gets the cold laser therapy…which I feel is a miracle wand. She is back dancing and feeling amazing. I am beyond grateful for Form and Function for getting my girl back doing what she loves.

Fantastic experience: Trevor is the absolute best! I came to him with some lower back problems which I was concerned about because I had recently begun taking weight lifting seriously. Trevor was able to help identify what was causing my problems, gave advice on certain workouts to add into my routine to prevent further injuries. Along with Trevor is his great staff who are all knowledgeable and friendly!

Fantastic experience: Trevor and the team at form and function are incredibly friendly and welcoming. Individuals in the medical field have an incredibly difficult task of on occasion having to give their patients news that they don’t necessarily want to hear, but even though this was the case Trevor put my mind at ease and prevented me from pursuing activities that would cause further injury. We then developed a treatment plan that has resulted in day by day improvement of my lower leg injury. I am incredibly grateful for the service and comfort provided and would highly recommend form and function to anybody who cares about their health.

Fantastic experience: This place is great, i came in and had a swollen foot and was on crutches for at least 3 weeks before coming here. My time here and my visits were the best thing for my ankle. Creating relationships with my trainers and getting my ankle back in motion. On my last visit I left with no boot and no crutches walking, running , and jumping on it. I’m grateful for them.

Fantastic experience: No regrets! Intially came to form and function for a second opinion after not feeling confident with my insurance provider p.t.s consultation/service and I am so glad I came. Trevor at form and function made me feel confident that he was going to do everything he could to get me back to were I needed to be after shattering my knee. At form and function I was treated like an important individual and not just another patient. I can go on and on but there is nothing like experiencing it for yourself. You won't regret it.

I injured my ankle last year (severe sprain) and after months of recovery I still had significant swelling and limited range of motion; After just one visit with Trevor I saw a reduction in swelling, increase in range of motion, and my ankle felt better than it had since I injured it. After three or four more visits with Trevor and Stacie, I had full range of motion back! Everyone there is super knowledgeable and friendly - highly recommend.

Fantastic experience: Hands down the best physical therapy team led by one of the best in the business. My 13 year old son had a pretty traumatic growth plate break to his femur and knee and they were the only ones I trusted to get him back to playing baseball and catching! They have been with us step by step and had a recovery plan for him that has helped him bounce back at the proper speed and make sure he didn’t damage his growth plate any further. I feel soo fortunate that we were recommended to come to Trevor and his team and will be forever grateful on how amazing they did getting my son back in the game!

Fantastic experience: I've been to 3 different places from massages, yoga, and cairopractors and this has been the only place where the pain on my body has went away and my muscles and body have been functioning again. The techniques they use really work especially the mix between acupuncture, exercise, and massages.

Fantastic experience: I am so Thankful for Form and Function, the Physical Therapist and employees. Was diagnosed with a pinched nerve around lower butox area. Unable to stand, walk or sit was in major pain and discomfort and with just a few treatments I've made awesome improvements. I would definitely go back for treatment and refer friends and family. Negative experience: Form and Function Physical Therapy I feel is the No. 1 PT clinic in the area. Have been to various PT Clinics in the area and none of them come close to Form and Function. Trevor is so knowledgeable its amazing. I would not go anywhere else for PT.

Fantastic experience: The Form & Function family has literally treated my entire family. My daughters play soccer and run cross county and the F&F team has always been able to diagnose and treat the injury and literally keep them in the game. I run marathons and they have treated me for severe planter fasciitis just three weeks before the marathon. The team is knowledgeable, experienced, and gets results.

Fantastic experience: I highly recommend Form & Function. I had ongoing foot pain includung a burning sensation on the bottom of both feet and a concentrated pain under the ball of my right foot. I experimented with different shoes and insoles but the pain kept getting worse. Trevor diagnosed the problem and after one visit I saw solid improvement. Now, after six visits my pain is completely gone. Thanks to F&F's therapies and "homework" exercises I am once again able to move freely without any pain.

Fantastic experience: The team at Form and Function have been a instrumental in helping me recover from frozen shoulder (also called adhesive capsulitis). Having a painful injury like this is not fun but I look forward to each visit because I have seen progress thanks to their focus on me. The other thing that helps me is getting acupuncture. I had never tried this before coming here and they showed me how much it can help with my recovery and it's NOT painful like people imagine. Last but not least Cupping has helped breakup scar tissue which was also a first for me. I highly recommend Form and Function for any rehab, strength and training needs. Call them ASAP.

Seeking treatment at Form & Function was the best decision we could have made to help her recover from a knee injury without surgery. As a soccer player and a long distance runner, she has sustained multiple injuries over the years and Trevor has always helped her recover quickly.

Fantastic experience: Took my athletic son in for an evaluation since he was having pain in his throwing arm. Trevor made some adjustments and after the 30min session my son had more mobility in his arm. He said his shoulder felt better. We purchased 12 sessions and my son hasn't had any issues.

Fantastic experience: Trevor and Jim are awesome. The Form & Function team has changed my opinion on PT post surgery. I had MPFL reconstruction surgery and went through the traditional PT with my HMO insurance and became discouraged and frustrated. So I took matters into my own hands and found Trevor. The Form & Function team have identified other issues that have been prohibiting my recovery. Since starting with the team, I've been able to regain some strength and mobility in my knee and quad. I'm extremely excited for the future and my ability to get back to being active. I highly recommend Form & Function.

Fantastic experience: I have intense back pain triggered by a car accident I was involved in. Regular movements were painful and uncomfortable, which made working and running my business uncomfortable. I started coming here for regular sessions multiple times a week. I've had so many different treatments on my back, needless to say nothing was helping my mobility. My girlfriend took me to EM speed & power for a session and it is the BEST physical therapy I've had. The staff is Amazing and extremely knowledgeable. They make you feel comfortable and confident with the recovery process. They also offer massage, acupuncture, and physical therapy that professional athletes use on their own bodies. I would highly recommend this company to anyone and everyone who is looking for a PT. If I could give them 10 stars it still wouldn't be enough. I have continued to do stretches and work outs you guys taught me. My back is stronger and my pain has become more manageable. Trevor was absolutely amazing and his entire team were nothing short of brilliant.

Fantastic experience: Trevor and his crew helped my ailing knee markedly. They were tough on me but that’s what I needed. Plus, they have a cold laser therapy machine which I had them use on me every single visit. I didn’t have complete knee replacement ( which is what the doctor recommended). In fact, I had no surgery. Does my knee pain me...yes, sometimes...but I believe what Trevor did for me allowed me to walk a mile every day with sometimes no pain at all. Thanks Form and Function.

Fantastic experience: My daughter had a terrible arm injury and couldn’t throw ten feet!! We had already been to a few places for recovery and had gotten her x-rays. We had been told just to let her rest. No one could tell us what was wrong or what caused the injury. We reached out to Trevor at Form and Function and immediately got her in to be evaluated. Trevor was able to diagnose her injury and got right to work. He has given her life long exercises for her entire body and she has been able to recover within 5 visits. We also have been made aware of what caused her arm injury and now can monitor and work towards maintaining her body for her future! Fantastic experience: Prior to visiting Trevor, I was having terrible hip and lower back issues from marathon-training. Fantastic experience: Trevor is amazing! His team and he are very knowledgeable and can fix and help our softball girls with whatever is bothering them. From injuries, tight and sore muscles to sprains, he will fix it! Fantastic experience: I am a 20+ year firefighter who suffered from an ongoing Achilles injury. After my surgery with an excellent orthopedic surgeon, I was looking for a physical therapy group that was equally professional and knowledgeable. I made several calls and visits to different locations. I found Form and Function PT to be a sports based PT group that was located within a multi-sport training facility. With my job being as physically demanding as it is I knew this was best suited for my needs. Trevor Fields gave me my consultation and I was off to my road to recovery. Fantastic experience: I am a baseball pitcher and went in with shoulder and elbow pain. After just the first session I felt relief. I was pain free after 3 sessions. I have tried other sports therapists for months at a time with no result. Trevor is by far the BEST physical therapist around!!!!

Fantastic experience: My son is a college pitcher in Kansas and was worried about some arm pain. He wasn’t getting the answers he needed and didn’t know who else to go to in Kansas. I called Trevor at Form and Function because we have been to him before and we trust his care. Trevor was able to help my son through a Zoom call and some help from a teammate (Trevor’s idea in order to move the arm in the positions Trevor needed to see) Trevor was able to video exercises to loosen tightness in the shoulder and send them to my son. Fantastic experience: Forget pain pills, go get physical therapy. I went to see Trevor several years ago with severe knee pain that I'd been suffering with for almost a year. I could hardly walk uphill, or do stairs without severe pain, forget even trying to run. At my age (over 50) I thought I might need surgery. He identified the issue & worked with me to build the muscles around the knee to support it. I continue with the exercises & strengthening & I haven't had any issue since, no surgery, no ice packs, no pain pills. I can hike, play sports & even run up stairs.

Fantastic experience: My sister recommended Form and Function. So my husband and I went here because we both had knee injuries. One week apart. I could barely walk my knee was so bad. Trevor was very helpful In treating my husband and I. He gave us mobility exercises we could do at home or at the gym. I went from limping to walking normal within six weeks with much less pain.
