FORM Hand Therapy

Negative experience: I saw patients leave because they had been waiting for over an hour. Doctor will tell you you are deformed. Doctor will not be honest about how much services will cost. Doctor wasn’t professional. Poor communication and customer service. The PT office kept on cancelling all of my appointments for almost 2 months so I actually never received treatment. On the bright side: the MRI team is phenomenal, friendly, and talented. I’d recommend MISSION PEAK ORTHOPEDICS!!

Fantastic experience: This place is wonderful! Serene atmosphere with background music, very clean and spacious. All the staff is courteous and friendly. The therapists are very well trained and knowledgeable. I highly recommend Form.

Fantastic experience: Literally Hands down the Besht physical therapy for your hands in Fremont not only is Dr. Besh the best surgeon, they also have the best physical therapists, and there supporting staff is also the best, this was my second time coming to this location and I HIGHLY RECOMMEND

Fantastic experience: Have been seeing this wonderful team of therapists , assistants and front desk staff from end of Jan 2022 for my both hands issues. Have to write my outstanding experience with this place that started with my first call to book appt with Dr Besh . It was Erica the clinic supervisor ! Erica B was extremely helpful , professional and very sweet to help me with appt and other info for my record transforming questions. So far I have seen just few of these outstanding staff members that I feel to acknowledge. Erica B, Kevin,Sophia,Jamie, Alejandra,Janina,April and May ! Thanks for this amazing patient care

Fantastic experience: The staff is completely friendly. They fix the sound in the picture my other hand is still a little messed up but that's the one that had surgery. But we'll see what happens in the future as I finish up the exercises they gave me I suggest if you need hand work this the place to come

Fantastic experience: I first came to Form with both of my wrists basically disabled of doing anything that required mild effort, and that frustrated me. But this staff with so much knowledge, passion, and effort in their job helped me significantly and brought both of my wrists almost to their perfect form. But what got to me the most, was the individuals of these people that I have gotten to know as time went by. *Sophia, Jalina, April, Melissa, Andrew, Josh, and the rest of the OCT staff, you guys are really wonderful and awesome people to get to know. I saw that every one of you were genuinely intrigued in getting to know me, and not just because it is your job. Because of that, all of you just made me feel so comfortable while still doing your job and helping me get to where you and I were aiming for on my wrists. Positive experience: Dr Besh is great, but waited 40 minutes from my appointment time to be seen for a five minute or less visit.

Fantastic experience: I would highly recommend Form Therapy, Dr.Besh was recommended by my Dr. I had cut the tendons In my thumb with a table saw and Dr.Besh was able to put my thumb back together so that I can use my thumb again. The therapy did a great job, Kevin was my therapist and he spent a lot of time working on my thumb to get the most movement and strength in my thumb and hand. Kevin has a lot of assistants that helped him and the other Therapist. Everyone there were very professional and were right there to help you. I was very impressed that everyone there were happy to help me to recover and kept me in a good and a happy mood.

Fantastic experience: I got a bad injury on my hand & I came to know about this place , then I visited Dr. Besh , he did my hand surgery very well, now I’m getting physical therapy from one of there great experience therapist Kevin , all of them taking care very well for my hand , big thanks to all of them , I’m strongly recommend this place for hand , elbow , wrist therapy

Fantastic experience: Wonderful atmosphere and a welcoming. I am an interpreter and am sent here quite frequently. The staff is professional, friendly and overall efficient. It is actually quite fun knowing I'm going to be assigned there. I always look forward to seeing the staff; they make me feel like part of the team. We work so well that the patients think I am actually part of the staff there... (that would be nice).

Fantastic experience: I would highly recommend Form Hand Theropy Clinic. Their staff is kind and professional. Their clinic is clean and very well equipted. They have a hand therapy machine, a Primus BTE unit that can replicate any possible hand function. If you need hand therapy, this is the place to go. To the staff, THANK YOU.

Fantastic experience: I broke my wrist in November and had a hard time finding a affordable hand therapist in Santa Cruz. My daughter worked at FORM and recommended that I make the drive to Fremont. What a lifesaver. It’s kind of a one stop shop for hand therapy with many therapists on site. They are also affordable, charging you only for the visits you make. The therapists are very friendly and knowledgeable. I worked with Sophia, Joshua and Andrew. Great people, great place.
