Fantastic experience: My dad had 2 surgeries recently and needed a facility to help regain his strength back after each. We are so thankful Pacific Haven was the place we chose to help him recover. The PT staff were so patient with him throughout his healing, the nursing and admin staff were always so kind and helpful. The facility itself is a little on the older side, but it is very clean and spacious with a nice garden to help residents get some sunshine. Keep up the good work Pacific Haven - your compassion and kindness is so greatly appreciated!

Fantastic experience: For my mother post injury therapy, I'm happy I went with Pacific Haven. The facility is clean, well-kept and roomy. Every member of the team including the nurses, CNAs, front desk employees, social services, maintenance and kitchen crew is kind and accommodating. Thank you very much for all the hard work and exceptional care that the physiotherapy team especially from Dan, Benjie, Ryan, Phoebe and Vivian had done for my mother. They have strong commitment to helping her becoming more mobile. People I would not forget to thank for their support and kindness include Nick Ho, Mayra Gomez, Jacqueline, Diane Tran, Van, Mai, Annabelle, Alex, Lucia, Daisy and Ruth. Thank you all.

Fantastic experience: My mom had knee replacement surgery and had a great experience staying at Pacific Haven for 2.5 weeks. The staff was friendly, supportive and the care was top notch. Rehab is right there at the center so it was very convenient. I was always greeted with a smile at the front desk whenever I visited. Thank you Pacific Haven for taking care of my mom! She will definitely be back when she has surgery on her other knee!

Positive experience: Thank you so much for having such caring staff , one particularly person whose name is Yesenia Serna (cna) who goes above and beyond from her duties. She’s one of the cna’s who’s taken care of my wife and I’ve got to say I’ve never been so please with the passion this young lady shows towards her patients. Fantastic experience: My Mom came back home today after staying at Pacific Haven Subacute and Healthcare Center for 2 months. Overall staff here took care of my Mom carefully, kept cleaning the room, bathroom, and bed very good and sanitized. As my Mom is only able to eat Asian food, they have menu specific for Asian, taste good and my Mom love it. Chief always listen and make my Mom’s favorite food as she asked for. Therapy staff are also very friendly and helped my Mom alot. Only one thing we concerned that one day, a CNA name “L****” was a bit rude attitude when my Mom complain about the beeping noise. My Mom forgave her, but just to remind that don’t treat other clients like that. Overall I would give 5 Starts to this place! Thank you everyone!!!!

Fantastic experience: Kind caring staff, great therapy and the food is delicious. Can’t think of a better place to spend a month or two recovering.

Negative experience: We went to visit a friend at this facility. Being it is Christmas time we brought him and the residents cookies. We had filled two giant Santa bags and two giant Christmas bags with cookies for all the residents to enjoy. When. We were leaving imagine our shock when we saw one of the nurses walk out with one of the Santa bags we brought and put it in the trunk of her car!!! Bad nurse stealing from the elderly and at Christmas time and bad administration to allow them to steal from the residents.

Negative experience: Today I went and visited my Grandpa with my family. We brought 4 santa sacks filled with cookies and crackers for all of the residence to enjoy for christmas time. As we were leaving one of the nurses STOLE a santa sack and put it into her trunk. The audacity she had to steal from the elderly and for this place to allow it.

Fantastic experience: Our mom was there for over 30 days the place was so clean always. The staff is more like family and mom loved each and everyone of them.No matter what time of the day or night call lights were answered very fast. Mom was in 9b. Everyone there was great. Mom loved her time there.

Fantastic experience: Thank you, Pacific Haven, for your fine treatment of my father over the past three weeks. Ryan and the other physical therapists helped my father vastly improve his mobility, with a return of walking strength to his legs. And every description of your facility and its services stated to me during the initial tour by the Marketing Director / Cultural Program Director proved entirely accurate.

Negative experience: I have a patient there since she moved in I wasn't able to see her even behind the window they wanted me to take COVID 19 test even though they have the copy of all my vaccination and couple of weeks after she moved in for threcommende first time she got COVID + ironically she did better when she wasn't there. Every time I call to know how she is doing they pass me to others and most of the time they hang up on me I have never seen a placeso disorganized I do not recommend this place to any one

Negative experience: Social Worker Terry Porfiana is not even social at all be careful with this place , they will make up excuses not to let you see your loved one . Even if you follow the Covid protocols and you will not be able to at least contact them also they will make you believe is 5 stars which is only a manipulated rating.

Negative experience: Cathy/Kathy at the reception desk has been so rude to my family and I. Someone please review her service because she's so rude to the patients. Even when she's around families she's rude to the patient so you have to wonder how she is around the patients at all times. I've seen her around other families as well and it seems she just has a rude attitude overall. Please check on her!!

Fantastic experience: My Mom was admitted to Pacific Haven February 7, 2022 after suffering a stroke the last week of January 2022. The care that she received while she was there was phenomenal. My Mom also has dementia and depression, so being in a nursing home made her feel like she was abandoned by her family, she would always cry everytime that we visit her. As days passed by, her crying episodes lessened..come to find out that the Filipino staffs there went out of their way to talk to her and encouraging her to do better in her physical therapy sessions. Their motivation for her was so she can go back home to the Philippines! My brother was also admitted here 2 yrs ago after suffering from respiratory distress and a heart attack. The physical therapy department did such a good job with him. I’m thankful for all the xtra care and compassion the staff have given to both my Mom and my brother. Since I used to work here, I know how how meticulous they are when it comes to care for their patients. To the Administrator (Melody Cruz), DON (Soledad Casino), ADON (Anabelle Duyon), Social Worker (Tere Alcaraz), Medical Record Asst. (Rosemarie), DSD (Albert), Rehab (Aslene, Rimando), RN Supervisor (Monina), LVNs/RNs (Gemma Duyon, Grace Alvarado, Mickey Nguyen, Vicky Sagun, Erwil Samoy), All shifts CNAs…from our family, we would like to express our sincere gratitude for taking care of our Mom❤️❤️❤️. This is the only facility that I can entrust the care of my family and friends..

Positive experience: Pacific Haven has dedicated staff that provide compassionate care to its patients. The team meetings to discuss our loved ones needs and interventions were very helpful. Debbie, the case manager is very polite and knowledgeable about quality patient care. We recommend this facility for those seeking moderate care for their family member. The monolingual staff is a plus to meet the multi-cultural needs of families. Thank you for the care you provided to my mother. May you continue to provide the highest quality of service to patients. Positive experience: I really like it here. The nurses are helpful some of them are pretty too. Mai wound nurse is excellent she took care of my wounds. I like that my room is always clean and always get clean linens. I also got my own tv. Always had my needs met. Food is good sometimes excellent, got coconut crispy shrimp and was so good. Fantastic experience: My dad has been transferred from a closed out subacute healthcare facility to this Pacific Haven. So, I would like to write reviews with more than 5 stars if I could of this facility to cast ANY DOUBTS of any people out there that PLEASE PLEASE don’t ever HESITATE to send your loved ones to either subacute or SNF section of this Pacific Haven!!! When the previous facility, where my dad stayed about 3.5 years, called the relatives of subacute residents to be looking for a new place within less than 3 weeks or 1 month, I was so shocked and nervous with couple of sleepless nights and with a question to ask myself where would be a new place for him? I was provided a list of nursing homes and I read Google, Yelp, Caring.com, SeniorCare.com reviews of all facilities on the list in OC. In the next couple days, I drove around OC to visit couple of them. Finally, that’s it and Thanks God that I have chosen a trusted nursing home for my dad because of the following things of this facility!!! 1) It has many strict infection control procedures in place that staff and visitors/caregivers have to follow. 2) It is a VERY VERY clean nursing home I ever visited. 3) All staff (from Administrator, DON, Infection control nurse, RNs in charge, LVNs, RTs and CNAs to Housing Keeping/Maintenance crews) are very nice and attentive to residents and they all make rounds to visit and check on subacute residents more often. 3) I like this facility very much that if a resident (like my dad’s case recently) has a health condition issue or problem, they called me right away no matter of day or night, and they also called the doctor too. 4) They check my dad and subacute residents Vital signs more often than previous nursing home did. Ladies and Gents… OMG, they do teeth cleanings for my dad and subacute residents twice a day!!!! This is absolutely amazing because it definitely protect subacute residents against infection and pneumonia!!! I loved that so much!!!! ❤️❤️ Finally, I am not only writing this review on public forums, but also writing to express our family gratitude for Staff that are currently taking care of subacute residents in general and our dad in particular with their professionalism, compassion, caring and love. Positive experience: My mom stayed at Pacific Haven two months ago and just Discharged yesterday. She just went home yesterday but already missed Pacific Haven and begging me to bring her back there . She missed all Pacific Haven staffs, all activities she did at Pacific Haven and especially the physical therapy staffs that worked with her & helped her in every way, plus Tere the social worker and Melody the Administrator who's conscientious, knowledgeable & advised when you needed her. They always take good care of my mom. I thanked & really appreciated all their kindness , caring and listening to my mom when she feel sad or emotional. The physical therapy team staffs they work from their heart. If you or family members need therapy rehab you definitely love to come here. It's very nice and clean place . So I highly recommend Pacific Haven. I also really thanks & appreciated for all the help from Melody and Tere . You are the best! We will definitely come back to visit your guys some days. Fantastic experience: Pacific Haven is a very great place to recover from your illness. I was treated with compassion, empathy, and professionalism. The food was excellent. They made sure that I had enough to eat by giving me double portions. I would recommend Pacific Haven to anyone needing long term rehab. They also do short term as well. Tere Alcaraz who runs this facility was efficient, courteous, and compassionate to meet my requests and needs. Tere is the social worker at this facility. All the staff was courteous, gentle,empathic to every patient. They were also very efficient at answering your questions and requests. Again, I enjoyed my stay here. It brings tears to my eyes to have to leave this facility because the staff were more like family.

Fantastic experience: My stay here at Pacific Haven Subacute has been wonderful. I came in here with the tubing in my throat, feeding tube,oxygen and I cannot even move and talk. And now I improved so much, thanks to wonderful staff here. I can now eat regular food and one time they gave me Pho soup and really liked it. And now after almost 3 months, i don't have the tube on my throat and feeding tube anymore, and I don't need oxygen which I needed to go home to my family. The therapists are all heaven sent, they worked really hard so I can improve my function. I can now walk by myself and I am so thankful. The social worker Tere helped me alot too, she was there whenever I need her and she also helped me with my discharge. I cannot thank them enough. This place is a good place to have recovery.

Fantastic experience: I would like to have a few words to thank Pacific Haven Subacute and their professional staff. This is my second time staying here and I really appreciated again for this time that I am being here (for 3 months). The facility is so clean, opening, from the floor to resident's room to the patio. The food is good and well-prepared. I love their lunch and dinner; they always being severed on time and warm. The whole Rehab department is excellent; The therapists are outstanding. They gone out of their way to help the patients' needs at all times, to make sure the patients are safe and happy. They served their customers (patients and their family) with love and care. When I first admitted to the facility, I was weak, no energy, unable to stand up, got fatigue easily. Now I am healthy, strong, be able to get out of bed, use the restroom by myself, and able to walk a long distance (more than 200 feet). I really appreciate all the hard work that they put on me to make I am today. They are my most favorite people to spend time with. Once again, thank you Pacific Haven Subacute! Fantastic experience: I was very pleased for be here. I had a great experience here and enjoy the rehab and the recreation and activity programs. All of the people here and nurses help me. Fantastic experience: I rarely ever write reviews, so this is a first for me. Pacific Haven has taken care of my husband for the better part of a month. They provided superior care and understanding for a person who suffers from neurological issues and cannot communicate well. They were kind, understanding, helpful and attentive. Their nursing and physical therapy staffs were wonderful to work with, and the administrative staff was always friendly and eager to provide whatever help you needed. They helped me understand his care needs at a time when I was in great need of that help. Needless to say, I highly recommend Pacific Haven.

Fantastic experience: I'm not a fan of writing these things, but I feel that credit should be given when it is due. I was admitted to this facility back in December 2021 after spending some time in the hospital. I had a very serious and lengthy procedure at the hospital and needed to recover with rehab and gain strength at Pacific Haven. I was already feeling down since I knew I'd be spending my holidays in the facility, but I had to get stronger so I could actually go home! This facility is definitely a well kept secret, since it is nicely hidden amongst some really big and old trees. Upon being admitted, the facility was nicely decorated throughout the hallways for the holidays. I was placed in a room with 2 other roommates, who kept to themselves for the most part. Due to COVID19 guidelines, it was nice to see the staff following all the proper precautions. The overall cleanliness of the facility was amazing from top to bottom. I would always see people cleaning and wiping things down from my bed. The overall staff of the facility were great. The CNAs were very attentive to my needs and I'm sure I had a lot! They always assured me someone would come back to assist me if that person was not able too. The nurses were very nice and caring. They were very attentive to my needs when I asked for medication. The Activities staff were really sweet and always offered me magazines, coloring material or books to read ... anything to keep me occupied. They arranged for all the managers to dress up in holiday attire and sing Christmas carols throughout the hallways of the facility too! It was great to have my spirits lifted. The Rehab department are great people. They are very patient and encouraging too. Sometimes, they would just sit with me and talk about life. This was their tactic to get me to participate with their exercises. The staff at this facility is overall very caring and family oriented. Everyone made my short stay bearable. I would definitely recommend this facility to family or friends needing a facility for rehab. I would only want to come back to this facility if needed in the future. Thank you Pacific Haven & Team!

Fantastic experience: I had an infection in my blood and stayed at UCI Hospital for 6 days, till I was finally cleared to leave but still needed to go to a SNF for IV antibiotics for a few more days. I chose to come to Pacific Haven Subacute from a list provided to me. I was a little unsure, because this was my first time going to a nursing home. Coming to Pacific Haven was nothing I had imagined, because it was MORE than what I thought. I had such a great experience coming here. The staff is so humble and caring with their patients and really encourage healing and self motivation. The rehab team was really good. They helped me regain strength in my legs, because when I first arrived to the facility ... my legs were really shaky and weak. The rehab team was able to help me get stronger, so I could go home. The activities department was really nice and helpful with keeping me busy. The food at the facility was really good and better than I thought hospital would be. I'm thankful the team and everyone at Pacific Haven Subacute has helped me heal and get stronger. I would highly & definitely recommend this facility to family and friends. I would definitely come back to this facility, if I ever needed to get rehab or treatment again in the future.

Fantastic experience: Hi my name is Bruce Smillie I was at Pacific Haven for three weeks after I had surgery to remove cancer from my lip and throat area. Just say I was taken care of very good the nurses are outstanding and the physical therapist outstanding as while as all the staff is a very caring. So I would recommend Pacific Haven

Fantastic experience: My father was sent to Pacific Haven from Kaiser needing skilled nursing care with trach care and close monitoring. After being here at the facility for about 3 months he was able to move on to the t-mask and by today he's been able to breathe pretty much on his own and is being discharged home. Every staff member (from RNs, RTs to CNAs) here are very passionate about their job and very welcoming when we would stop by during our window visits (Day & Night). The staffs were always helpful asking us if we needed anything or had any questions. Another plus is that this facility was always kept clean and they always groomed my dad nice and provided haircuts/shaving/showers keeping him looking nice and comfortable. The case managers here Debbie/Terri were always easy to reach too and would call us keeping us updated on any changes or tests that were being provided to him. Our family has been blessed to have him sent here to this facility and to receive the care that he had been given. Thanks again Pacific Haven! -Vu Family

Fantastic experience: My dear mother was a patient at Pacific Haven for 3 years. She had Alzheimer’s. I took care of her at home and did not want to let her go, but she needed skilled nursing care that I could not give her. I know that God led me to Pacific Haven because my mom surely landed right smack dab in the middle of a nest of angels. They treated my mom like she was their own relative, and they do this for all the residents. They are so professional, kind, caring and above all, patient. The staff are my heroes. When my friends have to face how to get their parents the best care, I always tell them—Pacific Haven!

Fantastic experience: I never thought I'd end up in a nursing home at the age of 26, but I did! I was in a traumatic accident and had multiple and severe injuries throughout my body back in October 2020. I was taken to UCI and had multiple surgeries, until I was stabilized and sent to Pacific Haven Subacute for the rest of my recovery and rehab care. At first, I was scared and anxious of being in a nursing home. My first night at the facility, the staff knew I was scared and did everything to comfort my fears and anxiety. The staff in the Subacute unit were so attentive and comforting to me. The RTs, CNAs and Nursing staff were really patient and attentive to my needs. The Subacute is a very busy unit, due to the level of care the patients (aside from myself) need. My first full day, I was visited by multiple people from every department checking on me and asking me questions. I was evaluated by the Dir. of Rehab, Ryan Cheng who was really nice and informed me of what the rehab department was going to do to get me strong and back home. Scott and Benjie worked with me on my OT and PT movements. I literally had to learn how to do everything all over again with my injuries. From getting dressed to just sitting up in my bed was an all new process for me. Little by little and day by day, I was able to gain strength to learn how to walk again. I even had to work with the Speech Therapist, Tina on how to eat by mouth again. I had my jaw wired shut for a couple of weeks and was only able to drink or swallow pureed foods. I eventually graduated to eating regular textured foods, which I didn't think or know I'd be able to do again. The food at the facility isn't like your typical hospital or nursing home food. The Dietary staff were always accommodating to my requests, even if it was a last minute request. I had daily visits from the Activities department staff. They were so nice in providing me with magazines and activities to do in my room, since I wasn't able to leave my room (aside from working with the rehab department). Even if it was just sitting next to me and having a conversation about life, it felt good to have someone there just to talk too. My Case Manager, Debbie and Social Worker, Tere were so good to me! They visited me daily and checked on me. They always made sure I was okay and had what I needed on a daily basis. Debbie was always in contact with my doctors at UCI for all my needed follow up visits and making sure my healing process was progressing. In all honesty, my recovery at Pacific Haven was the best place for me. My original goal was to discharge and go back home before the holidays, but the staff was always honest with me and my recovery so I wasn't too sad that I didn't get to leave when I wanted. So just after the New Year 2021, I was able to fully WALK out of the facility and go home! I am so thankful and grateful to everyone at Pacific Haven Subacute for all their hard work and efforts in healing me and getting me back home. I never thought anything as traumatic as my accident would happen to me, but it did. I never thought I'd be 26 years old and staying in a nursing home, but I did. I never thought I'd walk, talk or eat again ... but I am now! I would highly recommend Pacific Haven Subacute to all my family and friends, who need to recover from anything or need to have a family member placed. The staff is amazing and treat you like their family. I will never forget the staff at Pacific Haven Subacute for everything they did for me and giving me hope for a new life ahead. THANK YOU!

Fantastic experience: My husband had a massive stroke. He was released from UCI Medical Center in Orange & went directly to Pacific Haven. Richard could not talk, walk & his head hung to the side on his shoulder. Richard was at Pacific Haven for 9 months. The physical therapists & the speech therapists are miracle workers! The physical therapists taught Richard how to hold his head up, how to get out of bed, how to eat, & how to walk again. The speech therapists taught Richard how to speak again! With Richard’s condition, everyone was so patient with him & took great care of him. While I was at work during the day I did not have to worry about him because I knew he was in great hands! And knowing that - took a lot of stress off me. Pacific Haven is immaculate. The hallway floors shine as you walk down them & the same for the patients rooms. Everything thru out is spotless & very clean smelling! Richard’s hygiene was always perfect & he even got his haircut. Up to 2 months ago, Richard was still getting his haircut by the same lady who cut his hair while he was in Pacific Haven. The activities for the patients are wonderful. And the patients food is cooked fresh daily in their kitchen. From breakfast to dinner + snacks - their homemade food is very good! The entire staff is & are wonderful people. By the time your loved one leaves Pacific Haven - you feel like your own family has grown! We highly recommend Pacific Haven to everyone who has a family member who needs help. Your family member will receive excellent care here & everyone at Pacific Haven saved my husband’s life! We are so grateful & blessed that Pacific Haven cares about their patients!!

Fantastic experience: I was admitted on October 31st 2020 after being transferred from UCI due to a traumatic accident on the second of that October. I was there for about 2 1/2 months getting care from Nurses and CNAs, Physical Therapists Scott and Benji who have been there since I first sat up on the bed to walking again, Tina my speech therapist who oversaw the steps that I needed to take to eat solids and write again and, my case worker Debbie and Tere to help me get back on feet. it was tough, I did not have my family with me ,I was stubborn but most of all, I was alone trying to heal from what Ive been through. The staff knew that and in a way became my second family because of the amount of pain and suffering I endured. They were with me every step of the way and I appreciate them for all the things they have done. I highly recommend your loved ones if needed to go to Pacific Haven, they will take care of them at the best of their abilities.

Negative experience: My grandmother is still dealing with PTSD from being mistreated by staff here; do NOT bring loved ones here. There is a high likelihood they will be mistreated; my grandmother’s demands for help were actively mocked and not taken seriously. Definite neglectful behavior at this facility.

Negative experience: Horrible care and service. Horrible communication with family members. Horrible discharge process. Whoever schedules follow up care appointments for after discharge is incompetent. They prolonged my dad's stray there after nearly killing him while cleaning his tracheostomy so that they could get more money out of him. DO NOT LET ANYONE YOU KNOW BE SENT HERE!

Fantastic experience: On behalf of the Sciacca Family we want to thank the entire staff at Pacific Haven for taking care of our husband, father, friend. You all made a very trying and difficult time manageable and treated our loved one with love, kindness and compassion. Fantastic experience: I was sent here after 2 months at UCI Irvine Medical Center. I was looked after with great care by all the personnel. I did rehab for two months which really helped me recover. Food was good. Personnel friendly. The place was very clean. I recommend them highly.

Negative experience: This place is totally not set up to do joint replacement rehab. Proper equipment in bathroom not there. Aides don’t know movement precautions, food is awful and you may have up to 2 roommates who may be incapable of using their call light so you are getting help for them in the night. So much for healing sleep! Someone is making lots of money giving substandard care.

Negative experience: Took my dad there for rehabilitation. One day later spoked to social worker for more info. One week later my dad told me that Tammy To (social service assistant) was being rude to him and yelling at him to be quiet and threatening other patients to be quiet or will be kicked out of the facility. I am surprise this place is still open.

Negative experience: This place is a horrible. Do not take your loved ones here. It has 5 starts because they manipulate you to believe they are the best. the people here are all fake especially the administration.they all care about the medicare patients ,Tere Social worker is not even Social and & She treats the staff well if they like them and you are the Favorite otherwise She threatens & intimidates employees about not to comment anything in Social Media & treats CNAs like trash but when things starting to get sour? next thing you know they all fired. (got these from the employees) they don't care about the people. they all care about the money they would get from the government. i would not refer any of my family there again. they will welcome you at first then after your insurance days is gone? they will evict you and tell you that your therapy is done and etc. They worship money.

Fantastic experience: Wife there almost 6 weeks and after more surgery our family chose to return again. Clean, professional and caring. We never expected a SNF to be so caring. THANK YOU to all the cnas, nurses, case manager, social worker and management. Keep up the same standards you now hold to.

Negative experience: Horrible friend was there for therapy. The basically kicked him out after a week. He wasn't ready should have gone back to hospital. Was taken by 911 two days later.

Negative experience: Worst place ever! My grandma was put here for rehabilitation and I would never want her to come here again. Horrible food! Smells disgusting! And the staff is horrible! I will be reporting them! Many occasions were the nurses would steal my grandmas fruit, (I would take her cut up fruit once and a while) and they would eat it. Also on the last day my grandma had her clip stolen from one of the nurses while out on her walk and never returned. One of the nurses was trying to physically force my grandma to take 12 pills at one time! Complained and felt like Tere didn't even care. Also on two different occasions saw the nurses treating my grandmas roommates pretty horrible not tending to the poor ladies who were in pain or thirsty, Said they didn't care because they were practically dying. Food- told them many times my grandma doesn't drink milk and every time the meals came milk would be on her try. Food was extremely dry and dishes seemed dirty. Staff- one nurse didn't even wanna help my grandma out of bed to use the restroom, instead told her to just pee the bed and she would change it later. Many ladies in Spanish gossiped and talk badly about other patients. The Asian lady who took the paper work could hardly speak English. One of the nurses who came to ask my grandma would yell the questions out thinking she was deaf but my grandma has excellent hearing... when placing a complaint, it seemed to not be a concern to Tere. Please choose any other location! I read the reviews before my grandma was placed here but didn't have a choice since my grandmas doctor could only come to this one.. worst place ever!

Fantastic experience: Employees are awesome. OCD type clean with bleach daily. Student nurses highlights of my day. FML being hear sound mind and fully capable of getting better. Oh well its better than most jails or state prisons.
