Advanced Health Care of Glendale

Fantastic experience: We have used this organization 5 times (that is correct!) in 2024 and many more times by family for the past 6 years. This place is the benchmark! Great people and Wyatt the administrator is absolutely first class! Seeing an organization with all of the staff at this level is astounding. My hat is off (showing my bald head) to Wyatt, the staff and Advanced health care. This place is HIGHLY recommended. Charlie

Negative experience: After letting My father in Law fell twice at this facility . they failed to closely monitor his low blood pressure which was the main reason of his admission to this facility !! After he fell twice !… it was 8 hours for them to get a transfer to ICU bed ! This is NOT the professional care we expect from Advanced Care !!! No one at this facility will take ANY accountability ! Please Please Help me make this facility reported for their gross negligence!!!

Fantastic experience: This place is like staying at a 5 star resort. The facility is impeccably clean and the decor is conducive to a healing environment. Wyatt the administrator is loved and respected by the patients and staff. The nursing staff is prompt and receptive to the needs of the patients. The therapists are knowledgeable and assist with the needed safety precautions. Safety is the number one priority. The patient rooms are large and private. The dining room chandeliers and China farther add to the first class dining experience. The variety of the meals and the preparation of the food truly make the eating experience enjoyable. Advanced Healthcare of Glendale will be my number one recommendation for family and friends.

Negative experience: A family member was transferred to Advanced Health Care, and I am not impressed with how the staff is nowhere to be found, or how when you push the call button, and takes an hour for a staff member to respond to the call. Then the patient took a shower, and the towels were never cleaned up & or replaced and have been sitting in the bathroom on the floor for 24 hours. That is so unprofessional & unsanitary. Then found out that housekeeping services get started at @3:30am When patients are sleeping, they get woken up by the lights and get them talking to them. Then every 30 minutes another person or service came in to wake her up. Then to come to find that transportation team & or person, decided to change her pre scheduled appointments that she has already had set up for months, to push it out for 4 more weeks. This appointment is life threatening to her health. This place is not & never will be on my top 10000 list of places to attend & or recommend to anyone.

Fantastic experience: If you or a loved one needs some extra care from a nursing facility, this is your place! My mom just spent a week at this place and I can't tell you enough about the wonderful treatment she received from all staff! The people here are so caring and always treated my mom with dignity & respect. She felt 100% terrible and scared when she arrived and by the time she left a week later she was hugging staff goodbye. She came in needing help to go to the bathroom, showering, and moving around. The OT & PT (Doug & Dana especially) staff gave her hope and once she learned how to use the wheelchair and then the walker there literally was no stopping her. The nurses were equally helpful checking on her daily, administering care, and answering questions (Patricia & Andrew!) and a couple full time staff LPN'S in particular really stood out...Montarra, Jimmy, Kyle, and Reina (registry LPN). THANK YOU ALL for the care & concern you provided to my mom! My mom loved the food and said it was delicious. They have a full-time chef whom my mom also got to meet. The decor at this place is pristine...a very beautiful place inside and out. Mom's linens were changed regularly and her private bathroom and room were also kept clean. They have a gorgeous gym with up to date rehabilitation equipment and sidewalks and patios outside for more exercise or to just get done fresh air. The place is open 24/7 and none of our family had any problems visiting my mom or staying there as long as we wanted. The staff was also always very nice to our family. I have to admit that when it was happening, I was skeptical about my mom having to stay at a place like this, but now I am grateful we picked this place and will keep it in mind if any of us ever needs extra care outside of the home. Also, this place provides in home health care as the patient improves their condition & can go home. This place is located on 63rd Ave/Bell Rd. in the NW valley not from Arrowhead Mall. It's a very nice location and not fat from the 101, so it is easy to get to. They take Medicare & Tricare for sure. I'm sure they take other insurance but you'd have to call to find out for sure. Please know you or your loved one is in great hands at the facility! ❤️

Fantastic experience: I fell and broke several ribs and had other injuries and needed extra help. After the hospital, I went to this wonderful skilled nursing facility, Advanced Healthcare. All the rooms there are private rooms which helped me in healing quicker! The medical staff was top-tier as well as the Physical therapy department. They truly are so educated and so caring in what they do. They got me walking again, quickly. The nursing staff and the cna staff were very helpful and kind. There were a few blips of unkind staff which, if it were needed, I was able to share my experience with the Administration, who were very open to listening. The food was good. A few times I was served food that was on my "no" list, so that would be something to be aware of if taking an elderly family member here. If you need a Loving, gorgeous facility I recommend Advanced Healthcare!

Negative experience: Beautiful place. But their medical & staff is so horrible!! They do NOT answer patient calls, they’ve let my husband’s pain get out of control, they argue w you, didn’t get his regular meds on time either! I Discussed this w their supervisor & she promised me it would not happen again! WRONG!! After 2 & a half hours of my husband’s pain w/o any answers, I checked him out after only being there for 15 hrs!! Please, do NOT take your loved ones there!! This place just does NOT care!! Also, there food was terrible too! They don’t even give u menu! So u get what they feel like giving you!! They also took in a patient w Covid!! How safe is that?!?

Fantastic experience: This rehab company is a nice looking clean facility, therapists do their best to rehabilitate patients, they have a great team that all work well together. Overall this is not bad place if one day need it.

There is no competition when it comes to the amount of care, compassion, cleanliness, and attention that patients get when they go into one of these types of facilities. I have worked for dozens of nursing facilities as I travel every day to different nursing facilities. When you walk into advanced healthcare of Glendale the difference immediately stands out. At most other facilities, the moment you walk in, there are extremely unpleasant smells. NOT at advanced healthcare of Glendale. This is by far the cleanest facility I have ever seen. The nurse to patient ratio is half or less than half of most other facilities. This means that patients are getting the care and attention they deserve. Nurses and staff here have the time to go above and beyond to help patients with there needs, and they do! The meals they serve to patients here are mind-blowing. Patients have options of food to choose from and they really cater to satisfying the patients dietary needs. The staff at this company really care about the patients and you can see it. If I ever had to chose a nursing facility for my loved ones it would definitely be here at Advanced Health Care Glendale.

Fantastic experience: I have toured Advanced Health Care in Glendale. It is a very beautiful facility. The staff is very skilled and professional. I know several of the staff personally and know high much pride that they have working there and providing outstanding care for their patients. The management is very supportive of the staff providing five star care to their patients. If the need arises for me to need such a facility, I will be asking to rehab at AHC in Glendale.

Fantastic experience: This facility is one of the cleanest most best run facility I have ever walked in to. The staff is amazing, caring from the moment you walk in their doors. They really get to know their residence and families. It is such an amazing feeling when you can visit and leave without having to worry about your family member. The Director of Nursing is so knowledgeable with a heart of gold for each resident and their families no wonder her staff is as well educated, prepared and lovely as they are. You can rarely make it from the entrance to the residence room, or the dining room without the staff greeting you with a smile and by name. Very impressive. Would recommend this facility to anyone who would want too rate care for their loved one.

Negative experience: I have never know abuse like this place. My sister had back back surgery and came her to recover. She was punched and dropped on the bathroom floor causing her bracket on her back to shift. They lied about giving her pain meds causing her sever pain. There is too much that happened to even log the extent of it. If you love your loved ones, NEVER take them here, I beg you.

Fantastic experience: My mother was at Advanced Health Care last year after a fall and again this month due to a partial hip replacement. The staff is AMAZING! From the moment you walk in the door you feel welcome. The PT and OT staff are outstanding!

Negative experience: Infected my Mom with covid-19 then forced her to leave.

Negative experience: This facility gets all the stars for appearance. I kind of wish the grounds keepers were the care givers for my dad. They would have done a better job. He was admitted from Mayo to finish an anti-viral med and get strong enough to go home. Instead, he was neglected, and digressed. I can confidently state that had we been allowed in, his circumstances would have been far better. I respect the fact that, due to COVID, we can not enter the facility. They should take this into consideration, and up the staff and care of their patients. We were forced to visit through the window, and again, I understand that is out of everyone's control. He was in a South facing room, and the outside temperature was usually over 100. (August/September) The first day he arrived, I called to ask how he was adjusting and to see if he needed anything. The only reason dad's nurse returned my call (over 4 hours later) was to inform me (she was obligated to do so- her own words) that dad had fallen trying to get to the restroom unassisted. She assured me he was unharmed, and safely back to bed. She would make the same phone call a few days later- from her personal cell phone. I don't answer unknown, out of state numbers, and I feel it's rather unprofessional for staff at such a facility to call family from their personal cell phones. She left a message at 5:30 in the morning, . I then called the facility directly and it sounded to me as if someone in the background was helping her to figure out how to check her messages. Dad had stated that the suppers were "barely edible" So we took turns (my sister-in-law) and myself bringing him foods we knew he would enjoy. We did have to remind the attending staff to open the blinds so we could see that dad had gotten the food we brought, and we had to remind them to put it where he could get to it. They were pretty good about setting it out of his reach. And he was tucked into the bed so tight, he couldn't get up for a fire drill if he wanted to. I question why the blinds were closed once we got to the window and they came to help him move from the bed to the chair by the window. One male nurse did NOT close the blinds, and he seemed very kind and gentle. Dad even said this nurse was very pleasant and helpful. He didn't remember his name We were told daily that he needed clean clothes- so we dropped off a few sets of every thing he might need. I promise you - he did not get into clean clothes on a daily basis here. Dad's last day at the facility, the blinds were open, and we could see the food we had brought for him, but we couldn't SEE him, so we called the front desk to see if he was out of the room for PT/OT. Heather told us they stay in their rooms for that, and he should be in his room. His restroom door was open, and we could see that he was NOT IN HIS ROOM. We peered in the window and FINALLY he was moved by wheelchair closer to the window. He was in very bad shape. He couldn't lift his head to see us, and he couldn't talk on the phone. We left, really discouraged, thinking he must have been pretty tired. Low and behold - the facility tried calling- three times. I called back and finally spoke to Regina, who was noticeably upset, stating that my dad was unresponsive and she was calling an ambulance to get him to the closest hospital - he needed a brain scan (her words) Within a very short time, the ER concluded that he was dehydrated. He was then transferred to Mayo for treatment of pneumonia and COVID. This was Tuesday afternoon. I went back the following Friday to get his belongings, being certain he would NOT be going back to this facility. I kid you not - we received enough clothes for him to go on a month long cruise and never have to wash anything and THE GRILLED CHEESE AND TOMATO SOUP WE BROUGHT HIM ON TUESDAY. I would give a few more stars for the folks that answer the phone, and the one male nurse that was attentive and kind. But I can not get past the fact that DEHYDRATION - caused at the hands of my dads caregivers, was misdiagnosed as a stroke.

Negative experience: My 99 year old father was transferred here after hospital stay and having a fractured ankle. The end of April,2020. Because of the virus I was unable to go inside to see him. He was admitted on Friday And all weekend stayed in his bed, he was there for physical therapy. When I took him a bag of personal items they put it on a table he couldn’t reach. Kept calling to ask them to please give it to him Finally did that evening. Started therapy on Monday but by then he was deteriorating. Decided to bring him home with hospice, and when I got him home they had a filthy diaper on him with a terrible rash along with an extremely large bedsore. Not a good facility!

Negative experience: Not a very good place stay away!!!

Negative experience: Mom was a patient here for the fourth time due to weakness and frequent falls. Two days after release, my 95 year-old mother fell and broke her hip and femur. Back to facility again. However, things changed on this visit. Attitudes from the admission rep to the CNA were sarcastic and unfriendly. At one point, I went into the CEO's office and talked to him about their attitude and behavior. Not much change, but after a few weeks, my mother was told by the rehab person, that it was time to leave because of lack of progress required by Medicare. The day before discharge, I was handed a bill for $4224.00 because Mom's eligibility was cut short due to a previous visit. No one told us prior to this that Mom would have any financial responsibility. The person presenting the bill to me admitted she made a mistake in the software and didn't see it until that day. However, Mom has to pay it. Not right!

Fantastic experience: I will always remember how the wonderful people in Administration and all the dedicated Nurses and CNA's not to mention the cooks and cleaning staff who were all fantastic in every way. As I write this my husband is still a patient at Advanced Health Care of Glendale and its been a long road from the beginning at the hospital but I must say that since arriving at Advanced I have been able to sleep knowing that my husband is in such great care. This place should be considered the goal for all others to follow. I am so thankful and would like everyone to know how this experience has made me feel and that Advanced Health Care goes above and beyond and then some. Thank you so much to all of Advanced Health Care Team.

Fantastic experience: My mom was able to stay at Advanced Care of Glendale for 2 and 1/2 weeks after being released from the hospital. When she was discharged from AC, she was much stronger from PT/OT every day, happier because she was waited on, staff was always happy to see her, great food, clean and very comfortable. The staff was outstanding! Leaving there was like leaving family! Thank you Advanced Care of Glendale for the care of my mom.

Negative experience: Very gorgeous place... Staff was a little hard to track down for my dad. They sent him to the hospital after a high wbc. Transport that this facility uses had just left him by himself in the er with an envelope attached to him. He has a history of falling, short term memory loss, and stage 4 cancer. He was scared going to the hospital as is and my mom was on her way and THEY LEFT HIM BY HIMSELF. They were only going to hold his room for 24 HOURS. It's very sad they are more worried about filling beds. Do not take your family here.

Negative experience: This place was amazing at first until they need to send you to the hospital. My dad has been there for 3 weeks. They took his blood, realized there was an issue and sent him to the hospital via transport. While my mom was on her way, the transport left a man with short term memory problems, a history of falling and has stage 4 lung cancer alone in the ER with an envelope. While my mom is dealing with trying to get everything she needs to have him home, the care center decided that since he will be gone longer than 24 hours he would be losing his room there. This place is more concerned with a payment, which we have no problem paying than the well being of a human.

Negative experience: If you would have Asked me what I thought of this place back in 2014, I would have sworn by it. My mom broke her hip that time and she came to advance for rehab and they did a good job but she wasn't there long enough and had to be moved to Thunderbird due to insurance Billings. This time my mom was sent there after being sick, she had pneumonia and other stuff and became very weak and couldn't stand on her feet, we thought they can help her get her strength back. We noticed that it was not the same, the staff is always like a skeleton staff. The aids didn't really know what they're doing and a couple of times I have to stop them from picking her up from the bed because they were doing it wrong and were hurting her and when i tried to show them, they got an attitude. Putting all that aside it doesn't really matter, what matters is I came to visit my mom after not being there for 2 days due to the cold I had, I didn't want to pass it to her or anyone else, I'm found her slumped in her wheelchair. I didn't have to go inside her room I was in the doorway when I ran outside and I said she needs to go to the hospital, the nurse, Marianne, Was arguing with me. She's fine nothing was wrong with her she just tired and sleeping. I went back and looked at my mom closely, her hands were so swollen i looked at her face it was double the size, looked at her legs and feet her feet were like watermelons. I said no I need her to go to the hospital now she proceeded to argue with me and I got really upset so she went to call the doctor who said okay send her to the hospital. That night, I will never forget, we thought we lost my mom, the doctor said they don't know if they can do anything. We begged them to try and they gave her the necessary treatment she needed and put her in ICU. She was incoherent from Saturday until Monday night, and was in the ICU until Tuesday. They, at Advanced, had stopped giving her fluids for some reason which caused her kidneys to malfunction and were down to 12%. Now my mom has to take dialysis for the rest of her life on top of her other medical conditions. I'm assuming advance is good 4 people who where iniaccidents or recovering from surgery or something like that but not really sick people. Their staff are not trained to deal acute cases. My mom had physical therapy that morning, supposedly, and I say supposedly because she wasn't coherent that day so I don't know how did it and how they didn't notice her swelling up. This is something that doesn't happen in an hour or two. This happened on Feb 2nd. Today is Feb 12 and she's still in the hospital. Today she was taken back to rehab but not Advanced, she's going to a real acute rehab facility. I thank all the Drs and Nurses and staff members at Arrowhead Hospital for not giving up on her and giving her the best care ever. I was touched when the nurses and aids that took care of her came to her room to say goodbye to her and wish her the best. When we went back to Advanced to pick up her cloth and stuff, they had them packed. They put everything in bags and a box, mixing her nice clean cloth with her pee soaked cloth stuff and other dirty items. What a shame. The caring really showed. Sorry this is lengthy but I want you all to really pay attention when placing your loved ones in facilities to take care of them. Advanced is a beautiful place to look at but don't be fooled.

Fantastic experience: Was in for hip replacement. In March. Staff was warm and friendly. Food outstanding. Excellent care...physical therapy very good. Thank you all..for my care.

Fantastic experience: This rehab center is amazing! Not sure why others say different? My father was here for 2 weeks after his gall bladder surgery. He raved about the place. Great food. Caring people and they got him back on his feet in 2 weeks. I am now here with my father who is visiting his girlfriend who broke her pelvis a week ago. She says basically the same thing my father said - this place is awesome! Covered by Medicare too :)

Fantastic experience: My former Mother-In-Law is being cared for at the location of Glendale, Arizona and I wish I could give more stars! The entire establishment is completely over the top and the rooms are so comfy,cozy! The staff are friendly and so very helpful. I'm very impressed and can't say enough. My own Mother was in a "so-called" 5 star rehabilitation back home but couldn't even close to Advanced Healthcare.

Negative experience: Discharge planning nightmare! First wanted early discharge when my father was not ready. Then did not coordinate care. No meds given as he left and prescriptions not called in. No oxygen set up. This after Five days notice. Nursing was less then satisfactory as well, not noticing his legs were grotesquelly swollen until family brought the attention to it. Barely able to go through what medications he was on or tell me their purpose. One of the physical therapists was great and a few care staff but I would not recommend this facility.

Negative experience: My mom wanted to stay at advanced health care. The hospital gave a referral to advanced. Advanced health care got some miss information on my mom's condition. Would not take her. I called and asked Forrest Arnett head of admission to call hospital to confirm. Was told no . If they can't even take a minute to call to make sure the information is correct, then what type of place is this. Really , very disappointed would never recommend shame on you.

Negative experience: A beautiful facility and some of the personnel were excellent. However, we did not know that they contract out for the doctors & I am still fighting with a doctor's bill because she does not accept our insurance. Our insurance paid her but she is billing me for a balance that my insurance says I should not owe. More than once, our doctor's orders (who performed the surgery) were not followed. I had to stay on top of the PT & OT therapy when I found it being skipped. The machine that was to be used on my husband's knee was not used as ordered and I was given the excuse of not having someone there who could put it on or that it was not working properly - only to find that there was someone there to put it on and they got it to work. My peace of mind came after I brought my husband home and took him to outpatient therapy. The severe back pain my husband kept having was easily resolved by the physical therapist in the outpatient facility I took him to.

Negative experience: Stay away from this facility. Do not leave your loved one here. We did not like Dr. Patel. We had to beg to have a urinalysis after my mom finished a questionably short course of antibiotics for a urinary tract infection. Patel did an override on the prescription that the emergency room doctor prescribed. Had we not been pro-active, Advanced wouldn't have done a test after her antibiotic course to make sure the UTI was cleared up. Most of the nursing staff was rude, unhelpful and confrontational. My mom was discharged from Advanced with three urinary tract infections. She was rehospitilized the next day after leaving Advanced because of these serious infections. Months later, I find out that Patel ordered a completely unnecessary psych evaluation on my mom after I get a bill in the mail from a thrid party doctor. My mom doesn't remember this evaluation, giving consent for this evaluation, or anything about it. If you must use a skilled nursing facility, get a cot and stay in the room with your loved one at all times, because that is the only way you will ever fully know what is going on. You literally cannot leave their side. If the facility won't allow that, take your loved one home and do home health. These shenanigans will never happen on my watch ever again!

Fantastic experience: Great facility for my dad

Negative experience: A beautiful, clean facility. The room is quite comfortable. It may have been a rare mistake, but still one which should not have happened. It was only 3 days after major surgery, I was assisted to the bathroom then left while the attendant answered another patient's call. I won't go into detail but simply state that after a long wait I began to panic, cry, yell for help hysterically. The Emergency cord was flicked on and off 3 times. My legs and feet had gone numb, no feeling at all. I could not help myself, if I tried to stand alone I would have fallen. When help finally arrive and I was returned to a bedside chair I noted 35 minutes had passed. -- l immediately made arrangements to leave the facility;. I could not stay where I no longer felt comfortable, safe, or secure.
