Coliseum Nursing and Rehabilitation Center is located in Hampton, Virginia. Private and semi-private rooms are available, as are on-site rehabilitation facilities.

Dudley is the lead Administrator of Coliseum Nursing & Rehabilitation Center, a skilled and long-term community of Virginia Health Services. She is originally from West Virginia and moved to this area in 2009.  She has held various roles in the nursing field and pursued the Administrator in Training Program in 2013. Dudley graduated from WVU with a Bachelor of Arts in Chemistry, Bachelor of Science in Nursing. She also obtained a Master’s in Safety from Marshall University. Dudley enjoys spending time with her family, being outside and traveling.

Haley is Assistant Administrator for Coliseum Nursing and Rehabilitation Center. She assists in supervising the operation of the facility. Prior to this role, she was a Certified Therapeutic Recreation Specialist, providing person-centered and innovative programs for the older adult population. Haley graduated from Longwood University with a B.S. in Therapeutic Recreation.  In her spare time, Haley enjoys spending time with her husband and mini aussie exploring parks and walking trails. She also is an avid reader. Her favorite quote and words she lives by is, “Do what you love, and love what you do.” Haley is passionate about working with older adults and helping them live their best and most independent life. 

Yolanda oversees the Nursing Team at Coliseum Nursing & Rehabilitation Center. She is originally from New York and relocated to Virginia shortly after becoming a nurse. She joined the Virginia Health Service Team in 2015 as the DON. She attended a community college where she earned her ASN degree. Throughout her 25-year nursing career, she has served in many roles such as a Floor Nurse, Supervisor, Educator and Director of Nursing. Long-term care is her passion and she has worked with our elderly population for over 20 years. When she is not working, she enjoys spending quality time with her family.

Negative experience: I use to love this facility.of. everyday there is something to do with their patients.. my mother love the facility but now they are falling off.. short on nurses some of the nurses are rude and more...everytime I walk in that building I get mad ..

Negative experience: I am filing a Nursing Home Medical Neglect and Medical Malpractice Complaint against Coliseum Nursing & Rehabilitation Center for their mishandling of my father's care from 6/7/24 through 6/24/24. From day one the nursing home failed to meet my father's basic needs including proper medical care like medicine, supervision, food, water and his overall well-being. There have been several times we have called for help to get my father to change because he can't walk or stand at this time, it would take an hour or so before someone would come resulting in me or a family member having to change him ourselves. There have been so many times the nursing home didn't have the basics things in my father's room that he needed to be changed like diaper/brief's, wipe and cream. Many times, I have asked questions about my father's health and the nurse would get attitude or just didn't have the answer and would never get back to me about it I would then have to ask again. The lack of communication at the nursing home was unacceptable, no one seemed to know anything or be on the same page, and the nursing home is being paid by the Insurance company. Coliseum nursing home provided poor care and their breaches of duty have caused my father to stuffer. The nursing home failed to treat or prevent health problems properly. Coliseum nursing home failed to call a doctor when my father blood pressure drop down to a very dangerous low of 79/56 on 6/21/24 about 2:30am in the morning while I was were so l ask the nurse Emani to give him something to bring up his blood pressure and she told me that he couldn't get anything until 5am and at this time it was about 2:40am only a few hours away from 5am I ask why she said that when he is due for medication again. So, I told her to call the Charge Nurse, or the Nurse Practitioner and they will give her the ok to give it to him early. She lied and said she did but come the next day I call and spoke to the (NP) an ask did she get a call last night about my father blood pressure she told me NO. Coliseum nursing home is also unsanitary from the dirty floor, the smell and all rooms having an ant problem which was told to me by one of the aids that work there, when I ask if my father could be moved to another room because the room he has had ant crawling all over. My family and I had to go out and buy things to control the ant problem. The last and final thing that happened recently was on 6/24/24 when I walked in my father's room and found him sitting up in a wheelchair alone unresponsive, I called out to him he didn't respond. I left out the room and went to get the Nurse Practitioner to let her know what was going on and for her to come check him out. She took her own sweet time coming by time I got back down to his room he was drooling so bad, and mucus was coming from his nose at this point I told another nurse to call 911 because I wanted him to go to the hospital. When my father arrived at the hospital while still being checked out the doctor came to me and ask in the event your father heart stop should we resuscitate, and should he need to be put on a breathing machine should we do that because right now he is in critical condition I told him yes. As of 6/27/24 my father is now in the ICU unit at Riverside. Malnourished , dehydration and Infection is back because nursing home failed to stay on top of these things. The lack of care and treatment he receives at Coliseum nursing home has caused even more damage. The amount of anguish and stress this has cause on my family and l is outrageous. The inadequate staffing, inadequate policies and procedures, poor management, not well trained and lack of compression shown to patients explain why my father received the poor care he did. I am asking you please review my complaint and really investigate this nursing home not only because of my father's case but for all the other patients with no family to stand up and fight for their rights.

Negative experience: Do not send your loved ones here. Anytime there is a problem you get passed around and the buck gets passed. I am the MPOA for a family member and the doctor never speaks to me, changes meds and takes away vital meds without explanation. They also do bedside picc lines. I did not agree with that because it’s a tricky procedure that’s usually done in a special operating room in the hospital. My loved one ended up with a blood clot due to traumatic placement. If a patient is at the hospital when Medication call happens, they don’t get their meds when they get back. Most of the CNA and nurses have an attitude When you question their actions. He wasn’t getting enough insulin Even though I told them how much he was supposed to be on. He had insanely high blood sugar numbers. And Come to find out their diabetic tray contains a dessert with sugar in it usually!!! And then they wondered why he Was combative! This is somebody who has stroke Damage so he can get combative. He has called me probably 10 to 15 That he was sitting in Fecal matter and that he couldn’t get cleaned up. The nurses would accuse him of doing it on purpose. Has a short amount of time there left thank goodness. If you do, send your loved One to this facility, make sure you’re there Several times a week so that you can see for yourself what’s happening. It makes me sad to think there are people there without family members to advocate for them and I can’t imagine what their care is like. When I questioned why his blood sugar numbers were so high and why they were giving him a sugary dessert on a Diabetic Trey, they tried to blame me For bringing in snacks. All the snacks were sugar free Including the zero sugar soda, they tried to say Was causing his sugars to go up. They literally tried to blame us. Which is ridiculous. They also somehow convinced him Infection was coming out in his poop. A blood infection does not come out in your poop. Misinformation! This will be reported to the Virginia Department of health.

Fantastic experience: My father entered this facility for Skills Care. According to my father, everyone (reception (CeCe/Daisy -- always helpful and cheerful), nurses, PT, OT, Social Professional, cleaning people) were great. His only issue was the food; it was always cold. Considering how many rooms the staff has to serve and being at the end of the hall, well, maybe if you rotate the serving order, may allow those at the end of the serving line to get warm food at times. Overall: I would recommend Coliseum Nursing & Rehabilitation Center to anyone with special needs.

Negative experience: I walked into my aunt's room and found her room disgusting. He night stand was infested with ants. The floor and under the bed were filthy. Personal items under the bed was caked with dirt and dust. From what we could see, her room hadn't been cleaned in a long time. It was also buildup dirt on the floors. I ended up taking a large beg of trash to the nurses' station. I was in total disbelief to see such conditions.

Positive experience: I have mixed feelings about this place. The staff at coliseum Rehab are friendly. I recently had my mother discharged, she was a patient from Friday to the following Saturday (8/26/2023). However, my issues are 1) My mother never received a bath/shower. When I asked on Wednesday did she get one. The response was she had to be put on the schedule and patients get bath/shower 2x's a week. She was admitted on Friday evening I'm asking on a Wednesday night. Next, issue the communication. Again, my mother was admitted on a Friday evening I didn't learned of the day activities until Tuesday evening at 5pm from overhearing a conversation. When I asked about my mother going. The social worker put her on the schedule for her to attend the day activities. I had to leave a note for the social worker to call me. My mother has alzheimer's and dementia. I am trying to understand why wouldn't the social worker follow up with the patients Power of Attorney (POA) regarding any communication . My next issue when asked who was her treatment team? The doctor/NPcouldn't give me an answer. She looked confused. Simple question who was my mother's treatment . Who's tracking her progress? What goals would be in place to decide when she would be ready for discharge? How long is her stay? How many times a day? No one could give me an answer. Not one person at Coliseum Nursing/Rehab... now, my last issue. I learned the facility having several Covid cases on the long term side. Now, when I asked why wasn't we informed of the facility having covid cases, I received a dumb look. Then I was told the cases was on the long term side . My question was those patients had exposed others around them before being isolated and what about the nurses on that work on the long term side? I was told the nurses that work on the long term side do not work the skill unit (Rehab). That's incorrect, one the nurses who work the long term care unit , worked on the skill unit. Because, I asked that nurse because I remembered the voice from me calling the facility late nights coming to visit my mother.who let me in at the front door. So, if you are considering this place for a loved one or have a loved one at this facility I strongly advised you to stay active have a strong knowledge of how things operate. Because, these facility don't expect you to be knowledgeable, and activities so they will feed you b.s. don't visit the same time every day. Switch your visiting schedule. They made me remove my camera from my mother's room. Because it is a violation of privacy. Hell, how when you are entering a patients room? So, it's a violation of visitors and employees being recorded and watched on cameras when entering the building and walking the halls? But family members cannot put cameras in patients private rooms?

Fantastic experience: The rehab experience was excellent they are very helpful and show compassion for the pts. They make you feel very important. I highly recommend if u need rehab to seriously consider coliseum. Nursing has been good as well, overall a great experience.

Positive experience: My dad has been in this faciilty for 4 months and is treated very well, emotionally and physically. His room is clean and he enjoys eating in the dining room each day, socializing with the ladies and the food is good. When I report a problem, the staff take it seriously and work around his personal preferences. His medical needs are addressed quickly and the physicians are available and will talk to you. I can visit any time I want, talk to physical therapy, nurses, doctors, aides - and my dad's social worker practically lives there, day and night. The staff works with you, not against you, and he is treated as an individual with respect for his emotional needs. My dad enjoys the staff, activities (singing, cookouts, trips, etc.) and has made a good adjustment after a few rough months of not wanting to be there. This nursing home came was bought by Virginia Health Services (VHS) a few months ago (March 2013) and I have been happy with the changeover, physical updates in the building, training for the staff and my communications with the administrators and billing department.

Negative experience: This whole place is nasty needs to be rebuild . They just keep patching up holes and other things that need replacing. Water bugs everywhere. Floors in hallway are halfway kept clean. all behind residents bed . And dressers. Never get sweeping.housekeeping uses the same water in their bucket on every room they have mice. Traps and I have seen them the employees talk to the resident's like dogs.and talk about them to other staff members

Fantastic experience: Great time during physical therapy. Great staff ( Sam, Bridgett and Lyndsey). Enjoyed spending time with them. They are very knowledgeable, kind, and encouraging. They made stay pleasant, and I looked forward to working with them. Great job Physical Therapy.

Fantastic experience: My experience with the Colesium Rehab was a great experience. One thing about therapy was that they never let you give up, while being encouraging and not pushy.

Fantastic experience: Coliseum is a very friendly place and family oriented. This facility makes me feel safe and secure! The staff is very nice and friendly, they are like extended family to me now. They try to help every way possible, they listen to the residents concerns and alleviate those concerns. If you need help the nurses respond to the call bell timely. The office staff has been very helpful with my needs. I really like the beautician they bring in and will be continuing to visit her every week! The meals are good and medication is given with care. They make sure to tell you what medications they are giving you, I feel better knowing what I am taking.The room I am in is very clean with good lighting and comfy furniture! The facility does my laundry for me which helps out a lot at no cost to me. They help with supplies when possible. I definitely recommend Coliseum to anyone in need of rehab!

Negative experience: EDIT: I will say I am impressed with the response I received from the corporate management regarding this issue. The prompt response was greatly appreciated. My MIL was sent to this facility for physical rehab to increase her strength after a particularly nasty bout of pneumonia. She was moderately ambulatory, but still required medical supervision. Aside from the facility being unbearably hot, even in winter, she did OK for the first week. However, one night around midnight, she woke feeling very poorly. She called for medical assistance multiple times, but no one responded to her until 8am. At that point, she was so ill that she was rushed back to the hospital, where it turns out she has both Covid and double pneumonia. They let my sweet mother in law lie there, drowning in her own fluid filled lungs, for 8 hours. It would seem like common sense to me that if you have a patient with known lung and blood clot issues, you might take their inability to breathe as a serious medical emergency, and take action to provide appropriate medical care. TLDR: don’t send your loved one here. The care is non-existent. I will be filing a complaint with the state licensing board.

Positive experience: My mom was there for about a week; lucky we her children & other family members stay close by that been said she had no issues or concerns. She definitely wants to thank Steven & Bridget for all they have done for her. Her transition to home was smooth & she doing so well; you can’t keep a good hard working hard headed woman down.

Negative experience: Please be so careful With this place. I had my mom here for 6 days and it was absolutely horrible . She is bed bound with terminal cancer, mind you her body doesn’t work but her mind still does. Night one she was placed with a known violent dementia patient who came after her immediately after we got there. They knew this woman was violent and had assaulted other staff before. I had to tell them they needed to move my mom. They had my mom on meds and weren’t honest about what she was on. Valium was suppose to be when she asked for it. They where sneaking it and giving twice a day although she didn’t even know she was on it. One of the nurses a male older gentleman, got a serious attitude when my mom asked him what the 9 pills where he was giving her. My mom also smelled like urine the entire time she was there. There is also zero air conditioning in most of the patients rooms. My mom was in an 80 degree room her entire stay. She came home dehydrated and had gone down quick. She has a fractured pelvis, fractured spine and neck and was left sitting up right alone in a wheel chair for over two hours alone. She had no call button, or cell phone and was left in a very dangerous position. I promise you if you send your loved ones here they will die quicker then they would else where. Also the “nurses” walk around cursing, speaking on patients health loudly in the halls. I heard several of the residents medical history as well as being called a “fucking nightmare”. There are so many issues with this place it actually scares me. I feel so so bad for the residents here especially the ones that dine have family members to advocate for them. I have also reported them to the va state board of nursing. Please do not bring your loved ones here

Fantastic experience: My mother has been a resident at Coliseum Park for a year now. It was a very difficult transition for her, and she was uncooperative and difficult to handle in the beginning. She has Alzheimer’s and also needs 24/7 care physically. My sisters and I have been beyond pleased with her care in all aspects … we feel the staff is attentive to her, we are kept informed of her health needs by the nursing staff, we get answers to any questions we may have in a timely manner and she loves the food. I do want to specifically thank Sarah, Shawna, Haley and Kat for their attention to our mom and all of the residents. Their creativity in activity planning and personal encouragement to each resident makes all the difference in the world! Our mom has adjusted so nicely to being a Coliseum Park resident and looks forward to the many special activities. Most of all, she is happy, which makes us happy. Is her life perfect, no, but Coliseum Park is the perfect place for her! Thank you all for all you do!!

Negative experience: This is the worst nursing home facility. The people here are very unprofessional. I wouldn’t bring a dog to this place. The positive reviews that you see are probably fake. My father was a patient here to get physical therapy for his foot. His condition got worse and he was in a lot of pain he couldn’t walk on his foot . They wouldn’t even call an ambulance to take him to the hospital. We had to call 911 in a facility with 24 hr care. My father ended up passing away a couple of weeks later.

Fantastic experience: My mother has been a resident since January 2021. My sisters and I could not be more thrilled with the care that she has received. She suffers with Alzheimer’s and the staff has been so attentive to her needs. It always smells clean, my mother smells clean, and the staff is always so friendly. We asked them when she was admitted to keep us informed. They have gone above & beyond to help her and keep us informed. She loves the daily activities & staff. She is the happiest she has been in months. We are still extremely pleased with our decision to admit her there. Thank you.

Fantastic experience: My father was recently a resident here for Rehabilitation after a massive heart attack. I was very impressed with the thoroughness of his Therapist and the consistency they maintained for his progress. Staff was very friendly and helpful to my father and our family. They accomodated us well, his room was always clean with linen, towels and washcloths. He really liked the food he was given daily and they made sure they maintained his diabetes. The nurses were very professional and handled his medication well! He made it to and from his appointments with no issues. He had a social worker in the facility and she was very helpful with his discharge process and follow up with his primary care physician. Overall I truely appreciate the care my father was given, he is in great condition now thanks to the Therapy and continuous care he recieved by staff at Coliseum Convalescent. Thank you for everything!

Negative experience: It’s hard to give this place a good rating, there are select few who work here that actually care for the residents then there are others that cut corners and don’t care. I feel as though the residents arent getting proper care or enough care. Every now in then you will see roaches and from the outside of the Building you can see that the bugs are explained by the holes in the ac units. The privacy curtains are disgusting.

Negative experience: Some nurse aides are great, but don't trust the administration. People like Lisbeth Dudley Hass only care about protecting themselves from lawsuits, not your loved ones. They will lie to your face, change their story about events that happened, and then purposefully overcharge on your bill on top of everything. Never accept a bill from them without reading over carefully. They make 'mistakes' all the time. Staff that does the most work is underpaid and underappreciated. Administration and higher ups exploit them.

Negative experience: I'm not going to beat around the bush! My dad came to this God forsaken place just yesterday around 12:30 p.m. Needless to say, he's back home not even 24 hrs later!!! Before my sister and I left last night, which was around 8 p.m., our dad was about to get cleaned up for the night by CNA Alexander. I'm not sure what was done or wasn't done after we left! His shift was over at 10 p.m. My sister came to visit this morning and our dad had been sitting in a urine soiled diaper that was too big! His bed was soaked too! He had been laying in that diaper for at least 12 hrs!!!! He was cleaned up only after my sister said something! Some people that are in certain professions clearly don't need to be! If you don't want to do your job thoroughly then quit or do something else! No human being should have to sit in their own urine!! My dad and others that are still in that facility need to be treated with much better care than what they are receiving! So sad! Would you want your mom, dad, grandparents, etc., treated this way! I say HELL NO!!! Please don't place your loved ones in this place. I wouldn't even bring my dog here!!

Negative experience: I am only giving one star because I can't give anything less. One of the worst facilities I have ever encounter. Incompetent , unprofessional staff.

Negative experience: Alerted Nurse that my mother needed to be changed at approximately 5:30 pm....30 minutes passed by and no changing occurred.... between 6 and 6:I5, I asked again and he kind of acted like I was bothering him.... He finally got a CNA to do it...my caregiver said that she hadn't been changed since 2:45 pm that afternoon.... She and I changed my mom.... I washed my mom with soap and a wash cloth. When she first came into the room... she began looking for some wipes.... after 15 minutes she came back with just a handful.....I don't think it was her fault.... she was only using what she had. She was very nice. She noticed my mom had on the wrong sized diaper....it was too large....and she made sure she put on the correct size. Instead of a pad to place under patients to catch fluids and to protect the linen... a sheet was placed underneath my mom.... It was like this when my mom was admitted as well. I asked if they used bed pads.... she said they usually do..but they were out. The trash can within the room has no bag in it and it contains a saturated diaper as well. The floor looks unswept....and as if it hasn't been mopped..... remnants of the discarded diaper are on the floor. Dinner consisted of a chicken salad sandwich, green bean salad, potato salad, peaches and tea with no ice. Is this really a balanced meal? It did not look appetizing. I asked if the patients were given options and she said it depends on what they have.. like perhaps a grilled cheese instead. Old and rusty portable dinner tables with wheels are not appealing to the eye. The bedside tables and cabinetry could use updating.... carpet in the entryway could use a good cleaning......1 sink per 2 people... seems not to be cleaned regularly.

Negative experience: Under no circumstances should you trust this facility with your loved ones. My grandmother went in talking,walking and continent for REHAB. This facility literally watched my grandmother reach the brink of death over several days and when she become unresponsive then decided to transfer her to the ER where she died two hours later. She was severely dehydrated and malnourished In a facility with 24 hour staff. Reported to the state!

Negative experience: My dad was brought here. Place smelled of urine. Under the bed on the bed bars was dried streaks of urine, from previous tenant. Called nurse, she hung up on him then came 45 minutes later to finally ask what he needed. And acted like she couldn't be bothered by any problems. Gave every excuse as to why she lacked in attention to patients needs. Mom promptly left with dad and brought him home. She could not leave him there to die. He will live coming home.
