Negative experience: My 89-year old father was at Hayward Healthcare & Wellness Center to rehab his knee after a fall. I do not recommend this facility.

Negative experience: Calling from Sacramento asking a simple question about my Mom instead of driving up to Hayward, I'm at work, and the asian guy who answered the phone and the lady was so rude. They Hung up and I called few times no answer unprofessional

Fantastic experience: My mom was a patient at Hayward Health after knee surgery, she got the best care!! She appreciates each and everyone who assisted her in some way or another. I would like to name a few people who made a huge difference in her recovery….. Irma, Angela, Sol, Kelly, Kara, Tenise, Ranjini and the most outstanding who always made my mom laugh is Darren. Laleen the administrator was very approachable and was always very supportive.

Fantastic experience: So after a little adventure related to UTI in elderly women, my mom was brought here for her ease with recovery for the last one week. The entire staff of caregivers worked with me and my mom to hear our needs and to see what her best path to recovery may be. The facility is extremely friendly, clean (Kudos to the cleaning staff that took out garbage can multiple times a day) and there are always helping hands around from reception to hands on patient care. Importantly, the directors of all branches and administration are always open to conversations, respectful of individual needs and diligent about true concerns. Their food is always served on time and is plenty! My mom being a little princess, they kindly allowed her outside food as well. Location is close to Winton exit on 880 and there is plenty of convenient parking at and around the facility and very accommodating visitor times from 8am to 8pm. I had my daughter and my aunt accompany us along with additional helping friends. At no single point, including over the weekends, did we feel unwelcome. I see a very active PT room as well as interaction room, well staffed and oh! the ladies and gentlemen are knowledgeable, friendly, and trustworthy. The OT team were on top with the assessment and help. Nursing team and CNAs support is 24/7 high quality. My mom was transferred to this facility with some undue alterations to medications at the hospital. I cannot thank the nursing team enough, for communicating and working with my mom's medication prior to hospitalization. I am very thankful that my mom was brought here for the care she needed. This is a multi-cultural place of "care and open communication" toward patients meeting their realistic health goals, which in my mind makes this facility stand out! I myself am in the healthcare sector as a scientist in drug discovery for over 25 years. I am well aware of clinical protocols and care standards. I wish to express my positive experience here. Shout out to Ms. T, Kara, Laura, Angella, Irma, Rosa, Ratika, Herna and entire team at the facility!

Fantastic experience: Very friendly and accommodating staff. The facility is so clean . Food is ok. The care like OT,PT were so helpful and nice . Good place for rehab. Thank You Nona,( Kaiser Nurse Coordinator you made things easier , you’re the best and to Rochelle (Social Services) and to all the staff thank you

Negative experience: Take your family and run is all I have to say I give 5 stars only to the pt team period that place is a mickey mouse operation Don't know how to treat travel nurses bye bye I worked harder then your own staff some many incompetent lvns so sad only one is with it period get y'all facility together

Positive experience: My mom was in this rehabilitation facility for two weeks. The staff catered to my mom's needs. To be honest it wasn't a happy place to be in, however it was needed for her recovery and they did just that. The nurses and therapist did exactly what they needed to do, to get her out of there and home in a timely matter. Noemi one of the upper staff administrators and Nona the coordinator/ case manger were superb. Both ladies were very helpful, informal and super nice with all ofour questions and needs! To all the other staff working there and with my Mom thank you!

Negative experience: I don't think these people really do there job at all

Fantastic experience: My husband was there for only 4 days. The first day I had a few small reservations like room size, etc. but the next day things began improving. There was a doctor who visited the facility daily to check on patients and make the appropriate recommendations/decisions. Appreciated the facility had a clean smell. Not like disinfectants but no smell at all. The staff was accommodating and professional. As in regards to our loved one, of course you want everything done for them asap, however that’s unrealistic. Whenever when visiting, if my husband wanted something done and I requested it, they were fairly prompt addressing the request.

Fantastic experience: Sharing our loved ones experience at Hayward Healthcare. Our grandma was put in hospice and it was tough for all of us.the support my granny has gotten including ourselves was great. Only comment is the building is old. Common restrooms are old but aside from that staff were friendly and attentive.

Fantastic experience: My dad was here for rehab after he had a stroke and was needing rehab. We had minor concerns in the beginning but management was involved right away to rectify it. He received good care and therapy.

Negative experience: This place is horrible! My mom had knee surgery and this place was recommended for her by her doctor for recovery after surgery. The nurses or staff whatever those people called because I’m pretty sure they’re not nurse’s! They didn’t know why my mom was there for we kept telling them to follow what the doctor recommended to do for her like apply ice , pain pills, therapy etc,etc they kept asking is what was the reason my mom was there for. The nurse didn’t know what kind of medicine my mom was taking so she took the whole packs of pools to ask my mom if she remembered what she would take. Also the lady next to my mom her roommate fell and they left her on the floor for like 20 min my mom kept pressing the button to ask for help but no one would come in so my mom tried her best to help the lady and got off her bed and went into the hallway amd screamed for help! My mom could not believe this and FaceTimed is and we saw everything! The lady would scream for help and for her to be cleaned amd they wouldn’t no anything . Thankfully my sister was by mom most of the time and would help my mom . We decided that place was a nightmare and decided to bring my mom home. Please someone needs to do something about this place! I will make reports and show proof of all the things that are happening in this place! We can’t let these people treat these poor elderly, sick people like trash! It’s disgusting!

Negative experience: My grandmother told me She was in pain that not once the nurse emerson walked in her room to give her medication , she wasn’t given the correct medication since she got to the place that my grandma had to take her own, she was in pain and no one walked in her room when she wa asking Sunday from 4am to around 10am until no medicine until of her daughters showed up and asked for pain medication since the nurse never came in the room, eventually someone someone gave her a medicine and my grandma didn’t know what it was and she asked what type of medicine it was and they didn’t want to tell her what it was. when my aunt spoke to the nurse Emerson he tried to tell her to be quiet so he can explain that there was a camera to prove he did walk in the room 4 to 6 times to ask my grandma how she was feeling and to give her medication that night and my grandma said she had never met or talked to since him until she decided to leave the facility. Grace told my aunt that the pain medication norcos was only given unless the patient asks for it And when she was in pain no one was around and her red light was on for a while. Just to prove that no one was around when patients needed help My mom witnessed an accident that made her get up with pain from the bed to look for help because no one was around. My grandmother’s roommate who also had surgery done fell from her bed and was laying on the ground screaming for help for roughly 10-15 min before someone actually came my grandmother recorded the who ordeal which is when a nurse caught her recording and told her there was no need to record but the other patient was being awfully neglected. Not once was her family notified or anything

Negative experience: Very sad place, not sure how much you have to pay to get the help you need here. A lot of screaming for help coming out here.

Fantastic experience: My mom is having a very good experience here. Everyone we met here has been very nice!

Positive experience: It's a beautiful family oriented facility very clean. Staff is very caring. No waiting time when I needed help. Food I don't care for at all though, they really sincerely need to improve food as soon as possible, other than that everything else is great.

Fantastic experience: Staff here is wonderful they beyond to do a great job I appreciate the Administrator for excepting my husband to this facility Real family that works together I truly thank them for all their hard work

Fantastic experience: I am impressed with the facility being so clean. They have a five-star that takes care of my aunt. She is very happy with her care. The only thing she don't like the food.

Negative experience: Would give 0 stars if possible. Do not bring your loved ones to this facility. Wellness is only in the name, they could not treat someone worse or with more apathy. This place should be closed down.

Negative experience: One of the worst rehabilitation center. I am going to file official complaint about this center to county , State and Kaiser Permanente.

Negative experience: Not a good place, my mother got very sick here and had to be rushed back to the hospital and placed on a ventilator. we trusted she would get cared for here . Wish I would have read the reviews before we sent her to this place. Over crowded and not enough workers. Please think twice before sending a family member to this place.

Fantastic experience: I’m patient as myself to be in this nursing facility. After I got discharged from hospital, I have options either go home, or continue care and learn some physical therapy after my hips and hand operation. Every staff is really professional for what they’re practicing, including RN, CNA, case manager, therapist. I can say it’s has no difference compared with hospital. Also, don’t judge books from its covers. Even this place might seem out of date or old compared with modern hospital. I still satisfied the cares that I want including physically and mentally.

Negative experience: Not a good place. Seems overbooked and understaffed. Had a family member pass away there and another caught pneumonia there and had to be rushed back to an ICU. Take from these experiences what you like. Edit: The family member who caught pneumonia there also caught COVID-19 there as well. Also that same person is now in Hospice with sepsis and stage 4 bedsores. And the Hospice room had trash on the floor and was totally barren.

Positive experience: My mother came here after a hip replacement surgery. She stayed this facility for about three weeks. The staff at this place are 5 stars professional. Nice, responsive and they care about the patients. The reason why I deduct a star because the building and equipment seems old. However, the enviroment would considered clean tho....

Fantastic experience: In a time of need

Negative experience: My mom stayed here very short term. When she first arrived in the evening she came straight from Kaiser after a 9 day stay with only sponge baths. (Due to Covid we were unaware since we could not visit. )We visited her the next day and told the nurse that she needed a shower and explained why. She stated that it would be taken care of. It wasn't. Shower days are Tuesday and Thursdays? When Tuesday rolled around she still didn't have one. When we questioned we were asked if she refused? She did not. After complaining she was given one the next day. Also, she and her roommate would push the assistance button and it took forever for anyone to arrive. I understand they might be with another patient but what if its an emergency? Due to Covid we have to visit at her patio door. The first day they had her sitting on the edge of her bed. Why wasn't she put into a chair? or even sitting in in an upright position in the bed? She slightly moved and was coming off the edge. She couldn't reach her button to call so her roommate pushed hers. Took forever for someone to come. Every visit is different. First time we had to talk on our phones since the glass door was closed. Second visit we were able to talk to her through the screen. 3rd visit they placed a plastic covering over her screen from the inside then put up a small constructed gate with wheels and plastic in front of that. 4th visit we are back to talking through the screen. We were never given visitation hours. We usually visited her after work in the evenings. Then out of nowhere...We called ahead to say we were driving over so they could open her curtains for a visit when we were told that visiting hours changed that day and now ended at 4pm. WTH?

Fantastic experience: My friend was here for a few months. The DON and all other staff worked hard to meet all her needs from head to toe and did a great job. Thank you. I love this place.

Negative experience: I am calling and calling the main phone number and no one is answering the phone !!!

Positive experience: De todos os que eu já visitei em hayward, esse aparentemente era o mais arrumado, menos cheio de fezes, os enfermeiros eram educados e calmos. Aparentemente os quartos são limpos, o chão e os corredores também são limpos.
