Fantastic experience: My grandmother stayed here after her hospitalization and we had a great experience here. The nurses were very helpful and caring. My grandmother felt like they genuinely wanted her to get better. Along with this, the admissions leader, Antisha made the process so smooth and we had a pleasant experience working with her and her team. :)

Fantastic experience: well I can only speak for myself what I experienced! Cleanliness was fine, the food was pretty good and most of the staff was pleasant. I must add there was 3 carepeople that were more than great. Firsr was DianaRose, then Angelica and Erica. They were absolutely Stellar in their concerns, Johnny on the spot when the call button was pushed and extremely careful and gentle when changing my"diaper", My condition was my broken and dislocated hip which if not treated with care was extremely painful. The others didn't seem to understand how painful my hip was, Remember, they deserve a raise or award of some kind! Don't be afraid to come here.

Negative experience: This place is a toilet. If you have private insurance, do not come here. If you are workers compensation being paid by the State, you are hosed. Places like this live off State money. 40 minutes to respond to red button from bed. CNA shows up and says, "I am on break get to you later". Supervisor dose not bother to say hello or give any paperwork. My dinner was a Tuna sandwich from the day before and kool aid. The breakfast was 1 pancake, and something that resembles very watery gruel. See attached. Needless to say, still have not eaten since i have been here.The immediate staff is hard working and polite. The management is intimidating (Amy)and does not want you to think for yourself. Thier only interest is the long game from the State not what is on your best interests. Self reliance is frowned upon here.

Fantastic experience: I came here for tehabaltion theaphy and my theaphy was very challege Everyday but the staffs and CNA nurses and rehabaltion theaphy team was very helpful and encouragement me push hard eventhough I did'nt complete my goal to go home and be moving to another facility . Thank you all the staffs , nurses, and rehab therapist team for my stay as a patient. Event Drake

Fantastic experience: When I entered the building, I was greeted by friendly staff members. The facility looks clean and well-maintained, and I could tell that great care was taken to ensure the comfort and safety of the residents. Highly recommend this Skilled Nursing Facility if you or your loved ones need good nursing and rehab services.

Positive experience: My mom was at EBPA for a month and during that time the care she record from Antonio and most of the CNA staff was great. Her case manager, Anjani went above and beyond to help my mom and myself navigate through this process and transition my mom back home.

Fantastic experience: I came back to work here when it became the EBPA in 2016. It has become a facility that inspires staff to provide the best care to the residents. The staff here are professional, happy and respectful. I just love working here!!

Positive experience: I’m staying here currently, the nursing staff here are fantastic. Some CNA’s can use a refresher course in school on how to treat patients and take care of them. But other than that it has been very pleasant here. There were some issues with my insulin, but was resolved. They are now helping me find a place to live. So my stay now now has been very pleasant.

Negative experience: My friend's mother stayed there and it was a terable experience she left there malnururished stoled her jewelry they are bad people she died 3 weeks later.

Negative experience: Rarely does this facility answers the phone when we call to speak w my aunt. A receptionist would be advise to pick up the phone at ALL times. I previously made a call twice different occasions, nurse took my info w reassurance my aunt would call me. Never received a call back.

Fantastic experience: I work for an assisted living community and needed to come to do an assessment for a new resident moving in. I was able to make an an appointment EASILY by speaking with Andrea, the facility discharge planner. I deal with discharge planners frequently and have to say, she is by far one of the most professional I've ever encountered! Next, I needed to have additional medical documents filled out and signed. Jerica was who I needed to depend on to get these docs back to me quickly. I have never received documents back so quickly in the 8 plus years I've been in the industry! She's amazing. As for our new resident...the care he received was nothing less than amazing. Knowing the condition he had been admitted in and to see how much his health improved tells me that the entire staff truly cares for their patient's well-being. Their Administrator Erica is doing a Phenomenal job with her staff!!! Great leadership.

Negative experience: The food is so terrible and no fruit or greens are ever served.The rooms smell like urine, and they're supposed to be cleaned when required, but the patients have to keep harassing the stuff to clean them. It’s disgusting on so many levels. Do not put your loved ones here!

Negative experience: Worst place ever don’t put your love ones in there cna absolutely not good

Fantastic experience: I'm a threepeat patient ( two hips and shoulder replacement) and soon will go for a fourth and final knee replacement. They provided competent, professional and compassionate care. Keep up the great work EBPA.

Negative experience: Horrible place to go, online the rooms and the place look very nice but in person it is the worst thing ever. They treat you horrible here and I do not recommend. We had to wait hours just to get someone to come and check my family member out. That is ridiculous! This doesn’t deserve any of those 5 stars. DO NOT RECOMMEND!

Fantastic experience: I visit a family friend of mine there often and the staff are very friendly and professional. The facility is clean and seems to be very well managed. When i asked about the food, my friend mentioned that they love the variety and feel like they are at a five star resort. Overall i would highly recommend the facility to any individual who is looking for a Post-Acute Healthcare Center.

Fantastic experience: My Mom has been here for close to 8 months now and my sister and I are very impressed about the care-giving our mother is receiving. We recently met the new administrator, Shane, and while he is young, it's very apparent that the way he conducts things shows how serious he takes the position. He works hard to make sure all the patients here are satisfied and happy. We love how hard the nursing team works to make sure they communicate with us and that our mother receives the best care. Everything here has dramatically improved and I hope people come and give this place a chance and take a look at what they do here!

Fantastic experience: 1st off Great Staff!! 2nd. Very loving and patient. 3rd. more smiles than at school play ground 4th. Clean floors 5th Great parking across the street where I stop and pick up a Butterfinger every Wednesday and Saturday. I have been to 6 skilled nursing and rehab and I prefer EBPA more so than all of the others. Thank you! Lisa

Negative experience: terrible the food staff didnt get menue right not once i ate more oeanut butter and jelly and grilled cheese in the 10 days i was there than i care ti speak off.the nurses were great. the staff phoney and unprofestional rehab, good caring.but to rushed, sent my cloths to be washed labeled mind you, never returned. my items in safe never returned. untrustworthy
