Salem Village Nursing and Rehabilitation is a privately owned and operated comprehensive skilled nursing facility offering a wide range of services, such as physical therapy, short-term skilled nursing, and long-term care. We provide 24-hour medical care and support to our Residents.
At Salem Village Care Center, we are committed to providing our Residents with the resources and support they need to achieve their highest level of independence. We have a welcoming and inviting atmosphere, and our Residents are encouraged to make their rooms feel like home by adding their own decorations. Family, friends, and even pets are welcome to visit at any time.

Negative experience: This place has poor management
Negative experience: My son resided at this facility for five years before he passed away earlier this year. To be perfectly fair there are some skilled and caring staff members. However, there are many more who are not skilled and are definitely not caring! The facility is always dirty and poorly maintained. I definitely would not recommend!
Fantastic experience: I am a RN in the health care field.I want to say this place has great phone etiquette & professionalism. When I call this facility I can reach someone right away.Get transfered to other departments & reach someone right away.I have spoken to a day shift RN named Eric on the 4th floor & Amiqia RN who gave me lots me lots info I needed. Neither of these nurses hesitated to answer my questions. That's great to have accessibility to your loved ones care givers so easily. Bravo Salem employees
Fantastic experience: I have nothing but good things to say about the people who work at Salem Villages and the director of Memory Care. She has been very kind and patient with our family since we've learned to deal with our mother's dementia. During the Covid restrictions, it has been a relief to know that she is in good hands. Thanks, you guys!

Negative experience: First day at Salem village unable to walk Got into the room and waited 3 hours to get a cup of water Only after asking for one....when asked they said oh do you need water...
Negative experience: My friends have deteriorated after the state took over. Go somewhere else.
Negative experience: Treated my grandmother horribly. Worst experience ever!
Negative experience: They make it REALLY difficult to come and help care for your family members. My grandmother is becoming MORE depressed by the day! Need to make 24 hours in advance to visit, and last reservation taken is for 4pm. No children allowed. So in order to visit I need to take a day off work AND get a babysitter... just to visit and say hello! Find a better facitily for your loved ones... one that cares!!!
Fantastic experience: Every facility has its ups and downs and fair share of problems, however it is how you work as a team to get through it. The Salem Village staff works as a team to ensure the quality care and safety of all residents. From management to line staff, the different departments go above and beyond their scope of practice to promote an environment that is similar to home for the residents as well as providing a familial atmosphere when they may need it most.
Negative experience: Terrible place to put loved ones have roaches bed bugs and mice throughout the building and won’t get a exterminator. Unprofessional. Not very put together they didn’t know who was actually suppose to work cause they couldn’t keep up with the schedule. And rude employees. Sticky floors. And stinky hallways

Fantastic experience: I currently work at Salem Village. Staff is caring and want to give the best care to everyone in the facility. I am happy working here at Salem Village. I speak to many families in the community on a regular basis who give great reviews on how well Salem Village cared for their loved one.
Negative experience: NEVER SEND YOUR LOVED ONE HERE!!!!! Patients are sent to the hospital all the time because the staff doesn’t know what they are doing. This place is absolutely disgusting and needs to be taken down Immediately.

Negative experience: I can tell most of the reviews are by the staff lying. While there are truly ones who care there as CNA and Nurses most don't do their jobs and don't care. They have men changing the female elderly!!!! And the men I have seen are rough handling your female loved ones. My uncle is there and I'm doing everything I can to get him out. The elevator is unsafe. Halls smell bad. The room next to my uncles always stinks and i promise you he is drenched everytime I go there. I let medicare know and they will be sending out investigators undercover to visit patients like they are family and some are even visiting with the family. This court will have proof my uncle is being uncared for because I've recorded it 4 different times

Fantastic experience: From going to different facilities and actually seeing the care, this facility has given the best care I have seen with any other facility. From Nurse to patient ratio; CNA to patient ratio. They deserve 5 stars and has came a long way from 10+ yrs ago.
Negative experience: The elevators are all the time broken. I had a family member here, wouldn't recommend this place.
