Positive experience: My son was a patient here after he was released from the hospital after a catastrophic auto accident. The staff was really good, but there were a few of the staff that went above and beyond. Rosalie was simply put great. Now the two other staff personnel that were totally awesome were Jedon Brandon's day nurse whom was just the best nurse we could have ever hoped for, I can't put into words what she did for our family. Just a really really good nurse, but a better person. And the other employee was Brandon's Medical Technician/C&N Samantha G. I truly wish we would have gotten her last name. All I can say about Sam is WOW, we love her so much. There are no words that I will ever be able to find to let you know just how much Sam has done for us, she is the greatest and is well worth her weight in GOLD! Again the staff is really good we had absolutely no issues. Sam, Jedon, and Rosalie are very special people and are truly the best at what they do. Sam we love you. Positive experience: I had a relative at Mirage, she had been there for years. She used to talk to me about her friend Colleen who is in Social services there. She just loved her. When my relative got very sick and started going down hill Colleen did her best to keep me updated bc I live in another state. When she passed Collen made sure that I got her personal belongings. I thank her for making time to reach out to me.

Fantastic experience: following a car accident, I was so well taken care of of all by all the staff at Mirage, the rehab department helped me walk again in just a matter of 2 months. all of the CNA's were incredible. They always came in with a smile on their face and asked me "how are you doing." I'm so truly grateful and appreciative for everyone who saved my life

Fantastic experience: I came here following a brutal car accident, and I have just been amazed with the entire care process from Devon and Andrea in admissions, making me feel extremely welcomed, and a home like welcoming. Everyone in the rehabilitation department from Natalie to June to Claudia me with my recovery and healing. I had a broken sternum and a torn pectoral muscle. Mirage postacute has been nothing short of having a team of angels. Chad and Spencer the administrators have been so attentive to my care and plan of discharge. I’m extremely happy grateful that God placed me in good hands.

Negative experience: This is a horrible place to be. Everything they tell you is a lie. Patients here scream and eat their own diapers. I've never met my so called doctor or case manager. They won't even clean your body and often hold back your medication. Gross, cold and sad!

Positive experience: This facility is extremely busy. Our loved one was sent here after partial hip replacement surgery. Our social worker Colleen Kelly was very helpful. After a week she assisted us in transferring and transporting our loved one to the board and care home we had researched. Thank you, Colleen.

Negative experience: When I call to speak to mother this place answers sometimes, they transfer you to the station that you ask for and from there they never answer. I let the phone ring for over 30 min leaving me uncommunicated with mother, she lost her cell so for the past 2 weeks I have tried and tried im in Texas and this facility doesnt have any considerstion for long distance calls, they state there is only 2 phones for like 50 plus elderly people.

Negative experience: My husband was sent to Wellspring for "so-called" rehabilitation after he had hip surgery at AVH!!! What a joke!!!!!!!!! Where do I begin; huh!!!!!!!! All I'm going to say is "DON'T GO THERE"!!!!!!!! (So many of these "5" star reviews are fake!!!!!!! Just read through all of the reviews, and you'll see the deception!!!!!!!!)

Negative experience: I unfortunately stay at Wellsprings PAC and it is nighmare I can never wake up from. The most distressing part is the patients verbally abuse each other and other patients and staff 24|7. The entire facility is sub sub par in every way and I give it a minus 100% rating.

Positive experience: My elderly brother in under custodial care with Wellsprings Post Acute Center for a year now. I just came from a well-care meeting and I appreciate that those in charge of various services were present so I was able to get updates on my brother's care and I was able to get timely responses to my questions and concerns. I can see that my brother is well taken care of. There were lapses like delay in serving the meals, or hygiene care but I was able to address this during the meeting and I will monitor this situation. However, overall, I am satisfied with their level of care.

Positive experience: Need to do better with pts personal hygiene give a shower at lease 2-3 times a month because whipping the butt and maybe washing the face(maybe sometimes) just isn't good care. Causes skin problems. Just try Other than that doing a great job.

Positive experience: I just got back from a 5 weeks vacationing and stop by to visit mother then follow up with the social service to get an update. They were very kind to bring me up to date and provide me her file status on her medication and her condition. The staff were very helpful.

Positive experience: Staff were very good and caring. My biggest issue was the check in process where the machine yelled whether my temperature was normal or not. One time, because of the excessive heat outside it yelled out that my temperature wasn't normal. Everyone in the lobby seemed to panic. It was only because it was 114 outside and i had just gotten out of a hot car. I felt very violated every time it would yell out my status, even if normal. It is no one's business except the person at the front desk who may need to control whether people enter the building.

Fantastic experience: My Boyfriend was brought to Well Springs in February his condition had been declining after being cared for in a near by facility .This place is awesome they gave him the care he needed and the love the nurses and patients showed he started showing improvement right away he had 4 strokes in a short period of time he could not walk ,talk ,move his hands or arms properly,feed himself,had to wear a diaper and did not have a functioning brain last week he walked out of there he is walking ,talking,going to restroom by himself,moving all body parts and has a functioning brain thank you Well Springs I would highly recommend this place to care for your loved one Great Nurses That Go The Extra Mile!!!!

Fantastic experience: My brother recently stayed at this facility, and I couldn't be more pleased with the care he received. Although the aesthetics of the facility are older, the level of care and attention given to the patients is exceptional. From the very beginning, the admissions director was outstanding. She met with me and my brother at the hospital, communicated clearly, and made me feel comfortable choosing this facility. The admissions director followed up with me almost every day, addressing any questions or concerns I had. Initially, I was apprehensive due to a friend's family experience years ago, but the new management has significantly improved the quality of care. They are proactive and responsive to any issues. The wonderful care my brother received and his successful recovery are testaments to the dedication of the staff. While the facility may not be the prettiest, the staff's kindness and professionalism shine through. They truly deserve five stars. ⭐⭐⭐⭐⭐

Positive experience: My dad is doing so much better since he was placed in wellsprings. He looks so much healthier and is more lucid than I’ve seen in a long time. They don’t dictate the activity and how time is spent, so my dad is actually being cooperative and hasn’t been upset. The only thing that he doesn't like is the roommate situation but he is grateful that they at least have curtains between beds that provide a little bit of privacy. They seem to be very well staffed and having a lovely outdoor area for smokers and lots of room to walk around and explore and make friends. They also have a gym which is super neat.

Negative experience: We’ve been trying to admit my father from the hospital for three days. The admitting staff isn’t there on weekends. So if you’re in the hospital being released; good luck trying to reach anyone. You’ll end up paying extra to the hospital, petitioning your medical group or being transferred to places that actually respond (even if they’re not the greatest). I was told by three different people they’d call back in an hour. All three times I wasn’t called back. Even the case worker from the hospital and medical group had no luck getting someone to transfer. They said they had no transfer request but they don’t call back the person who is sending the request. Then I call on Monday. Surely someone is there in the mornings on Monday? That’s when doctors do their rounds. I was just told by the operator that they don’t come in until 9AM and the person that does admitting is often late. If he ever gets transferred I’ll edit to add the experience from the actual stay but right now, it’s not a good first impression.

Fantastic experience: I have to say a year ago this center was a huge failure. You walked in and it was dark, dirty and you could tell there was a lack of care being provided. They were understaffed, you would find three to four patients in a room and physical therapy was failing. I manage a senior living community and our resident was referred to Wellspings by the hospital. I was horrified by the conditions these people were having to endure. My resident was to be having physical therapy and that was not happening, I had to visit weekly and complain to get them to do their jobs. She was left in her bed unclean and no one to get her up and going for the day. It was heartbreaking. We had another resident that was there who had to have oxygen and the oxygen tank was empty the nurse told the family that the gauge was broken! Unacceptable, how in the heck would anyone know if it was even working! At some point during a visit I saw Davone Florence. I knew him as he would do wound therapy for a resident in my building. I learned he was now working to bring Wellspings back from the pits of hell! A year later and I can say I am so happy and proud to see the improvements that have been made. It is clean, sanitary, staff and patients seem to be well cared for. I have a resident there now and could not be happier with my residents progress and how Wellsprings has really transformed. I am not worrying and concerned for my residents that are being helped there any longer. I see the huge changes and am so thankful on behalf of my residents and families that they now have the right team of people in place who work hard to make a difference in the lives of those they now are treating. Great job!!

Fantastic experience: I Kirk came to this facility for rehabilitation as I arrived I was given awesome care by Cassandra, Erica and Denise from admissions. While my stay here I was constantly checked on daily had physical therapy daily. Helped me get on my feet walking, very pleased with all the staff here. Negative experience: This is the most disgusting understaffed facility I have ever been in. It smells of urine and they are all liars. I would not recommend this facility to anyone. I was there for five weeks of Rehabilitation and they are sorely understaffed with OTS and PTs. They are all so grossly understaffed with the nursing assistants. Lights do not get answered and it is a horrid place. I would not put any loved one in this facility.

Negative experience: I only gave a one star , because i could not post this review with a star . This place is decspicsble !! Residents who have no family and are not able to do things for them selves , bed ridden . Are ignored and not cared for . They’re infested with scabies , left in bed to sit in their own feces for hrs upon hrs . They do not get any type of bath what so ever . Their scabies are not treated . Bed sore beyond belief . They do not answer the phone at the nurses stations . There are no phones in the rooms . Absolutely deplorable.

Negative experience: So After I was sent and completed intake my wife left. Passed that point it took about eight hours to receive any water, food was on a paper plate (squished)sandwich, and a 2oz juice around 9pm and zero medication(s)… Staff was loud and abusive and yelled at me as if I was challenged or suffered from dementia. Negative experience: If you love your family do not send them here!! They let them die and do not care for their patients my grandma was transferred to this home form A wonderful place. One month later after she was transferred she passed away. She had bruises all over her body they claimed she fell. Understaffed they cannot monitor all patients they leave people lying in A bed all day. This place smelled very bad through the halls I do not know if DMHC is involved.

Fantastic experience: It was great, I didn’t wanna leave, Melody was top notch along with others, Jesse and Ellise from wound care were also top notch. Lilia from housekeeping was wonderful! Anna the Nursing Supervisor was amazing as well. Rachel G, Tiffany, and Melody of course were outstanding too! Kianna (LVN) and Grace including Andre were top notch! Elena was so welcoming and friendly! Cassandra also! Miguel as well! Cris from dietary and Mariana came on her day off and decorated the station for Christmas! But Melody was just outstanding and top notch! I am a big believer to always recognize the hard work of people and not only the bad.

Fantastic experience: Over the last four and a half years as a spiritual care pastor at Grace Chapel, I have had many members whose loved ones stayed at Wellsprings. I have visited many under the care of Wellsprings and upon my visits I truly enjoyed and appreciated the kindness of the nurses, the joy, even though many are faced with life threatening situations to care for, not to mention that many often have unrealistic expectations placed upon them when it comes to caring for their loved ones. I have visited this facility over 100 times, and I am grateful to every single member of the staff, dietitian, groundskeeper, custodian, and doctor. Wellsprings is a bright light in our community, bringing hope to those who are desperate for care by providing light and encouragement. Thanks for being a light in our community. Negative experience: I called 19 times.... ALL IN THE SAME SITTING.... to speak to someone I haven't heard from in 25 years, in Room 413, and not ONE of your so-called "highly skilled" professionals could get me through to him. ABSOLUTELY UNPROFESSIONAL. Y'all should be ashamed of yourselves.

Negative experience: The facility appears rundown. The hallways smell of urine and feces. The staff are inattentive and can be rude. The latter pertaining to the night shift. All meals were lukewarm at best but in general, “cold”. The “nursing staff” don’t appear to do anything but sit at their nurses station. Everything looks to fall upon the CNA’s who, in my opinion, do the best that they can. Does the state even require random/surprise inspections? By the condition of the facility? I think it’s a valid question/concern.

Negative experience: Well Springs runs a scam with a company that charges 3k to supposedly assist in helping patients families get Medical! It’s ALL A SCAM!! They called my mom 5 days later and stated they will not be able to help as my dad did not qualify as he made too much! They robbed my elderly mother of 3k in 5 days with lies saying they 100% could help but actually sweet talked her and never did a thing! I will follow up with said companies name!! Listen to these reviews.. they ONLY have two stars for a reason! Well Springs IS A JOLE and they run a scam to steal seniors savings!

Negative experience: My nephew, 55, was transferred here from AVH. Awake, alert. Two (2) days later, transferred back to AVH, almost in a comatose state and passed a few hours later! Contact Wellspring to notify and set appointment to pick up his things. It’s easier to make it rain in AV, than get anyone on phone without 10 times put on hold and eventually Someone else picks the phone up and asks once again, why are you calling, who do you want to talk to, let me put you on hold and transfer, yeah right! So I went there, According to the staff, my nephew did not have anything to pick up. All the things that were transferred from AV HOSPITAL with my nephew to Wellsprings, disappeared! Wallet, cell phone, glasses, clothes, paperwork, GONE! But I still get a bill each month and now, the threatening letters! I have called each month, for 11 months since my nephew passed, asking for Billing Dept., transferred 11 times to the voice mail of Daphne the bookkeeper. 11 times I have left a message for Daphne to call me back. 11 times Daphne has failed to call. Got another bill today and again I called, but this time I did not ask to speak with Daphne, I wanted to speak with the administrator, Rafeal Reyes. Yeah right! Receptionist wanted a detailed message of why I was calling to speak with the administrator before putting me on hold, three minutes later someone else picked the phone up wanted a detailed message, put on hold! three more times my call was picked up and put on hold before I finally said No more! If you guys cannot get your act together, I will write reviews, I will contact your corporate office, I will do anything and everything i can possibly do to maybe make you guys answer the phones or return phone calls! If this is how family is treated, how do you think they are treating your loved ones? Especially the ones that cannot defend themselves? Horrid place!

Negative experience: If I could give it less I would. The food is bad, everything is broken or needs wd40. Don't come here unless you have to.

Negative experience: My father is here because of a broken hip. Hes received fair treatment. That being said, its Extremely difficult to get an appointment. Their phone lines are pretty unprofessional and unorganized. Disappointed

Fantastic experience: My mom is taking care of with very good Nurses and CNAs. This is the second time she has been there. We have taken care of her at home as long as we could. Now she is taking care of knowledgeable staff. I am really glad she is in a place where I trust my moms care to others. I don’t have any complaints with her care. I will recommend this facility.

Negative experience: It is very difficult to get someone on the phone to make an appointment ... nobody calls me back after 6 attempts ... very sad !!!

Positive experience: I just want to take a moment to give a shout out to Kathy Giba for the tremendous help she has been during my mother-in-law's stay. Due to covid and our inability to visit with my family member it was hard to have a good understanding on what her treatment was like, how she was doing, etc. While we were able to call into the facility and talk directly with our love one, we never got the chance to speak with a doctor. Kathy was very helpful in making sure we were up to date with important information pertaining to my love ones insurance and discharge process. Negative experience: My mother in law was here about two years when recently decided to house COVID patience. They didn’t let my husband(her son) who is the one in charge if he wanted to take her home before they housed the covid patience since she was on oxygen and very poor health condition. She contracted COVID and two weeks later she passed away. I would not recommend this place at all. Should not house seniors with COVID patients one or the other. Just a horrible place! I wouldn’t even give it one star
