Fantastic experience: Administrator is very attentive to the Residents and their families. Always on the floor making rounds. Tim is always making sure staff is doing their job . Danny the DON is Awesome. The Therapy department is amazing. My grandfather was here and had the BEST care. Recommend this facility.
Negative experience: Some of the most disrespectful employees and higher ups that dont take accountability when issues occur. Employees even quoted to say... "they wouldnt have their own parents in this facility".... what does that tell you. Very sad
Negative experience: Well I need to get this out. This place needs to be shut down and the sister company too...Lubbock Hospitality. My grandmother moved in may 1th of this year. The next day my grandmother got out and was missing for 6 hours. She was walking not knowing anything where she was going. We got a call telling us they can't find her. My grandmother wasn't in there for a full 24 hours and she is missing. And all the family gets is I'm sorry and we don't know what happened. To make matters worse. They want her to move to there sister company. They a lock down part and they have a bed open. We didn't want to move her there but we was told that with her get out she couldn't stay there. So she was moved to the sister company and that place was hell. My grandmother was a month in one place and a month in the other. She clothes came up missing her bed stuff was on other people's beds. She look bad. Smelled bad. She was losing weight. Not sleeping. So when the family took her out of there. And took her home she had a shower she had a bug on her she was mad and didn't know where she was. I never seen my grandmother like that. So if u are thinking about doing this DONT DO THESES TWO PLACES!!!!!! 2 months and my grandmother will never be the same.
Negative experience: This nursing facility is very neglectful and not helpful with patients, they leave their patients in need of help for 30 minutes or even more, they do not help with changing, they are rude and disrespectful to family members who are just their to support their own, they are an insurance scam essentially if they state they provide care, there is no such thing as care provided. The nurses are neglectful, abusive, and need to be exploited.

Fantastic experience: At first, some of my dad's belongings went missing. I reported that, and all belongings somehow returned. I don't know if some of the staff were not well educated on removing his electric razor, personal pillows and clothes. It got better. I really think Tim made a big difference. I so enjoyed having a wonderful early Thanksgiving dinner the facility put on for the residents and family members. The nurses and aids are very attentive. I am notified of any issues with my dad's condition and care. I believe he is treated very well. I see how they also in general see after the residents with care. I don't see everything of course, but do believe that is the best place he can be at this time in his life. I see him as much as possible. He is confused, but seems generally happy.
Negative experience: Do not send your loved one to this facility, especially the south wing! My friend was treated horribly in her last days of life.
Negative experience: My aunt has been here for a few days they will not answer my calls it's a recording they will not talk to me please don't bring your family here cuz I'm fixing to get mine out.
Negative experience: This was the worst place I could have taken my mom to be for a few day's it was a night mare !! My mom was treated horribly! And to top it off She was Given Not 1 Not 2 Not 3 BUT 9 Blood pressure medications plus a Tylenol For a blood pressure reading 108 over 60 She had Dialysis that day and because of their mistake Dialysis was not able to pull any fluids off , Now Here we are Sunday and my mom is having trouble breathing, They don't think it's a big deal oxygen reading at 65 I called 911 So because Of their mistakes and not caring if she lived or died My mom is back in the Hospital! This place shouldn't even be Open, You guys if you have a loved one needling nursing Care Don't Don't I repeat Don't care them their! I wouldn't Wish This On Anyone ' This broke my heart, So many BAD Things took place in just 5 days and i wasn't their to defend My Mother
Negative experience: Very unprofessional,I lost my job because the manager showed favoritism to her daughter I didn't know she was a assistant manager until today the administration threatened me in didn't let me explain very racist there and everything
Fantastic experience: The one of the best facility I have work for plus my grandma got great care there
Negative experience: This facility is very rude and disrespectful, even going as far as cursing out delivery drivers who are just doing their jobs. The entire complex smells like urine and most of the patients there, look miserable
Negative experience: Thieves, sickness and isolation. Steer clear of this awful place. My sweet friend was here for a short time and they stole her jewelry, cash, credit cards and cell phone. There are many here with COVID and the do not quarantine them. They took her cell phone and would not charge it so she couldn't communicate with anyone outside the facility, then it disappeared completely. "No phones in the rooms allowed" BEWARE!!
Fantastic experience: You can tell there is new leadership! They are attentive and caring along with all the staff. Keep up the good work.
Fantastic experience: Tim and Quentin were fantastic! The newly renovated areas are beautiful. HIGHLY recommend.
Negative experience: Having my father there was the worse. They forgot to feed him, didn't give him his medicine and this happen more then once. When I was pulling him out of there they had the nerve to ask me to sign papers that were supposed to be signed upon admission. The place was dirty and never enough staff. Walking down the hall would a sight to see. I can keep going and going this was the worst nursing home experience and most disgusting place that I have been in.

Negative experience: If anyone has a loved one in this facility please move them to where they will get taken care of ….this place don’t have a phone so you can talk to your loved one and it’s a piece of s*** it smells . I had my mom there for therapy and instead of them doing her therapy they just let her lay in bed and sleep, they do not need to have their doors open !!!!!!! My GOD and I will take better care of her than heritage oak!!!!!!!!!

Negative experience: My husband took his Mom a Little Caesar’s Pizza for her Mother’s Day surprise. She was asleep, so he left it on her bedside table along with a diet drink. He stopped by the Nurses Station an told the 2 nurses that he had left the pizza for his Mom by her bed. The both nurses assured him that they would heat the pizza for her when she woke up. He spoke with his Mama a few minutes ago and she never saw the pizza, the drink, nor heard it was brought to her by my husband. Today is Saturday, so of course yesterday’s DON nor the facility manager are working. The only person I could speak to about this was the weekend On Duty Nurse. She is of no help. Is it out of line to expect my MIL’s Mother’s Day surprise be replaced? Or my husband reimbursed to buy his Mom another Pizza and drink or something else she likes? But by Monday, when we can speak to someone in charge of this facility, Mother’s Day will have gone by. Grrrr!!!!! It is theft, no matter what it was. My MIL should have at least been told about it.

Fantastic experience: I had to place my Mother in Heritage oaks for Rehab after she broke her leg. The rehab team was awesome and always had an open line of communication. The staff is phenominal and helped me with financial resources when I had to make the decision to place her permanently . I would recommend their services on the future!

Fantastic experience: We have a friend that is a residence at heritage Oak, The staff is always willing to do for him, since he is bad fast… We live out of town and if we cannot get a hold of him on his phone, they’re always happy to you go check and make sure that he is OK…We are very grateful he’s taken care of!
Fantastic experience: Heritage Oaks is a great rehab facility. Their director of rehab Dickie is a Superman in helping the patients get stronger and recover.

Fantastic experience: I work for this wonderful company but on a personal note I have placed my own Nanna and mom there for short term and long term care which says it all! I’ve gotten to see the delicate side of being a family member of a loved one and I can’t say enough about the amazing care my own family received. I would recommend Heritage Oaks to anyone seeking a wonderful place to work and also to anyone who is needing care for themselves or their loved ones❤️

Fantastic experience: My grandmother who has Alzheimer’s has recently been admitted to facility from another nursing home and with in 2 weeks she is getting great nursing care and therapy daily. She has shown a great improvement in just such short period of time

Negative experience: My mothers afternoon nurse called me on 4/22/22 and informed me that she found a bruise on her right arm and her right side near her rib cage. She said re was no documentation on it. I went the following morning to see for myself and the bruise on her arm was on the inside of her bicep and covered the entire thing! How can anyone see this and not report it. The nurse that told me this is very professional and said she is very upset about it. She said she will start investigating to see what had happened.

Negative experience: The nurses are unprofessional and always "forget" to feed my aunt . I would like to also point out that she has been hit multiple times by nurses .when we confronted them about it they say she feel or hit herself . I hate the nurses and I am always getting into arguments. They never move her from her bed , not even to bath her. They are always saying my aunt is going to chock but when she eats with us she eats just fine . I do not recomend this place to anyone r and hope they shut down .

Negative experience: One of my grandparents was just admitted to this place and they forgot to feed her. She's only mentioned one time, but she isn't looking good. We are in the process of getting her out of Heritage Oaks in Lubbock because their care is so poorly given!

Negative experience: My sister is on her death bed because they never checked on her over night . Her sugar level went down to a 20. Now she has 24 hrs to improve to be saved . DON'T BRING YOUR FAMILY HERE
Negative experience: Paid a $90,000 fine because one of their aids was caught abusing a resident there on a hidden camera. They stood by the aid. Soooo if you want your loving family member to die.... Bring them here.
Negative experience: This horrible dump is a nightmare. They have staff leaving this place faster than those twin towers went down on 9/11. A nurse had a job interview here on the same day they were audited. She drove all the way down there, only to be turned away. We DO have cell phones. She could have been notified. However, she was not. Her appointment was rescheduled. They had no time for her after that. That patio was junky and cluttered, and the receptionist made promises she could not keep. STAY AWAY!
