Fantastic experience: My wife needed rehab during the time Hurricane Milton hit us and we are fortunate she was able to be at Lexington. It was comforting to know she was safe with O2 even during a power outage and nurses were available 24/7 during the entire experience. I went by at least once a day until she was able to come home and I was pleased with the positive healing vibes that were evident during my wife's stay. This was an intense period and everybody could still smile. My wife was comfortable and felt very secure. My impression was there are some great leaders at this place as all of the employees I ran into appeared to be focused on the residents' well-being. Nice job, Lexington!

Positive experience: Their "rapid" rehab is great due to the exceptional unit manager (Hubba) and the therapy department. As far as long term units go ... No, I don't recommend those: noisy and smelly. Nursing staff is decent. Aides? Depends on the day/luck. Administration? Revolving door. Nursing director? You'd find more caring in a prison guard. They do seem to pay their vendors, though.

Fantastic experience: I am pleased to leave a review for Lexington where I recently stayed. The room was spacious and totally clean. The food was good and nutritionally healthy. They offered a menu selection daily. The serving sizes were adequate and they offered snacks during the day. Most importantly, the staff were excellent and attentive. I was checked on several times a day and a nurse was always available when needed. The call button was answered within five minutes no matter what time of day or night. I will request to stay at Lexington next time I require a rehab and gladly recommend it to anyone I know.

Negative experience: My father passed away peacefully on June 30th and was cared for by Hospice and the Lexington nurses that he adored. Since his passing I have been unable to talk to anyone in the administrative departments including the Administrator. It has been 11 days and no return calls or I am told they are not available. Completely unacceptable!!

Negative experience: This place has too many ups & down. With each change of management staff, hopes run high but the disappointment continues when they are suddenly gone. This has been happening every since Greystone handed facility over to new owners! Time & time again there are issues with patients not getting balanced meals or enough food. Where is the food going. I have seen patients receive one item on their plate at breakfast & lunch. No vegetables, no bread.- just an egg or a piece of meat with a glass of juice. Anemia & weight loss have resulted for some patients. AHCA should investigate this! Administrators have come & gone. I haven't met the latest one but I'm hoping for good change. The inside of the building is at least being cleaned. Please help with the food situation. Patients are suffering. I pray that your response is true & results will be seen!

Fantastic experience: I would like to congratulate the entire staff for working as a Team to Care for the Residents. As an Acute Care Hospital CEO who for many years operated SNF’s, I was impressed with the Facility Leadership Team, along with my fathers Nurse Manager, RN’s, LVN’s, CNA’s, Therapy Team, Dietary, Housekeeping, Front Desk and maintenance. Staff listened and was extremely helpful at all times. My Dad experienced Hope, Healing and Recovery. Thank you to the entire Team at Lexington Health Rehabilitation Center in St. Petersburg, FL. Negative experience: Absolutely horrified after I called my grandfather and found out none of the nurses or staff is caring for him. The poor man is in there needing aggressive antibiotics TWICE a day. He’s lucky if they come in once! We have already called and left complaints, with no change at all. Absolutely disgusting. Unprofessional. Not sure how this place is even open. Don’t ever send any of your family members here!! And if you do, have your attorney on speed dial!

Negative experience: I definately have seen the worst of the worst in employees and rooms! From attitudes to staff IGNORING needs of patients! They transferred my dad from hospital, put him in a room with no beds made...finally 1 gentleman made the bed...6hrs later, I have to call there at 10pm and ask why nobody has been in to see him!! He never felt like such a nobody! His sugar, BP , nothing was done! Antibiotics...nothing...pain medicine...12hrs later they finally give him 1! What kind of place is this???!!! Lets not.leave out how he has other patients trying to get into his room in middle of night! Other patients yelling all hours of night about cheating and constant swearing back and forth ..really?!! No supervision at night! Staff there for a paycheck and nothing more! Shows 100%

Negative experience: This place lacks professionalism, empathy, compassion and any kind of respect for the field of Nursing Care. My mom been in here one week and had to call 911 to get help from staff that are right in the building with her. That should never have to happen if the staff are there to work and care for patients that they are assigned to on a daily basis. If this continues I will post staff names as I see it in my next post. This place has a great environment for patients to live in, but it lacks people who really are in the nursing world that care about ppl.

Negative experience: They have a LOT of "cleaning up " to do from staff to the place itself. The few good staff that is there can not make up for all the horrible staff and lack of care that happens there. Their bad reputation is accurate and I hope for the present residents and future that the new lady in charge turns things around for them

Fantastic experience: My dad is here and he's been here for a year and a half the staff is excellent and the care they give is great, the only thing that could be better is the food.

Positive experience: l just got here late last night. It's ok, but nothing like BayHealth. The EMS took me to BayHealth since I have been falling, so after a Cat scan, they sent me to Lexington Rehab Facility to build up my strength in my legs.

Negative experience: Pictures very deceiving building old and needs good cleaning and painting inside. They have direct tv the cheapest of programs no menu of stations. The food is ok they have a menu daily to pick from however you don’t know what your getting I guess they run out of food . Staples like all beef hotdogs they run out of or don’t make enough. Thier lunch which is the big meal is usually 3 oz of pasta fish chicken or something else . I don’t know about anyone else but 3 oz isn’t enough to fill a 6 year old plus 3 oz veggies supper is usually sandwiches ham turkey 1 slice 2 pieces bread . The breakfast is the best fill up on it but it’s limited to usually 2/3 oz. Eggs then crearals toast and juices 4 oz . I thought we were here to strengthen ourselves . The cnns aren’t bad but most of them only do what they are suppose to do . They don’t work together like a team . Ex if I call to have them empty a urnal or change me they will say I’ll get your cnn and walks away if light is on they do this all the time . You do have a few that are very good and go out of thier way to help extra. Pt is good they want to help and get you better . As it stands now I would not recommend because of those reasons. And more . Thank you for your time . Top of post got taken away because I changed rating . It basically said building not clean rooms need paint and food is mediocre. Out of menu items only 2/3 oz of each 4oz veggies not enough proteins on menu not enough to fill a 6 year old

Fantastic experience: Above and beyond my expectations!! I was on the rapid rehab hall and I can honestly say that I was blown away with everything from the welcoming that I received when I arrived, to the staff’s thoughtful, kind, caring, and heartwarming regard they had for me from the moment I walked in the doors until the time I left!! I was treated like a first class citizen and the staff really went out of their way to accommodate to my needs. My light was always answered promptly, housekeeping made their presence known and did what was asked of them, the therapy staff was vigilant to how I was feeling and worked with my schedule to eliminate stress and discomfort during my sessions. I just can’t say enough about this place because I was truly surprised by the level and quality of care, which for me has been a very hard thing to find in these places.I’ve unfortunately had to frequent hospitals and rehabs more than a few times and never have I said I would return to any of them but, believe it or not, I would come back here hands down if I had to! ( god forbid) To the families trying to decide where to place their loved one, please take my advice and trust in this facility. I know everyone has a different of opinions and experiences but, this was a place that god sent me to heal and recover so all I can tell you is if the opportunity is presented to you, trust that it’ll be the right fit for your loved one. Thank you so much again to all the staff for the support, love, and care given to me during my stay, you blew me away with your kindness and definitely made an impression on me that’ll stay with me forever. Also, as a bonus, I made a few friends that I didn’t have before my visit here where, in most cases, I couldn’t wait to get away from the majority if not all workers at the other places! One thing I can say that needs some improvement is the food! Could absolutely use a little love in the kitchen! Well, I hope this helps make your decision a little easier as it’s not a situation you can take lightly. Good luck in your choice, and the recovery and future of your family member and god bless!

Fantastic experience: This place is not perfect but it has been a great stay for me. I ask God to place me in a place that is clean and cares about my well being. He placed me at Lexington. The leadership listens and cares. I’ve had a couple situations to handle the good far out weighs the couple.I’ve been at peace and met some wonderful people. Therapy has been a tremendous boost for my growth. I would definitely recommend Lexington if you’re looking to rehab and feel safe. Thank you to all the leadership and staff. You make it work.

Positive experience: A wonderful facility. Very pleasant and welcoming Staff. My loved one was recently admitted there for a short term stay and everyone went above and beyond to make sure she was happy and comfortable. From the new administrator to the entire nursing staff. Communication was on point with our family throughout her stay(That was a plus), even the Doctor communicated well. If asked would recommend this facility to my family or friends, Definitely a BIG yes!!!

Negative experience: A lot of the reviews about this place say the same thing, literally. I think they bought some bots to make reviews. Visit on-site and use your own judgment. I think it's too sketchy for my family.

Negative experience: A good friend has been there for several months. He reports difficulty getting attention from staff, less than courteous behavior from several staff, and difficulty arrranging M.D. visits there or appointments outside. As a friend trying to call him, either the phone is not picked up, disconnected, or sent to the wrong room, especially any time not between 9 and 4. I wouldn't send my dog there, and my bud is trying to arrange a transfer. The best he has said of the place is 'the food is good.'

Negative experience: If I could give zero stars, I would. Absolutely horrible. My loved one was there recently and he contracted rashes and sores due to neglect. We had to practically force the employees to do the bare minimum (shower, change his clothes, walk him around). The place was filthier than the hospital and staff were hard to find. Everyone was clearly under cared for. It was difficult to get him out of the facility due to the employees’ lack of communication and effort. It took them days to get my loved one his medications which caused him more problems! I wouldn’t wish this place upon my worst enemy. If you care about the well being of your loved ones DO NOT take them here!

Negative experience: This was one of the most stressful experiences for myself and my loved one. It's not just that the place is not clean or well-maintained. It took days to get meds and basic care resulting in skin rashes and various health issues. I was genuinely afraid. The best way I can describe it is gross negligence, poor to almost no communication, and empty promises. It is a terrible feeling to leave your loved one with people you feel you cannot trust. It took an act of God and massive tenacity to get out.

Negative experience: The place is remarkably bad. Finally had to transfer my father to a different facility. He was there 6 weeks. I never once got a call from anyone and when you call the facility the phone just rings and rings. There is basically no therapy or rehab done of any kind.

Negative experience: my wife went to Lexington for rehab after a fall back in April 2022 she was sent there after a bad experience at another greystone facility but this was the only facility that had an opening that would take her insurance. The first week wasn't bad but it got worst. the food was terrible, the bathroom floor was always sticky, she was lift laying in bed with a wet diaper for most of the day, call bells were never answered in time sometimes for almost 45 minutes or longer. one day she had a cup of water on her bed table that was dated from 3 days ago. if you have to talk to a nurse then you have to leave a message with the front desk and hope the nurse calls you back which usually never happens, the same with the administrator and most of the time the phone just keeps ringing and no one answers. My wife ended up getting COVID and also a UTI from sitting in a wet diaper most of the day. My wife is a type one diabetic and needs to have her blood sugar checked and insulin given at pretty much the same time each day which was not done, most of the time the nurses did it on their own schedule. They also did not follow doctors orders for the amount of insulin she was suppose to get and ended up having her blood sugar levels drop three days in a row. when my wife told the nurses and aids her blood sugar was dropping they argued with her saying it wasn't . My wife ended up being rushed to the Emergency room one Friday morning with a blood sugar level of 12 and was semi unconscious. When I talked to the doctor that had sent the orders to Lexington for the amount of insulin she was suppose to get he told me it was suppose to be 18 units but when I spoke with the nurse in charge of that unit at Lexington she said they were giving her 25 units which was too much. you would think that after three days of her blood sugar dropping that they might notify the doctor but I guess that is asking someone to do their job. My wife spent four days in the hospital recovering from the mistakes Lexington did. the hospital wanted to send her back to Lexington to finish her rehab but I told them no way is she going back there! I would never recommend Lexington to anyone and if you can do the rehab at home it is a lot safer.

Negative experience: My experience was not positive. Pictures shown here must be 70 years old. They are nothing like the Lexington Center I saw. Lexington of today (June 2022) is like the set of a horror movie. I actually feared for my health. I was there less than 4 hours before i called an Uber and left.

Negative experience: Filthy... Stinks... Insofistion work habit... If you Lucky you might see your nurse.. staff is lazy. Night crew walk around in slippers and noses I'm their phones sounds like it's party time .. extremely loud all night long

Negative experience: Nope! You first walk in and the front few halls are fine. Then they walk you back to the "rehab section" and it's like walking through a 50's horror movie. Everything gets more dirty and outdated as you go. The room and bathroom were filthy. Bugs flying around. Plaster falling off the walls. The absorbent pad on the bed was not disposable just some old mini blanket. The chairs in the room were dirty and stained. The old bed was rusty. They didn't have a walker for her, or bedside commode for her. We told them she couldn't get up and walk very far and they said they'll go try to get a catheter request from the doctor. They didn't have her medications for which she was due. I could see the fear in her eyes from taking it all in. We called 911 and had her readmitted to the hospital. DO NOT LEAVE YOUR LOVED ONES HERE!!!

Negative experience: This place is Absolutely Disgusting!!! The "nurses" don't care and are Very RUDE!!! My Mom was taken there for rehab after a Major back surgery. The room was filthy, bugs flying around, dirt and black mold on the floor, around the shower and even on her phone!!! Nats flying around and she has open wounds!! They had No pain meds or her high blood pressure meds. There food was uneatable. They didn't wear masks and was too busy to help us because they were playing on their phones. We had to call 911 to get her out of there!! NEVER bring your loved ones there!!! We have a LOT of evidence to prove how UNEXCEPTABLE this place is!!! They need to be shut down!!!! We will be calling the Health Department and the BBB!!!!.

Negative experience: This place is disgusting! There are bugs in every single room, everywhere. There are used hair products and dirty bobby pins in the bathroom, and there is no soap in the dispensers. Mold is growing on the ceiling and on the walls. There are signs saying that masks are mandatory, but nobody was wearing a mask. You cannot get inside the building and when you call the number provided they will hang up on you. There is a sign saying that the door is locked from 8pm-8am for the security and safety of patients but is opened with the push of a big button. Do not take your loved ones here as this place is an absolute nightmare.

Fantastic experience: Wonderful facility! Clean and welcoming. My Grandmother was a patient for over 6 weeks. Everyone went above and beyond to make sure she was happy and comfortable. Her doctor, Dr. Shah, communicated with our family throughout her stay. I would never hesitate to recommend this facility to family or friends!

Negative experience: Big mistake choosing this rehab for my Mom! A friend was there 4 yrs ago and very happy with care. A lot has changed since then! Did not get therapy as scheduled, did not get personal care as needed, I took care of it almost every day when I came to check on her. Mom is not a crier, she called twice crying about the treatment she received. I tried to call the nurses desk and was hung up on. Called again and got no answer. Most of the staff are rude and unfriendly. There is one wonderful aide, he took great care of my Mom! Tried to get her transferred out. Do NOT allow your loved one to go here!!!

Negative experience: No meds given. I would not recommend. Bayfront could have done better. I had to beg for my meds. . When I told them I wanted out. I was told at 9am the Doctor had to order it. She got here at 345pm and then she told me that the customer service rep had to do the paperwork. He has known for over 6 hours.and he does not work on weekends. This place is a joke.

Negative experience: Not sure what the kick backs were for these 5 star reviews. This place is a disgrace! From the CEO down to housekeeping, they do not care about your loved one. Complaints went in one ear and out of the other. Not just my loved one but her roommate as well. Several occasions medication was found in the hall and brought to the attention of several staff members. I started taking pictures of the neglect my mom was receiving, the filthy rooms and halls. I bought a mop and cleaned the room myself. Clean linens and clothes were only given when I asked or just went and got them myself. The food or lack there of, was not edible and usually had an odor that was not pleasant. Nurses and CNA’s unplug the call light on the night shift as to not bother their social time or cell phone usage. I have proof of that as well. The higher ups and especially the DON will look you right in your face and lie. Police reports were filed a few times. DCF got involved along with the States Attorneys Office. You don’t have to believe anything written but I would hate for another Elder to be neglected and abused like my mom was and disregarded like she meant nothing. This place needs to be closed down and shame on every person that works at Lexington.

Negative experience: From the time my mother was admitted there has been poor or lack of communication. 11/13/21 my mother fell in the evening, facility never informed the family. Now I find out on 11/14/21 I still cannot speak to a supervisor. How do you trust these people with your family member. The nurse on duty states no documentation from the hospital of what they did to treat in ER, she actually stated never getting in her report that my mother was taken to hospital. I am furious!

Negative experience: Very unprofessional I got ripped off my paychecks the staff does not take care of the patients how they're supposed to the patients are always in bed the patients are dirty the rooms are smelly the CNAs do not take care of the patients how they're supposed to the patients complain about how they want to get dressed and nobody wants to give them showers or dress them the activities director Chris is very unprofessional he was being very unappropriate he sat there and had a full-blown conversation about inappropriate things I felt very uncomfortable he is so unprofessional I don't understand why would they even have him working as a activities director very ghetto staff sorry to say I will never ever recommend someone to live there or work there I am beyond upset with the front desk lady she was very rude on professional and thought that she can run anyone I am highly upset that nothing was done when I brought it to their attention I don't feel like should bring their loved ones to a facility that does not care about patients I am highly upset that nothing has been done they've stolen hours from me they have not wrote down any of my hours I haven't received any calls I had to call about my paycheck I am over this company and they need to be shut down especially when they're not taking care of the residents that's what we're there to do but unfortunately some people don't care they only want the money sincerely naila beganovic

Positive experience: Lexington Health and Rehab has no many caring staff. They pour their heart and soul into providing patient center care to their residents. There are so many wonderful changes happening and coming in the future. I can not wait to see how the building blossoms and grows as they provide care and services in this community.

Fantastic experience: My mom was admitted to this facility for rehab after her hemoglobin dropped to dangerous levels . She had received amazing services from all the staff . Place was also clean. Scheduled visits were allowed to ensure patients’ safety against COVID. She had worked with Anu from therapy who she adored. She was also raving about Marsha who was her CNA. Mom is so much better now , walking and finally gaining some weight after being at risk for failure to thrive . I would definitely recommend Lexington to anyone .

Fantastic experience: My experience was great, more than I had ever imagined. When I first came I was bedridden, and very malnourished. I had a number of infections, as well as a heart monitor. I also had depression and very little will to live. The staff here made a big difference in all of that. I was embarrassed because I couldn't make it to the bathroom. They would clean me up with no problem, talk to me with respect, and made me believe in myself. When physical therapy started, there were times, I was unable to participate, but they were patient and let me work at my own pace. I am very grateful for the staff and workers here. The concern and genuine care made me want to do and be better.

Negative experience: On January 5th my 90 year old Mother entered Lexington Rehab facility, I was told by Lorraine Wexler that medicare payed for the first 20 days, after that there would be a copay, but that my Mother's secondary insurance would pay a sizable chunk of this. My Mother was in there for 7 WEEKS. During this time there was virtually no communication to me about her progress, nor any indication of how long her stay would last. During the coming weeks, they screwed up transportation scheduling for several outside doctor's appointments , including a colonoscopy, after which my Mother was discharged on Feb. 25th. The day before, in a phone conversation, Denise Fleming told me how well my Mother had done in rehab, and asked what durable medical equipment I had at the house. I told her I had among other things, a wheel chair. She thought that was good in terms of distances greater than 10 or 12 feet. When my Mother arrived home the next day, she was in the same condition as before she entered the Lexington; unable to stand or walk on her own. On top of that, she came home with a urinary tract infection, and was back in St. Pete General that same night. Some months later, she was billed $5,750.50 for her time there. Her secondary insurance covered nothing, and I was never informed of this. Chris, the young man at the door, and RN Karen Montgomery, were my only sources of information the entire time. They are both good people who always did what they said would do. Beyond that, I would counsel anyone to avoid this place.

Negative experience: Place your loved one anywhere but here. There was sub adequate wound care that left my father in law with 2 massive infections including MRSA. Visitation was limited to 45 minutes a week per PATIENT and in public areas so you never see their rooms or the cleanliness of patient areas. As he is now placed in a much better rehab, with none of the issues, I can only think the staff is too lazy to care for the patients and treat them as humans.

Negative experience: We were told our loved one saw a doctor. She complained as did we for a week about an ear infection. No one has called us once to tell us if she actually saw the doctor. We live out of state and their main number has been busy or with a disconnected message for 5 days. Someone local has gone down there and told them multiple times. They even used a cell phone to verify they were down in front of her on Thursday. Our loved one can barely here us on the phone and we have no way of knowing if she is OK or not.

Negative experience: My mom (who has severe dementia) just got out of this place. She was admitted after falling and breaking her sacrum then the day before she was to leave the first time, she managed to wheel herself into another patient's room and tried to stand resulting and in her breaking her hip. Every time I visited her she was sitting in a dirty brief and had feces under her fingernails. She was also brought the wrong order at meal times, and she left with either a cyst or a bed sore which they neglected to mention. They sent her home unbathed and half of her belongings were missing. Some (not all) of the staff are rude as I asked for a box of tissues and one of the nurses was busy on her phone and told me to get them myself from the supply closet. She made no progress physically or mentally. Imo, patient's are neglected and the difficult ones are almost completely ignored. I do not recommend Lexington to anyone!

Negative experience: We understand that Covid has changed the way things are. But, when we are lied to repeatedly and can’t get anyone to tell us anything it is not acceptable. We have a relative there and we were told that he had seen the doctor and that the doctor would call us after rounds- no one has called. (He has been there over a week) When we ask our relative he says he hasn’t seen anyone. We brought him food hours ago and they still haven’t given it to him. They don’t answer the phone when we call. It is so frustrating not knowing what is going on.

Negative experience: I had an 85 yr old family member stay at Lexington in 2019 and he received proper care an we enjoyed the staff. Nurses and physical therapist were very nice, caring, and personable. His original stay was only for about a week or so and then he was released back home. Due to medical issues he was taken back to Lexington a few days ago after a stay in the hospital while he awaits surgery. I understand that during Covid adjustments have been made to keep everyone safe, but the care here has gone down hill DRAMATICALLY!

Fantastic experience: My mother has been at Lexington for over 2 years. The one on one attention from staff is very nurturing. For example Jeanie from physical therapy visits my mom just to check in and give her a hug. That type of attention is invaluable. I also like to thank the nursing staff, CNA's, and anyone involved in the day to day care of my mother. The combined efforts of all involved in the facilitation of her care is greatly appreciated. Negative experience: The owner of HZC M71, B.D. In July. Car make and model, a Chrysler 200, 4 door silver gray car tried to kill me and my son 3 times. The female was a tall black woman with blue hair, wearing dark blue scrubs on bottom, with a white and green top. This female got out of her vehicle in the parking lot of Lexington health and rehabilitation center smelling like weed , smoke just bellowing out of her vehicle she just hot boxed, threatened to shoot me and my minor son dead. She continued to approach my vehicle, smelling like weed. She clearly works there, she was driving under the influence of weed, went in high as a kite to work at Lexington health and rehabilitation center. Black female with blue hair was laughing acting completely inappropriate, while wearing a name tag with the company’s name on it. My son and I both suffered severe injuries, I called and spoke to Louis the manager, who said he would call me back as soon as he identified her. Louis never called back. My attorney went to take more pictures and gather more evidence since I plan to sue both parties for damages and abuse of a disabled person, not to mention she threatened to kill us and had a weapon. To our surprise her Chrysler was still parked in front of the facility. She had backed in to conceal her tag. I am posting this so you can see Lexington health care and rehabilitation does not care if there workers abuse you or drugs, only about money they can con out of disabled people. Louis stated there are cameras on the front of the building, than why is she not in jail or fired? No one in any state is allowed to work, driving under the influence of drugs. Just another violation of our rights. I plan to make sure this black, 5’9 female is held accountable. I am calling in this health center for a complete audit. Please demand drug testing here. Call your government and report abuse to your loved ones today.

Negative experience: The physical and occupational therapist here are fabulous. However, we have been very dissatisfied with the way the basic needs are being met. Missed pain medicines, not getting toilet paper, and rashes due to neglect.

Negative experience: Interior is very clean and well maintained. The dietary department can't find me in the system. No menus assigned to me. When passing out trays I finally get food that I have randomly picked for me. Today I was told pork chops, mashed potatoes with gravy, mixed veggies but then I get a fruit salad with cottage cheese... Room 208A

Negative experience: My mother was just “housed” there. No medical attention, only “meds” to feel no pain. Both legs swollen for a week...no diuretics until my 91 year old father TOLD them to give them to her. After three weeks, insurance ran out and they released her to go home. 24 hours later, a Registered Nurse was sent out that dad requested from the insurance company. RN listened to her heart. RN told my dad to have someone take her to the hospital immediately. Turns out, she has Congestive Heart Failure! Thank God her insurance ran out, had to go home, RN came in, said she needed to go to a hospital ASAP! Dad said had he let one more week go by and kept her at Lexington, she would have died there!

Fantastic experience: I am a current employee of Lexington Rehabilitation Center. What drew me to their family was the confidence they have in their Vision -BELIEVE- B-est practices ,by opening the facility to nursing programs to bring future nurses and cna's in to be educated hands on. E-xceeding expectations by choosing me based on experience in Social work,a nurse whom have over 20years experience and a cna over 23years to advocate for over 100 Resident's if needed to be their voice!! So I recommend family,friends and Loved ones whom need strengthen after a set back Lexington Rehabilitation Center is the place to come.

Fantastic experience: I visited two days ago. And found it to be clean, neat , well-appointed with very friendly staff. My friend there who is a patient, says she loves it. I found nothing wrong with it. In fact it was nicer than my house. I've made numerous comments on different social media about how impressed I was with the Lexington.

Negative experience: Don't take anyone there if you care about them at all. I have been dealing with them for months now and they keep saying they will do better but they don't. My mother has deteriorated every time she is there, she goes to the hospital and gets better, then back there to go back downhill again and round and round. We had a debit card stolen from her purse and $1,000 taken from her account. Other personal items also have gone missing. I get phone calls from my mother begging me to call them to get someone to help her cause they won't answer the call button. We timed it one night and it was over an hour before they went to help her. And they only went then because I made 4 phone calls to the Asst Director! They have left her sitting in waste for hours causing terrible sores. I can't even begin to tell you all the pain and misery this place has caused my mother. I had heard good things about them but I haven't seen any sign of it. I am moving my mother as soon as I get her out of the hospital again. She is 81 and has health issues but that doesn't mean it's ok for them to mistreat her and just stand by and let her die.

Positive experience: The staff at Lexington Rehab were helpful and were really concerned with my well being. The staff was introduced to me upon arrival and each one has a part in my healing process. I was made to feel important and was listened to. Thanks to all the staff that treated me like family and not just a person occupying a bed.

Negative experience: I'm not going to go into the gory details, but I will tell you that the incompetency at this facility is unbelievable, from top management on down. They will give lip service to whatever the issue is, walk out of the room and do nothing. No communication between shifts. A dangerous and unsafe facility.

Positive experience: So beautiful on the gulf coast. Auntie is feeling better, she had a bad day today with BP and CH F. O2 running at 2 L. Using bed pan NWB on art. Leg. I offered her a prayer She will be better tomorrow. Staff is wonderful , met her CNA's Talked with Nsg staff too. Seem pleased I have arrived in Seminole.

Negative experience: Spent 3 weeks in rehab. Non-smokers are NOT welcome anywhere on property. Food fit for dogs and pigs. Nutrition purposefully lacking to save money. Physical Rehap awesome. A few excellent administrators. Some CNA excellent. One was on drugs. Some plain dumb. Needs to be shut down. Mission statement full of lies.

Negative experience: My father in law fell and had a minor break in his hip. After a short stay in the hospital, he went to Lexington for rehab. During the week or so that he was there, the care and attention was terrible. He fell several times, got scabies and another infection. He ended up back in the hospital after his blood levels for coumadin were not properly monitored. About two weeks later he died. I could not and would not recommend this facility. Negative experience: This is one of the filthiest places I have ever been in my life. There is caked on dirt, food, feces etc.. in the hallways and rooms, the PT room has blood or another dried substance on the wall near the sanitizing station. My husband (hip replacement therapy) was supposed to be admitted Sunday 2/15/15 first thing in the am. The transport driver came to BAYFRONT hospital to pick him up at 8:30am and was told to turn around and go back as the room at Lexington Rehab Rapid Recovery had to be sanitized. When he finally was admitted at 4:30pm to the "sanitized" room, the wallpaper was peeling from the walls, bugs were climbing on the walls and the baseboards and corners of the room were black from months and months of filth. I HAVE PICTURES OF ALL OF THIS. The food portions are miniscule and the food is all processed loaded with salt, no fresh veggies or fruit is served. Bfast today was corned beef hash with toast and oj. Who eats corned beef hash these days. This facility must be experiencing financial difficulties. All but a few of the nurses were helpful, everyone walks around looking like they are busy but most of them are sleeping in the TV room which is also filthy and cups from starbucks are sitting there from days. We asked to see the Head Nurse to get an early discharged since he had been there since Sunday and had only 2 therapy sessions. The staff kept telling us they called her and 24 hours went by and we never heard from Barbara and finally I had to raise my voice to get some answers and she came to see us and told us the staff never contacted her. We got the runaround the whole time he was there. We had to request he have his bandages changed. I was under the impression that when a patient is leaving a hospital to be discharged to a rehab facility that a list of facilities need to be presented to the patient. Christine who Bayfront hires to assist patients in this procedure never presented a list, she mentioned Lexington, Apollo and one other, not up to par facility. If my husband did not have me an advocate (squeaky wheel) to get some action to this shoddy facility I am sure he would have still been in there. Don't allow your loved ones to go to this place.
