Fantastic experience: When I toured the facility for my mom, I was greeted with smiles and everyone was helpful. While touring I met some of the nurses and they seemed very nice and down to earth. The facility was clean and organized and some of the mobile residents were playing games. I loved the intimate feel to this place!

You made this Time in This facility Trying To get back to where I was So much easier. No 1 wants to be away from home on a trip like this But you made it easy. Of all of the people that are here in this facility I never met anyone that wasn't kind And caring. I am so glad to be going home today But I will miss all of you. Always seeing smiling faces Helps a person To continue on everyday. I will say This has been the cleanest nursing home I've been in a while. Only on visiting Other homes. And what can I say about therapy? They were caring concerned and light hearted. But they got results from people here in the facility and from people from out on a temporary basis like me. I'm not much of a speaker. But I do have 1 request Please don't any of you change. Don't let this world change you. You are great as you are. Thank you all

Fantastic experience: From my experience with this facility ,people do their jobs to the best of their abilities. From all depts, they try to get the help each person is in need of, the business dept works very hard in getting answers for the clients. Sometimes it's different than what people expect,but they do work together to follow thru. I recommend this nursing facility for your family members, to get the help they need.

Fantastic experience: My grandfather was at this facility for around 4 months and the staff was always super friendly and attentive. They always helped him in any way they could. I would recommend this facility to anyone with a family member in need.

Negative experience: My dad was only there for 20 days and boy they were something else. I always try to allow people to do their job but when you start violating people's rights that's it. We'll Ms Roxanne (Roxy) takes it upon herself to contact her own personal VA contact to ask questions. The VA did not cover any of my dad's expenses there so she should not have communicated with them at all. When I stepped in to get things in order, she was giving me push back. Baby this is my daddy let me do this. I told her what the VA was telling me about them possibly paying for 30 days and she insisted I was wrong. Well she then came back and said I need to call someone when I had done the work already. She did not do anything about it and would not give me the social worker name. We'll God sits high and looks low, on that third day I went to ask had she did it and her office was closed so I walked across and guess who office I was in, you guessed it the social worker. Yessica was all that and a bag of chips.(Thank you) She helped me figure out the whole situation and I was able to get my dad transferred out because he was in the wrong place to begin with. There is so much more but for sake of time, DONT BRING YOUR LOVED ONES HERE. I will add this too the nurse called the night before trqnsfer and tried to "drug" my dad up and we said no, so what she did was add the med to his chart so the receiving facility would administer it, good thing we were on it and caught it the first day and told them we did not authorize that med. A dirty shame. Huge shout out to Shana and Yessica that is how I was able to tolerate that place.

Negative experience: The kitchen supervisor is the worst...the kitchen stays nasty none of the cooks can cook and no one reads the menu..we need state in the kitchen at Jefferson like asap..the one they call chi needs to be terminated asap... The smell when u walk in the building will turn your stomach that further more let's me know they are not cleaning these ppl

Negative experience: My husband of 33 years went to this place after having hip replacement surgery. He wasn't there for 6 hours before he started to have problems they ignored him and left him to die all alone Then they called me the next day to tell me he had passed away and when I got there he had never been moved out of the bed no ambulance was called it was terrible do not take your love ones there unless you want them to die

Fantastic experience: During my first day at orientation I was greeted nicely. Everyone said hi that we walked by! The best feature is that they are adamant about abuse and how it is not allowed! I absolutely love that. Im completely against abuse and I finally found a nursing home that forces this correctly! I'm glad I can be part of the nursing staff.

Fantastic experience: I find the previous reviews of this facility not my experience. My Mother-in-law was admitted a few days ago after suffering a stroke. The nurses check on her frequently, are cheerful and competent. The facility is modern and clean. Their goal is to get her functioning on her own as soon as possible. She is already sitting up and doing small actions on her own for herself. The other residents seem happy and cared for too. At night it is quiet and during the day it is busy but orderly. I am very glad we picked this facility for her rehabilitation.

Negative experience: Didn’t hook my loved ones oxygen for hours then forgot her in the bathroom within 5 hours of being there. Don’t send your loved ones here unless you want them to die.

Negative experience: My mom has been there two days now and I’m already thinking about taking her to another facility…they didn’t give me no type of orientation I had to ask them questions…I feel that it’s no type of organization…also this is a business you supposed to have at least talk to the family and let them know how the facility is ran the different types of activities they do with the residents, how often the doctor sees the residents ,transportation etc was not explained to me instead when I mentioned it to the aid that was on my mothers wing I had to inform the manger that when I arrived there with my mother for admission she was Covid tested and when we were notified that she was negative I was told to bring her to her room… now I was like no one came to explain anything on how the facility is ran and no tour was given to me

Negative experience: How dare this place try and act as if they care about their patients. Having the call button on the floor out of reach… coffee stains all over the sheets. Do better. I won’t stop until you respect the elders who come to y’all for help. Have some respect and common human decency. I bet you wouldn’t like to sleep in “coffee” stained sheets. Sickening you could treat others this way.

Fantastic experience: Jefferson Nursing and Rehab cares about it's residents! They are here for it's residents and families! They know when to step; up to the plate and address any issues. They also go as far as giving out their cell phone numbers so we may reach them even after hours. If you want to feel at home stop by to speak to the staff and give them a chance to show you what they are made of by taking care of your loved one.

Fantastic experience: I love working at Jefferson rehabilitation. The staff is caring and goes extra miles to satisfy our residents. I am a dietary manager, and I am working to provide food for the residents according to their diets and desire. Communication is strong and that's what makes me happy.

Fantastic experience: Jefferson has the Veterans Contract for the community and I love Danny the DON. Kindred Hospice had a great Veteran's Event there which honored the residents and staff members that has served our country. I have known many of the staff members for years. In these trying times they have continually strived and work very hard at giving great care for our community. Keep it up my friends !!!!!!!!!

They both demonstrate the core values of excellence and honesty for the residents and co-workers. Negative experience: My brother is a resident there and he’s not treated very well, he hadn’t gotten a shower in a week we had to inform the staff that he needs a shower.

Negative experience: This review is from Pamela D., not Tayla D. My father was in rehab at this place and it was awful. On the first day, when I walked through the front door, there was an awful smell. The second day was not that much better. He had an excellent CNA, think her name was Tamika or Shamika. And I bet she makes the least amount of money at this place. My father fell twice (that I know of) and it was not reported until he told one of his children he fell, then they made a report in his file. My father said on the second day there, he did not get his medications, although it was noted that he did receive meds. He is an older gentlemen, but he has a memory like an elephant, and after seeing what little care is given to patients, I believe him. Supposed to be in rehab every day, or so I thought, yet on one occasion, he was not given rehabilitation (don't know why), could not find anyone to ask most. Sister brought his medication list to the nurse, yet told my brother that one medication they said they did not have at the place. Room was clean most of the time, but sometimes not clean. We told them that he did not eat chicken and almost everyday they had something with chicken in it, so they would bring him a sandwich which consist of two slices of bread and meat, no chips, no nothing. As a matter of fact, lady brought sandwich in her hand (it was wrapped), but not on a tray. This place never even bothered to find out what foods he eat. Insurance companies pay these places well I am sure, and the customers are not getting quality care. While in his room on 10/31/18, I noticed his speech was not clear. My brother went and got a nurse and he said he was not familiar with my father and did not know how he talked before, so he was going to go get his regular nurse. Regular nurse never came, nurse took his blood pressure and said it was fine, brother asked that father's doctor be called, so attending nurse called my daddy's doctor and doctor told them to call ambulance to get him to hospital. At this very moment he is in intensive care. In my opinion, not a good place to rehab. Also, you would think in the area it is located in, it would be immaculate. Need to hire more people or train people already employed there. If he is release from hospital, and has to go back to rehab, it will not be Jefferson Nursing and Rehab.

Negative experience: My mom was put here for respite care. Care really was lacking here. Lost her meds, didn't clean bed side potty for hours. When called, no one showed til repeatedly called. When she was admitted she came by ambulance, had a blanket around her, that got added into their stuff. Looked for it til we decided that it wasn't worth it. Overall place wasn't too clean. Rooms super crowded. Bottom line....NOT A GOOD EXPERIENCE!

Negative experience: Horrible!!! They treat their CNA as if they are in slavery !! The Administrator is very unprofessional and he treats employer's like prisoners it's just not a good place to work.. Nice facility and residents. but their Administration team are awful.. and no I'm not the right fit for them!!
