Negative experience: This facility does not deserve one star. My father resided here for approximately four months in the second half of 2025, and the level of care was unacceptable. The facility is severely understaffed, and as a result, residents are routinely neglected. On multiple occasions, my father was left sitting in soiled diapers for extended periods of time—even after notifying staff that he needed assistance. He would call me from his cell phone asking if I could come change him because no one was available. We timed these delays on several occasions, often exceeding an hour. During his stay, he suffered repeated UTIs, which his physicians confirmed were caused by prolonged exposure to soiled diapers. I reported these issues to HHS due to the frequency and severity. Bed linens were also frequently left soiled for most of the day. During visits, I often waited over an hour for staff to assist, and I ultimately had to locate clean sheets and supplies myself to clean his mattress and change his bed so he could lie down. I regularly had to change his diapers personally because staff was unavailable, and the unit was frequently short on basic supplies. Hygiene care was routinely missed. My father often missed scheduled showers, and his wheelchair frequently smelled of urine and feces and was visibly soiled. I repeatedly requested that the wheelchair be properly cleaned and sanitized. When staff finally addressed it, they were only using soap and water, which is not adequate sanitation for bodily waste. I asked twice for proper wheelchair sanitation to be written into his care plan, but it never was. Medical oversight was also deeply concerning. The physician who oversees this facility comes from out of town only once per week and often does not see all residents. Communication between the nursing staff and the physician was extremely poor. At one point, my father had persistent diarrhea for nearly three weeks. Staff suspected C. diff but were unable to obtain an adequate stool sample for testing and did not follow standard C. diff infection-control protocols. Despite repeated failures to obtain the sample, they refused to notify the physician that the requested testing had not been completed. When my father began having blood in his diaper in addition to the ongoing diarrhea, the facility had no clear plan of action and still did not escalate the issue appropriately. At that point, I transported him to the emergency room myself because the staff was unable or unwilling to manage the situation safely. He ended up being negative for c-diff but did have an advanced UTI which nursing staff at the home never suspected or tested for. Care plan management was another major issue. Despite multiple care plan meetings, changes discussed were often never documented. I had to repeatedly follow up just to obtain copies of the care plan, only to find that agreed-upon items were missing. When I requested updates, I was ignored for longer than the federally required timeline for updates and had to threaten to get HHS involved again just to hear back. The facility ultimately involved the state Ombudsman because they disagreed with my requested changes—yet the Ombudsman agreed with me, making the entire process unnecessary and prolonged. Staff turnover was constant. There were multiple overnight shifts when only one CNA was present because scheduled staff failed to show up. Every visit confirmed the same issue: chronic understaffing. The administrator dismissed concerns by stating that “no one wants to work” and that all nursing homes are experiencing this. That has not been our experience. My father has been in two other nursing homes—both before and after this facility—and neither had staffing or care issues like this. His current facility is adequately staffed and meets his needs consistently. Whether due to poor management or budget constraints, this facility fails its residents. No one should have to endure the neglect my father experienced here.
Fantastic experience: My grandmother took an unexpected fall at her Independent Living community and ended up in the hospital for a short term stay. She was discharged to a Rehab facility and Sandi Beard took the reins from us and guided us through the entire process, making it as smooth and easy as possible. It was a great experience from start to finish with Sandi and Anita were there the whole way to answer any questions or concerns that we had. Thank you guys for all you do and have done for our family!

Fantastic experience: What can I say about this place other than it’s just such a wonderful place with wonderful caring people and staff. With the new administration the place has become an even better care facility. As a person with family in this particular facility I would highly recommend the oaks for all your long term care needs. Negative experience: the residents are poorly taken care of there’s not enough CNA’s to watch everyone they leave their station, my dad had clothes and blankets there and they mixed everyone’s clothes up they threw away his blankets as well, me and my tia and grandma came to a meeting to “find ways to help my dad” we came with a solution to help him and they didn’t do what they said they would and now that’s why my dad isn’t here with us today

Negative experience: The smell of the place almost made me sick. We see the type of person you are if you stick your loved ones here..
Negative experience: My dad stayed there 5 days with Hospice. They were terrible. He set in a wheelchair all day. They took all his clothes I bought him after I told them I would come every day and get his dirty clothes and bring them back. Everyday when I went all his dirty clothes were gone and they just said sorry. So we left with the clothes on his back. He had bruises and sores on his arms. I told them I would get him at 5 on Friday but when I got there they said they thought he was leaving on Saturday so he wasn’t ready. He was dirty and they put a mattress on the floor and let him sleep on it. He is 84. That is a disgrace.

Fantastic experience: This is my 2nd nursing home facility i’ve worked at and i love working here so much. The residents, the leadership team, the staff are all so Welcoming and full of compassion. This facility has had some ups and downs like any other facility but the way it has become and continue to become such a happy and bright place, its Amazing. Everyone is very resilient and extremely optimistic!!
Fantastic experience: WOW!! Is what I have to say about the Oaks! My momma is there for a skill stay she doesn’t speak English and has dimentia. Ofcourse I was so nervous at first but the whole staff I mean the whole staff starting from the director Sandy Beard to the social service Emmy the CNA’s LVN’s RNA’s the physical therapist, Shelly the director of nursing Synde human service coordinator, house keeping EVERYONE IS AMAZING!!! Anytime I need a little break with being a caregiver to my mom I will bring her here for restpit! This is the place I will bring my momma when she has to be in a place long term for sure. I haven’t seen my momma so relaxed with everyone in a long time. They are giving her a safe space and I’m so grateful for them!
Fantastic experience: The Oaks at Radford Hills staff, nurses and therapists have been amazing for my mother in law. She has not had one single complaint—from the food being so good she hasn’t even asked for her favorite Chinese take out to the therapists working with her to get her stronger—everyone has been so caring and attentive. This place has not only met but greatly exceeded our expectations. We are so grateful for the hard work of everyone here to make this process as easy and positive as possible.
Negative experience: My mom stayed there 20 days for rehab. During that time they failed to inform us she wasn't eating or taking all her medicine especially her liver medication. They leave call alarms going constantly. My mom called me many times when they failed to change her in a timely manner. They never listed her dietary needs in her chart. Wouldn't fight to get her help from the ER. I'm certain they caused my mom's liver condition to worsen and speed up. She lost about 30 lbs there, was diagnosed with severe malnutrition once discharged. They tried to discharge her without oxygen when she needed it. Not only that they took her off without doctors orders, even lied to me about her being weaned off oxygen. I went same day to see her and she was on oxygen. No one could give a straight answer. I got her home in July and she just kept going down hill. She even developed a lvl 3 bed sore in their care that they some how missed with daily changes Don't trust this place. Rip mom

Negative experience: The only reason this place has a 3.8 rating is because the employees are speaking highly of themselves. I just went to deliver a pizza and the nurse on shift in the middle was quite rude. As someone with a bachelors degree (BIS) and delivers pizzas as a side job, I do not appreciate being spoken to like an idiot. I am highly educated, and would like to file a formal complaint. As I left one of the residents told me they are also rude to the residence and treat them like retards.

Negative experience: +The LVNs were all good and friendly but understaffed like most places. One of the food service girls new she was having a hard day and prayed with her. That says a lot! Thank you! The seniors that can function on there own would probably do good there. -The management needs to take lessons from the staff on how to be friendly and helpful. They never call you back, they don't introduce themselves when they come into the room, never complete the paperwork for long term care. The rooms are nasty, roaches, mice, food and spilt drinks on the floor, dirty bathrooms with no soap or paper towels for the 2 months she was there. Even after requesting them. - She came into the facility on Hospice with an unknown life span. Hospice would give orders, but somehow, they never all got given . She passed away suffering.

Negative experience: This place is an absolute joke!! I have a friend that is in there right now and is in very bad shape. He's not receiving his medications and has had to call an ambulance to be taken back to the hospital to receive them. The staff that has been in there to care for him leave their trash all over the bedside table instead of putting it in the trash and the floor looks like it hasn't been mopped in a week. I understand that you may be short staffed but this is UNACCEPTABLE!! I will be reporting this place tomorrow to the state and if I have to I will go further!

Negative experience: There are a lot of roaches. They did not clean my loved one and my love one is in the hospital with a major UTI. They do not take well care of the patients and they neglect them.

Negative experience: Used to be a decent place to do work for as a vendor, until they got bought out and their big parent corporation decided to quit paying their bills. They don't care about the long term relationships with their vendors.

Negative experience: While the staff is *friendly*, my father in law did not receive the care we were told he would have. Also, the ROACHES there are awful! We are still battling them after 3 weeks and we've NEVER had roaches in our home before! STAY AWAY FROM THIS ROACH MOTEL! Im going to have to call Orkin AGAIN to come spray my home! Disgusting place!!!!!!

Positive experience: My Dad has been here for almost a year. He loves the staff. Everyone treats him great and with much respect. This is a clean nursing home. Food could be a little better but overall its pretty good.
Fantastic experience: My was supposed to die. The hospital discharged him to go to the nursing home on hospice with counseling that without dialysis he would die. That was last year in April. He has since been released home and is doing better than ever. I just want to give a shout out to the staff of The Oaks at Radford Hills for doing such a great job during the pandemic! Y'all Rock!

Fantastic experience: In the 20 years I've been here I've worn many 'hats' here at The Oaks of Radford hills, from CNA. RNA..CNA staff coordinator..Medical Records, Central Supply & CMA. !..It is a pleasure to serve these Wonderful Ladies & Gentlemen here for so many years! .they & my peers here are my extended family !...We have such a loving,devoted , professional staff who just love to love on our Residents!.. ❤
Fantastic experience: I’ve been working here since March and this has been the best place and the friendliest environment to work in. The residents are wonderful to take care of and I enjoy coming to work each day!
Negative experience: I can’t tell you how disappointed I am in the way my immobile grandmother was treated in this facility. We frequently had to drive to the facility every few hours to be sure she was cared for. Looking back we should have moved her to a different facility. It tells you a lot when the employees have to give reviews to bolster the overall rating of a facility.

Fantastic experience: The staff at the Oaks at Radford Hills were wonderful to my dad while he was there. The therapy staff especially helped with the difficult transition for my dad. Sandi, Ginnifer, and Lisa just took my dad in and went above and beyond to help him. I have to say a very special thank you to Sandi Beard director of therapy for helping us with the transaction and making my dads time there great. We could not have made it through this time without her. She was a blessing to my dad and a Gift from God to the family. Thank you doesn’t seem like enough for all you have done.

Though I have been with Senior Living Properties (SLP) for 1 1/2 years, I have been an Administrator for almost 15 yrs. From the top down, SLP is the best company I have ever worked for. Their commitment, dedication, support and compassion is unmatched. The great leaders and caring hearts that comprise my team at The Oaks makes a world of difference to the residents and their families. The Oaks at Radford Hills will successfully collaborate with local healthcare organizations to ensure that continuity of care is followed through in order to achieve the highest level of medical treatment for each individual. It is my commitment and promise to ensure we deliver the highest quality of care to our residents. Thus, I strive to make a positive difference with those I work with and those that we care for the most.....our Residents.
Negative experience: Hiring dept doesn’t have their act together. Got called for an interview then it was took away, their explanation was we are sorry but position is now filled.
Negative experience: Extremity bad management, staff doesn't even give all the equipment to the patients. Graveyard shift is terrible
Negative experience: The weekend crew don't give the love ones their meds. Your loved one will end up in and out of the hospital. That is why I give them one star. Also the admin and the ones above don't listen to the families concerns about their loved ones. They call you and tell you we can't have them back after you place you concern about your loved one. DONT SEND YOUR LOVED ONES HERE!!
Negative experience: I only rate 1 star because I have to in order to let everyone know how very,very unprofessional, rude,uncaring this place is. It starts at the top and goes all the way down to the bottom. If you are just trying to hide someone because they have become a burden on the family then this place is for you but if you love your family member then this is not the place for them. Read the government's report, citations and the fines the have put on this place.

Negative experience: Residents are neglected. "Nurses" do not care about their patients. The fake nurses do what they please without regard to patient welfare. Nursing management is horrible. Poor supervision leads to terrible care. Would not send my dying dog here
Negative experience: Horrible facility with awful administrative and kitchen employees! The kitchen fails to provide proper meals for residents such as, cereal with no milk, incomplete meals without proper accommodation to disabled seniors such as unprepared pb&j sandwiches that residents, who are unable to do, must make themselves. Residents are left still hungry and feeling like inmates due to improper proportions. To my understanding the administration and management does little to nothing to improve these issues. Very careless and unprofessional in my opinion, with little care for residents. I DO NOT RECOMMEND THIS FACILITY!
Negative experience: Kayla Brown works here and that's why she gave 5 stars. Don't be fooled. This place neglects patients and from my experience they don't care about the residents. Keep your loved ones away!! Numerous instances where their "care" is questionable! This place should be shut down!
