Fantastic experience: This office is wonderful! All staff I’ve encountered over the last several months have gone above and beyond, and have been a pleasure to work with. Especially my sons pediatric PT, Bianca! She was absolutely wonderful. She was dedicated to helping my infant son with his torticolis & her plan was very effective. She is certainly an asset to this team. Thank you all so much!

Fantastic experience: Great place to work. I was a Speech Therapy intern during summer 2022. My supervisor, Alyssen, was so incredible and kind during my time there. All the therapists and workers at this clinic made me feel so welcome always. I love the fact that this clinic has many students during the year- I think this provides fantastic opportunities for students to learn from different environments/therapists. I'd love to work at this clinic at some point; Brooks Rehab was a blessing.

Fantastic experience: I would LOVE to take the time and give my review for Patrick Pham, my son with just 13 years old had a ACL injury. After having a not good experience on another Brooks location we had the blessing in ending on Patrick Pham miracle hands and work ethic. He was just an angel on our lives. He has passion, focus, love in helping others. My son was up and running within months playing football again and getting his security back. Having someone who really cares and loves what he does makes a positive impact. We love him and will recommend him to anyone! Thank you Patrick Pham!

Fantastic experience: I entered physical therapy a year ago in order to help my flat feet. Walking/standing without pain was very difficult before starting PT. My physical therapist, Patrick Pham, did an amazing job of strengthening my feet and legs. My arches on my feet have raised so much due to his training. He provided me with such helpful exercises in order for me to heal. Before PT, I could not even walk a quarter of a mile without pain. Just a year later, after intense PT with Patrick, I walk over 4 miles each day in a big city with virtually no pain. In addition to Patrick's professional expertise, he is such a compassionate and understanding person. He establishes caring relationships with each and every one of his patients. An appointment with Patrick does not feel like an obligation; instead, it feels like a meeting with a friend who just cares about improving your health and wellbeing. Patrick gradually pushed me to levels I didn't know I could handle, while persistently encouraging and believing in me. Because of Patrick, I can live my life doing what I love without terrible foot pain. Overall, Brooks Rehabilitation Outpatient Clinic-San Pablo has such a fun and caring atmosphere that one cannot help but smile and enjoy their appointments. It is so evident that each physical therapist cares greatly for their patients. I am so grateful for Patrick Pham and Brooks Rehabilitation San Pablo for such an amazing experience.

Fantastic experience: Brooks Rehabilitation in San Pablo, Beach Blvd ,Jacksonville Beach Florida is great. All the staff are awesome they are nice and respectful. The therapist they are doing excellent job. Specially Patrick Pham and Pat Gaffigan. In just 2nd sessions my son start moving his toes and 3rd session he start walking.. I'm 100% recommend them to my friends and relatives.

Fantastic experience: After having hardware installed in my ankle and not walking for months I started attending this Brooks and it was great. I worked with Patrick from the start and he made my feel very welcome. He was always asking the right questions and worked out a solution for when something seemed off. I also met Pat Gaffigan and he was awsome too, really helping me put more weight on my ankle and ironing out things wrong with how I was walking. I really felt like they were both very professional, everyone knew what to do and everything went smoothly. I would recommend anybody that needs PT to go here. Thanks Pham!

Fantastic experience: Patrick has made my recovery from bilateral knee replacement a truly rewarding experience. He guided me to push myself and the results are amazing. I haven't felt this good in years. I almost hate to end my regular visits but Patrick has given me the ability to continue my exercises at home to keep in the best shape ever. Fantastic experience: I have used this Brooks location twice now. Once for my neck spinal fusion and PT related to an accident that injured my neck after fusion. I had the absolute pleasure to have Patrick P. as my therapist on both occasions. I've unfortunately had a lot of injuries/issues over the years and spent quite a lot of time in PT and Patrick by far has been the best to work with. He is extremely knowledgeable, very creative with exercise plans, but more importantly he listens! He cares about you getting back to full strength, asks the right questions and listens to the feedback, even when I don't know exactly how to convey things. The other folks there all have had positive attitudes and are super approachable. All in all 10/10. I hope I don't need PT again, but if I do, I'll be coming back here.

Fantastic experience: Just recently completed physical therapy on a knee injury. I can't thank my therapist Patrick Pham enough! Patrick was professional, thorough, caring, and kind! He made sure I had the necessary exercises to get a full and complete recovery. Ask for him by name, you will be glad you did. I also need to mention Emily who filled in when Patrick had other patients, she made sure I continued with the regimen of exercise for continued improvement.

Fantastic experience: A year ago the VA sent me to Brooks Rehabilitation on San Pablo and Beach to work on my left shoulder and neck that had very limited range of motion and numbness. The postoperative treatment was needed after having stage 4 head and neck cancer whi h included a neck dissection. A year later after extensive work with my physical therapist Patrick Pham, I no longer have numbness in my left hand and about 90% range of motion back in my left shoulder and neck. Patrick used a partnership/team approach that yield significant progress as we journeyed through my rehabilitation program. I am grateful that I have Patrick as my physical therapist, and consider him the best physical therapist I have ever had. Additional, the entire staff at Brooks Rehabilitation is great and I have always been greeted by a warm welcoming smile. I have been most certainly blessed by this experience and highly recommend Patrick and this Brooks Rehabilitation location to anyone who wants the best physical therapy possible any where. Fantastic experience: I apologize for not having submitted this a long time ago. I tried on numerous occasions on the computer and cell phone and couldn’t figure out how to get to writing the review section. Patrick Pham was upbeat, friendly, patient and helpful on all of my visits; more importantly, he took the time to explain the various exercises and why I was doing them and always checked to make sure I was doing ok and not overdoing it. Fantastic experience: I have been treated at Brooks Rehab San Pablo by Patrick Pham for several ortho problems. He is the most professional and knowledgable practitioner I have ever had the pleasure to experience. Unlike all other PT clinics I have utilized; Patrick takes a personal interest in your recovery and does not run the patient through a “standardized” regimine of exercizes. I have had 100% positive results from his treatment and highly recommend Mr. Pham.

Fantastic experience: Patrick and Marcus did a great job of assessing what was causing me right hip and glute pain. They really listened well to what I had to say as a patient. I learned simple exercises from them that I can do to help relieve the pain and tingling sensations I’ve been experiencing. I would highly recommend this location to anybody.

Fantastic experience: I have had 2 knee replacements in the past year and gone to Brooks Rehab on Beach and San Pablo and my therapist, Mallory is absolutely the best....I had not walked without a wheelchair, walker or cane in over 12 years but by the time I was through I rang the bell and walked out. I no longer need any of those things...I highly recommend going there for your needs as everyone is great.

Fantastic experience: Having had Patrick as my physical therapist, there’s nothing about the experience that i would change. The environment held at Brooks was extremely positive and made me feel safe and supported everytime i went. Not only was the care focused on my physical health, but my mental health as well. Patrick always ensured that Brooks was a safe space for me and was very flexible and understanding regarding every situation!

Fantastic experience: Patrick has brought me from walker to walking. February 16, 2022 I had a total knee replacement. After 5 weeks therapy with Patrick I rang the bell and walked out on my own. Patrick and the staff at Brooks San Pablo are the best. If you need agressive, caring therapy of any kind come to Brooks Rehab at San Pablo.

Fantastic experience: Words can not express the gratitude I have for Mallory. My daughter was seeing Mallory for Physical Therapy following a PARS Fracture and was experiencing a ton of back pain. She was "down in the dumps" because she was unable to pitch and play softball. Mallory provided a great plan of care for Hayley and always considered her personal goals. Hayley improved each week and followed the home exercise program Mallory provided. Fast forward to now, Hayley is now playing softball and pitching again without pain! We are forever grateful and thank God for Mallory's expertise! She IS ONE IN A MILLION!

Fantastic experience: I JUST FINISHED 6 WEEKS OF PT FOR MY NECK, MY THERAPIST WAS PATRICK PHAM WITH JOANNA AS MY ALTERNATE THERAPIST. I DO 3 MONTHS OF THERAPY EVERY YEAR ALWAYS AT SAN PABLO. EVERY YEAR I LEARN SOMETHING NEW TO ADD TO MY AT HOME ROUTINE. PATRICK WAS EXCELLENT EXPLAINING THE HOW AND WHY OF EVERYTHING AND I LEARNED WHICH EXERCISES WOULD WORK THE BEST TO CONTINUE DOING AT HOME, BROOKS IS THE VERY BEST OF THE BEST AND EVERY THERAPIST IN SAN PABLO WILL GO 100% TO HELP YOU. Fantastic experience: I had the opportunity to receive treatment for lower back pain at this facility. For many years, I have gone to many of these therapies in other locations throughout Jacksonville. This one stands out from the rest. I attribute this feeling of satisfaction and gratitude to 2 particular therapists at that location, Patrick Pham and Johanna Fonder. From the start, they were able to identify accurately what my level of pain and discomfort was and waht it would take to improve it. In just 10 treatments, I felt tremendous relief. The exercises were precise to say the least. They hit the target and the results are great. I will take these exercises with me to continue to feel this relief in the future. I thank both Patrick and Johanna for their care and the friendly atmosphere they are displaying evry time I walk in for a treatment. Thank you.

Fantastic experience: I truly cannot say enough good things about this place. Patrick Pham is one of the most kind and caring people I have ever known. I owe him more than words could say. I have suffered with chronic back pain for over 10 years that was so debilitating it got to the point where I was struggling to walk. I had not worked in over a year and a half when I went to go see him at Brooks therapy. Every single visit, Patrick took the time necessary to understand what was going on with me that day. Took the time to figure out how I had responded to therapy from the last session. Catered my therapy according to the pain that I was in, and my needs and limits for that day It wasn’t just treatment of my body, but my soul as well. I was being treated as a person. Not as a number. Not as a case. But as a human being. My mental condition was taken into account, not just my physical condition. How my pain affected my life and what I wanted out of life were the primary drivers of my PT. THEY LISTENED. HE LISTENED. And I’m getting better for it. I am proud to say that I am well on the road to recovery. My lower back has been restored. A back the doctors said would never work properly again. And now I am learning how to walk again, how to run. How to lift how to squat. How to hold my core properly. I am working part time. I anticipate by the end of this year I will be working full-time. Where I was not able to walk for more than a few blocks before, I can walk for over a mile, and it gets better every day. I’m getting better. I’m getting my life back. I’m so grateful to every person there who helped me. All of the staff were extremely professional. Extremely courteous. All of them seem to genuinely be there to help, to uplift, to heal. If all doctors treated their patients the way I was treated at this Brooks, the world would be a healthier happier more wholesome place. Thank you to all of you who played a hand in my recovery. And thank you to Patrick. You gave me a second chance at life

Fantastic experience: I have been to Brooks for both my ankle and my knee injuries. Monika was tremendous. Living an active life and playing with my grandchildren is my main priority. I wouldn’t be able to exercise without her help! The entire staff was friendly and accommodating. I always felt welcome and at home. Very professional and courteous. I wouldn’t go anywhere else for my therapy!

Fantastic experience: My experience with Brooks has been the GREATEST! The personal attention given has been unlike any I've received. I'm being treated for lower body strenght and balance issues and my progress has been better than I expected. They have therapists who specialize in certain problem areas so as I moved from strength to balance, I also came under the care of a different therapist. They gave me a "tool set" of excercises that I do at home without any special equipment so that once I'm finished at Brooks, I can maintain all the fitness improvements I've made.

Fantastic experience: Where do I begin to sing the praises of Brooks Rehabilitation? A hip fracture brought me to Brooks and was blessed with the most professional and patient therapist one could pray for. Patrick Pham not only has the patience of a Saint, but took the time to ask how I was doing in general. Not only was he focused on my care, he generally cared for me as person. His guidance and thorough explananation of work to do, got me up and running in no time. What can I say. Patrick is just "The Best". Have I mentioned you leave with a smile on your face? The entire facility is run smoothly and professionally from start to finish.The entire team joins in to assist others so that patients and staff have a smooth and efficient experience. Exercise equipment is pristene and wiped down after each patient. The entire area is kept clean and tidy with loads of smiles and encourgement from the entire staff. I will be forever greatful to all at Brooks, but especially to my awesome therapist and now friend Patrick Pham. As they say, "He's da Bomb!" And so is Brooks Rehab!

Fantastic experience: After I had the misfortune of having surgery for a broken femur bone, I was sent to Brooks Rehab for my therapy. The entire staff was very professional, friendly, and they really care about their patients. The facility was very clean and they were always keeping the equipment wiped down. My therapist was Patrick Pham and he was amazing. He was the upmost professional, explaining the exercises and having great patience with me. Patrick always had words of encouragement for me, and for that I will always be grateful, as he helped me to walk again.

Fantastic experience: I've had several great experiences at Brooks! Teresa at the front desk was kind and helped me find an appointment before work. The women's health PT set me up with a home exercise program to keep me confidently exercising and working throughout both of my pregnancies.The pediatric PT, Kate, taught us positioning strategies to help with my daughter's mild torticollis. Both of my parents loved the orthopedic PT team and were thrilled with their therapy results (for a total knee replacement and ankle injury).

Fantastic experience: I love Brooks Therapy. I had home therapy at first and am now going to the outpatient services. It is a new facility, very large in the back and has many working stations for the therapists. The people seem concerned and really worked with my bad leg. I have multiple sclerosis and my legs are very weak. I would recommend Brooks home Therapy and the outpatient services. The only complaint I do have is that- I never see the same therapist. When I had Brooks coming to my house. I had the same therapist every time and she worked with me for an entire hour. She charted after she worked with me. She worked with both sides, my right and my left legs. The brooks out patient only works with my left side and does not have a set time. They also chart the entire time I am there. They are very nice though. But if I had a choice I would rather stay with the Brooks home physical therapist.
